지방의료원의 연구기록관리: 인천광역시의료원 임상연구윤리위원회(IRB) 사례 (Research Records Management in Regional Public Hospital: Focusing on the Cases of Institutional Review Board (IRB) of Incheon Medical Center)

  • 공공기관이자 의료기관인 인천광역시의료원의 특성을 반영하는 부서인 인천광역시의료원 임상연구윤리심의위원회(이하 IRB)는 2013년 설치되었으며, 의료원에서 시행된 연구의 윤리성을 심의·검증함으로써 연구자와 연구대상자를 보호하고 지역책임의료기관 역할 수행에 기여하고 있다. IRB는 지금까지 총 80여 건의 인간대상연구 및 인체유래물연구 계획을 검토하였으며, 현재 의료원장 직속의 독립된 처리과로 존재하고 있다. 본 고에서는 IRB 관련 기록물의 등록 및 보존, 기록관리기준표 및 관련 규정, 문서고의 정비에 관하여 설명하고, 기록물 공개와 활용, 분류에 있어 한계 및 개선 사항을 제시하고자 한다.

여성의 건강을 위한 간호전달체계 모형개발 - 조산원 중심으로 - (Model Development of Nursing Care System for Women's Health : Based on Nurse-Midwifery Clinic)

  • The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.

공공부문 분만개조 사업 : 평가 및 발전방향 (Maternal Child Health : Toward Better Performance)

  • Health of a nation is quite often represented by the statistics such as infant death rate and maternal mortality rate. It is indisputable that maternal child health(MCH) is the basis of health of a nation. MCH is also one of the cardinal component of primary health care. The importance of MCH is conspicuous especially in the developing countries. In Korea, People in the rural communities still have high access barrier to basic health care needs, including MCH services. Access to quality care during pregnancy and delivery seems to be the crucial factor in preventing deaths in women and children. The beneficial effects of prenatal and postnatal care on the outcome of pregnancy for mother and child, and those of health professional-attended institutional delivery on the health of mother and child have been well documented in many studies. Recognizing these effects, the government of Korea received IBRD loan of $30 million in 1979 for th purpose of constructing 89 rural MCH centers. The construction is complete now and all 89 MCH centers are under operation ti imporve primary health care for mothers and children in Korea. However, it has been observed over time that overall performance of public MCH centers is declining. The decline has been attributed partly to low quality services by public MCH centers, poor management by health center mangers, competition with for-profit private clinics, and to the development of national health insurance. This study investigates the utilization by rural communities in Korea of MCH services provided by public sector health centers deemed to be physically and financially accessible to the community but suboptimally used. It seeks also to determine the factors that influence people's utilizations. This study sets out to discover a desirable form of MCH center from among alternative forms of centers, thereby to construct a MCH model.

정신장애인 가족의 보호부담과 삶의 질과의 관계에서 가족탄력성과 사회적 지지의 조절효과 (Moderating Effect of Family Resilience and Social Support on Relationship between Burden of Care for Families with Mentally Disabled and Quality of Life)

  • 본 연구는 정신장애인 가족의 부담감이 삶의 질에 미치는 영향과 가족탄력성과 사회적 지지가 조절효과가 있는지 검증하는데 목적이 있다. 이를 위해 본 연구는 서울과 경기, 인천 지역의 사회복귀시설과 정신건강증진센터에 등록된 정신장애인 가족 102명을 분석에 사용하였다. 연구결과, 첫째, 정신장애인의 가족이 그들 자녀를 돌보면서 경험하는 보호부담은 삶의 질에 부정적 영향을 미치는 것으로 나타났다, 둘째, 가족 탄력성은 정신장애인 가족의 보호부담과 삶의 질 간 관계에 있어 조절효과가 있는 것으로 조사되었으며, 하위요인 모두에서 완충효과가 있는 것으로 제시되었다. 끝으로 사회적 지지는 보호부담과 삶의 질 관계에 있어 조절효과가 있음이 입증되었다. 이를 근거로 가족탄력성 증진을 위한 정보제공의 필요성을 인식하면서, 정신장애인 가족의 사회적 지지망 강화 노력과 이들의 경제적 부담을 완화할 수 있는 제도적 차원의 지원이 필요하다는 것을 제안하였다.

장기요양기관 노인들의 우울증에 영향을 미치는 요인에 관한 연구 (Research on Impacts of Depression Among the Aged in the Long-Term Care)

  • 본 논문은 노인 장기 요양기관에서 노인들의 우울증에 영향을 미치는 요인에 관한 연구로서 서울시에 소재하며 노인 장기요인시설에 수용되어 있는 360여 명의 노인을 대상으로 설문조사를 실시하였다. 한국노인복지학회의 통계에 의하면 65세 이상 노인 우울증환자의 수는 13%에 이른다고 보고한 바 있다. 여기에 사회적 지지가 이러한 정서적 불안을 해소하는데 주요한 관건으로 보고 있다. 그 결과 인지된 사회적 지지와 우울증과의 관계가 강한 정을 나타내고, 그 다음으로 건강상태와 우울증이정(+)의 관계였다. 그리고 높은 건강상태는 낮은 정도의 우울증을 유발하고 네트워크의 크기나 접근의 빈도는 노인우울증과는 관련이 없는 것으로 나타났다. 이러한 결과는 노인 장기요양기관에 종사하는 사회복지사에게 사회사업실천에 있어 사회적 지지 프로그램을 어떻게 실천하여야 하는가를 암시하고 있다.

