Gypenoside XVII protects against myocardial ischemia and reperfusion injury by inhibiting ER stress-induced mitochondrial injury

  • Background: Effective strategies are dramatically needed to prevent and improve the recovery from myocardial ischemia and reperfusion (I/R) injury. Direct interactions between the mitochondria and endoplasmic reticulum (ER) during heart diseases have been recently investigated. This study was designed to explore the cardioprotective effects of gypenoside XVII (GP-17) against I/R injury. The roles of ER stress, mitochondrial injury, and their crosstalk within I/R injury and in GP-17einduced cardioprotection are also explored. Methods: Cardiac contractility function was recorded in Langendorff-perfused rat hearts. The effects of GP-17 on mitochondrial function including mitochondrial permeability transition pore opening, reactive oxygen species production, and respiratory function were determined using fluorescence detection kits on mitochondria isolated from the rat hearts. H9c2 cardiomyocytes were used to explore the effects of GP-17 on hypoxia/reoxygenation. Results: We found that GP-17 inhibits myocardial apoptosis, reduces cardiac dysfunction, and improves contractile recovery in rat hearts. Our results also demonstrate that apoptosis induced by I/R is predominantly mediated by ER stress and associated with mitochondrial injury. Moreover, the cardioprotective effects of GP-17 are controlled by the PI3K/AKT and P38 signaling pathways. Conclusion: GP-17 inhibits I/R-induced mitochondrial injury by delaying the onset of ER stress through the PI3K/AKT and P38 signaling pathways.

Optimal Ratio of Wnt3a Expression in Human Mesenchymal Stem Cells Promotes Axonal Regeneration in Spinal Cord Injured Rat Model

  • Objective : Through our previous clinical trials, the demonstrated therapeutic effects of MSC in chronic spinal cord injury (SCI) were found to be not sufficient. Therefore, the need to develop stem cell agent with enhanced efficacy is increased. We transplanted enhanced Wnt3-asecreting human mesenchymal stem cells (hMSC) into injured spines at 6 weeks after SCI to improve axonal regeneration in a rat model of chronic SCI. We hypothesized that enhanced Wnt3a protein expression could augment neuro-regeneration after SCI. Methods : Thirty-six Sprague-Dawley rats were injured using an Infinite Horizon (IH) impactor at the T9-10 vertebrae and separated into five groups : 1) phosphate-buffered saline injection (injury only group, n=7); 2) hMSC transplantation (MSC, n=7); 3) hMSC transfected with pLenti vector (without Wnt3a gene) transplantation (pLenti-MSC, n=7); 4) hMSC transfected with Wnt3a gene transplantation (Wnt3a-MSC, n=7); and 5) hMSC transfected with enhanced Wnt3a gene (1.7 fold Wnt3a mRNA expression) transplantation (1.7 Wnt3a-MSC, n=8). Six weeks after SCI, each 5×105 cells/15 µL at 2 points were injected using stereotactic and microsyringe pump. To evaluate functional recovery from SCI, rats underwent Basso-Beattie-Bresnahan (BBB) locomotor test on the first, second, and third days post-injury and then weekly for 14 weeks. Axonal regeneration was assessed using growth-associated protein 43 (GAP43), microtubule-associated protein 2 (MAP2), and neurofilament (NF) immunostaining. Results : Fourteen weeks after injury (8 weeks after transplantation), BBB score of the 1.7 Wnt3a-MSC group (15.0±0.28) was significantly higher than that of the injury only (10.0±0.48), MSC (12.57±0.48), pLenti-MSC (12.42±0.48), and Wnt3a-MSC (13.71±0.61) groups (p<0.05). Immunostaining revealed increased expression of axonal regeneration markers GAP43, MAP2, and NF in the Wnt3a-MSC and 1.7 Wnt3a-MSC groups. Conclusion : Our results showed that enhanced gene expression of Wnt3a in hMSC can potentiate axonal regeneration and improve functional recovery in a rat model of chronic SCI.

