승모판 협착증 환자에서 운동부하 심장풀스캔을 이용한 좌심실기능의 평가 (Exercise Radionuclide Ventriculography in Patients with Mitral Stenosis)

  • We performed exercise radionuclide ventriculography to evaluate left ventricular (LV) function in 35 patients with pure mitral stenosis (MS). There were 6 males and 29 females ranging from 21 to 63 years of age (mean $37{\pm}10$ years). We also studied 8 healthy men as control group (mean age $27{\pm}5$ years). Each patients was evaluated at rest and during maximal exercise on an isokinetic bicycle ergometer. Peak filling rate (PFR), peak ejection rate (PER), ejection fracion (EF), end-diastolic volume (EDV), end-systolic volume (ESV), stroke volume (SV), and cardiac output (CO) were determined by the radionuclide technique. The results were summarized as follows: 1) LV systolic dysfunction and reduced PFR were noted in patients with MS. 2) EDV as well as SV decreased with exercise in patients with MS. 3) No significant increase in EF during exercise compared to rest value was observed because it was caused by reduced EDV and SV during exercise in patients with MS. 4) CO increased with exercise was significantly lower than normal in patients with MS. 5) Patients with MS were divided into two groups according to whether SF increased more than 5% druing exercise compared to resting state or decreased. Significant difference were found between these two groups. Patients with a fall in EF with exercise were older, had larger left atrial size, and had lesser decreased in ESV during exercise.

3 차아민의 4 차화반응에 관한 연구 (제1보). Phenethyltosylate 와 치환 피리딘류의 반응에 관한 반응 속도론적 연구 (Studies on the Quaternization of Tertiary Amines (I). Kinetics and Mechanism for the Reaction of Phenethyltosylate with Substituted Pyridines)

  • Phenethyltosylate와 치환 피리딘의 반응속도를 전기전도법으로 아세트니트릴 용매에서 50, 60, 및 70$^{\circ}C$에서 측정하였다. 이 반응에서의 피리딘의 치환기 효과를 검토한 결과 피리딘의 전자주는 능력이 강할 수록 반응은 빨랐다. $E_{\alpha}$${\Delta}S^{\neq}$ 사이에는 동속관계가 성립되었고 동속온도는 240$^{\circ}$K 였다. Bronsted 도시는 4-아미노피리딘을 제외하고는 1ogk=0.22pKa-3.7l (r=0.986)의 직선이 얻어졌다. Hammett 식에서도 역시 4-아미노피리딘을 제외하고 logk=-1.33${\sigma}$+0.08 (r=-0 987)의 직선이었다. 이와 같이 4-아미노피리딘의 편차는 용매효과와 공명효과 및 ${\sigma}$ 값 그 자체에 기인된다고 생각된다. 이상의 결과로서 본 반응은 전이상태에서 C…N 결합형성에 의하여 반응이 결정되는 $S_N2$ 반응메카니즘으로 진행된다는 것을 알았다.

러시안 전류 자극이 하지 골절 후 회복기 환자의 넙다리네갈래근육의 근수행력과 근활성도에 미치는 영향 (Effect of Russian Current Stimulation on Muscular Performance and Muscle Activity of Quadriceps Femoris Muscle of Convalescent Patient after Leg Fracture)

  • 본 연구에서는 하지 골절 후 회복기 환자에게 러시안전류 자극이 넙다리네갈래근의 근수행력과 활성도에 미치는 영향을 알아보기 위하여 실시하였다. 본 연구는 2013년 1월 9일에서 2013년 3월 16일까지 실시하였으며, 하지 골절 후 회복기에 있는 환자 20명을 대상으로 실험을 실시하였다. 러시아 전류자극기의 큰 도자를 넙다리네갈래근의 근위부에 위치하고, 작은 도자는 원위부에 부착시키고 한 주에 세 번, 6주 동안 적용하였다. 대상자의 넙다리네갈래근의 근활성도를 측정하기 위해 근전도가 이용되었고, 근수행력을 측정하기 위해 등속성기기가 이용되었다. 연구결과 기간별 넙다리네갈래근의 근수행력 차이는 러시안전류 치료 전과 비교하여 4주 후와 6주 후에서 유의한 근수행력의 증가를 나타내었다. 또한 넙다리곧은근, 안쪽넓은근, 가쪽넓은근의 근활성도는 치료 전과 비교하여 4주 후와 6주에서 유의한 증가를 나타내었다. 이와 같은 결과는 러시안 전류가 하지 골절 등과 같은 문제로 인해 약화된 하지 근육의 근력 강화를 위해 선택될 수 있는 효과적 치료 방법이라 할 수 있다.