한국 전문간호사제도의 현황과 활성화 전략 (Current Status and Vitalizing Strategies of Advanced Practice Nurses in Korea)

  • Purpose: This study aimed to provide information on the developmental process and current status of Advanced Practice Nurses (APN) and to suggest ways for activating APN roles in Korea. Methods: We reviewed literatures to explore domestic and international trends related to the roles, practice settings, scope of practice, regulatory policies, and political environment of APN. Results: Interest in the APN role has been gaining ground worldwide. While the roles of APN in the USA, Taiwan and Japan have been changed in terms of legislation and institutional regulations, the role of APN in 12 fields except for APN in home care has not been legislated since qualification was specified in Korean Medical Law. Many problems were found in vitalizing APN roles: absence of legal regulations on the scope of practice or health insurance coverage for APN, increase the number of medical support staffs who are not qualified, and a disagreement regarding the qualification field and practical field. Conclusion: This study presents snapshots and vitalizing strategies of practice, and regulation for APN to develop their unique roles in Korea. The tasks and duties of APN should be identified specifically to fulfill their roles. In terms of national health insurance policy APN's Nursing care should be reimbursed just as nursing care of APN in home care.

노인장기요양기관에 근무하는 간호사의 근무실태 및 직무만족도 영향 요인 (A Study on Working Conditions and Factors Related to Job Satisfaction of Nurses in the Long-term Care facilities for the Elderly)

  • Purpose: Th study is to explore the working conditions and factors related to job satisfaction of nurses in long-term care facilities for the elderly. Methods: A total of 127 nurses working long-term care facilities, from June to November 2014, completed questionnaires. Face to face interviews were conducted. The questionnaire consists of a total of 32 questions, including 5 questions related to general characteristics, 15 questions related to working conditions, and 12 questions related to job satisfaction(likert scale of 1 to 5). Results: The majority of nurses were employed in institutional care facilities, in urban area, and in privately owned facilities. On average, employed nurses worked 170.7 hours per month. Only 40.9% of them got more than two million won for their monthly salary. They were satisfied with the nature and worth of their work and the workplace relationship, but not with their wages and promotion opportunity 40.2% of nurses reported satisfaction with job. In multiple logistic regression analysis, age was a statistically significant influencing factor on general job satisfaction. Conclusion: The findings suggest that policies must be to improve nurses' working conditions, for wages, and to support them with education and training for skills development.

방문구강건강관리사업에서 정신적 장애인의 구강건강관리시간에 영향을 미치는 요인 (A STUDY ON FACTORS AFFECTING DENTAL TREATMENT TIME OF PATIENTS WITH MENTAL DISABILITY IN VISITING DENTAL SERVICES)

  • Patients with mental disability who are classified as dental severely disabled, have poor oral health status and many difficulties in taking health services. The purpose of this study was to investigate factors of influencing in visiting oral health care services. After receiving approval from institutional review board in Seoul National University, 39 participants were recruited and the oral health examination and questionnaire were taken. Data were analyzed using one-way ANOVA, independent t-test, hierarchical linear regression for predicting influence of each participants' characteristics on oral examination time and care time. In regression model, the higher the disability grade and the lower the degree of cooperation, the more oral examination time increased. However possibility of communication was not significantly influenced. Also, the higher the disability grade and the lower the degree of cooperation, the more oral care time increased. However possibility of communication was not significantly influenced. These results are expected to provide objective data for introduction and establishment of visiting dental care.

방문요양서비스 기관 평가의 효과성 : 이용자 관점에서 (Effectiveness of Evaluation for Visiting Care Service Institution: From the User's Point of View)

  • 본 연구의 목적은 이용자의 관점에서 노인장기요양보험 방문요양서비스 기관 평가의 효과성을 확인하는 것이다. 이를 위해, 전라북도 내 지역별(14개 시·군) 할당표집을 통해 수집한 이용자(266명) 설문자료와 기관(47개) 자료를 결합하여 다층모형으로 분석하였다. 주요 연구결과는 다음과 같다. 첫째, 5개 평가영역 중 이용자에게 권리의식을 갖게하고, 이용자를 존중하는지에 대한 '권리·책임' 영역의 점수가 높을수록 서비스 품질과 만족도도 높게 나타났다. 둘째, 5개 평가영역 중 '권리·책임' 영역과 '급여제공결과' 영역이 충성도에 영향을 미치는 것으로 나타났다. 셋째, '기관운영', '환경·안전', '급여제공과정' 영역은 서비스 품질, 만족도, 충성도에 영향을 미치지 않는 것으로 나타났다. 이러한 결과를 바탕으로 형식적이고 정량적인 평가지표보다는 이용자가 인식할 수 있고, 방문요양서비스의 기본 목적에 충실한 내용으로 평가지표를 재구성할 것 등을 제안하였다.

북한의 보육 정책 및 현황 (Current Situation and Policies of Early Childhood Care and Education in North Korea)

  • In order to resolve the decline in population due to low birthrates, the South Korean government is expanding its free child care policies with an increased budget. In anticipating the effects and problems of our system, it will be worthy of attention to refer to the child care systems of other countries. In this paper, we reviewed the past and present policies and the current situation of the child care system in North Korea. North Korea started its free child care system earlier than that of South Korea, for the purpose of utilizing the women's labor force and rearing children to be revolutionary men of Juche type (Kimilsungism), in order to construct a communistic society. 'Child Care Education Law', which is the legal foundation of the child care system, regulates institutions for nursery schools and kindergarten and informs people that the country is responsible for support of child care. Despite their interest and progress in both quantity and quality in the child care system until the 1980s, the free child care system was partially disrupted, and discrepancies between ideology and actual situation were revealed due to economic difficulties from the 1990s. Because people's survival and physical health have been threatened, it is barely possible to find any study investigating the effect of institutional child care from early childhood and the instillation of unique ideology by group education from the preschool period on mental health.