Prognostic Factors of Neurocognitive and Functional Outcomes in Junior and Senior Elderly Patients with Traumatic Brain Injury Undergoing Disability Evaluation or Appointed Disability Evaluation

  • Objective : This study explored the relationships among demographic (DVs) and clinical variables (CVs), neurocognitive (NOs) and functional outcome (FO) that could be used as prognostic factors for old aged patients with traumatic brain injury (TBI) undergoing or appointed disability evaluation (DE) after treatment. Methods : A total of 162 subjects with TBI above the age of 55 years undergoing DE or appointed to do so after treatments were selected. The patients were divided into two subgroups according to age : a junior elderly group 55 to 64 years old and a senior elderly group over the age of 65. NOs and FO were evaluated using the Seoul Neuropsychological Screening Battery and Clinical Dementia Rating scale. Results : Gender, age, and education level were shown to significantly impact the recovery of NOs after TBI. Other DVs and CVs such as area of residency, occupation, type of injury, or loss of consciousness were not found to significantly affect the recovery of NOs after TBI. Analysis of the relationships among DVs, CVs and NOs demonstrated that gender, age, and education level contributed to the variance of NOs. In FO, loss of consciousness (LOC) was included to prognostic factor. Conclusion : Gender, age and education level significantly influence the NOs of elderly patients with TBI. LOC may also serve as a meaningful prognostic factor in FO. Unlike younger adult patients with TBI, old aged patients with TBI did not show global faking-bad or malingering attitudes to DE for compensation, but assume that they could faking their performance in a test set available visual feedback.

  • Objectives : Applying herbal-acupuncture using Ursi Fet into Zusanli (UZ) on to the atopic dermatitis (AD) in mice to study changes in external dermal formation, change of leukocytes in vasculature, change of lipid formation in stratum corneum and distribution of ceramide. This study was done through forcing an injury to the mice's back skin which damages the lipid protection formation in the stratum corneum. Methods : The AD which was caused intentionally using the external application on the mice's back skin was treated with VB; the change of leukocytes in the vasculature was identified through optima 5.2 and Student's t-test and the results were made into a dermal formation graph. Results : After dispensing UZ into the AD, the dermal injury decreased. The recovery of the lipid protection formation which includes lipid and ceramide in the stratum comeum (for suppressing acute inflammation due to factors such as PKC, $TNF-\alpha,\;IL-1\beta$, which controlled the secretion of the relating inflammatory cytokine) also went on to show a decrease of both angiogenesis and degranulated mast cells. In addition, the decrease of epithelial injury also caused the growth of cells to decrease in the stratum basale and cytoclasis. In the vasculature, the leukocytes were also decreased na this could relate to a decrease in AD. Conclusions : UZ has an effect on AD by suppressing dermal injury through the recovery of the lipid protection formation in the stratum corneum.

Evaluation of the Combination of Methylprednisolone and Tranilast after Spinal Cord Injury in Rat Models

  • Objective : The aim of our study was to evaluate the neuroprotective functions of the combination therapy using methylprednisolone (MP) and tranilast (TR) after spinal cord injury (SCI) in adult rats. Methods : Spinal cord compression injury model was achieved using Yasargil aneurysm clip. Rats were divided into control group, MP group, TR group, and combination therapy group using TR and MP. Rat models were assessed for locomotor functional recovery using Basso, Beattie, and Bresnahan (BBB) score, spinal cord water content and myeloperoxidase (MPO) activity 24 hours post SCI, haematoxylin and eosin staining and glial fibrillary acid protein (GFAP) staining at 7 and 14 days post SCI. Results : The spinal cord water content and MPO activity in the combination therapy group was significantly lower than the control group and the individual therapy groups p<0.05. The combination therapy group had significantly higher BBB scores than control group and individual therapy groups (p<0.05). At one week after SCI, GFAP expression in the combination group was significantly lower than the control group (p<0.05) but there was no significant difference compared to the individual therapy groups (p>0.05). At 2 weeks after SCI there was a slight decrease in GFAP expression compared to the first week but the difference was not statistically significant (p>0.05), GFAP expression between the groups was not statistically significant p>0.05. Conclusion : Combining MP and TR is therapeutically more effective in improving functional recovery, inhibiting inflammation and glial scar formation after acute SCI.

