내부마케팅이 간호사의 직무만족도 및 이직의도에 미치는 효과 - 일개 대학병원을 대상으로 - (The Effect of the Intramural Marketing Activities on Nurses' Job Satisfaction and Turnover Intention)

  • Purpose: The purpose of this study was to evaluate the importance and the performance of intramural marketing in the hospital and to examine the effect of intramural marketing on nurses' job satisfaction and turnover intention. Method: The participants of this study were 203 nurses, who had worked over 3 months in the general or special units in C university hospital located in Seoul. Results: The participants' perceived performance of the intramural marketing was 2.35 on average. The importance of the intramural marketing recognized by the participants was 4.02. The score of the job satisfaction of the participants was 2.74 and that of the turnover intention was 3.67 on average. The intramural marketing demonstrates statistically significant correlations with job satisfaction and turnover intention. Conclusion: The findings of the study demonstrate the importance of the intramural marketing in the hospital for nurse retention. In order to increase nurses' job satisfaction and reducing their turnover intention, the intramural marketing activities should be expanded further.

항응고제에 의한 자발성 장관 벽내 혈종 1예 (A Case of Anticoagulant-induced Spontaneous Intramural Intestinal Hematoma)

  • Spontaneous intramural hematoma is a rare complication of oral anticoagulants, and its incidence is expected to increase because of the increasing number of elderly patients undergoing anticoagulant therapy. Clinical manifestations of spontaneous intramural hematoma vary from mild abdominal pain to intestinal obstruction or acute abdomen. Early diagnosis is important because most patients can be treated successfully without surgery. The role of endoscopy in the diagnosis of intramural hematoma is not well established because almost all cases are diagnosed non-invasively with computed tomography scans. However, confirmation of the intramural hematoma through direct visualization of the involved bowel mucosa is helpful, if the imaging diagnosis is uncertain. We report a case of anticoagulant-induced spontaneous intramural hematoma, which was diagnosed using endoscopy, with relevant literature review.

대동맥벽내 혈종의 수술치험-증례보고- (Surgical Treatment of Intramural Hematoma of the Aorta Case Report)

  • 대동맥벽내 혈종은 진성 대동맥박리증과 구별되는 질환으로 아직 정확한 예후나 치료지침이 알려져 있지 않다. 저자들은 57세 여자환자에서 상행대동맥에 발생한 대동맥 벽내 혈종 1례를 수술 치험하였다. 환자는 질식감과 함께 심한 전흉부동통이 견갑골간으로 전파되는 것이 주소였다. 술전 흉부 전산화 단층왈영에서 흉부대동맥전체와 복부대동맥 일부의 대동맥 벽내에 초생달 형태의 혈종이 관찰되었고, 내벽의 파열 은 관찰되지 않았다. 저자들은 완전순환정지와 역행성 뇌 순환하에서 상행대동맥 치환술을 시행하였다. 우측관상동맥입구 의 손상이 있었으나 복재정 맥을 이용한 우회문합술로 수술하였다. 환자는술후4주만에 퇴원하였으며, 창상감염, 심낭삼출액 등의 합병증외에는 심각한 합병증은 없었다.

Delayed Manifestation of Isolated Intramural Hematoma of the Duodenum Resulting from Blunt Abdominal Trauma

  • Duodenal injury following blunt abdominal trauma is a relatively unusual complication, and it may sometimes be difficult to distinguish a duodenal hematoma from duodenal perforation. According to recent reports, intramural hematomas typically resolve spontaneously with conservative treatment. Surgery, however, is occasionally necessary in some cases if the diagnosis is delayed, conservative therapy fails, or a high degree of suspicion of duodenal injury persists. We experienced a case of delayed manifestation of a duodenal intramural hematoma that was surgically treated.

  • Mitochondria are cytosolic organelles essential for generating energy and maintaining cell homeostasis. Despite their critical function, the handful of proteins expressed by the mitochondrial genome is insufficient to maintain mitochondrial structure or activity. Accordingly, mitochondrial metabolism is fully dependent on factors encoded by the nuclear DNA, including many proteins synthesized in the cytosol and imported into mitochondria via established mechanisms. However, there is growing evidence that mammalian mitochondria can also import cytosolic noncoding RNA via poorly understood processes. Here, we summarize our knowledge of mitochondrial RNA, discuss recent progress in understanding the molecular mechanisms and functional impact of RNA import into mitochondria, and identify rising challenges and opportunities in this rapidly evolving field.

