원발성 족근관절 퇴행성 관절염에 시행한 경골원위부 절골술 - 증례 보고 - (Low Tibial Osteotomy For Primary Osteoarthritis of The Ankle - Cases Report -)

  • Primary osteoarthritis of the ankle without history of trauma is rare. We are reporting the use of a low tibial osteotomy on 2 ankles in one patient. The osteotomy is designed to correct the varus tilt and anterior opening of the distal tibial joint surface where it has been shown by weight-bearing radiographs. Follow-up at 8 months the result showed to be excellent, by Takakura's scoring system. We experienced that slight overcorrection of deformity by low tibial osteotomy is effective in treating intermediate-stage primary osteoarthritis of the ankle.

로봇 손을 위한 인간 손가락의 관절간 운동특성 고찰 (A Study on Characteristics of Inter-Articular Coordination of Human Fingers for Robotic Hands)

  • One of challenging topics for humanoid hands is to modulate a human-like motion of humanoid fingers handling an object. To this end, recognizing the motion behavior of human fingers is very important aspect. Based on this concept, this paper identifies the .joint trajectories of human fingers for an operation of hand opening and closing, and specifies an empirical model that coordinates an inter-articular relationship of human fingers doing the given motion. It is expected that the inter-articular model presented in this paper is applicable for humanoid fingers to mimic the natural motion of human fingers.

  • The purpose of this study was to evaluate the effect of vegetation on the cutting slope applied by vegetation system in the whole land. The engineering characteristics of slopes were investigated using the face mapping and physical tests. From the statistical methods, a relationship between a geologic stratum and degree of covering was independent. Therefore, the afforestation of slope was affected more by condition of vegetation as direction and opening of joints, and a topsoil state than by conventional classification of layer. It was concluded that the adjustable vegetation on the slope considering condition of geometric shape of slope, joint data and spontaneous type could make useful early afforestation.

  • The zygomaticomaxillary complex (ZMC) functions as a buttress for the face and is the cornerstone to a person's aesthetic appearance, by both setting the midfacial width and providing prominence to the cheek. Malar deficiency is often acquired by blunt injury incurred in a traumatic accident, resulting in ZMC fracture. A 48-year-old male patient presented a right ZMC fracture after contusion injury by a baseball. He only received conservative management and later he suffered discomfort during mouth opening at the moment of mastication, due to trismus involving the temporomandibular joint. In the current case, we describe a surgical technique, by which the malar body is shifted anteriorly and laterally after combined oblique-vertical osteotomy. The technique presented, eventually restored the former aesthetic position of the malar complex and symmetry, and, moreover, improved mastication function.

  • Condylar process of mandible is an important and fuctionally versatile part of the mandible. There were quite large amount of investigations on the functional and anatomical adaptation of the temporomandibular joint(TMJ) to the surrounding tissues. But controversies on the mechanism of functional adaptation of the joint still exist. In this research, we investigated changes in the TMJ by the lateral deviation of the maxillary incisor to shift the mandible right, and bone the undecalcified microscopic sections with fluorescent microscope and von Kossa staining with bright field microscope. Results were as follows: 1. Lateral deviation rendered shifting and tilting of the mandible, There were, compressions in the right joint and opening of the left joint space at early stage. At the same time, both condyles shifted slightly to anterior. 2. After $2{\sim}4$ weeks, left condyle showed anterior displacement and compressions in the joint space. Right condyle showed only slight shift to the anterior. 3. Regardless of the direction of the lateral shift, anterior bite plate compressed both condyle heads until 2 weeks. 4. There are bone resorptions in the anterior aspect of the condyle head and apposition of posterior border. Bone remodeling were observed between 3 and 4 weeks. 5. After 8 weeks of the experiment, there were little differences in condylar morphology between experimental and control group, though slight shifting and compression were still present in the experimental group. Lateral deviation of mandible evoked active remodeling of the TMJ until functional and anatomical reconstruction of TMJ position was achieved.