고등학교 엘리트 카약 선수들의 등속성 근기능·균형성·유연성 측정 및 평가 (Assessment of Isokinetic Muscle Function, Balance and Flexibility in Elite High School Kayakers)

  • 본 연구는 고등학교 엘리트 카약 선수들의 등속성 근기능, 균형성, 유연성, 신체 조성 등을 검사 및 진단하여 선수들의 경기력 향상과 부상 예방을 위한 훈련 방법론에 대한 조언을 하고자 실시되었다. 11명의 선수들을 대상으로, Biodex system IV, Biodex stabilizer system, Inbody 3.0, CNP-5403을 사용하여 근기능(peak torque, average power, work fatigue) 동적 균형 신체 성분 유연성 검사를 하였다. 선수들의 BMI 평균은 $22.57kg/m^2$, 체지방률은 10.90%, 체전굴은 16.95cm, 밸런스 점수는 좌우 각각 1.55, 1.49로 나타났다. S4, S5, S8, S10, S11은 어깨 근력과 파워 훈련이, S5, S6, S10, S11은 어깨 근지구력 훈련이 필요한 것으로 평가되었다. S5, S7, S8, S10, S11은 무릎 근력과 파워 훈련이, S1, S4, S5, S10은 무릎 근지구력 훈련이 요구되었다. S3, S10, S11은 손목 근지구력 강화 훈련이 필요했다. 추후 근기능 불균형으로 인한 경기력 저하와 잠재적 부상의 위험을 줄이기 위해선 획일화된 근력 훈련 프로그램을 지양하고, 정기적인 근기능 검사와 함께 개인별로 특화된 훈련 프로그램을 수립해야 한다.

한방복합치료를 병행한 도인운동요법이 요통환자의 통증 및 기능 개선에 미치는 영향 : 후향적 관찰 연구 (Effect of The Daoyin Exercise Therapy Combined with Complex Korean Medicine Treatment on Pain and Function Improvement of Low Back Pain Patients : A Retrospective Observational Study)

  • This study was designed to prove the effect of the Daoyin exercise therapy combined with complex korean medicine treatment on pain and function improvement of low back pain patients. A retrospective observational study. 30 low back pain patients, checked numeric rating scale(NRS) over 5, were treated with the Daoyin exercise therapy combined with complex korean medicine treatment. NRS, roland & morris disability questionnaire(RMDQ), range of motion(ROM), isokinetic muscle strength, and euroqol five demension questionnaire(EQ-5D) were checked to evaluate patients. The Daoyin exercise therapy combined with complex korean medicine treatment reduced NRS $5.67{\pm}1.40$ to $3.73{\pm}1.72$, improved RMDQ $10.33{\pm}4.91$ to $8.17{\pm}5.40$, strengthened lumbar muscle's peak torque and improved EQ-5D $0.56{\pm}0.20$ to $0.63{\pm}0.15$ and euroqol visual analogue scale(EQVAS) $46.80{\pm}19.95$ to $57.50{\pm}19.93$ but had no effect on ROM. NRS decreased significantly as the number of trials increased. No one had serious adverse reactions. As described above, the Daoyin exercise therapy combined with complex korean medicine treatment can help reducing low back pain, improving lumbar function, strengthening lumbar muscle power and stamina and improving quality of life of low back pain patients.

태권도 선수와 일반인의 등척성 무릎신전 토크-각도 관계 특성 비교 분석 (Comparison of Isometric Knee Extension Torque-Angle Relationship between Taekwondo Athletes and Normal Adults)

