Successful Intubation with Intra-Oral Camera in Difficult Intubation Case (치과치료를 위한 전신마취 환자 중 기관삽관이 어려운 환자에서 구강카메라를 이용한 증례)

  • Dental treatment under general anesthesia is considered a good way for behavioral control in disabled patients. General anesthesia for disabled patients sometimes requires difficult airway management. In this case, intra-oral camera is great helpful to intubation. Originally, the intra-oral camera is designed to facilitate diagnosis dental disease. The intra-oral camera is very efficient on intubation. Laryngeal photo taken by Intra-oral camera can be good guide for difficult intubation. In this report, an 11-year-old boy with chromosomal anomaly received dental care under general anesthesia. And he had a potentially difficult airway. We achieved nasotracheal intubation successfully with intra-oral camera.

Comparison of accuracy of digital data obtained by intra-oral scanner and extra-oral scanner (구강 내 스캐너와 구강 외 스캐너를 사용하여 취득된 스캔 데이터 정확도 비교)

  • Purpose: The purpose of this study was to compare the accuracy of the scan data acquired by the extra-oral and intra-oral scanner. Methods: The maxillary right first molar was made of polymethyl methacrylate(PMMA) specimen. This PMMA specimen was scanned with a engineering scanner and intra-oral scanner. Meanwhile, extra-oral scanner scanned stone die duplicated from PMMA master die. Trueness and precision of scan datas was measured by 3-dimensinal inspection. Independent t-test was conduct to analysis the significant difference(a=0.05). Results: In the trueness analysis, mean of discrepancies were 13.82um for intra oral scanner and 16.84 um for extra-oral scanner. In the precision analysis, mean of discrepancies were 11.72 for inta-oral scanner and 9.2 for extra-oral scanner. Both trueness and precision showed a statistically significant difference (Table 1, p<0.05). Conclusion: Intra-oral scanner can show higher trueness than extra-oral scanner, it has lower precision.

Prophylactic antibiotics in intra-oral bone grafting procedures: a prospective, randomized, double-blind clinical trial

  • Objectives: This study was conducted in order to assess the efficacy of 1st generation cephalosporin as use as a single-dose preoperative prophylactic antibiotic for surgical wound infections resulting from intra-oral bone grafting procedures. Materials and Methods: A total of 23 patients who were to undergo intra-oral bone graft procedures participated in this study. After randomization, 2 grams of 1st generation cephalosporin was orally administered to both the experimental and placebo groups one hour prior to surgery in a double-blind fashion. Post-operatively, the experimental group (12 patients) was orally administered placebo three times a day for three days. The control group (11 patients) was orally administered 1st generation cephalosporin three times a day for three days. The postoperative course was observed for one month including the clinical parameters associated with infection. Results: Postoperative infections were noted in 1 out of 11 patients in the experimental group. No infections occurred in the control group. Conclusion: There was no significant difference in the incidence of postoperative infections between the two groups. Two grams of 1st generation cephalosporin administered orally one hour before surgery served as an effective prophylactic antibiotics therapy for intra-oral bone graft surgery.

Evaluation of validity of polyurethane model fabricated by intra-oral scanning method (구강 내 스캐닝 방법에 의해 제작된 폴리우레탄 모형의 정확도 평가)

  • Purpose: The purpose of this study was evaluate the validity of polyurethane model fabricated by intra oral scanning method. Methods: Ten sam cases of stone models were manufactured from master model, and polyurethane models were made with the intra oral scanning and CNC milling method. One examiner individually measured 6 distances(intercanine distance, intermolar distance, two dental arch lengths(right, left) and two diagonal of dental arch lengths(right, left) on the stone models and the polyurethane models. The Mann-Whitney U test(${\alpha}$=0.05) were used for statistical analysis. Results: The mean difference between measurements made directly on the stone models and those made on the polyurethane models was 0.31-0.38mm. No statistically differences between the two groups were founded 4 distances(p>0.05), but 2 distances were statistically significant(p<0.05). Conclusion: Stone models showed larger than polyurethane models fabricated by intra oral scanning method.

