여군간부 자녀 보육제도 개선방안에 관한 연구 (A Study on the Improvement of Child Care System for a woman soldier)

  • 오늘날 여군들은 군 특성상 복무기간 중, 훈련, 당직근무, 잦은 야근과 이사 등의 직업적 환경과 가족구성에서 발생하는 자녀 양육문제 등 수많은 고충을 감내하며, 극심한 스트레스를 호소하고 있다. 또한 여군들은 군대의 고유한 임무와 끊임없는 훈련의 반복 및 잦은 야간근무로 인한 고충은 지속 심화되어가고 있는 현상이다. 이러한 이유에서 자녀 보육을 담당해야 할 여군의 막중한 책임과 스트레스는 극에 달하고 있다. 따라서 본 연구는 육아기간 여군의 복무여건을 보장하고, 양육문제를 개선하기 위한 제도적 방안을 연구할 필요성이 있다.

가정외보호청소년의 사회적자본이 학교적응을 매개로 자립의지에 미치는 영향 (The relationship between social capital of adolescents in out-of-home care and their self-reliance: Focusing on the mediating effects of school adjustment)

  • 본 연구의 목적은 가정외보호청소년(양육시설, 그룹홈, 가정위탁)의 사회적자본이 그들의 자립의지에 어떠한 영향을 미치는지를 살펴보고, 이 경로에서 학교적응의 매개효과를 검증하는데 있다. 본 연구를 위해 전국의 양육 시설, 그룹홈, 가정위탁 청소년들 가운데 중 고등학교에 재학 중인 만 13세 이상 만 19세 이하의 청소년을 연구 대상으로 하여 표집 하였고, 총 358명의 가정외보호청소년이 연구에 참여하였다. 주요 통계 방법으로는 구조방정식 모형 분석을 활용하였으며, 매개효과 검증을 위해 Bootstrapping 방법을 사용하였다. 구조방정식 모형 분석 결과, 가정외보호청소년의 사회적자본이 자립의지에 직접적인 영향을 미치지 않았으나 학교적응을 통해 간접적으로 자립의지에 영향을 미치는 것으로 드러났다. 이러한 연구 결과를 바탕으로 가정외 보호청소년의 사회적자본과 학교적응, 그리고 자립의지를 높일 수 있는 정책적 실천적 함의를 논하였다.

한국과 일본의 노인 대상 지역사회 재활서비스 비교 연구: 노인장기요양보험 제도를 중심으로 (Comparison of Community Rehabilitation Services for the Elderly in South Korea and Japan: Focusing on the Long-Term Care Insurance System)

  • Background: As South Korea enters an aged society, the government has emphasized the need for a soft landing of the older adults into the community after the acute and recovery periods under a national policy of "community care." However, the institutionalization of community rehabilitation services to implement this is insufficient. Japan had already entered an aged society when the Long-Term Care Insurance System was introduced in 2000. Thus, the case of Japan's institutionalization of the system is expected to have implications for us in supplementing a suitable system for the aged society. Objects: This study compared the institutionalization process of the Long-Term Care Insurance System in South Korea and Japan and the services currently being implemented in each country. Methods: To examine the institutionalization process and services of the system, related legal rules and regulations, government reports, and articles were reviewed. To examine the operation status of the system, statistical data provided by each country's government were analyzed. Results: Japan recognized the importance of community rehabilitation even before the enactment of Long-Term Care Insurance. Thus, community rehabilitation services, such as home-visit rehabilitation and health facilities, were already stipulated in the law. Under such institutional legacy, Long-Term Care Insurance was able to establish a service system, which balanced welfare and health-related services, including various types of services with enhanced rehabilitation functions. In South Korea, rehabilitation policies were not much considered in the process of institutionalizing the system; thus, it was composed mainly of services focusing on care and recuperation. Conclusion: In order to realize community care, rehabilitation services need to be developed in Long-Term Care Insurance System in various forms such as home-visit services, daily services, short stay, and facility services.

Nitrous oxide splurge in a tertiary health care center and its environmental impact: No more laughing stock