  • Objective : In order for Taekwondo athletes to perform destructive kicking performance, they are expected to have Taekwondo-specific muscle properties such as high muscle strength and power. The purpose of this study was to investigate the joint angle-dependent force-producing property of Taekwondo athletes' knee extensor muscles, which is one of the primary muscle groups involved in kicking performance. Method : Ten Taekwondo male athletes (age: $19.9{\pm}0.7yrs$, height: $180.6{\pm}6.2cm$, body mass: $75.9{\pm}8.9kg$, career: $9.2{\pm}2.9yrs$.) and 10 healthy male non-athletes (age: $26.3{\pm}2.6yrs$, height: $174.2{\pm}4.8cm$, body mass: $72.8{\pm}7.7kg$) participated in this study. Subjects performed maximum isometric knee extension at knee joint angles of $40^{\circ}$, $60^{\circ}$, $80^{\circ}$, and $100^{\circ}$ (the full knee extension was set to $0^{\circ}$) with the hip joint angles of $0^{\circ}$ and $80^{\circ}$ (the full extension was set to $0^{\circ}$). During the contractions, knee extension torque using an isokinetic dynamometer simultaneously with muscle activities of the rectus femoris (RF), and the vastus lateralis (VL) and vastus medialis (VM) using surface electromyography were recorded. Based on the torque values at systematically different knee-hip joint angles, the joint torque-angle relationships were established and then the optimal joint angle for the knee extensor was estimated. Results : The results of this study showed that the isometric knee extension torque values were greater for the Taekwondo athletes compared with the non-athlete group at all hip-knee joint angle combinations (p<.05). When the hip joint was set at $80^{\circ}$, the peak isometric torque was greater for the Taekwondo athletes compared with the non-athlete group ($313.61{\pm}36.79Nm$ and $221.43{\pm}35.92Nm$, respectively; p<.05) but the estimated optimum knee joint angles were similar ($62.33{\pm}5.71^{\circ}$ and $62.30{\pm}4.67^{\circ}$ for the Taekwondo athletes and non-athlete group, respectively). When the hip joint was set at $0^{\circ}$, the peak isometric torque was greater for the Taekwondo athletes compared with the non-athlete group ($296.29{\pm}45.13Nm$ and $199.58{\pm}25.23Nm$, respectively; p<.05) and the estimated optimum knee joint angle was larger for the Taekwondo athletes compared with the non-athlete group ($78.47{\pm}5.14^{\circ}$ and $67.54{\pm}5.77^{\circ}$, respectively; p<.05). Conclusion : The results of this study suggests that, compared with non-athletes, Taekwondo athletes have stronger knee extensor strength at all hip-knee joint angle combinations as well as longer optimum muscle length, which might be optimized for the event-specific required performance through prolonged training period.

8주간 데드리프트 운동 시 전신진동운동이 재활스포츠 선수의 근 기능 특성에 미치는 영향 (The Effect in the Muscle Function Following 8-Week Dead-lift training with Whole-body Vibration in Rehabilitation for Sports Players)

  • Objective : The purpose of this study was to investigate the effects in the muscle function following 8-week dead-lift training with Whole-body Vibration(WBV) in rehabilitation for sports players. Method : Twenty young sports players. Each subjects were randomly assigned to a resistance training with Whole-body vibration group(TG, n=10), a resistance training without Whole-body vibration group(CG, N=10). The measurements which physical fitness test and joint torque test were performed before the randomization and after the 4-week and 8-week. The WBV group performed the dynamic Dead-lift exercise on a vibration platform during one minute. The CG group performed the equal training without vibration. The WBV and CON group repeated 5 set and trained two times weekly for 8-weeks. Paired t-test was used to test for differences between the groups at baseline and after 4-weeks and 8-Weeks. And independent t-test was used to test for differences between the groups at TG and CG. All analyses were executed using SPSS software 18.0. The level of significance was set at p<.050. Results : Following the 8-Weeks training sessions, an increase in the back-muscle strength was found to be greater for the TG compared with the CG group(p<.05). Muscle endurance was significantly decreased after training than before training only for the CG(p<.05). Isometric Hip/Lumber Extension/Flextion measurement was found to be significantly greater for the TG compared with the CG group(p<.05). The finding indicates that WBV effects as an efficient training stimulus to enhance muscle function by facilitating neural control trail. Following muscle activation in motor unit synchronization of the co-contraction of the muscles. Conclusion : The results imply that the WBV training may have enhanced muscle function in rehabilitation for sports players.

착지 높이와 무릎관절 근육 피로가 착지 후 방향 전환 동작 시 하지관절의 움직임에 미치는 영향 (Effects of Landing Height and Knee Joint Muscle Fatigue on Movement of the Lower Extremity during Cutting After Landing)