Effects of Larval Extracts from P. brevitarsis seulensison Cytokine and Diagnostic Marker in Carbon Tetrachloride-Administered SD Rats

  • This study was designed to determine the effects of larval extracts from Protaetia brevitarsis seulensis shade dried larva from Korea and China on liver tissues of hepatic injury groups. The experimental groups are divided into five groups; Normal, carbon tetrachloride single injection ($CCl_4$), Pb-CHI ($CCl_4$ + P. brevitarsis shade dried larva from China), Pb-KOR ($CCl_4$ + P. brevitarsis shade dried larva from Korea) and SIL ($CCl_4$ + 0.35% silymarin) groups. Sprague Dawley rats were oral injected with $CCl_4$ at a dose l mg/kg (20% in corn oil) for induction of liver damage for 4 weeks. Each experimental group was fed with a dose 50 mg/kg of larval extracts based on medicinal preparations from 3 weeks to 4 weeks after $CCl_4$ treatment. At the end of 4 weeks, we evaluated the serum levels of glutamate-oxaloacetate transaminase (GOT), glutamate-pyruvate transferase (GPT), alkaline phosphatase (ALP) and blood urea nitrogen (BUN) in serum and the cytokine levels of tumor necrosis factor-${\alpha}$) (TNF-${\alpha}$) and transforming growth factor-${\beta}$ (TGF-${\beta}$) in the cells isolated from spleen and liver. The histological analysis was also conducted. The $CCl_4$ injection reduced body weight, induced congestion of middle lobe and hepatocytic degeneration, resulting in disintegration of hepatic cords, and increased biochemical markers of blood related to hepatic injury. On the other hand, the Pb-CHI and Pb-KOR group decreased the levels of biochemical markers in blood and cytokine levels in spleen and liver. Especially, the Pb-KOR group facilitated the recovery of biochemical values of blood related to hepatic injury, hepatic lesions and fibrosis. Taken together, larval extract from P. brevitarsis might prevent acute hepatotoxicity and enhance the recovery of liver fibrosis and cirrhosis induced by $CCl_4$, and the ingredients could be a promising candidate for the prevention and treatment of hepatic disorders.

Ginseng extract and ginsenosides improve neurological function and promote antioxidant effects in rats with spinal cord injury: A meta-analysis and systematic review

  • Spinal cord injury (SCI) is defined as damage to the spinal cord that temporarily or permanently changes its function. There is no definite treatment established for neurological complete injury patients. This study investigated the effect of ginseng extract and ginsenosides on neurological recovery and antioxidant efficacies in rat models following SCI and explore the appropriate dosage. Searches were done on PubMed, Embase, and Chinese databases, and animal studies matches the inclusion criteria were selected. Pair-wise meta-analysis and subgroup analysis were performed. Ten studies were included, and the overall methodological qualities were low quality. The result showed ginseng extract and ginsenosides significantly improve neurological function, through the Basso, Beattie, and Bresnahan (BBB) locomotor rating scale (pooled MD = 4.40; 95% CI = 3.92 to 4.88; p < 0.00001), significantly decrease malondialdehyde (MDA) (n = 290; pooled MD = -2.19; 95% CI = -3.16 to 1.22; p < 0.0001) and increase superoxide dismutase (SOD) levels (n = 290; pooled MD = 2.14; 95% CI = 1.45 to 2.83; p < 0.00001). Both low (<25 mg/kg) and high dosage (25 mg/kg) showed significant improvement in the motor function recovery in SCI rats. Collectively, this review suggests ginseng extract and ginsenosides has a protective effect on SCI, with good safety and a clear mechanism of action and may be suitable for future clinical trials and applications.

대학 태권도 선수의 스포츠상해 실태와 대처능력 방안 (The Current State of College Taekwondo Athletes' Sport Injury and the Measures to Improve Their Coping Ability)

  • 대학 태권도 선수들의 스포츠 상해 실태 중 발생시기와 주요원인은 체급과 선수경력에서 차이가 나타났으며, 발생상황은 학년과 체급, 선수경력에서 차이가 나타났다. 스포츠 상해 대처능력 방안 중 응급처치는 학년에서 차이가 나타났으며, 상해예방은 학년과 선수경력에서 차이가 나타났다. 치료방법은 차이가 없는 것으로 나타났다. 대학 태권도 선수들은 연습 시가 스포츠상해 발생시기이며, 넘어지는 상황에서 스포츠 상해가 가장 높게 나타난다. 또한 기술부족이 스포츠 상해의 주요원인이 된다는 것이 스포츠 상해 실태의 결론이다. 더불어 스포츠 상해의 대처능력 방안은 얼음찜질로 응급처치를 하고, 본인 마사지로 치료하였다. 또한 상해예방을 위해서는 충분한 피로회복이 상해예방의 최선으로 인지하였다.

