Nutritional Status among Elderly Korean Women and Related Factors (간이영양상태조사지(Mini Nutritional Assessment)로 조사한 일개 복지관 여성노인의 영양상태 평가와 관련요인 분석)

  • Purpose: The purpose of this study was to evaluate the nutritional status of elderly Korean women and to investigate related factors. Methods: A total of 100 elderly women attending a local elderly welfare center were recruited for study. Evaluation criteria included demographics, clinical status, nutritional status (using a mini-nutritional assessment), food intake, and nutritional bioparameters, including homocysteine, transferrin, prealbumin and lymphocyte levels, and their antioxidant power status. Results: Among the test group, 83% of the subjects were classified as undernourished, and they often had hyperlipidemia and a lower intake of fiber ($p$ <.05, respectively) and vegetables than normal subjects. In a correlation analysis, their nutritional status was associated with abdominal circumference, the Korean Geriatric Depression Scale score and the Enriched Social Support Inventory score. Nutritional bioparameters and antioxidant power were associated with age among undernourished subjects. Old age, the prevalence of hyperlipidemia, and poor social support significantly increased the risk of undernutrition. Poor social support, the prevalence of diabetes mellitus and depression significantly increased the risk of nutritional deficiency after controlling for age, educational and economic variables. Conclusion: More strategies to increase social support in the Korean female elderly population may improve their nutritional status.

Dietary Habits and Factors Associated with Depression in Yangpyeong-gun Elderly (양평군 노인에서 우울 지수, 영양불량과 식생활 실천 요인)

  • This study was conducted to analyze relationships between depression indices, mini nutritional assessment scores, and nutritional quotients among 80 elderly in Yangpyeong-gun and to identify factors that help prevent depression and malnutrition. Nutrition assessment scores were low in the high-risk group (PHQ-9 score ≥10), and nutritional quotient scores were lower in the high-risk group than in the normal group (PHQ-9 score ≤4). Interestingly, the consumption frequencies of fruits, eggs, and nuts were low in the high-risk group, and subjective health awareness, dental condition, and sleep were poorer. The total PHQ-9 score was correlated with malnutrition, body mass index, calf circumference, weight change, independent daily living, reduced meal amount, water intake, and the need for help when eating. Analysis of correlations between items of the PHQ-9 and nutritional status evaluation indices showed that a self-perceived feeling of depression, low energy, difficulty controlling sleep or appetite, negative thoughts (e.g., failure, disappointment), and difficulty concentrating were negatively correlated with total nutritional status scores. These results show that attention is required when food or water intake decreases and that deviation from normal sleep and appetite cycles flags the need to prepare guidelines to prevent depression.

Risk Assessment on Nitrate and Nitrite in Vegetables Available in Korean Diet

  • Nitrate is a naturally occurring compound in plant and can be converted to nitrite in the nitrogen cycle and vice versa. Therefore, it is easy to find nitrate in plants including vegetables. Especially, it is known that high levels of nitrate found in leafy vegetables. Nitrate itself is relatively non-toxic but its metabolites and reaction products such as nitrite, nitric oxide and N-nitroso compounds, may produce adverse health effects such as methaemoglobinaemia and carcinogenesis. To execute the risk assessment of dietary nitrate and nitrite for the intake of vegetables, it is investigated that the levels of nitrate and nitrite in 23 vegetables (798 samples). The range of concentration were 0-6,719mg/kg for nitrate and 0-1,635mg/kg for nitrite, respectively. The Estimated daily intakes (EDI) were 0.85-1.38 mg/kg body weight/day for nitrate and 0.02-0.03 mg/kg body weight/day for nitrite by ages. We conclude that there are no health concerns for eating various vegetables since the EDI were below the Tolerable Daily Intake (3.7 mg/kg body weight/day for nitrate, 0.06 mg/kg body weight/day for nitrite) level established by WHO.