  • Objective : The purpose of this study was to investigate the effects of landing height and knee joint muscle fatigue on the movement of the lower extremity during cutting after landing. Method : Subjects included 29 adults (age: $20.83{\pm}1.56years$, height: $172.42{\pm}9.51cm$, weight: $65.07{\pm}10.18kg$). The subjects were asked to stand on their dominant lower limb on jump stands that were 30 and 40 cm in height and jump from each stand to land with the dominant lower limb on a force plate making a side step cutting move at a $45^{\circ}$ angle with the non-dominant lower limb. The fatigue level at 30% of the knee extension peak torque using an isokinetic dynamometer. Results : The results showed that the difference of landing height increased maximum range of motion and angular velocity of hip, knee, and ankle joints in the sagittal plane, and in the angular velocity of motion of the hip joint in the sagittal plane. The maximum range of motion of the knee joint in the sagittal plane and the frontal plane decreased on landing from both heights after the fatigue exercise. The angular velocity of the hip joint in the sagittal plane, and the maximum range of motion of the hip joint in the transverse plane decreased for both landing heights after the fatigue exercise. The angular velocity of the hip joint in the frontal plane decreased for the 30 cm landing height after the fatigue exercise. On the other hand, the angular velocity and maximum range of motion of the ankle joint in the sagittal plane for both landing heights, and the angular velocity and maximum range of motion of the ankle joint in the frontal plane increased on landing from the 40 cm height after the fatigue exercise. Conclusion : Different landing heights of 30 and 40 cm and 30% fatigue of peak torque of knee extensor found a forefoot and stiff landing strategy, when cutting after landing. These results might be due to decline in the shock absorption capability of the knee joint and the movement capability related to cutting while increasing the contribution of the ankle joint, which may cause increased ankle joint injuries.

프로 축구팀의 시즌 전 체력검사 (Pre-seasonal Physical Examination of Professional Soccer Team)

  • 목적: 국내 프로 축구팀에서 시즌 전 체력검사를 시행하여 포지션에 따른 자료를 분석하고 표준화하고자 했다. 대상 및 방법: 34명으로 구성된 한 개의 축구팀을 대상으로 시행하였으며 평균 나이 24.9세였다. 설문지를 통한과거 병력 조사, 이학적 검사를 통한 근 골격계 검사, 등속성 근력기기를 이용한 근력 검사, 생리적 검사, 기능적 검사를 시행하고 포지션에 따른 차이를 분석하였다. 결과: 과거력 상 슬관절 손상이 23예 중 11 예(47%)로 가장 빈번했고, 이학적 검사 결과 총 40예 중 족관절 손상이 14예 (46%)로 슬관절 손상 11예 (24%)보다 많은 손상이 있었다. 위치 별로 공격수는 장무지 굴곡 건염, 미드필드는 만성 족관절 불안정성 수비수는 아킬레스 건염이 많았고 근력 검사상 우세 $60^{\circ}$ 신전/굴곡 체중당 최대 염력비 평균이 55.1%였다. 생리학적 검사상 미드필드 체 지방율이 17.8% (표준편차 2.9)로 높게 측정되었고 기능적 검사 결과 배근력 125.5N (표준편차 28.3), 서전트 점프 59.6cm, 반응시간 208 5초, 유연성 19.2 m/s, 사이드 스텝 39.9 cm였다. 결론: 시즌 전 체력검사를 통하여 선수들의 체력을 평가하고 이를 기초로 하여 재 손상에 대한 예방적 수단을 강구 할 수 있다.

Thera-Band를 이용한 단축성 & 신장성 수축 저항 훈련 프로그램이 어깨의 최대 토크와 최대 파워에 미치는 영향 (Effects of Combined Resistance Training Program of Concentric and Eccentric Contraction Using Theraband on Shoulder Rotation Torque Max and Peak Power)

  • 본 연구는 Thera-Band를 이용해 단축성 수축과 신장성 수축이 혼합된 저항운동을 실시하는 것이 단축성 수축 저항 운동만을 실시하는 것보다 근력증진에 더 효과적인지를 알아보고자 하는 논문이다. 연구대상자는 부산광역시 소재 S 대학교에 재학 중인 학생 30명 이었으며, 30명을 무작위로 단축성 수축과 신장성 수축 운동군, 단축성 수축 운동군, 무운동군으로 나누어 4주 동안 운동을 실시하였다. 근력 측정은 등속성 운동기기인 CON-TERX를 이용하였으며, 각속도는 $60^{\circ}$/sec, $120^{\circ}$/sec이었다. 4주 운동 후, 단축성 수축과 신장성 수축 운동군의 경우 각속도 $60^{\circ}$/sec에서 단축성과 신장성 최대 토크와 최대 파워, 각속도 $120^{\circ}$/sec에서 단축성 최대 파워가 통계적으로 유의하게 증가하였다. 반면 단축성 수축 운동군은 각속도는 $60^{\circ}$/sec와 $120^{\circ}$/sec의 최대 파워에서만 유의한 증가가 있었다. 반면 무운동군은 변화가 없었다. 따라서 Thera-Band를 이용해 단축성 수축과 신장성 수축이 혼합된 운동을 실시하는 것이 단축성 수축 운동만을 실시하는 것보다 근력증진에 더 효과적임을 알 수 있었다.