The Musculoskeletal Pain and Inconvenient Feeling During Hand Instruments with Mannequin and Intra-oral Cavity in Dental Hygiene Students (치위생학과 학생들의 구강 내와 마네킹에서 기구 조작시 근골격계 통증과 불편감)

  • Purpose : The purpose of this study was to analyze some factors that can cause incidence of the muscle and skeletal system trouble, by which Dental Hygiene Students examine the pain and the inconvenient feeling according to kinds in manual implements and the usability in the level of manipulating and maintaining the manual implement, in the actual training of the intra oral cavity along with mannequin given the scaling practice. Methods : Targeting 18 female juniors for the department of dental hygiene, who had directly practiced oral prophylaxis for 2 years, the questionnaire research was carried out right after the mutual practice in the intra oral cavity along with mannequin. Results : It was indicated that there is no big difficulty both in intra oral cavity and mannequin in terms of manipulating implement with a method of maintaining the rightly hand fixing or of grasping the trans formative pencil and of controlling force given manipulating the implement. How to grasp a trans formative pencil was indicated to be more difficult in the intra-oral manipulation(p<.05). Pain and inconvenient-feeling level, which occur in muscle and skeletal system during practicing the scaling in mannequin, were indicated to be in order of shoulder, wrist, neck, waist, elbow, and headache. Conclusion : There was no big difference in the pain and the inconvenient feeling in muscle and skeletal system according to mannequin and intra-oral environment given manipulating tile manual implement. However, there was difference in manipulating the implement with a method of grasping transformative pencil. The pain and inconvenient feeling in muscle and skeletal system were the highest both in shoulder and wrist.

Evaluating intra- and inter-examiner reproducibility in histometric measurement: one-wall intrabony periodontal defects in beagle dogs

  • Purpose: Accurate and exact measurement is an important factor for generating meaningful results in any properly designed study. If all the participating examiners are able to yield similar results, it will be possible to evaluate the objective results of the study more easily and quickly. The purpose of this study was to evaluate the intra- and inter-examiner reproducibility of histometric measurements in the intrabony periodontal defect model. Methods: One wall intrabony defects were surgically created at the distal aspect of the second and the medial aspect of the fourth mandibular premolars in the right and left jaw quadrants in twenty beagle dogs and the defect sites received the following ${\beta}$-tri calcium phosphate, growth differentiation factor-0, growth differentiation factor-100 and sham surgery. Histometric analysis was performed after 8 weeks. Histometric parameters were recorded and repeated at three months interval by three examiners. Intra- and inter-examiner reproducibility was assessed. Results: Most parameters of all the groups showed high intra- and inter-examiner reproducibility. Parameters including defect height, bone regeneration height, cementum regeneration height, and formation of junctional epithelium yielded interexaminer correlation ${\geq}0.9$. The intra-examiner reproducibility showed a high result, over 0.9. Conclusions: Histometric evaluation of the one-wall intra-alveolar periodontal defect model showed high reproducibility not only for a single given examiner but also among the three examiners.

Development of Image-space Telecentric Lens for Intra-Oral 3D Scanner

  • An image-space telecentric lens for an intra-oral 3D scanner was designed and fabricated for dental application. Since a telecentric function can provide the same results regardless of image plane position, it helps to realize a more accurate image for an intra-oral scanner. The performance of the designed lens meets the required properties for HD resolution. In particular, lateral color is corrected within 1 pixel. This system achieves depth of focus of more than 3 mm. For user convenience, the developed system consists of a prism part and an imaging part. Both parts are optimized to reduce the front size and weight of the system. In order to make the parallax sights, parallax angle was determined to be 8 degrees between two optical systems.

