파킨슨병 환자의 골밀도 영향요인 (Factors Affecting Bone Mineral Density of the Femur in Parkinson's Disease Patients)

  • Purpose: The purpose of this study is to explore the level of bone mineral densities of the femur neck and to identify factors affecting bone mineral density of the femur in Parkinson's disease (PD) patients. Methods: Participants were 121 PD patients visiting the outpatient clinic of D University Hospital in B City. Bone mineral density was measured at the femur neck by using a dual-energy x-ray absorptiometry. The serum vitamin D level, the amounts of milk intake, caffeine intake, cigaret smoking, and alcohol consumption, and the number of steps taken daily were measured. Past and present disease history, the medication history, and duration of the disease were also collected. The level of disability was obtained by neurologists using the Hoehn and Yahr stage and the Unified Parkinson's Disease Rating Scale. Results: Among PD patients, 57% had osteopenia and 15.7% had osteoporosis. The bone mineral density of the femur was associated with weight, serum vitamin D level, age, and working status. Conclusion: The identification of weight and serum vitamin D level as important predictive factors emphasizes the importance of balanced nutrition for PD patients.

손목 관절의 해부학 (Normal Anatomy of the Wrist Joint)

  • 손목 관절은 신체의 관절 중 가장 많은 골들과 다양한 인대 및 근육 의해 형성되는 복합 관절로 그 해부학적 구조가 복잡하여 이해하기 힘든 관절이다. 손목 관절은 하나의 관절 공간이 아닌 요척골과 근위 수근골 사이에 형성되는 요수근 관절(radiocarpal joint)과 근위 수근골과 원위 수근골 사이에 형성되는 중수근 관절(midcarpal joint) 그리고 원위 요골과 척골두에 의해 구성되는 원위 요척 관절(distal radioulnar joint)로 구성되며 각각의 관절은 정상적으로 각기 분리되어 있다. 손목 관절에 발생하는 질환과 외상에 대해 치료하기 위해서는 손목관절의 정상 해부학에 대한 정확한 이해가 필수적이다. 본 종설에서는 손목관절의 해부학에 대해 자세히 알아 보고자 하였다.

두개하악장애환자의 교합안정장치에 의한 치료후 Sonopak을 이용한 악관절음 변화 (A Comparative Study on the Temporomandibular Joint Sounds before and after Occlusal Splint Therapy Using Electrovibratography)

  • This study was performed to compare the TMJ sounds by means of vibration-related items by Sonopak such as integral, high integral, above 300/(0-300) ratio, peak amplitude, peak frequency and median frequency before and after occlusa1 splint therapy as well as counselling, physical modalities. For this study 22 patients with craniomandibular disorders (CMDs) were selected and examined by routine diagnostic procedure for CMDs including Transcranial and Panoramic radiographs and were classified into 3 CMDs subgroups : disc displacement with reduction, disc displacement without reduction, and degenerative joint disease. Visual analogue scale (VAS) about joint sound was recorded during treatment period and VAS treatment index (VAS Ti) was calculated from the VAS data and treatment duration. The author evaluated and compared treatment results by several parmeters such as symptom duration, timing of joint sound, parafunctional habits, trauma, and diagnostic classification. The obtained results were as follows : 1. Before the treatment, the highest value of peak amplitude was observed in disc displacement with reduction group and value of median frequency was highest in degenerative joint disease group. In addition the highest values of peak frequency and ratio ware observed in degenerative joint disease group, though they were not significant. Furthermore the lowest value of high integral was observed in disc displacement without reduction group and though it was not significant, value of integral was lowest in that group. 2. Among 3CMDs subgroups disc displacement with reduction group showed the significantly decreased value of high integral and degenerative joint disease group had the significantly decreased value of integral after conservative treatment including occlusal splint therapy. Conclusively conservative treatment including occlusal splint therapy vay be effective in the treatment of CMDs including TMJ sound. 3. Fair prognosis for conservative treatment was observed in acute group under 6 months than chronic group, 6 months over in symptom duration but there was no statistical difference. The result for conservative treatment was observed slightly poor in subjects with bruxism, clenching, unilateral chewing habit and trauma history but there were no statistical differences.

  • Neuropathic shoulder arthropathy or Charcot's shoulder is an extremely rare disease, and sometimes it is associated with cervical syringomyelia. Clinical symptoms of the disease include edema of the shoulder and restriction in range of motion. Radiological diagnosis can be made through plain radiography through a characteristic, atrophic destruction of the joint. We experienced a Charcot's joint of the shoulder wherein destruction of the joint progressed extremely quickly and reviewed the literature concerning this condition.

개에서 3단계 골스캔을 이용한 골병변의 진단 (Detection of Orthopedic Disease Using Three Phase Radionuclide Bone Scan in the Dog)

  • Specific diagnosis of orthopedic disease can be diffcult in canine practice. Failure to detect the clinical signs of a disorder during physical examination of dogs with acute or chronic lameness is the most common reason for failure to make specific diagnosis. A 6-month-old, female doberman with history of swelling and non-weight-bearing lameness in the left forelimb was referred to Beterinary Teaching Hospital of Chungbuk National University. Physical examination, plain radiography, and conventional three-phase radionuclide bone scan were performed in the patient. Based on the physical exam and radiography, this case was diagnosed as elbow strain and subluxation. Conventional three-phase bone scan detected soft tissue inflammation and osteochondral lesions of elbow joint, and revealed good agreement with clinical findings. Therefore, conventional three-phase bone scan was able to provide the precise information about inflammation of soft tissue and osteochondral lesions of joint.

