Altered Motor Control in Patients With Neck Pain and Prospective Research Work (경부통증 환자의 변형된 운동조절 특성 분석과 향후 활용 방안)

  • Background: It is necessary to find and develop the effective way of intervention for patients with neck pain, since the neck pain is becoming increasingly common throughout the world. To identify the altered motor control in patient with neck pain would be informative to find and develop the effective way of intervention. Objects: The aim of this study was to review literature regarding the altered motor control in patients with neck pain, measured by using surface electromyography (sEMG), ultrasonography, and functional magnetic resonance imaging (fMRI), and to suggest prospective research work on neck pain. Methods: Case-control (neck pain/healthy) studies published between 2004 and 2015 that investigated neck muscle activation, thickness, cross-sectional area, and fat infiltrate were searched in Scopus, PubMed, and ScienceDirect. Twenty-eight articles were included in this study. Results: sEMG, ultrasonography, and fMRI were used complementarily to investigate the altered superficial and deep neck muscle activation, thickness, cross-sectional area, and fat infiltrate in patients with neck pain. They showed the following altered motor control when compared retrospectively with healthy subjects or during specific functional tasks: (1) increased superficial muscle activation, (2) lesser deep muscle thickness, (3) smaller cross-sectional area of the deep muscle, and (4) greater fat infiltrate in deep muscles. In particular, among the women, the office workers showed higher muscle activation of superficial neck muscles during functional tasks, although they did not have neck pain, than those who were not office workers. Conclusion: Studies revealed that patients with neck pain showed an altered motor control when compared with healthy subjects by using various assessment modalities. Understanding this phenomenon would help researchers design an effective intervention for alleviating neck pain or to evaluate the effectiveness of the intervention. In addition, we recommend that female office workers take measures to care for their necks before developing neck pain.

Th17 Cell and Inflammatory Infiltrate Interactions in Cutaneous Leishmaniasis: Unraveling Immunopathogenic Mechanisms

  • The inflammatory response during cutaneous leishmaniasis (CL) involves immune and non-immune cell cooperation to contain and eliminate Leishmania parasites. The orchestration of these responses is coordinated primarily by CD4+ T cells; however, the disease outcome depends on the Th cell predominant phenotype. Although Th1 and Th2 phenotypes are the most addressed as steers for the resolution or perpetuation of the disease, Th17 cell activities, especially IL-17 release, are recognized to be vital during CL development. Th17 cells perform vital functions during both acute and chronic phases of CL. Overall, Th17 cells induce the migration of phagocytes (neutrophils, macrophages) to the infection site and CD8+ T cells and NK cell activation. They also provoke granzyme and perforin secretion from CD8+ T cells, macrophage differentiation towards an M2 phenotype, and expansion of B and Treg cells. Likewise, immune cells from the inflammatory infiltrate have modulatory activities over Th17 cells involving their differentiation from naive CD4+ T cells and further expansion by generating a microenvironment rich in optimal cytokines such as IL-1β, TGF-β, IL-6, and IL-21. Th17 cell activities and synergies are crucial for the resistance of the infection during the early and acute stages; however, if unchecked, Th17 cells might lead to a chronic stage. This review discusses the synergies between Th17 cells and the inflammatory infiltrate and how these interactions might destine the course of CL.

A Hydrologic Analysis for the Infiltration Storages Planned on Jeju-do (제주도에 계획된 침투저류지의 수문학적 분석 사례)

  • An infiltration storage can be installed as a method of reducing runoff from catchment and increasing stream flow during the dry period by recharging groundwater. However, there is no proper model and method that can be used to design storage capacity of an infiltration storage in Korea. The purpose of the study is to evaluate capacities of infiltration storages planned on Jeju-do in Korea by modifying Storm Water Management Model (SWMM). The basic equations for the infiltration storage are same as those of the infiltration trench used in MIDUSS. Infiltration rates of the infiltration storages were first measured by double ring infiltrometers, and then the modified model was applied to evaluate adequacy for the capacities of three infiltration storages planned on Jeju-do in Korea. The application results show that the two infiltration storages with higher infiltration rates have adequate capacities to infiltrate the total water inflow to the storages. However, the other infiltration storage with lower infiltration rates has not sufficient capacity to infiltrate the total water inflow to the storage and release occurs to the downstream region. The simulation model and method applied can be used for capacity evaluation of future infiltration storages on highly pervious areas in Jeju-do.