Participation of IL-1β in temporomandibular nociception in rats with CFA-induced inflammation

  • The aim of the present study was to develop an animal model for evaluation of temporomandibular (TMJ) nociception under TMJ inflammation. We also investigated the participation of $IL-1{\beta}$ in inflammation-induced TMJ nociception. Experiments were carried out using male Sprague-Dawley rats. Intra-articular injection of 3% formalin was administered to evaluate hyperalgesia 3 days after CFA injection. Intra-articular injection of 3% formalin did not produce nociceptive behavior in normal rats. Although intra-articular injection of 3 doses of CFA produced TMJ inflammation, only 1:3 diluted CFA produced hyperalgesia when formalin was injected intra-articularly 3 days after CFA injection. Co-administration of IL-1 receptor inhibitor with formalin into the TMJ cavity 3 days after CFA injection was performed. Co-administration of IL-1 receptor inhibitor significantly inhibited formalin-induced hyperalgesia in rats with CFA-induced TMJ inflammation. These results suggested that intra-articular injection of formalin produced hyperalgesia under chronic TMJ inflammation. Moreover, $IL-1{\beta}$ plays an important role in TMJ hyperalgesia under chronic inflammation and blockade of $IL-1{\beta}$ is a potential therapeutic target for inflammatory TMJ pain.

Comparison of the accuracy of domestic dental intra-oral scanner(e-scanner) and model scanner (국산 치과용 구강스캐너(e-scanner)와 모델스캐너의 정확도 비교)

  • Purpose: The purpose of this study is to evaluate the discrepancy of scan process in dental intra oral scanner by comparing model scanner and anticipate possibility to introduce intra oral scan technique. Methods: 3D superimposition test was conducted to compare the scan discrepancy. The scanners used in this study are the e-oral scanner, the D750 model scanner, and the high precision CMM(3D Coordinate Measuring Machine). The standard of accuracy verification is ISO 5725-1; trueness and precision. Master model was manufactured by dental stone and scanned 5 times by intra oral, model scanner. Reference data was scanned 5 times by high accuracy CMM to evaluate the trueness. Results: Trueness of D750 scanner were $7.4{\mu}m$ $5.1{\mu}m$ $6.8{\mu}m$ at an abutment, an occluasal, a specific area. and trueness of e-scanner were $20.2{\mu}m$ $27.4{\mu}m$ $37.8{\mu}m$ at an abutment, an occluasal, a specific area. Precision of D750 scanner was $7.04{\mu}m$, e-scanner was $15.95{\mu}m$. Conclusion: When conducting in vitro test, The mean difference of trueness between e-scanner and D750 were $12.8{\mu}m$ at an abutment area, $22.3{\mu}m$ at an occlusal area, $31.0{\mu}m$ at a specific area and $8.91{\mu}m$ in precision. The scan discrepancies are within the range of clinical acceptance.

A Study of Radiation Dose Evaluation and Optimization Methods for Intra Oral Dental X-ray in Pediatric Patient (소아 구내촬영 시 방사선량 평가 및 최적화 방안에 대한 연구)

  • Although intra oral dental x-ray is a lower dose than other radiological examinations, pediatric patients are known to have a higher risk of radiation damage than adults. For this reason, pediatric dental x-ray requires management of dose evaluation and imaging conditions during the examination. In this study, the dose calculation program ALARA-Dental(child/adult) was used to evaluate the organ dose and effective dose exposed to each examination site during intra oral imaging of children during dental radiographic examination, and dose analysis according to the imaging conditions was performed. As a result, the highest organ dose distribution was shown at 0.044 ~ 0.097 mGy in all are as of the mucous membrane of oral cavity except for the maxillary incisors and canines. Also, in the case of the thyroid gland, the maxillary canine and maxillary premolar examination showed 0.027 and 0.020 mGy, respectively, and the dose distribution was 15.4% to 70.0% higher than that of the mandibular examination. As for the effective dose calculated during intra oral imaging, the maxillary anterior and canine examinations showed the highest effective doses of 0.005 and 0.004 mSv, respectively, and the maxillary area examination showed a higher dose distribution on average than the mandible.