Clinical experience in managing temporomandibular joint ankylosis: five-year appraisal in a Nigerian subpopulation

  • Objectives: Temporomandibular joint ankylosis (TMJA) is a joint pathology caused by bony and/or fibrous adhesion of the joint apparatus, resulting in partial or total loss of function. Materials and Methods: This is a retrospective study conducted between 2012 and 2016 in the northwest region of Nigeria. The data retrieved includes gender, age, etiology of ankylosis, duration of ankylosis, laterality of ankylosis, type of imaging technique, type of airway management, types of incision, surgical procedure, mouth opening, interpositional materials used, and complications. Results were presented as simple frequencies and descriptive statistics. Results: Thirty-six patients with TMJA were evaluated during the study period. There were 21 males (58.3%) and 15 females (41.7%), yielding a male:female ratio of 1.4:1. The patients' age ranged from 5 to 33 years with $mean{\pm}standard$ deviation ($13.8{\pm}6.6years$). Thirty-five cases (97.2%) were determined to be true/bony ankylosis, while only 1 case (2.8%) was false/fibrous ankylosis. Most of the TMJA cases (16 cases, 44.4%) were secondary to a fall. In our series, the most commonly utilized incision was the Bramley-Al-Kayat (15 cases, 41.7%). The mostly commonly performed procedures were condylectomies and upper ramus ostectomies (12 cases each, 33.3%), while the most commonly used interpositional material was temporalis fascia (14 cases, 38.9%). The complications that developed included 4 cases (11.1%) of severe hemorrhage, 1 case (2.8%) of facial nerve palsy, and 1 case (2.8%) of re-ankylosis. Conclusion: Plain radiographs, with their shortcomings, still have significant roles in investigating TMJA. Aggressive postoperative physiotherapy for a minimum of 6 months is paramount for successful treatment.

류마티스성 관절염 환자에서 나타나는 측두하악관절장애의 임상적 및 방사선학적 특징 (The Clinical and Radiographic Features of Temporomandibular Joint Dysfunction in Patients with Rheumatoid Arthritis)

  • 이 논문의 목적은 류마티스성 관절염 환자에게서 나타나는 측두하악관절장애에 대하여 임상적 및 방사선학적 특징을 평가하는 것이다. 경북대학교병원 류마치스내과에서 류마치스성 관절염을 가지고 있는 것으로 판명된 환자 중에 측두하악관절 기능장애의 증상을 나타내는 환자 15명을 실험군으로 하고, 경북대학교병원 구강내과에서 측두하악관절장애로 진단된 환자 20명을 대조군으로 하였다. 류마티스성 관절염 환자에서 근막동통, 아침경직감, 측두하악관절 운동범위의 감소, 염발음의 발생빈도가 각각 86.7%, 73.3%, 80.0%, 73.3%이었으나, 대조군에서는 각각 55.0%, 15.0%, 40.0%, 30.0%로 나타나 류마티스성 관절염 환자에서 측두하악관절장애의 증상의 발생 빈도가 훨씬 높게 나타났다. 방사선학적 특징 중에 두 군 간에 발생빈도에 있어서 유의한 차이를 나타내는 소견은 과두의 평탄화와 경화, 변연골증식, 관절와의 침식 등이었으며 모두 류마티스성 관절염 환자에게서 많이 나타났다. 또한 MRI 사진에 의해서 확인된 바에 의하면, 관절원판의 변위와 관절원판의 파괴, 피질골의 침식이나 증식 등의 소견이 류마티스성 관절염 환자에게서 유의하게 높은 빈도로 나타났다. 결론적으로 류마티스성 관절염 환자에서 측두하악장애의 발생빈도가 매우 높고 관절의 구조적 변화도 일반적인 측두하악장애 환자에 비해 더 심하게 나타남을 알 수 있었다.