Evaluation of a Dietary Assessment Method Using Photography for Portion Size Estimation (사진촬영을 이용한 1회 섭취 분량 추정의 타당도 연구)

  • This study was conducted to explore portion size estimation by comparing the measurement errors of food and macronutrient intake by photography estimation and 24-hour recall from weighing method. A total of 28 female participants aged 20-24 years old were provided a meal that included rice, chard soup with soybean paste, stir-fried squid, japchae, stir-fried boiled fish paste, lettuce geotjeori, and kimchi. Each portion was measured accurately, and food intake was measured using three dietary assessment methods. For the photography method, trained researchers estimated remaining food amount by analyzing photographs using a mean of four times. The measurement errors for the stir-fried squid and japchae were larger by the 24-hour recall method, while the error for kimchi was larger by the photography method. The correlation coefficients for each food, except chard soup with soybean paste, between the weighing method and photography method were 0.77~0.99 (p<0.001). The correlation coefficients of energy and intake of macronutrients between the weighing method and photography method were 0.85~0.86 (p<0.001). The results of this study demonstrate substantially less measurement error using the photography method than the 24-hour recall method. However, further research is needed to standardize various kinds of foods, photograph angles, serving plates, and serving style for establishment of reliable and valid portion size estimation using the photography method.

Human Multi-route Exposure Assessment of Lead and Cadmium for Korean Volunteers (일부 자원자를 통한 납, 카드뮴의 다경로 인체 노출평가)

  • Objectives: We wanted to evaluate the main route of exposure to lead and cadmium for the general population in Korea by using multi-route and multi-media exposure assessment Methods: Samples of air at the homes, samples of the food and water and peripheral blood samples were collected from thirty volunteers living in Seoul (the metropolitan area), Yong-in (the suburban area) and Ansan (the industrial area) in 2001. Graphite furnace atomic absorption spectrometric methods were used for the determination of the Pb and Cd levels in the air, food, water and blood samples. Results: The average intake of lead through the air, drinking water and food were $5.06{\mu}g/day\;(26.3%)\;0.002{\mu}g/day\;(0.1%),\;and\;16.4{\mu}g/day\;(73.6%)$, respectively. The average intake of cadmium through the air, drinking water and food were $0.082{\mu}g/day\;(0.9%),\;0.001{\mu}g/day\;(0.007%),\;and\;12.61 {\mu}g/day\;(99.0%)$, respectively. The blood lead level was statistically higher in the male subjects than in the female subjects (3.39 and $2.22{\mu}g/dl$, respectively), and only gender was a significant variable on the multiple regression analysis for blood lead. Conclusions: This study showed that the intake of lead and cadmium through food was the major route of exposure. A policy to reduce the pollutants according to the exposure routes should be established. However, more studies will be needed to support these data for the general population.

Perspectives on the systematic review for the 2020 Dietary Reference Intakes for Koreans for calcium

  • An accurate assessment of the recommended calcium (Ca) intake may contribute to reducing the risk of fractures and chronic diseases, ultimately improving quality of life. This review was performed to summarize key findings of Ca studies, investigate the effect of Ca intake on health outcomes, and determine the adequacy of evidence to revise the 2015 Dietary Reference Intakes for Koreans (KDRIs) for Ca in 2020. Databases were searched for intervention studies that assessed health outcomes by providing Ca in diets or as supplements. The framework of the systematic review comprised conducting literature searches, data extraction, quality assessment of the literature, and summarizing key findings relevant to set the Estimated Average Requirement (EAR) and Tolerable Upper Intake Level (UL) for Ca for the 2020 KDRI. The final search was performed in June 2019. A total of 13,309 studies were identified through databases and manual search. Sixtyfive studies were included in the final quality assessment and were summarized according to health indicators. As bone health was used as an indicator of the EAR for Ca, literature reports on bone health were further categorized by the life-cycle stage of the participants. This systematic review did not find new evidence that could be applied to the general Korean adult population, including postmenopausal women, for defining a new EAR for Ca in the 2020 KDRIs. Evidence in most of the reviewed literature was considered weak; however, some evidence was found that could improve the criteria on how the EAR for Ca was determined in children and adolescents. A review of the literature for the 2020 KDRIs for Ca did not find strong evidence in order to change the recommended values of the 2015 KDRIs. More clinical interventions are required among Koreans to strengthen the body of evidence to warrant the revision of the KDRIs.

