The Research Trends on the Acupuncture Treatment of Lumbar Herniated Intervertebral Disc Using PubMed Database (PubMed 검색을 통한 요추 추간판 탈출증의 침치료 연구 동향)

  • Objectives The purpose of this study is to research current trends of acupuncture treatment of lumbar herniated intervertebral disc using the PubMed database. Methods We set up the search strategy and investigated clinical trials on acupuncture treatment of lumbar herniated intervertebral disc through PubMed search. This study analyzed previous researched papers published from January 1st, 2000 to April 30th, 2014, and classified them by publication year, journal names, types of literature, treatment methods and evaluation scales. To assess the quality of the reviewed literature, randomized controlled trial (RCT) studies were assessed by Cochrane's risk of bias (ROB) tool and non-RCT studies were assessed by risk of bias for non-randomized studies (RoBANS). Results We found 35 studies on the acupuncture treatment of lumbar herniated intervertebral disc. Papers on this topic have been published, on average, three to four times annually in 9 journals since the mid-2000's. The journal with the largest number of publications was Chinese Acupuncture & Moxibustion, and most of articles were classified as RCT. Acupuncture treatment was performed individually or together with other treatments. The most frequently used pain evaluation index was visual analogue scale (VAS). The index of effective rate was used frequently but there was a lack of objectivity. In regards to the quality of the studies, outcome assessment in RCT showed that random sequence generation, allocation concealment, and the blinding of participants and personnel increase potential of risk of bias. For non-RCT assessment, outcome showed that confounding variable, measurement of intervention were at high risk of bias. Conclusions In order to obtain objective clinical evidence of acupuncture treatment of lumbar herniated intervertebral disc, further clinical studies should be designed to minimize the risk of bias, using STRICTA with larger sample sizes.

An Intradiscal Granuloma Due to a Retained Wooden Foreign Body

  • We report a patient with a wooden foreign body granuloma in the intervertebral disc space being symptomatic 17 years after a paraspinal penetrant trauma. According to the our result of the search for wooden foreign body granulomas, this is the first case suffered from a wooden foreign body granuloma in the intervertebral disc space that reported in the literature. In this report, we emphasized the importance of rigorous examination and follow up in paraspinal wooden penetrant traumas.

Clinical Usefulness of Somatosensory Evoked Potentials (SSEP) in Dogs with Intervertebral Disc Diseases (추간판질환 개에 Somatosensory Evoked Potential(SSEP)의 임상활용)

  • Clinical usefulness of somatosensory evoked potentials (SSEP) as a prognostic tool was evaluated with three dogs showing clinical signs associated with intervertebral disc diseases. Prior to measure SSEP, history taking, physical examination, radiological study and neurological examination were performed. In case 1, poor prognosis was predicted because deep pain was not observed and loss of sensory function was observed in SSEP. And the clinical signs persisted with the conservative treatment. However, in cases 2 and 3, good prognoses were predicted by normal conduction velocity in SSEP that meant the presence of sensory function. The clinical signs of cases 2 and 3 disappeared at days 18 and 13 after treatment, respectively. These results suggest SSEP be used clinically as a prognostic tool in dogs with intervertebral disc diseases.

A Review of Lumbar Herniated Intervertebral Disc (요추 추간판 탈출증에 대한 고찰)

  • Purpose : The purpose of the study was to examine lumbar pain through literature reviews about herniated intervertebral disc (HIVD). Methods : Diagnosis was made by patient's history, physical examination, simple radiography, myelography, spinal computerized tomography, electromyography, somatosensory evoked potential, etc. Results : There are two treatment modalities, one is conservative and the other is operative. About 90% of patients with HIVD have been recovered only by conservative treatment and therapeutic exercise was the most effective one among these conservative treatments. Conclusion : We consider HIVD can be treated with therapeutic exercise which can correct muscular imbalance and facet joint problems and make the degenerative process of intervertebral disc slower.

The Effect of Conservative Therapy on Thoracolumbar Intervertebral Disc Disease on 15 dogs (소형견 15예의 흉요추추간판질환에 대한 보존치료)

  • Fifteen dogs with clinical signs of paraparesis, paraplegia and urinary dysfunction were referred to Veterinary Medical leaching Hospital, College of Veterinary Medicine, Konkuk University. According to physical and neurologic examination, survey radiograph and myelography, these patients were diagnosed as thoracolumbar intervertebral disc disease. All of them were treated with medical (prednisolone or carprofen, antibiotics), acupuncture (Ji Zhong, Bai Hui, Zhong Shu, Pang Guang Shu, Zu San Li, Huan Tiao) and physical therapy (hydrotherapy, thermotherapy, massage, manipulation, swimming). Of twelve patients were recovered of neurologic deficits within 21 days after the onset of treatment. However, three dogs were not resolved, which were found to have other spinal cord disease concomitantly. All of patients showed normal urination after treatment. Until 21 months after treatment, there were no recurrence in twelve patients. This study indicated that the conservative therapy could be effectively managed in paraparesis or paraplegia and urinary dysfunction which caused by intervertebral disc disease.