하악신경 손상 후 발생한 감각부전 환자들에 대한 분석 (Analysis of Patients with Dysesthesia after Mandibular Nerve Injury)

  • 하악신경은 삼차신경의 한 분지로 발치, 임플란트 수술 등의 치과치료에 의하여 손상이 유발될 수 있으며, 다양한 신경병증을 보일 수 있다. 본 연구의 목적은 하악신경 손상 후 발생한 감각부전 환자들에 대한 분석을 통하여 감각부전의 이해에 필요한 기초 자료를 제시하고 의미를 파악해 보는 것이다. 2007년 1월부터 2009년 7월까지 하악신경 손상에 의한 감각부전으로 진단받은 환자 59명의 의무기록을 분석하여 다음과 같은 결과를 얻었다. 1. 가장 빈번한 손상 원인은 임플란트 수술에 의한 손상(59%)이었고, 가장 빈번한 손상 분지는 하치조신경(81%)이었다. 2. 신경 손상이 발생한 후 경과한 기간이 6개월 이상인 경우, 6개월 미만인 경우에 비하여 통증 강도(Visual Analogue Scale; VAS)가 4.82에서 6.91로 유의하게 증가해 있었다. 3. 약물치료 등 보존적 치료를 시작한 시기에 따른 감각부전의 회복 정도는 조기에 치료를 시작한 경우 증상의 회복을 보인 환자의 비율이 증가하는 경향을 보였으나, 통계학적으로 유의한 상관관계를 나타내지는 않았다. 4. 전산화단층촬영 영상에서 하치조신경관의 침범 수준에 따라 통증 강도나 감각부전의 회복 정도는 통계학적으로 유의한 상관관계를 나타내지 않았다. 결론적으로, 하악신경의 감각부전 환자들에서 치과 임플란트로 인한 하치조신경 손상이 가장 많은 비중을 차지하고 있었다. 하치조신경에 대한 침범 수준이 감각부전을 유발하는 가장 중요한 요인이겠지만 환자가 느끼는 주관적인 통증 강도나 치료에 대한 반응 등에는 신경의 손상 정도 이외에도 통증에 대한 환자의 반응, 심리상태, 치료의 시작 시기 등 많은 요인의 영향이 있을 것으로 생각된다. 따라서 조기에 약물치료 등 적극적인 보존적 치료를 시작하는 것이 증상의 회복을 위해 바람직할 것으로 생각된다. 또한 전산화단층촬영 영상에서 신경관을 직접 침범하지 않고, 신경관에 근접한 경우에도 감각부전이 나타난 경우가 적지 않은 것으로 보아 임플란트 수술 시 감각 부전의 방지를 위하여 충분한 안전거리 확보가 필수적일 것으로 사료된다.

소아의 척추 외상 (Spinal Cord Injury without Radiographic Abnormalities in Children)

  • Spinal cord injury in child often occurs without evidence of fracture or dislocation. The mechanisms of neural damage in this syndrome of spinal cord injury without radiographic abnormality(SCIWORA) include flexion, hyperextension, longitudinal distraction, and ischemia. Inherent elasticity of the vertebral column in infants and young children, among other age-related anatomical peculiarities, render the pediatric spine exceedingly vulnerable to deforming forces. The neurological lesions encountered in this syndrome include a high incidence of complete and severe partial cord lesions. Children younger than 8years old sustain more serious neurological damage and suffer a larger number of upper cervical cord lesions than children aged over 8 years. Of the children with SCIWORA. 52% have delayed onset of paralysis up to 4 days after injury, and most of these children recall transient paresthesia, numbness, or subjective paralysis. The long-term prognosis in cases of SCIWORA is grim. Most children with complete and severe lesions do not recover; only those with initially mild neural injuries make satisfactory neurological recovery.