A Study on Relation among Habitual Isoflavone Intake, Blood Pressure, and Serum Lipid Parameters in Korean Men and Women over 20 Years Old (20세 이상 남녀의 일상 식이중 이소플라본 섭취와 혈압, 혈중 지질과의 관련성)

  • There is some evidence that soy isoflavone has beneficial effects on the concentration of blood lipids. In this study, we investigated habitual isoflavone intake levels and the relation among dietary isoflavone intake, blood pressure, and blood lipids of adult men (n = 149) and women (n = 205). Anthropometric measurements including blood pressure, dietary in-take assessment using 24-hour recall method, and biochemical assessment using blood were conducted. The average age, height, weight, and BMI were 54.7 years, 168.5 cm, 67.3 kg, and 24.5 $kg/m^2$ for men and 53.9 years, 153.8 cm, 59.2 kg, and 25.0 $kg/m^2$ for women, respectively. The mean daily intakes of total food and energy were 1219.1 g and 1740.9 kcal for men and 1071.3 g and 1432.6 kcal for women, respectively. The mean daily isoflavones (daidzein + genistein) intake of men and women were 20.0 mg and 14.2 mg, respectively. Blood pressure of the subjects was 128.3/75.5 mmHg for men and 124.1/73.7 mmHg for women. Serum lipids of men and women were 180.2 and 184.9 mg/dL for total cholesterol, 160.8 and 137.6 mg/dL for triglyceride, 41.5 and 44.7 mg/dL for HDL-cholesterol, 106.6 and 112.7 mg/dL for LDL-cholesterol, and 3.5 and 3.3 for atherogenic index, respectively. Serum triglyceride and atherogenic index of men were significantly higher than those of women. In men, isoflavone intake and the level of total serum cholesterol were negatively (p < 0.05) correlated, after adjusted age. In women, there were significant correlations between isoflavone intake and blood pressure (systolic and diastolic), after adjusted age (p < 0.05). Based on these results, we concluded that higher isoflavone intake seemed to be related to a better lipid profile in men and lower blood pressure in women. But more epidemiological studies and controlled clinical trials would help to confirm the optimal amount required for the prevention and treatment of cardiovascular disease.

Study on Energy and Nutrient Intake and Food Preference of the Elderly in Care Facilities (요양시설 거주 노인의 에너지와 영양소 섭취 실태 및 식품선호도 조사)

  • Objectives: The purpose of this study was to assess energy and nutritional intake and investigate the preference for food and cooking methods of the residents in elderly care facilities. Methods: Data were collected from 72 residents (10 males and 62 females) aged ${\geq}70$ years in elderly care facilities using questionnaires, food photographs for estimating dietary intake and records for daily physical activity. Results: Average age of the study participants was 85.0 years and 41, 36 and 8 had dementia, hypertension and diabetes mellitus, respectively. 15%, 65% and 19% of subjects were physically mobile, enervated, and immobile, respectively. Daily energy intake was 1360.2 kcal in men and 1378.0 kcal in women, which were 68.0% and 86.1% of the estimated energy requirement (EER) of dietary reference intake for Koreans (KDRI) for ${\geq}75$ year old individuals, respectively. Estimated energy expenditure (EEE) of subjects calculated using formula from KDRI was 1361.9 kcal and EER calculated using estimated daily physical activity (EDPA) was 1232.9 kcal. Energy intake and EEE from KDRI were higher than EER from EDPA. Dietary intake of dietary fiber, calcium, potassium, zinc, vitamin $B_2$, niacin, vitamin C were lower, and protein, phosphorous, iron, sodium, vitamin A, vitamin $B_1$, vitamin $B_6$, vitamin E were higher than the corresponding ones of KDRI. Subjects liked meats, fishes and shellfish, and fruits, while subjects disliked milk, seaweeds and salted fish and salted vegetables. Cooked rice, soybean paste soup, beef, cooked sliced radish strip, and yogurt were favorite foods, with steam being a favorite cooking method. Subjects considered nutrition as the most important factor for improving food service quality. Conclusions: Results of this study could be utilized for improving food-service for the residents in elderly care facilities, and provide a basis for setting reference intake of energy and nutrients of the elderly having very low activity levels.

