Study of Management and Environmental Factors Affecting Medical Expense Reduction (의료기관 운영요인과 환경요인이 진료비 삭감율에 미치는 영향에 관한 연구)

  • This study aimed to determine the management and environmental factors affecting medical expense reduction. For analysis, medical expenses were divided into hospitalization expenses and outpatient treatment expenses, and the rate of medical expense reduction was classified into initial and final reduction rates. Data were collected through a direct survey among 205 directors of independent health insurance review departments of hospital-level medical institutions in Korea. The results of the study are discussed below: In the analysis, differences in the initial and final reduction rates of hospitalization expenses and outpatient treatment expenses were compared. The results showed that, in hospitalization expenses, the initial and final reduction rates were both significantly affected by the following management factors: number of beds, number of departments, number of personnel reviewing health insurance cases, and total number of employees. Further, in outpatient treatment expenses, the initial and final reduction rates were both affected significantly by the following management factors: management of medical records, number of beds, number of departments, number of personnel reviewing health insurance, and total number of employees. The management factors significantly affecting both the initial and final reduction rates were higher number of beds for hospitalization expenses and electronic medical record management for outpatient treatment expenses. The environmental factors significantly affecting both the initial and final reduction rates of hospitalization expenses were a highly cooperative work environment, better implementation of indicator management systems, and overtime pay. Better implementation of indicator management system and a committee for handling medical expenses had significant effects on the initial reduction rate for outpatient treatment expenses. A highly cooperative work environment, better implementation of indicator management system, and overtime pay had significant effects on the final reduction rate for outpatient treatment expenses.

  • This study researched the effect of initial expenses for food service business and shop characteristics on the motive of starting business through questionnaires(from April to May, 2008). The results are as follows. First, for the hypothesis, 'the motive of starting business depends on its initial expenses', the significance probability of good commercial area was .015 and that of retirement was .020, showing statistically significant difference. F values were 2,889 and 2.734 each, which showed significant difference by 0.001. Second, for the hypothesis, 'the motive of starting business depends on shop characteristics', the significance probability of the accomplishment desire, the future stability and the retirement were .011, .009, and .025 each, showing statistically significant difference. F values were 2,616, 3.034 and 3.177 each, showing significant difference by .001. Lastly, initial expenses for business and shop characteristics showed significant difference according to the demographical variables on the motive of starting business. This study shows the problem of starting business with no extra funds. It is important to reflect the opinions of founders about the commercial building lease protection law for them to concentrate on business. The founders should research for menu and systems and be aware of the tendency of the industry for sure.

An Effects of Education Expenditures on the Economic Growth (교육비 지출이 성장에 미치는 영향에 관한 연구)

  • The purpose of this paper is to examine a causal relationship and interaction among payment of education expenses, economic growth, imports and exports. The results of 'the Granger causality test' suggest that payment of education expenses are affected by imports and exports; and the other factors are not related. In addition, payment of education expenses and the relationship among variables confirmed by IRF show that payment of education expenses has a minus(-) effect on exports at the beginning and then it turns into plus(+) as a certain time passes. On the other hand, it has a minus(-) effect on imports at the beginning, and this gradually changes to zero(0). Lastly, it turns out that it has a plus(+) effect on GDP. It is therefore estimated that the demand for high-quality manpower is expected to grow and new investments for education to rise with increases in imports and exports with initial acceleration. The findings show that government education investment in foreign languages and research manpower must take precedence in order to meet the demand for high-quality manpower.

  • Reducing global warming potential has become important, and as one of those methods for reducing it, economic evaluation by applying ice thermal storage air conditioning system was performed. The floor area and height of the subject building was assumed $5,000m^2$ and 20 m. Absorption chillerheater system and air source heat pump system was used for comparing to the subject system, and payback period method was used to perform economic evaluation. Although the running cost of ice thermal storage system is reduced compared to two systems, the ratio is not significant compared to the increase of initial construction expenses, and payback period was calculated to be about 7.7 and 79.3 years. However, the heat storage system should be approached from the viewpoint of long term rather than the economic standard in the present standard.

  • The GIS(Geographic Information System) projects require big initial investment and cost of maintenance. Decision makers would not launch GIS projects unless they are convinced of the benefits of the projects. Therefore the purpose of this paper is to develop an appropriate model for GIS project evaluation. We proposed a GIS project evaluation model and summarize the steps involved in Cost-Benefit Analysis(CBA). The costs of a GIS implementation include hardware and software cost, costs of database development, training expenses, annual maintenance expenses, and other annual expenses. The benefit by using GIS has been assessed into Type I(direct benefits), Type II(indirect benefits), Type III(benefits that result from the sale of information services), Type IV(intangible benefits).