  • Background: Nitrous oxide has been an integral part of surgical anesthesia for many years in the developed world and is still used in developing countries such as India. The other main concerns in low-resource countries are the lack of an advanced anesthesia gas-scavenging system and modular surgical theatres. As a greenhouse gas that has been present in the atmosphere for more than 100 years and damages the ozone layer, nitrous oxide is three times worse than sevoflurane. Here, we conducted an observational study to quantify the annual nitrous oxide consumption and its environmental impact in terms of carbon dioxide equivalence in one of busiest tertiary health care and research centers in Northern India. Methods: Data related to nitrous oxide expenditure' from the operation theatre and manifold complex of our tertiary care hospital and research center from 2018 to 2021 were collected monthly and analyzed. The outcomes were extracted from our observational study, which was approved by our institutional ethics board (INT/IEC/2017/1372 Dated 25.11.2017) and registered prospectively under the Central Registry (CTRI/2018/07/014745 Dated 05.07.2018). Results: The annual nitrous oxide consumption in our tertiary care hospital was 22,081.00, 22,904.00, 17,456.00, and 18,392.00 m3 (cubic meters) in 2018, 2019, 2020, and 2021, respectively. This indicates that the environmental impact of nitrous oxide (in terms of CO2 equivalents) from our hospital in 2018, 2019, 2020, and 2021 was 13,016.64, 13,287.82, 10,289.94, and 10,841.24 tons, respectively. Conclusion: This huge amount of nitrous oxide splurge is no longer a matter of laughter, and serious efforts should be made at every central and peripheral health center level to reduce it.

21세기 신생아 전문 간호사의 역할과 전망 (The role of the Neonatal Nurse Specialist in 21st Century)

  • The role of the neonatal nurse specialist has been well established over the past decade and now reform in 21st century. Neonatal nurse specialists responsibilities in caring for critically and long-term chronically ill infants and their families are very important. Neonatal nurse specialists have a two fold responsibility in caring for these infants. First, through acquiring advanced practice education in complex neonatal care and diagnostic skills, neonatal nurse specialists meet the physiologic needs of the infant. Second, neonatal nurse specialists provide a more holistic approach to their care through evaluating the family in treatment plans and involving the family in discharge planning for the infant. In some institutions, neonatal nurse specialists are directly involved in institutional and/or home follow-up care and case management also. It is the neonatal nurse specialists responsibility to function collaboratively with the multidisciplinary team in managing critically or chronically ill infants from admission to discharge. The role of the neonatal nurse specialist case manager can be described as one that focuses on individualized care of the infant, while providing continuity of care to both the infant and family. The neonatal nurse specialist's role will vary depending on the neonatal intensive care unit(NICU). Therefore, the multidisciplinary collaborative approach to long-term management of infants in the NICU is extremely important to provide successful transition to home or to long-term rehabilitative care facilities because care for the chronically ill infant is complex and multifaceted. I suggest the role of neonatal nurse specialist in 21st century are as follows. 1. Diagnostic/patient assessment 2. Management of patient health/illness 3. Administering/monitoring therapeutic interventions and regimens 4. Monitoring/ensuring quality of health care practices 5. Organization and work role 6. Helping role 7. Teaching/coaching role 8. Management of rapidly changing situations 9. Consulting role The advanced practice nursing model of care delivered by neonatal nurse specialist's in the NICU incorporates medical and nursing role functions and emphasizes holism, caring, and a health perspective for critically and chronically ill neonates and their families.

방문간호사의 생애말기 간호에 대한 경험: 질적 내용분석 (Experience of Visiting Nurse's End-of-life Care: A Qualitative Contents Analysis)

  • Purpose: This study aims to describe and explore the experiences of visiting nurses so as to providing high-quality end-of-life care at home. Methods: Data was collected and recorded through in-depth interviews with 11 visiting nurses and transcribed verbatim. The data were analyzed with qualitative contents analysis. Data were collected from April 8 to July 30, 2021. Results: The results revealed the following four themes and ten sub-themes: The main themes were 'Dignified end-of-life caring practice at home', 'Providing integrated support of end-of-life family caregivers', 'Confronting the limits of visiting nursing at the end of life' and 'Reflection on life and feeling rewarded'. Visiting nurses have performed dignified end-of-life care at home and integrated support for family caregivers. Although the visiting nurse sometimes faced the limits of visiting nursing during end-of-life care, it was surveyed that they rewarded on their lives and felt rewarding through the end-of-life nursing experience. Conclusion: Practical and systematic training is needed to positively change the end-of-life care of visiting nurses. We propose programs, such as expert counseling, to improve coping skills in end-of-life care practice at home. Additionally, we propose various institutional supports which can support the limitations of end-of-life care provision.