Food Ingestion Factors of the Korean Exposure Factors Handbook

  • The purpose of this study was to establish food ingestion factors needed to assess exposure to contaminants through food ingestion. The study reclassified the raw data of the Korean National Health and Nutrition Examination Survey in 2001 into 12 subcategories including grain products, meat products, fish and shellfish, and vegetables for international comparability of exposure evaluation. The criteria for food intake calculation were unified according to the characteristics of food groups, and recommended values for food ingestion factors were calculated through moisture correction and recategorization of cooked, processed, and mixed foods for each group. The average intake rate for grain and grain products was 6.25 g/kg-d per capita and the men's intake rate was approximately 8% higher than that of the women. The average intake rate of meat and meat products was 1.62 g/kg-d per capita and the men's intake rate was 30% higher than that of the women, on average. The average intake rate of fish and shellfish was 1.53 g/kg-d per capita, and the age groups of 1 to 2 and 3 to 6 recorded higher capita intake rates than other age groups, 2.62 g/kg-d and 2.25 g/kg-d, respectively. The average intake rate of vegetables was 6.47 g/kg-d per capita, with the age group of 1 to 2 recording the highest per capita intake rate of 9.79 g/kg-d and that of 13 to 19 recording the lowest mean. The study also offers recommended values for food ingestion factors of other food groups by gender, age, and region. The food ingestion exposure factors will need future updates in consideration of ongoing changes in food consumption behavior.

Nutritional Assessment of the Continuous Ambulatory Peritoneal Dialysis Patients (지속성 외래 복막투석환자의 영양상태에 관한 연구)

  • This study investigated the nutritional status of 32 CAPD patients. Their weight, height, triceps, mid arm circumference were measured and their dietary intake and the blood profiles were evaluated. They were 56.0${\pm}$11.8 years old. Their intake of energy and protein were 78% and 93% of RDA respectively. The energy from dextrose was 376.1${\pm}$83.2kcal. The men's and women's intake of protein was 84.6% and 102.1% of RDA respectively, According to the distribution of BMI, 23.5% of the men and 6.1% of the women were underweight and 5.9% of the men and 20% of the women were overweight. The serum albumin levels of the men and women were 3.30 and 3.71g/41 respectively. However, the average amount of serum ferritin, as a whole, comes within the normal range, which shows that stored iron was not decreased and that their anemia was not caused by iron deficiency. The subjects were divided into three groups according to the level of albumin, and their intakes of nutrients were compared with one another, The group with the high level of albumin showed that energy and protein intake was significantly larger and that BMI was also significantly higher than the other groups. There was a positive correlation between BMI and energy intake. Serum total protein had a positive correlation to energy intake ; hematocrit, to carbohydrate intake. BMI had a positive correlation to energy intake. A relative magnitude of factors affecting albumin level was analyzed by Stepwise multiple regression analysis. Overall results about relative influence of independent variables to dependent variable(albumin) indicated that the blood total protein(p <0.0001) was the most significantly correlated with serum albumin level in all subjects,1311owe4 by creatinine and total cholesterol.
