Conceptual Model for Women s Health (여성건강을 위한 개념적 모형)

  • There has recently been an increased interest in women's health from, various disciplines, with different perspectives presented according to each profession's academic background. This has led to many instances of incorrectly defining, or misinterpretation, of the issues even among professionals. Nurse scholars as well as practitioners who work in women's health care need to have a clear conceptual understanding of women's health in order to build a body of knowledge, delineate curricular activities, and set directions for professional nursing interventions. In addition, a conceptual model that may be directly utilized in practice is needed to maintain and promote women's health issues. The purpose of this study was to apply a Hybrid model, analyzing conceptual definitions and discussions related to women's health gathered from review of the literature. Further to compare analyticals the concepts and properties observed from field work, so as to present a final definition of women's health and, build a conceptual framework for a united comprehensive perspective on the concept as well as on nursing practice. Data collection and analysis consisted of a theoretical stage, field work stage, and final analysis. A heterogeneous group of professionals and lay persons, 39 in all, participated in the field work. Study findings Include several subconcepts under the concept of women's health : a woman's whole life, holistic health, quality of life, awareness of being a woman, individual nursing, self care ability, reproductive health, and family health. Thus, a comprehensive definition was built, 1. e., "Women's health care be defined as improvement in the quality of life of women through attainment of holistic health throughout the life span. With reproductive health at the core, the concept is directly related to family and national health, and includes taking care of one's own health based on awareness of being a woman and utilizing self care activities. Women's health care issues are unique and allow various responses, therefore women's health professionals need to apply individual approaches to reach solutions in attaining holistic health and improving quality of life." The constructual factors of women's health were found to be reproductive functions, diseases more common in woman, self actualization, mental health, women's health policies, sexuality, midlife changes, and marital relations, with each factor having more than three properties. Positive factors affecting women's health were found to be a normal childbearing process, a healthy lifestyle, active health management, health information, support, and resources, and interpersonal relationships. Negative factors were found to be overwhelming role stress, cultural oppression, gender inequality, distorted sexual identity, economic difficulties, misuse and/or abuse of substances, and stress. The model of women's health may be visualized as a balance scale set upon a woman's life, supporting 4 concentric circles. The innermost circle and second circle incorporate conceptual definitions of women's health, and the outer two circles represent the constructional factors and properties of women's health. Each circle has its own color that symbolizes the conceptual meaning. Positive and negative factors are represented as weights at either end of the scale, and are affected by nursing intervention, i. e., health and wellness increase when positive factors are stronger, whereas disease and illness increase when negative factors are stronger. This model is only a preliminary effort and requires much discussion and testing to be further developed. Continuous research is also required.

Discomfort of Donors associated with Bone Marrow Donation (골수공여자들의 불편감)

  • The study was designed to provide the fundamental information for understanding discomfort of bone marrow donors and for promoting an individual comfort by comparing the difference on discomfort between unrelated donors and related donors. The subject of the study was fifty related donors and thirty unrelated donors who was in the C University Hospital. This survey had been carried out and collected from October, 1998 to March, 1999. The scale of discomfort of donors associated with bone marrow donation were assessed by the questionnaire deviced by Kim Sang Dol and amended by the researcher. Data was analyzed by $x^2-test$, t-test, two-way ANOVA, and Pearson Correlation Coefficients. The results were as follows. 1. Considering the general characteristics of bone marrow donors according to gender, male was consisted of $60\%$ and female was consisted of $40\%$. Of those related donors are consisted for $62.5\%$ and accounted for $37.5\%$ of those unrelated donors. Considering the classification to the job, employee of company were major donors which was $35\%$, and next order was student, individual businessman, and housewife. Considering the education level. college students were $48.7\%$ and students who have less than high school level were $42.5\%$. 2. According to the above the results regarding discomfort of bone marrow donation, it is especially shown that the major cause for discomfort of bone marrow donors is on physical factor. The concrete examples for physical factor are pain in the region of bone marrow harvest and pain in the injection part by fluid therapy and blood-sampling, an immovability of the body after bone marrow harvest, and difficulties on walking. Considering physiological factor, there are an uneasiness about leading to injure their health, vague fear about the hospital. and a tedium at hospital. Environmental factors for discomfort of bone marrow donors are insufficient explanation for needle gauge and procedure of bone marrow donation and difficulty on following medical schedule. Therefore. it is necessary to establish more effective and systematically organized program for nursing intervention based on the research results. An effective program is only useful in getting rid of discomfort of bone marrow donors.