Evaluation of Managerial Needs for Palliative Care Centers: Perspectives of Medical Directors

  • Background: Palliative therapies have an important role in increasing the quality of healthcare and in dealing with physical and psychosocial problems due to cancer. We here aimed to evaluate the managerial perspectives and opinions of the hospital managers and clinical directors about specialized palliative care centers. Materials and Methods: This study was conducted in two large-scale hospitals in which oncology care is given with medical directors (n:70). A questionnaire developed by the researchers asking about demographic characteristics and professional experience, opinions and suggestions of medical directors about providing and integrating palliative care into healthcare was used and responses were analyzed. Results: Potential barriers in providing palliative care (PC) and integrating PC into health systems were perceived as institutional by most of the doctors (97%) and nurses (96%). Social barriers were reported by 54% of doctors and 82% of nurses. Barriers due to interest and knowledge of health professionals about PC were reported by 76% of doctors and 75% of nurses. Among encouragement ideas to provide PC were dealing with staff educational needs (72%), improved working conditions (77%) and establishing a special PC unit (49)%. An independent PC unit was suggested by 27.7% of participants and there was no difference between the hospitals. To overcome the barriers for integration of PC into health systems, providing education for health professionals and patient relatives, raising awareness in society, financial arrangements and providing infrastructure were suggested. The necessity for planning and programming were emphasized. Conclusions: In our study, the opinions and perspectives of hospital managers and clinical directors were similar to current approaches. Managerial needs for treating cancer in efficient cancer centers, increasing the capacity of health professionals to provide care in every stage of cancer, effective education planning and patient care management were emphasized.

보육교사의 사회적 지지, 직무스트레스, 소진 간 구조관계 (Structural Relationship Between Social Support, Job Stress, and Child Care Teachers' Burnout)

  • 본 연구의 목적은 보육교사의 소진에 영향을 미치는 사회적 지지, 직무스트레스 변인 간 구조관계를 분석하는데 있다. 연구대상은 어린이집에 재직 중인 보육교사 432명으로 하였다. 수집된 자료는 SPSS 21. 0 및 AMOS 22.0 통계프로그램을 이용하여 구조방정식 모델을 설정하고 분석하였다. 본 연구의 주요 결과는 첫째, 사회적 지지는 보육교사의 직무스트레스에 직접적인 영향을 미치는 것으로 나타났다. 둘째, 사회적 지지는 보육교사의 소진에 직접 효과는 나타나지 않고 간접효과가 있는 것으로 나타났다. 셋째, 보육교사의 직무스트레스는 보육교사의 소진에 직접효과가 있는 것으로 나타났다. 이상의 분석결과를 통해 보육교사의 소진 감소 방안으로는 구성원의 사회적 지지 강화 및 정부의 제도적 지원이 필요시 된다.

노인복지시설 유형별 지역적 편차에 관한 연구 (A Study on Local Variations of Elderly Welfare Facilities by Care Type)

  • This study purposed to examine elderly welfare facilities by type, to analyze their local variations, and ultimately to contribute to the expansion of elderly welfare infrastructure. The results are expected to help inspect elderly welfare infrastructure for providing the aged with social services before the execution of 'the insurance for elderly long term care' and establish welfare facilities by area in the future. For these purposes, we used the national data "The Current State of Elderly Welfare Facilities in 2007" produced by the Ministry of Health and Welfare. We digitized elderly welfare facilities in 163 cities and counties by type and analyzed them by area. We also examined the differences in the local distribution of representative elderly welfare facilities such as elderly welfare centers, home based facilities (home helper centers), asylums for the aged and elderly care facilities in 16 cities and provinces. Furthermore, we analyzed differences and problems in their local distribution urban areas, mixed areas of urban and rural communities, and rural areas. In addition, we studied the current state of institutionalized care and home based care, which are two major directions of current elderly welfare policies, based on the local distribution of facilities and analyzed differences in the trends according to area. According to these results, the urban had more home based care facilities than the rural. However, the rural had more institutionalized care facilities than urban. Also, each local self-governing body showed unique characteristics. Therefore, these results suggest that we need to establish elderly welfare policies based on the distribution of facility types by area.

Elderly Welfare System and Role of Dental Hygienists in Korea and Japan

  • In accordance with the aged society, oral health care for the elderly is considered important to maintain general health. Although the role of dental hygienists is essential for proper health management of the oral cavity, research on the care system for the elderly people's oral health and on the role of dental hygienists in the field of elderly welfare is still insufficient. Hence, the aim of this study is to investigate the status of Korean elderly welfare system and dental hygienists by comparing them with those in Japan, a precedent of aged society. First, we compared and investigated the Japanese long-term insurance system, which provides an institutional basis for a long-term care system for the elderly in Korea. Second, the elderly welfare law and care system, focusing on oral care, were examined. Lastly, in elderly care, we analyzed the distinctions between Korea and Japan regarding dental hygienists' role and scope of work. Taken together, as a precedent of aged society, Japan has shown well-specialized and systematic welfare for the elderly compared with Korea. With the development of the welfare system for the elderly in Japan, the role and the work scope of dental hygienists have been expanded to improve quality of life of elderly people, as a key professional for elderly oral care. Therefore, we should perceive the need for improvement of long-term care insurance and the expansion of dental hygienists' work in Korea. In conclusion, these results could be used as basic data for improving the elderly welfare system and developing dental hygienists in Korea.