폐구성 악관절 과두걸림환자의 하악회전운동에 관한 연구 (A study on the Rotational Torque Movement of Mandible in TMJ Closed Lock)

  • This study was performed for Investigation of the magnitude of mandibular positional change in maximum mouth opening. protrusion, lateral excursion, gum and peanut chewing with BioPAK system(Bioresearch Inc. USA) which can analyze mandibular rotational torque movements. For this study 17 female patients with Temporomandibular joint(TMJ) closed lock and 18 female control without any Temporomandibular disorders(TMDs) signs and premature occlusal contact were included. The obtained results were as follows : 1. In maximum mouth opening, the mandibular rotational angle and distance of patients were significantly greater than those of control group in horizontal plane(P<0.05). 2. In protrusion, the mandibular rotational angle and distance of patients were significantly greater than those of control group in frontal and horizontal plane(P<0.01, P<0.05). 3. The mandibular rotational angle and 야stance in lateral excursion to affected side of patients were significantly greater than those in lateral excursion to non-affected side in frontal plane(P<0.05). 4. The mandibular rotational angle in gum chewing to affected side of patients was significantly greater than that in gum chewing to non-affected side in frontal plane. 5. The mandibular rotational angle and distance in peanut chewing to affected side of patients were significantly greater than those in peanut chewing to non-affected side in frontal and horizontal plane. 6. The mandibular rotational angle and distance in peanut chewing to affected side of patients were greater than those in gum chewing, and was the same result in control group in frontal and horizontal plane.

경두개방사선사진과 측방 개별화 단층방사선사진을 이용한 하악과두의 형태와 위치에 관한 연구 (A RADIOGRAPHIC STUDY OF MANDIBULAR CONDYLE SHAPE AND POSITION: A COMPARISON OF TRANSCRANIAL RADIOGRAMS AND INDIVIDUALIZED CORRECTED TOMOGRAMS)

  • The purpose of this study was to observe mandibular condyle shape and position in an asymptomatic population. In order to carry out this study, 142 temporomandibular joints in 71 adults(35 males, 36 females), who were asymptomatic for temporomandibular joint disorders and had no history of prosthodontic or orthodontic treatments, were selected, and radiographed using the Accurd-200 head holder(Denar Co., U.S.A) for transcranial radiograms and the Sectography(Denar Co., U.S.A) for lateral individualized corrected tomograms. Mandibular condyles were classified morphologically and evaluated in positional relationships with articular fossae and articular eminences at centric occlusion and 1 inch mouth opening. The obtained results were as follows; 1. In the classification of mandibular condyle shape, the convex type was more prevalent in transcranial radiograms and tomograms taken at medial, central, and lateral locations. 2. In the mandibular condyle position at centric occlusion, the mandibular condyles were placed posterior to the center of articular fossae in transcranial radiograms and anterior to the center of articular fossae in tomograms taken at medial, central, and lateral locations. 3. In the mandibular condyle position in right and left TMJs at centric occlusion, the mandibular condyles were placed bilateral asymmetric relationships to the articular fossae in transcranial radiograms and tomograms taken at medial, central, and lateral locations. 4. In the mandibular condyle position at 1 inch mouth opening, the mandibular condyles were placed anterior to the articular eminences in transcranial radiograms and tomograms taken at central location and posterior to the articular eminences in tomograms taken at medial and lateral locations.

Assessment of functional improvement with temporalis myofascial flap after condylectomy in elderly patients with anterior disc displacement without reduction and an erosive condylar surface

  • Background: The purpose of this study was to investigate the functional effects of temporalis myofascial flap after condylectomy, with or without disc removal, in elderly patients with anterior disc displacement (ADD) without reduction and an erosive condylar surface of the temporomandibular joint (TMJ). Methods: A total of 15 joints from 11 elderly patients (71-78 years old) were included. The patients had pain, mandibular dysfunction symptoms, and unilateral or bilateral ADD as well as an erosive condylar surface of the TMJ. All patients underwent temporalis myofascial flap reconstruction after condylectomy, with or without disc removal. If the maximal mouth opening (MMO) remained <35 mm after condylectomy, coronoidotomy was also performed. Self-assessed pain and mandibular function, including MMO and protrusive and lateral movements, were evaluated. Results: No patient experienced serious complications. Most measurements improved significantly after surgery compared to preoperatively. Most patients achieved nearly-normal mouth opening at 4 weeks after surgery. Although most patients felt discomfort during active postoperative physiotherapy, no patient reported serious pain during the follow-up period. Conclusion: Although nonsurgical therapy is often the first treatment choice for ADD without reduction of the TMJ, surgical intervention involving condylectomy and temporalis myofascial flap reconstruction may be a reasonable first option for elderly patients with an erosive condylar surface of the TMJ.