Effects of Aquatic Exercise Program on the Shoulder Joint Function, Physical Symptom and Quality of Life in Postmastectomy Patients (수중운동 프로그램이 유방절제술 환자의 어깨관절 기능, 신체적 자각증상 및 삶의 질에 미치는 효과)

  • This study was designed to investigate the effects of an aquatic exercise program on the shoulder joint function. physical symptom. quality of life and stress among the patients who received modified radical mastectomy between 6 to 12 months prior to their visits. The subjects were 31 women aged between 40 and 60 who visited the out-patient department at Kang Nam St. Mary's Hospital for follow-up care. and were not under the treatment of intravenous cancer chemotherapy or radiation therapy. and had no complications. Twelve of them were assigned to the control group. while nineteen subjects to the experimental group. The aquatic exercise program was developed by the author with the assistance of exercise specialists. The program includes warming uP. aerobic and cooling down exercises in water. The aquatic exercise program for the experimental group was carried out 3 times a week with 60 minutes in each time for 8 weeks from September 20th to November 15th. 1995 in a regular swimming pool in Seoul. Changes in the range of motion of the shoulder joint. muscle strength. physical symptom. quality of life. and stress were examined after the completion of treatment. The data were collected through isokinetic muscle strength evaluation and questionnaire survey before and after the treatment. Paired and unpaired t-test were adopted to analyze the data. The results were as follows ; 1. The increment in the range of motion of the shoulder joint in the experimental group after the exercise was significantly greater than those in the control group. 2. The peak torque of shoulder girdle muscles increased significantly after the exercise in the experimental group only. 3. The physical symptom score decreased significantly after the exercise in the experimental group only. 4. The experimental group revealed significantly higher level of quality of life and lower level of stress after the exercise compared with those before the exercise. whereas the control group showed no significant changes in those levels. These findings may indicate that the aquatic exercise program is effective in increasing the range of motion of the shoulder joint and muscle strength and quality of life. and also effective in decreasing physical symptoms. and the level of stress in postmastectomy patients. Accordingly. the acquatic exercise program' can be adopted as an effective nursing intervention for postmastectomy rehabilitation.

  • This study investigated the effects of response prompting through 3-steps compliance training to reaction time for child with Asperger's syndrome(AS). The participant was 3 and 8 year-old boy who was diagnostic As with non-compliant, delayed receptive language. Study design was multiple-baseline across behaviors. Target Behaviors were hands-up, following direction, and answering behavior. Dependent variable was latency reaction time during compliance training. This results mean that reaction time was increased raise hands-up behavior, compliance behavior and response ask questions. During intervention, the participant improve the rate on-task behavior as well as reduce off-task behaviors.

A Convergence study on the effect of Caregiving Burden on Interpersonal problem of Dementia Caregivers : Focusing on moderating effect of Family Support (치매노인보호자의 부양부담이 대인관계문제에 미치는 영향에 관한 융합연구 : 가족지지의 조절효과를 중심으로)

  • The purpose of this study is to examine the effects of caregiving burden on the caregiver's interpersonal problem. In addition, mediating effect of family support was examined. In order to accomplish these purposes, a total of 341 sons and daughters in law who are caregivers of dementia elderly using self-report questionnaires. The analysis result of dementia elderly caregiver's symptoms showed that prevalence of caregiving burden were 50.9% and interpersonal problem were 43.6%. As a result, it was confirmed that the caregiving burden had a statistically significant effect on interpersonal problem. Also, Family support was found to moderate the relationship between caregiving burden and interpersonal problem(p< .05). Based on this result, I suggested the various policies and the necessity of social welfare intervention for caregivers of dementia elderly.

