What Else Is Needed in the Korean Government's Master Plan for People With Developmental Disabilities?

  • On September 12, 2018, President Jae-In Moon announced the Comprehensive Plan for Lifelong Care for People with Developmental Disabilities, with representatives from the associated government branches (Ministry of Health and Welfare, Ministry of Education, and Ministry of Employment and Labor) in attendance. The goals of this plan are to provide health, medical, rehabilitative, special education, and social welfare services according to the life-stages of the affected individuals; to reduce parental pressure; to promote social interventions; and to enhance community-level participation in order to create a 'welfare society in harmony.' However, in order for the plan to succeed, additional efforts must be made in the following areas. First, an epidemiological survey is needed to understand the scale, prevalence, and incidence of developmental disabilities and to establish an evidence base to support policy development. Second, accurate definitions of developmental disabilities must be established in order to avoid policy discrimination based on impairment type and age. Third, personal evaluations to assess disabled individuals' unmet needs and customized service designs to deliver those needs are required. Fourth, the plan must fulfill the goals of accessibility and fairness that the government intends to provide. Fifth, the government should consider an integrated financial support system and to propose a detailed plan for monetary distributions. Finally, an integrated system that links health, medical, employment, educational, and welfare services must be constructed.

Observational Motor Skill Learning in Individuals with Intellectual Disabilities (지적장애인의 관찰적 운동기술 학습)

  • The purpose of this study was to determine the influence of video modeling observational learning intervention on the learning and performance of a bowling skill in adolescents with intellectual disabilities. Thirty special middle school students whose ages ranged from 14 to 16 years were recruited from Daegu, Korea. Intellectual disabilities of the participants were assessed by Korean version of the Wechsler intelligence scale for adolescent and a social maturity scale. During the experiment, participants repeatedly watched the one-minute action observation film for three minutes before beginning each frame and played 60 frames. Statistical comparisons were performed using a 2 (groups) ${\times}$ 6 (trials) ANOVA, with repeated measures on the last factor of the acquisition stage (p<0.05). Factors of the retention stage scores were analyzed by one-way ANOVA. The sources of any significant main effects were tested using a Tukey's HSD (honest significant difference) approach. SPSS 21.0 was used for statistical analyses. The performance scores of the action observation group were significantly higher than those of the control group. The findings showed that observational learning in the form video modeling has the potential to enhance acquisition and learning of a bowling sport skill in intellectual disability individuals; however, these findings are limited to adolescents with moderate intellectual disabilities.

The Converge Effects of Long-term Weight-bearing Exercise on Lumbar, Femur Neck BMD and Body Compositions in Person with Intellectual Disabilities and Normal Men (지적장애와 일반인 남성의 장기간 체중부하운동 참여가 요추와 대퇴골경부 골밀도 및 신체구성에 미치는 융합적 효과)

  • The purpose of this study was to determined the converge effects of weight-bearing exercise on lumbar, femur neck BMD and body compositions in intellectual disabilities and normal men. Nineteen men were participated for this study, they are divided into two groups(intellectual disabilities, n=9, and normal men, n=10). All subjects are accomplished on weight-bearing exercise (music rope-jumping) program for 60 minutes a day, 3 days a week throughout 12 weeks. Two-way ANOVA with repeated measures was used to determine differences between and within the intellectual and normal groups for dependent variables. The statistically significant level was set at 0.05. In this study, there were a significant differences on the bone mineral density of lumbar and femur neck level after exercise training in both groups (respectively p<.01, p<.001). Also, I found that there were effectively decreased on body weight, percent of body fat, and BMI levels after exercise training for 12 weeks in both groups (respectively p<.001). Therefore, music rope-jump exercise program may suggested to be one of the ideal training methods for enhancing and the convergence positive effect of bone mineral density and body compositions in intellectual and normal person.