  • The goal of total disc replacement is to restore pain-free mobility to a diseased functional spinal unit, by replacing the degenerated disc with a mobile bearing prosthesis. SB Charite III is named commercial product as the Artificial Intervertebral Disc (AID). SB Charite III consists of sliding core and endplate made by Ultra-high Molecular Weight Polyethylene (UHMWPE) and cobalt chrome alloy, respectively. To evaluate the effect of von-Mises stress in AID, and three-dimensional finite element model of AID analysis was preformed for four different loading types of sliding core. Consequently, endplate was compared with a compressive preload at 400N and flexion moment at $3{\sim}9Nm4. Therefore, this research has obtained result that von-Mises stress of sliding core in AID disc by radius curvature.

  • Objective : The purpose of this study was to suggest (diagnostic) criteria for lumbar disc space and see the correlation between lumbar disc space narrowing and the level of HIVD. Methods : We looked at the 113 patients diagnosed disc space narrowing with lumbar x-ray and MRI films who have visited Jaseng Hospital of Oriental Medicine with low back pain in October,2013. The width of lumbar disc space and the severity of Herniated Intervertebral Disc(HIVD) on MRI films were collected and stastically analyzed using one-way ANOVA. Results : In this study, measured value(%) for lumbar disc space of the group diagnosed serious HIVD, such as protrusion, extrusion and migration was significantly decresed compared to the normal group and the bulging group. Conclusion : There was a significant correlation between lumbar disc space narrowing and the level of HIVD.

Percutaneous Discectomy of Herniated Intervertebral Disc with a Dekompressor(R) (요추 추간판 탈출증에서 Dekompressor(R)를 이용한 경피적 수핵 감압술)

  • Background: Radicular pain, associated with herniated intervertebral disc, has been treated with either conservative treatments or a traditional surgical discectomy. Several modalities for minimally invasive percutaneous procedures have been developed as alternatives to a surgical discectomy. Percutaneous decompression using a $Dekompressor^{(R)}$ probe has been recently introduced. Herein, we report the outcome of percutaneous decompression with a $Dekompressor^{(R)}$ for the treatment of a herniated intervertebral disc. Methods: Between August 2004 and April 2005, percutaneous decompression was performed using a $Dekompressor^{(R)}$, 1.5-mm percutaneous lumbar discectomy probe in 17 patients with a herniated lumbar intervertebral disc resistant to conservative treatments, with the results reviewed retrospectively. The procedure was performed under fluoroscopic guidance after local anesthesia. Disc access was gained with a posterolateral approach on the symptomatic side and intradiscal placement of the discectomy probe in the herniated disc confirmed from the anteroposterior and lateral views on the fluoroscopy. Results: We obtained satisfactory clinical results in 14 patients with a decrease in the initial Visual analogue scale (VAS) of more than 55% and the elimination or reduction of analgesic medication, with a follow-up of 3 to 11 months. Conclusion: We concluded that a percutaneous discectomy with a $Dekompressor^{(R)}$ probe might be an effective alternative for the treatments of painful disc herniations resistant to conservative managements when performed under proper selection criteria.

Role of Expression of Inflammatory Mediators in Primary and Recurrent Lumbar Disc Herniation

  • Objective: To assess role of some inflammatory mediators in patients with primary and recurrent lumbar disc herniation. Expression of IL-6, transforming growth factor (TGF)-1, insulin-like growth factor (IGF)-1, and Bcl-2-associated X protein (BAX) have been shown to be more intense in the primary group than the recurrent goup, but this mediators may be important aspects prognostic. Methods: 19 patients underwent primary and revision operations between June 1, 2009 and June 1, 2014, and they were included in this study. The 19 patients' intervertebral disc specimens obtained from the primary procedures and reoperations were evaluated. Expression of IL-6, TGF-1, IGF-1, and BAX were examined immunohistochemically in the 38 biopsy tissues obtained from the primary and recurrent herniated intervertebral discs during the operation. Results: For IL-6 expression in the intervertebral disc specimens, there was no difference between the groups. The immunohistochemical study showed that the intervertebral disc specimens in the primary group were stained intensely by TGF-1 compared with the recurrent group. Expression of IGF-1 in the primary group was found moderate. In contrast, in the recurrent group of patients was mild expression of IGF-1. The primary group intervertebral disc specimens were stained moderately by BAX compared with the recurrent group. Conclusion: The results of our prognostic evaluation of patients in the recurrent group who were operated due to disc herniation suggest that mediators may be important parameters.