농촌주민의 주소증에 대한 조사연구 (A Study on the Chief Complaints among Farmers in the Rural Community)

  • Objectives and Methods : This study was conducted to investigate chief complaints among famers. The subjects of this study were 640 famers who live in the rural area in Wanju-gun Jeonbuk province. They were received medical treatment by oriental doctors who visited each village from July 1996 to June 1999. The data of this study were collected from their medical charts . Results and Conclusions : The number of female patients was 449(70.16%) and the number of male patients was 191(29.84%). The group of patients above 70 years was 32.81%, 60-69 years 32.03%, 50-59 years 20.15%, 40-49 years 8.90%, and bellow 39 years 6.09%. The most frequent complaint in farmers was low back pain(27.16%) the second was knee joint pain(16.14%), the third was shoulder Pain(12.06%), and the fourth was headache-dizziness(9.59%). The frequency of musculoskeletal disease(64.48%) was most high, and cardiovascular disease(11.33%) was the second, gstrointestinal disease(10.73%) was the third, respiratory disease(3.42%) was the fourth. It is necessary to pay attention to musculoskeletal disease-especially low back pain and knee joint pain- in study and treatment of farmer's disease.

Calcium pyrophosphate dihydrate deposition disease in the temporomandibular joint: diagnosis and treatment

  • Background: Calcium pyrophosphate dihydrate deposition disease (CPDD) is a rare disease in the temporomandibular joint (TMJ) space. It forms a calcified crystal mass and induces a limitation of joint movement. Case presentation: The calcified mass in our case was occupied in the left TMJ area and extended to the infratemporal and middle cranial fossa. For a complete excision of this mass, we performed a vertical ramus osteotomy and resected the mass around the mandibular condyle. The calcified mass in the infratemporal fossa was carefully excised, and the segmented mandible was anatomically repositioned. Scanning electronic microscopy (SEM)/energy-dispersive X-ray spectroscopy (EDS) microanalysis was performed to evaluate the calcified mass. The result of SEM/EDS showed that the crystal mass was completely composed of calcium pyrophosphate dihydrate. This result strongly suggested that the calcified mass was CPDD in the TMJ area. Conclusions: CPDD in the TMJ is a rare disease and is difficult to differentially diagnose from other neoplasms. A histological examination and quantitative microanalysis are required to confirm the diagnosis. In our patient, CPDD in the TMJ was successfully removed via the extracorporeal approach. SEM/EDS microanalysis was used for the differential diagnosis.

류마티스 관절염 환자에서 지각된 질병의거 변화(illness intrusiveness)가 심리사회적 상태에 미치는 영향 (Illness Intrusiveness and Psychosocial Impact in Rheumatoid Arthritis Patients)

  • Chronic disease such as rheumatoid arthritis is believed to induce a significant psychosocial stressors. The concept of illness intrusiveness-illness induced life style disruptions-was hypothesized to affect psychosocial status. And demographic, socioeconomic, disease and social characteristics were hypotehsized to affect illness intrusiveness. Hierarchial multiple regression analyses were used. As a result, among the demographic factors gender was identified as contributor to directly increase the psychosocial stress and education level was identified to affect the psychosocial stress through illness intrusiveness. Among socioeconomic factors, the burden about the cost of medical treatment was found to indirectly affect the psychosocial stress through the illness intrusiveness. Also income and job were found to affect directly the psychosocial status. Among the disease characteristics, only the pain level was identified to increase the psychosocial stress through the illness intrusiveness. Among the social characteristics, perceived social support is identified to increase the psychosocial stress through the illness intrusiveness. Based upon these results, some suggestions were made for minimizing illness intrusiveness in rheumatoid arthritis and future research.

간경변증 환자의 식이이행과 영양상태 및 질병상태와의 관계 (Relationships among Dietary Compliance, Nutritional Status and Stages of Disease in Patients with Liver Cirrhosis)

  • Purpose: The purpose of this study was to investigate the degree of dietary compliance, nutritional status, and stages of disease in patients with liver cirrhosis and to identify the relationships among those variables. Methods: This study used a cross-sectional design and conducted a survey of 100 patients who were treated at D-university hospital in Busan. For data analysis, descriptive statistics and Pearson's correlation coefficients were performed using SPSS/WINdow 20.0 program. Results: The mean score of dietary compliance was 3.50 in the a 5-point scale. Stage of disease was significantly correlated with dietary compliance (r=.20, p=.048), and protein as one of nutritional status (r=-.44, p<.001). Conclusion: These findings suggest that dietary compliance of patients with liver cirrhosis can have an influence on prognosis of disease. Therefore, patients should be encouraged to improve dietary compliance from the early stage of liver cirrhosis.

한센병력자의 낙인, 자아존중감 및 삶의 질 (Stigma, Self-esteem and Quality of Life of People with Hansen's Disease)

  • Purpose: The purpose of this study was to identify levels of perceived stigma, self-esteem, quality of life and factors influencing on quality of life in people with Hansens' disease. Methods: The subjects were 128 people in Sorokdo National Hospital, two nursing homes, and three settlements. The stigma scale, self-esteem scale, and World Health Organization's quality of life instrument were used. Data were collected through self-reported questionnaires from July to August, 2010 and analyzed by t-test, ANOVA, Ducan's multiple range test, Pearson's correlation coefficients, and stepwise multiple regression by SAS. Results: The level of perceived stigma was 3.93, self-esteem was 26.2, quality of life was 2.78. Self-esteem was positively associated with quality of life. Through the multiple regression, it was found that self-esteem and job were factors influencing of quality of life with 43% of predictability. Conclusion: It was found that self-esteem and job were important for people with Hansen's disease. To improve the quality of life in people with Hansen's disease, it is necessary to develop nursing interventions fostering self-esteem and decreasing perceived stigma.