Analysis of the Convergence Pulmonary Function in the 20s Men of Mild Intellectual Disabilities with Obesity According to Position Changes (경도 지적장애를 가진 20대 남성 비만인의 자세변화에 따른 융합적인 폐기능에 대한 비교)

  • The purpose of this study was to determine whether changes of position might effect the convergence pulmonary function of the 20s men of mild intellectual disabilities with obesity. Ten subjects of mild intellectual disabilities with obesity(fat group) and ten contrary subjects without obesity(control group) were participated in the experiment. Subjects were assessed for their pulmonary function by using Fit mate according to the position changes(supine position, 45 lean sitting position, 90 sitting position). One-way repeated ANOVA analyzed each region data of pulmonary function of both the fat group and the control group according to their position changes. The result of the experiment showed that the 20s men of mild intellectual disabilities with obesity have lower pulmonary function than the contrary subjects in the each given posture. In comparison with three experimental positions, supine position was the lowest in the pulmonary function. This study showed the 20s men of mild intellectual disabilities with obesity have lower function of pulmonary volume than the control group, and thus, it suggests that the pulmonary functional data of 20s men of mild intellectual disabilities with obesity in this experiment can be used as a basic respiratory one for the various exercise programs in the area of the physical activities.

A Study about the Changes of the Writing Ability and Hand Function of the Children of Intellectual Disabilities According to the White Noise (백색소음의 적용에 따른 지적장애 아동의 쓰기 능력과 손 기능의 변화에 관한 연구)

  • The purpose of this study was to analysis of the changes of the white noise on the change of the writing ability and hand function of the children with the intellectual disabilities and then provide the basic information about that. The subjects was 12 children with intellectual disabilities. White noise was applied to analyze the subjects' writing ability and hand function before and after application. The provision of the white noise was continuous and uniform through the white noise generator. The analysis of the writing ability was performed by using the KNISE-BAAT assessment and the writing, vocabulary and composing ability were evaluated for the writing ability of the subjects. Also, the analysis of the hand function was performed by using the pegboard sub-item of the Manual Function Test. The results of the writing ability showed the statistically significant increase of the writing and vocabulary ability, but in the case of the composing ability, there was no statistically significant increase in the composing ability. Also, the results of the hand function showed the statistically significant increase in the both hands. The use of the white noise should be considered as a compensatory approach to improve the writing ability and hand function of the children with intellectual disabilities. Also, in order to improve the level of the performance, learning level, and academic achievement of the children of the intellectual disabilities, the application of the white noise in the living and learning environment should be needed to consider.

TREATMENT OF OPENBITE WITH MYOFUNCTIONAL THERAPY IN MOYAMOYA DISEASE PATIENT (근기능요법을 통한 모야모야병 환자의 개방 교합 치료)

  • Moyamoya disease is characterized by progressive bilateral narrowing of the internal carotid arteries(ICA) and their main branches, and formation of a fine vascular network (moyamoya vessels) at the base of brain. Mild intellectual or motor impairment, or both can be seen in moyamoya disease patient. Patients having intellectual disability usually have malocclusions associated with oral habits such as tongue thrusting, bruxism, or clenching. A patient with moyamoya disease and cerebral palsy visited Chonbuk National University hospital for dental examination. She showed mild openbite on anterior teeth, and had oral habits such as mouth breathing, tongue thrusting, and drooling. She was treated with myofunctional therapy(MFT) whose purpose was to strengthen orofacial muscles. Lip closing force and bite force were recorded to evaluate the muscle tone. Tongue crib was delivered to restrict tongue thrusting habit, since her compliance to the MFT was decreased after 6 months of treatment. With the MFT and tongue crib, the openbite was fairly improved. When myofunctional therapy has difficulty in obtaining good compliances, the treatment should not be effective enough. Although intellectually disabled patients are not compliant enough, thus a habit breaking device should be needed to treat the openbite caused by oral habits, dentist should make an effort to do MFT, since it could reduce a treatment period, and enhance the stability.