Oral Status of Middle-aged Orthodontic Patients and Their Treatment Modality; Comparison with Young-aged Adult Patients (지상보수교육강좌 1 - 중장년 성인교정환자의 구강상태 및 치료양태에 관한 연구; 젊은 성인교정환자와의 비교분석)

  • Orthodontic treatment for middle-aged patients has become more commonplace with various reasons including improved socioeconomic status. Understanding of oral status and treatment modalities of middle-aged patients is mandatory for accurate diagnosis and proper treatment planning. This study investigated 100 consecutive patients aged 40s and 50s and 100 aged 20s who had been examined and diagnosed at the Department of Orthodontics, Chonnam National University Dental Hospital. The results were obtained as follows; 1. Gender distribution showed female outnumbered male patients in young-aged adult patients, but middle-aged patients showed similar male and female distribution. 2. The major concern seeking orthodontic treatment was esthetics not only in young-aged but also in middle-aged adult patients, and a number of middle-aged patients were concerned about oral health as well. 3. Considerable number of middle-aged patients were referred by other dental specialties while young-aged adult patients were more self-motivated for orthodontic treatment. 4. Middle-aged adult patients had more missing teeth and periodontal disease than young-aged adults. 5. The most frequently-observed problem was dental spacing in middle-aged patients while dental crowding in young-aged adult patients. Middle-aged patients showed higher prevalence of deep overbite and overjet while most of young-aged adults presented opposite direction of problem in overbite and overjet. 6. Limited orthodontic treatment was required rather than comprehensive treatment in middle-aged patients, and the most common tooth moving area was anterior part of dentition in case of limited treatment. Need of interdisciplinary therapy with other dental specialties was more common in middle-aged patients. 7. Intervention of specific technique such as invisible TP, passive bracketing, passive wire bonding, and lingual orthodontics was more required in middle-aged patients. Considering that middle-aged patients have different characteristics than young-aged adults, the results of the present study suggest that different treatment modalities are required in middle-aged orthodontic patients in order to manage them properly and efficiently.

Regional Characteristics of Bird Community in Nakdong River Basin (낙동강 하류 권역별 조류군집의 특성)

  • The survey of birds in Nakdong River Basin was conducted from March in 2003 to February in 2004. This study shows that the population of birds was 94,481 individuals and 114 species. The bird recorded in Daemadeung (Ⅰ) was 21,781 individuals and 55 species. It is surrounded with silt to have a better condition for a habitat and the rest place than other areas in the Nakdong River Basin. In the traits of the groups, Grebes were well established in the lower estuary(Eulsook islet), Cormorants in the main stream of the Nakdong River and around West Nakdong River (F). Swans, Shelduck and Shorebirds were using silt in the lower estuary where the depth of water is shallow, dabbling ducks were also established in West Nakdong River (F) and Yummak (C), diving ducks in the upper Nosan Watergate in West Nakdong River (F). Besides, land birds was distributed in the Yummak (C), Eulsook islet (A) and West-Nakdong River (F). Because they are connected with the artifical forest in Eulsook Islet Garbage Field and Ⅱwoong islet, it is good for the land birds and the water birds to have a habitat in this kinds of vacant lot. The Nakdong River Basin is not only an important area as the habitat and rest place for ducks and geese but also one of the areas where we can not get out of the temptation to development. To preserve effectively the natural habitat from human s intervention, it needs to keep supervising in Daejeo Watergate (E) and the upper area Noksan watergate in West Nakdong River (F) and Yummak (C).

Comparison of Effects between MSAT on Piriformis Muscle and Non-MSAT on Piriformis Muscle in Patients with HLD with Leg Radiating Pain: A Retrospective Analysis (하지방사통을 동반한 요추 추간판 탈출증 환자에 대한 이상근 MSAT 치료군과 비치료군의 후향적 호전도 비교 연구)