  • The surface-drainage system, which consists of bio-swale and detention-infiltration Basins and carries out the function of temporary detention-infiltration of runoff, is defined as the "natural drainage system". It is an environmentally sound and economically beneficial practice to reduce run-off by retaining it in swales as much as possible and letting run-off infiltrate into the ground. In order to estimate appropriate capacity of swales, it is necessary to know how long will it take for certain depths of water to infiltrate. The ponding times, or infiltration times, of various depths and of various soil textures, could be estimated with the Green-Ampt Infiltration Model. Included soil textures are loamy sand, sandy loam, loam, silty loam, sandy clay loam and clay loam. Ponding depths are from 10cm to 100cm intervals. Newton-Raphson method is used for the solution of the Green-Ampt equation by a computer program. The computer program was written with the FORTRAN Developer 4.0 v.. Selected ponding depth is acceptable when the sum of the ponding time and the breeding time of mosquitoes is less than the tolerance period of innundation of grasses and trees.and trees.

  • As impervious area increases due to urbanization, rainfall on the impervious area does not infiltrate into the ground, and stormwater drains quickly. Low impact development (LID) practices have been suggested as alternatives to infiltrate and store water in soil layers. The practices in South Korea is applied to urban development projects, urban renewal projects, urban regeneration projects, etc., it is required to perform literature research, watershed survey, soil quality, etc. for the LID practices implementation. Prior to the LID implementation at fields, there is a need to simulate its' effect on watershed hydrology, and Storm Water Management Model (SWMM) provides an opportunity to simulate LID practices. The LIDs applied in South Korea are infiltration-based practices, vegetation-based practices, rainwater-harvesting practices, etc. Vegetation-based practices includes bio-retention cell and rain garden, bio-retention cells are mostly employed in the model, adjusting the model parameters to simulate various practices. The bio-retention cell requires inputs regarding surface layer, soil layer, and drain layer, but the inputs for the drain layer are applied without sufficient examination, while the model parameters or inputs are somewhat influential to the practice effects. Thus, the approach to simulate vegetation-based LID practices in SWMM uses was explored and suggested for better LID simulation in South Korea.

  • The development of dental materials has widened the scope of materials by changes in processing methods. CAD/CAM processing enables the use of zirconia as a dental material. Recent esthetic materials development has been made. For aesthetic purposes, a block for CAD/CAM processing by mixing polymer and ceramic materials are fabricated. However there is no guideline of how these materials should be used in actual clinical practice. Mechanical properties, wear and clinical studies were reviewed.

  • A 39 year-old woman presenting with a hemoptysis, pulmonary infiltrate was found to have thoracic actinomycosis by the specimens of excised lung. Recently, pulmonary actinomycosis is very rare by the widespread use of antibiotics. Clinical, radiological, and microbiological findings can be nonspecific. The diagnosis is dependent on a high index of suspicion. Chances for cure are excellent with lengthy antibiotic administration. The purpose of this paper is to review our experience and to remind us of pulmonary actinomycosis.

  • W/WC-Cu/Ag contacts of 60wt%-40wt% base and contacts with additives(Ni, Co, C) of 1wt% below were prepared by a press-sinter-infiltrate process to compare with their physical properties and arc erosion characteristics. In physical properties, electrical conductivity of contacts with additive is lower than that of base contacts but hardness is higher. The results of arc test show that the erosion rate of contact with -0.1wt% Ni is decreased.