THE ADDITIONAL POINT SYSTEM OF NATIONAL HEALTH INSURANCE FOR DENTAL TREATMENT IN PATIENTS WITH A SPECIAL HEALTH CARE NEED IN KOREA (한국의 장애인 환자 치과 진료를 위한 국민 건강 보험 가산제도의 종류 및 청구 현황)

  • In order to increase the accessibility of dental care for people with disabilities, National Health Insurance Service has implemented an additional point system of National Dental Insurance for dental treatment of patients with a special health care need (AID). The purpose of this study is to investigate the types and status of AID in Korea using data of the Health Insurance Review and Assessment Service from 2011 to 2017. The basic consultation fee is increased by 9.03 points (713 won) for brain disorder, intellectual disability, mental disability, or autistic disorder. From 2011 to 2015, the number of claims with a basic consultation fee increased from 90,456 to 141,179. Dental treatment and surgical treatment fee is increased by 100% of the defined insurance score for each of the 15 items. During the five years from 2012 to 2016, the number and amount of claims for each item increased steadily. Of the total claims for 5 years, endodontic treatment was highest, with 107,477 cases, followed by 51,641 cases of scaling. There are two types of dental safety observation fee, simple and complex. The simple safety observation fee is 10,370 won per day, and the complex safety observation fee is 20,750 won per day. Dental safety observation fees were charged 34 times in 2015, 14 times in 2016, and 41 times through May 2017. From 2011 to 2017, the number and amount of claims using AID for dental care for people with disabilities increased. However, considering that the number of registered dental users with disability was about 560,000 in 2016, the number of claims using AID is 1-20,000, which is less than 2% of registered dental users with disability. Therefore, it is necessary to expand dental services for people with disabilities including AID.

A Case Study of Oral Healthcare Program for Some Students with Intellectual Disabilities (일부 지적장애 학생을 위한 구강건강관리 프로그램에 관한 사례연구)

  • This study is to develop an oral healthcare program for students with intellectual disabilities, who were in high-school grades in Special School C located in North Chungcheong Province, and to examine its effect after applying the program. The program effect was the index of oral environment care ability and the score of dental caries risk per each session. In addition, images were taken for phase-contrast microscopy. The PHP index before and after the oral healthcare program and the risk of dental caries decreased but showed no significant difference. An observation on phase-contrast microscopy revealed that the number of cocci decreased at the third session compared to that in the first session, and almost no bacillus was observed. The oral healthcare program, would be able to improve the oral environment care ability of students with intellectual disabilities and lower the risk of dental caries.

A Study on the Real Condition of Teachers' Safety Accident and Methods of Preventive Education in Special Schools for Students with Mental Disorder (지적장애 특수학교 교사의 안전사고 실태 및 예방교육 방안에 관한 연구)

  • This study is the purpose of effective safety education measures for special school teachers with intellectual disabilities on the status of safety accidents and preventive education. In summary, the results show that there is a statistically significant positive correlation between the safety education activities of teachers at special schools with intellectual disabilities, safety education environment, safety education recognition, safety education knowledge, safety resources and training experience, and internal and external factors. Overall, the experience of safety accidents by teachers is higher than expected, and the practice of safety education to prevent the occurrence of various places, times, and types such as classrooms, staircases, auditoriums, and rides. The practice of safety education to prevent this also indicated that the safety resources of special school teachers and opportunities for training experiences should be expanded more by presenting safety data, knowledge, awareness, and education activities in order.

DENTAL MANAGEMENT OF A PATIENT WITH KABUKI SYNDROME : A CASE REPORT (가부키 증후군(Kabuki syndrome) 환자의 치과적 관리의 치험례)

  • Kabuki syndrome is characterized by typical facial features (elongated palpebral fissures with eversion of the lateral third of the lower eyelid; arched and broad eyebrows; short columella with depressed nasal tip; large, prominent, or cupped ears), minor skeletal anomalies, persistence of fetal fingertip pads, mild to moderate intellectual disability, and postnatal growth deficiency. A 6-year-old male with kabuki syndrome was referred from the local dental clinic for left facial swelling and dental caries on all primary molars. He was treated for acute periapical abscess with incision and drainage under physical restraint, and left maxillary second primary molar was extracted. Other caries treatment was performed under general anesthesia. As the syndrome involves many different medical problems, special cares should be considered. Dental treatment should be carried out in comprehensive consultation system.