  • Objectives The purpose of this study was to compare the effects of MSAT (Mostion style acuputure treatment) on piriformis muscle regarding leg radiating pain with HIVD of L-SPINE. Methods Forty patients who received inpatient treatment from Febuarary 1 to March 31, 2014 in the Daejon-Jaseng oriental medicine hospital were divided into 2 groups by blocked randomization : Group A (n=20)=routine treatment on HIVD of L-SPINE with MSAT on piriformis muscle and Group B (n=20)=routine treatment on HIVD of L-SPINE (acupuncture, Shinbaro pharmacopuncture, herb medication, chuna treatment and physiotherapy). The MSAT (Mostion style acuputure treatment) on piriformis muscle was administered each day and both groups were received routine treatment each day. Evaluations were made before treatment, after 3 days and 7 days, and before discharge using the NRS (numeric rating scale) and ODI (oswestry disability index). The statistically significance was evaluated by SPSS 18.0 for windows. Results Compared to before treatment, the NRS (numeric rating scale) and ODI (oswestry disability index) of both A and B groups after 3, 7 days and before discharge of treatment significantly decreased (p<0.05). Regarding group comparison, the NRS (numeric rating scale) and ODI (oswestry disability index) of A group was significantly lower than the B group at 3, 7 days, before dischanre (p<0.05). Regarding improvement of the NRS (numeric rating scale) and ODI (oswestry disability index), A group was significantly lower than the B group at 3 day (NRS), 7day (ODI) (p=0.000) but there was no significant difference between the two groups at 7 days (NRS) and before discharge (p>0.05). Conclusions Compared to just routine treatment, the MSAT (Mostion style acuputure treatment) on piriformis muscle significantly improved the NRS (numeric rating scale) and ODI (oswestry disability index) after 3, 7 days, and before discharge. Because early intervention determines the grade of Leg radiating pain with HIVD of L-SPINE, the MSAT (Mostion style acuputure treatment) on piriformisat the early stage will be clinically helpful to patients.

DENS INVAGINATUS AND A VITAL MAXILLARY LATERAL INCISOR WITH LATERAL PERIODONTAL ABSCESS (생활력이 있는 상악측절치에서 치내치로 인한 측방치주농양이 형성된 증례)

  • Dens invaginatus is a developmental anomaly resulting from an invagination of the enamel organ. The incidence is highest with maxillary permanent lateral incisors. The reported occurrence ranges from 0.04 to 10%. This anomaly may involve the pulp and periapical tissues and cause pulpal inflammation, loss of vitality, apical and lateral periodontitis, periapical abscesses and cysts and stimulate internal resorption. Oehlers describes dens invaginatus as occurrence in three forms. In treating type 3 invaginatus, treatment strategy can be determined by considering the complexity and accessibility of invagination. In this case, showing simple invagination, it could be treated by simple endodontic treament confining to invagination without loss of vitality of tooth. After treatment of the present case, the results were as follows: 1. In type 3 dens invagiantus, if the tooth is vital and there is no evidence of communicating between invagination and pulp, we can save the vitality of the tooth and resolve the lesion by endodontic treament confining to the invagination. 2. In the invagination with opened apex, the closure of apex can be induced by apexification procedure doing this, we can avoid the neccessity of surgical intervention.

SIALOLITHIASIS ON THE MOUTH FLOOR IN A CHILD (소아의 구강저에 발생한 타석증)

  • Sialolithiasis is the formation of calcific concretions within the ductal system of major or minor salivary glands. The submandibular gland is most involved because of its high viscosity of the saliva and the long, curved duct. It may occur at any age but, it is most common in middle-aged adults and rare in childhood. Clinical symptoms in sialolithiasis are variable but, swelling is the most common, followed by the pain. Clinical examination and radiographic examination(panoramic and mandibular occlusal radiographs, sialography, intraoral-, extraoral- ultrasound, CT scan, MRI and sialoendoscopy) can help to confirm a diagnosis and localize a stone. The treatment is surgical intervention, either removal of the sialolith or sialoadenectomy. However, non-invasive techniques including shock-wave lithotripsy, $CO_2$ laser and endoscopic treatment used in selected cases. A 5-Year-old girl referred from private practice for evaluation of a yellowish mass on the floor of the mouth. She complained that it had became three times bigger than four months ago when it was found for the first time and she had some pain on submandibular gland area occasionally. On physical examination, a firm and yellowish mass could be seen at the orifice of the submandibular duct. Diagnosis is the submandibular sialolithiasis in the anterior Wharton`s duct. Under local anesthesia, stone was removed.