A Study on the Economic Evaluation of Thermal Spray Methods for the Corrosion Protection of Steel (금속용사 방식공법의 경제성 평가에 관한 연구)

  • Generally, as corrosive protection processing of a steel structure, zinc galvanizing and heavy duty coating paint are applied. However, zinc galvanizing has the difficulty of restriction of a size, or on-site construction. Moreover, heavy duty coating paint has a problem with many administrative and maintenance expenses with short problem of adhesion, corrosion generating of a damage portion, and maintenance management cycle. In this study, a salt water spray test, CASS test, and the electrochemistry examination were carried out for the thermal metal spray method of construction for corrosive protection performance evaluation. Moreover, the corrosive protection life of a thermal metal spray method of construction was quantitatively calculated on the basis of this experiment. in consideration of LCC, the economical efficiency of a general corrosive protection method of construction and a thermal metal corrosive protection method of construction was compared. Consequently, although initial construction expense was estimated 16 to $30\%$ high, as for a thermal metal spray method of construction, it turns out that the administrative and maintenance expenses for 100 years became cheap 9.3 to 13 or more times.

The Life Expectancy Making Model for Construction Equipment (건설장비 수명결정 모델)

  • Life analysis is conducted for economic analysis of equipment or facilities. The purpose of life analysis is to predict future indicators for scrapping construction equipment, and establish and utilize a wide variety of business strategies according to data predictions. First, this study shows the methods to figure out average life, life expectancy and life prediction of construction equipment and the analysis of life making methods, using survival curves. Second, the study proposes and examines the life expectancy making model depending on revenues and expenses. The result of the study reveals that the economic life of the same equipment varies with expenses, revenues and the initial cost. The life expectancy making model for construction equipment reflects respective management status for equipment and will help efficient management for companies.

  • In January 2005, the BTL private investment project was introduced in the Korean construction market as part of the plan to provide high-quality public service and expand the required facilities in a timely manner. Nonetheless, problems such as the low earning rate at the beginning of the business, burden of service compared to the cost of the proposed business, and limitations of the local small and medium-sized companies in relation to their participation in the project arose. The LCC analysis system for the BTL projects was developed as part of efforts toward efficiently investigating the investment eligibility. Specifically, methods for LCC analysis were selected for each stage of the BTL project in relation to the requests of experts for military residential facilities and public educational facilities. Variables were then extracted to derive an accurate analysis value, LCC for the 5 cost items (initial investment cost, operating expenses, maintenance expenses, energy cost, and disposal cost), analyzed, and system enabling comparative analysis for single and multiple initiatives by year and item, developed. Thus, we have to clearly require the accumulation of data to examine the appropriateness of the results of LCC analysis based on data and results.

  • Large area members such as foundation concrete of underground structures in power plants have an effect on structural stability and durability of the structure due to danger of crack occurrence and shrinkage crack that occur owing to the difference of temperature by heat of hydration between inside and outside of the members at initial age. And a construction for waterproofness is performed additionally to protect marine structures from osmosis of seawater because the structures adjoin below the surface of sea. So, if a rise of the heat of hydration, crack, and corrosion of bars are controled effectively using a composite such as fluosilicate salt in concrete production process of a initial construction, expenses are cut down and construction hours are reduced by securing durability through improvement of watertightness. The property tests of adiabatic temperature by hydration are carried out at initial age about standard concrete and test concrete using a fluosilicate salt composite to evaluate an effect on improvement of watertightness for concrete structures in this study. And the experiments such as a permeability test of hardened concrete, a water absorption test, a compression strength test and a elongation test are carried out and the results from these are described.

Costs of Initial Cancer Care and its Affecting Factors (암 환자의 발생 초기 의료비와 이에 영향을 미치는 요인)

  • Objectives : The purposes of this study is to estimate the cost of cancer care after its diagnosis and to identify factors that can influence the cost of cancer care. Methods : The study subjects were patients with an initial diagnosis one of four selected tumors and had their first two-years of cancer care at a national cancer center. The data were obtained from medical records and patient surveys. We classified cancer care costs into medical and nonmedical costs, and each cost was analyzed for burden type, medical service, and cancer stage according to cancer types. Factors affecting cancer care costs for the initial phase included demographic variables, socioeconomic status and clinical variables. Results : Cancer care costs for the initial year following diagnosis were higher than the costs for the following successive year after diagnosis. Lung cancer (25,648,000 won) had higher costs than the other three cancer types. Of the total costs, patent burden was more than 50% and medical costs accounted for more than 60%. Inpatient costs accounted for more than 60% of the medical costs for stomach and liver cancer in the initial phase. Care for latestage cancer was more expensive than care for early-stage cancer. Nonmedical costs were estimated to be between 4,500,000 to 6,000,000 won with expenses for the caregiver being the highest. The factors affecting cancer care costs were treatment type and cancer stage. Conclusions : The cancer care costs after diagnosis are substantial and vary by cancer site, cancer stage and treatment type. It is useful for policy makers and researchers to identify tumor-specific medical and nonmedical costs. The effort to reduce cancer costs and early detection for cancer can reduce the burden to society and improve quality of life for the cancer patients.
