  • Objectives : The aim of this study was to evaluate the effects of EFT-I(EFT program for insomnia) for insomnia in the elderly as a preliminary study. Methods : This study was a single group pre-post comparative study that involved 10 elderly women(mean age=$76.3{\pm}4.29$), who visited a senior welfare center, complained of insomnia symptoms. Subjects received 8 sessions(twice a week for 4 weeks and 1 hour for each session) of EFT-I group training. Insomnia severity, depression, state-anxiety, and life satisfaction of each subject were evaluated twice at pre and post of EFT-I. Korean Sleep Scale, Short form of Geriatric Depression Scale(SGDS), State-Trait Anxiety Inventory(STAI), and life satisfaction scale were used as evaluation tools. The data were analyzed using paired-samples T-test. Results : Insomnia severity, life satisfaction, depression, and state-anxiety were significantly improved by EFT-I. Conclusions : Result of this study showed that EFT-I can be a useful treatment program for elderly insomnia. Larger clinical trials are needed to verify effect of EFT-I as a community based insomnia management program for the elderly.

  • Medication alone is not sufficient to treat insomnia. In addition, the side effects of sleep medications themselves cannot be ignored during treatment. Insomnia begins with poor sleep quality and discomfort, but as it continues, patients fall into a vicious circle of insomnia with negative thoughts and dysfunctional and distorted perceptions related to sleep. Mindfulness-based intervention for insomnia corrects these sequential cognitive and behavioral processes. The mindfulness technique basically recognizes all the thoughts, feelings, and experiences that occur to us as they are, nonjudgmentally, and then trains them to return to the senses of our body. In this way, while noticing all the processes of the sequential vicious cycle and training them to return to our bodies (e.g., breathing), mindfulness determines whether we are really sleepy or just fatigued. This mindfulness-based intervention can be a useful nonpharmaceutical intervention for insomnia, and its stability and efficacy has been proven by many studies.

  • Relatively little is known about the neurobiology of insomnia, despite its wide prevalence and broad medical impact. Although much is still to be learned about the pathophysiology of the disorder, identification, systematic assessment, and appropriate treatment are clearly beneficial to patients. Recent research, using quantitative EEG, polysomnography (PSG), multiple sleep latency test (MSLT) and neuroimaging techniques, suggests that some broad areas can be identified as possible pathophysiological models. Sleep-wake homeostat model hypothesizes a failure in homeostatic regulation of sleep, an attenuated increase in sleep drive with time awake, and/or defective sensing of sleep need. Circadian clock model hypothesizes a dysfunctional circadian clock, resulting in changes in the timing of sleep-wake propensity that are incompatible with normal sleep. Intrinsic sleep-wake state mechanism model suggests that abnormal function of insomnia comprises the systems responsible for expression of the sleep states themselves. Extrinsic over-ride mechanism (stress-response) model suggests that insomnia reflects the consequences of overactivity of one of the systems considered "extrinsic" to normal sleep-wake control. Many current therapies for insomnia are based on these physiological models. Several attempts have been made to create a physiological model that would explain this disorder and could be used as a foundation for treatment. However, it appeared that no model can fully explain and clarify all aspects of insomnia. Future research should be necessary to expand our knowledge on the biological dimensions of insomnia.

  • Insomnia is a disorder of initiation and maintenance of sleep that is derived from multiple factors such as psychologic, physiological and environmental problems. A number of stroke patients suffer from insomnia classified as one of the sleep disorders associated with physical illness and on the contrary insomnia may have profound deleterious effects on the natural course of stroke. Sedative-hypnotics including benzodiazepine and non-benzodiazepine have widely been used in chronic insomniacs. However, most hypnotics cause dependence, tolerance, impaired daytime function and rebound insomnia. Therefore, we are looking forward to proposing an effective oriental treatment for insomnia. We report two cases of insomniacs, treated with Changpoulgeumtang. After the treatment, insomnia and other accompanied symptoms were improved.

  • Insomnia is defined as an experience of inadequate or poor-quality sleeping habit, coupled with more than one of the following symptoms : having difficulty sleeping, waking up too early in the morning or not having sound sleep. Insomnia is one of the most common complications that stroke patients experience, which interfere the patients' recovery. The authors observed two improved cases of insomnia in stroke patients by administrating traditional Korean medicine. The patients suffered from insomnia with nocturnal irritation, constipation, petechia of the tongue, thready or weak pulse. These symptoms, were diagnosed as blood stasis and we administered Hyulboochucke-tang three times a day, to improve blood circulation to remove blood stasis. After the administration, nocturnal irritation disappeared and the insomnia improved. Based on this experience, it is considered that enhancing blood circulation to remove blood stasis can be applied to the treatment of insomnia that stroke patients suffer.

  • Insomnia is common disease generating about 30% incidence in Korea. It describes not only a state of a lack of sleep but also a causative factor for both psychological and physical disorders. According to Oriental Medicine theory, there are many causes of insomnia, and Wibulhwa(胃不和) is one of causes. In this study, we diagnosed as Wibulhwa(胃不和) for three patients and mainly treated with Modified Yukgunja-tang. We compared with before and after Insomnia Severity Index(ISI) treatment. As a result, ISI score falled, and it showed an improvement in insomnia. So we suggest that Modified Yukgunja-tang might be usefully applied to the treatment of insomnia caused by Wibulhwa(胃不和).

  • Purpose: This study was conducted to exam the effects of the Abbreviated Cognitive Behavioral Therapy(ACBT) on chronic insomnia. Methods: Study was one-group interrupted time series study that involved 13 adults(mean age=51.7, aged 25-77 years) with chronic primary insomnia who visited sleep disorder clinic of S Hospital from November 2004 to October 2005. The subjects received 2-session individual ACBT with 2 week-interval($1^{st}$: 1.5- 2hrs, $2^{nd}$: 20-30min). To measure the subjective insomnia severity and sleep patterns, 3 times of insomnia severity index and sleep logs were completed(before ACBT, after ACBT, and 3-month after ACBT). The main outcomes were subjective insomnia severity and sleep patterns(sleep onset latency, waking after sleep onset, and total sleep time, sleep efficiency). The data were analyzed with SPSS 10.0 version program by Friedman test, Wilcoxon signed rank test with Bonferroni correction. Results: There were statistically significant decrease in insomnia severity index, sleep onset latency, and waking after sleep onset, and increase total sleep time and sleep efficiency. Conclusion: ACBT was effective in reducing subjective insomnia severity and improving sleep patterns. Sleep improvement was better sustained over time with ACBT.