  • Intramural pregnancy is an unusual ectopic gestation located within the uterine wall, completely surrounded by myometrium and separate from the uterine cavity, fallopian tube, or round ligament. It is known to be difficult to diagnose, and associated with a high rate of uterine rupture. We report a case of intramural pregnancy in which early diagnosis was made and successful treatment was done by dilatation and curettage. Diagnostic laparoscopy confirmed the absence of uterine rupture during the procedure. Therefore, conservation of fertility can be possible with early diagnosis of intramural pregnancy. To our knowledge, this is the first case report of intramural pregnancy following IVF-ET in Korea.

  • 점막하 식도 박리는 드문 질환으로 식도 천공 없이 점막하 출혈 및 혈종에 의해 점막하층과 근육층이 박리되는 상태를 의미하며, Mallory-Weiss, Boerhaave's syndrome과 함께 제3의 급성 식도 손상질환이다. 금식과 수액 요법 등의 보존적 치료만으로도 비교적 예후가 좋다. 하지만 출혈 소인이 있는 질환이나 정상적인 지혈에 영향을 미치는 약제(anticoagulants, anti-platelets, thrombolytic agents)를 복용중인 환자에서, 흉통 및 연하통 혹은 연하곤란 등을 호소하는 경우에 감별 진단으로 염두에 두어야 부적절한 치료를 피할 수 있다. 본 증례는 항혈소판 제재를 복용 중에 발생한 점막하 식도 박리를 보고하는 바이다.

  • 폐동맥 외막주위 혈종은 Stanford A형 대동맥벽내 혈종의 드문 합병증이다. 근위부 상행 대동맥과 폐동맥은 공통된 혈관외막을 공유하고 있기 때문에 대동맥벽내 혈종의 혈액은 폐동맥으로 확산될 수 있다. 저자들은 급성 흉통을 호소하는 66세 남성에게서 보인 Stanford A형 대동맥벽내 혈종과 연관된 폐동맥 외막주위 혈종의 증례를 보고하고자 한다.

Arterial Switch Operation in Patients with Intramural Coronary Artery: Early and Mid-term Results

  • Background: The intramural coronary artery has been known as a risk factor for early death after an arterial switch operation (ASO). We reviewed the morphological characteristics and evaluated the early and mid-term results of ASO for patients with an intramural coronary artery. Materials and Methods: From March 1994 to September 15th 2010, 158 patients underwent ASO at Dong-A and Pusan National University Hospitals for repair of transposition of the great arteries and double outlet right ventricle. Among these patients, 14 patients (8.9%) had an intramural coronary artery. Mean age at operation was $13.4{\pm}10.2$ days (4 to 39 days) and mean body weight was $3.48{\pm}0.33$ kg (2.88 to 3.88 kg). All patients except one were male. Eight patients had TGA/IVS and 4 patients had an aortic arch anomaly. Two patients (14.3%) had side-by-side great artery relation, of whom one had an intramural right coronary artery and the other had an intramural left anterior descending coronary artery. Twelve patients had anterior-posterior relation, all of whom had an intramural left coronary artery (LCA). The aortocoronary flap technique was used in coronary transfer in 8 patients, of whom one patient required a switch to the individual coronary button technique 2 days after operation because of myocardial ischemia. An individual coronary button implantation technique was adopted in 6, of whom 2 patients required left subclavian artery free graft to LCA during the same operation due to LCA injury during coronary button mobilization and LCA torsion. Results: There was 1 operative death (7.1%), which occurred in the first patient in our series. This patient underwent an aortocoronary flap procedure for coronary transfer combining aortic arch repair. Overall operative mortality for 144 patients without an intramural coronary artery was 13.2% (19/144). There was no statistical difference in operative mortality between the patients with and without an intramural coronary artery (p>0.1). There was no late death. The mean follow-up duration was $52.1{\pm}43.0$ months (0.5 to 132 months). One patient who had a subclavian artery free graft required LCA stenting 6.5 years after surgery for LCA anastomotic site stenosis. No other surviving patient needed any intervention for coronary problems. All patients had normal ventricular function at latest echocardiography and were in NYHA class 1. Conclusion: The arterial switch operation in Transposition of Great Arteries or Double Outlet Right Ventricle patients with intramural coronary can be performed with low mortality; however, there is a high incidence of intraoperative or postoperative coronary problems, which can be managed with conversion to the individual coronary button technique and a bypass procedure using a left subclavian free graft. Both aortocoronary flap and individual coronary button implantation techniques for coronary transfer have excellent mid-term results.
