Despite the latest advancement made in its techniques and devices/apparatuses and the resulting rising expectation in the field of dental surgery, apicoectomy performed in the molar teeth remains a technical challenge and lacks evidence substantiated by long-term follow-up studies. This study sought to investigate the treatment outcomes and post-operative success rate in the root-end resected molar teeth accompanied by a high level of surgical risks due to their close proximity to the mandibular canal and maxillary sinus. A total of 68 patients who received treatment at Livingwell Dental Hospital between 2004 and 2010 and underwent apical surgery in the maxillary or mandibular molar area were enrolled in this study. A total of 160 roots collected from 75 molar teeth were subjected to surgical endodontic treatment and subsequently evaluated clinically as well as radiographically. Based on the results of the study, the clinical success rate was found to be 78.8% in cases involving radiological healing. Likewise, 90.7% of the roots recorded a robust clinical survival rate, but with incomplete healing as shown by radiography. The results indicate that the apical procedure involving molar teeth is a prognosis-friendly method that promises positive outcomes and higher success rate based on long-term follow-up observations.
Background: The primary objectives of mandibular surgery are to achieve optimal occlusion, low sensory disturbance, and adequate fixation with early movement. In-depth knowledge of the mandibular structure is required to achieve these goals. This study used computed tomography (CT) to evaluate the mandibular cortical thickness and cancellous space according to age and sex. Methods: We enrolled 230 consecutive patients, aged 20 to 50 years, who underwent CT scanning. The cortex and cancellous space centered around the inferior alveolar nerve (IAN) canal were measured at two specific locations: the lingula and second molar region. Statistical analysis of differences according to increasing age and sex was performed. Results: The t-test revealed that the cancellous space and cortical thickness differed significantly with respect to the threshold of 35 years of age. Both cortical thickness and cancellous space in the molar region were negatively correlated with age. Meanwhile, both cortical thickness and cancellous space in the lingula region showed a positive correlation with age. With respect to sex, significant differences in the cancellous space at the molar region and the cortical thickness at the lingula were observed. However, no further statistically significant differences were observed in other variables with respect to sex. The sum of each measurement on the mandibular body reflected the safe distance from the surface of the outer cortex to the IAN canal. The safe distances also showed statistically significant differences between those above and below 35 years of age. Conclusion: Knowledge of the anatomical structure of the mandible and of changes in bone structure is crucial to ensure optimal surgical outcomes and avoid damage to the IAN. CT examination is useful to identify changes in the bone structure, and these should be taken into account in the planning of surgery for older patients.
Background: Local anesthetics alone or in combination with adjuncts, such as oral medications, have routinely been used for pain control during endodontic treatment. The best clinical choice amongst the vast numbers of agents and techniques available for pain control for irreversible pulpitis is unclear. This network meta-analysis combined the available evidence on agents and techniques for pulpal anesthesia in the maxilla and mandible, in order to identify the best amongst these approaches statistically, as a basis for future clinical trials. Methods: Randomized trials in MEDLINE, DARE, and COCHRANE databases were screened based on inclusion criteria and data were extracted. Heterogeneity was assessed and odds ratios were used to estimate effects. Inconsistencies between direct and indirect pooled estimates were evaluated by H-statistics. The Grading of Recommendation, Assessment, Development, and Evaluation working group approach was used to assess evidence quality. Results: Sixty-two studies (nine studies in the maxilla and 53 studies in the mandible) were included in the meta-analysis. Increased mandibular pulpal anesthesia success was observed on premedication with aceclofenac + paracetamol or supplemental 4% articaine buccal infiltration or ibuprofen+paracetamol premedication, all the above mentioned with 2% lignocaine inferior alveolar nerve block (IANB). No significant difference was noted for any of the agents investigated in terms of the success rate of maxillary pulpal anesthesia. Conclusion: Direct and indirect comparisons indicated that some combinations of IANB with premedication and/or supplemental infiltration had a greater chance of producing successful mandibular pulpal anesthesia. No ideal technique for maxillary anesthesia emerged. Randomized clinical trials with increased sample size may be needed to provide more conclusive data. Our findings suggest that further high-quality studies are required in order to provide definitive direction to clinicians regarding the best agents and techniques to use for mandibular and maxillary anesthesia for irreversible pulpitis.

This study was conducted for the purpose of suggestion of the new technique of sagittal split ramus osteotomy pararell to the true sagittal plane. This pararellism is the important concept of the sagittal split ramus osteotomy to reduce the condylar sagging including mandibular hypomobility, temporomandibular disorder, occlusal relapse and other complications. We used 26 adult dry manibles(52 rami), and obtained the computed tomographs through the sagittal, horizontal and coronal sections. The results were obtained as follows. 1. On sagittal section, mean area of S1 was $8.63{\pm}2.10cm^2$, S2 was $8.93{\pm}1.94cm^2$, S3 was $9.49{\pm}2.15cm^2$, S6 was $10.72{\pm}2.22cm^2$. The wider area of sagittal section, the more lateral section, But, no significant differency between the areas of the sagittal sections(P>0.05). 2. On horizontal section, The distance between the inferior alveolar canal and the lateral cortical plate of the mandibular ramus were $6.73{\pm}1.24mm$ minum, $7.70{\pm}1.44mm$ maximum. 3. On coronal section, Outer mandibular angle were $4.84{\pm}2.37^{\circ}$ right side, $4.93{\pm}2.12^{\circ}$ left side. 4. The design of the ideal true sagittal split ramus osteotomy is that posterior border of osteotomy must be limited vertically, at the right posterior point of lingula mandibularis and anterior of osteotomy must be extended to mandibular body, anteroinferiorly.
Purpose: This study aimed to identify the prevalence and extension of the anterior loop (AL) of the mental nerve in different populations and according to different cone-beam computed tomography (CBCT) imaging settings. Materials and Methods: Medline/PubMed, Embase, Scopus, Web of Science, and ProQuest were searched. The main inclusion criterion was ALs evaluated in CBCT images. The quality of studies was assessed with the Joanna Briggs Institute risk of bias checklist. Subgroup analyses were conducted for sex, side, continent, voxel size, field of view, and type of CBCT-reconstruction images with a random-effects model. Results: Sixty-three studies with 13,743 participants (27,075 hemimandibles) were included. An AL was found in 40.6% (95% CI: 32.8%-48.9%, P<0.05) of participants and 36.0% (95% CI: 27.5%-45.5%, P<0.05) of hemimandibles, in 34.9% (95% CI: 25.1%-46.2%, P<0.05) of males and 34.5% (95% CI: 23.5%-47.4%, P<0.05) of females. The average length of ALs was 2.39 mm (95% CI: 2.07-2.70 mm, P<0.05). Their extension was 2.13 mm (95% CI: 1.54-2.73 mm, P<0.05) in males and 1.85 mm (95% CI: 1.35-2.36 mm, P<0.05) in females. Significant differences were observed regarding the prevalence and length of ALs among continents and for its measured length on different CBCT-reconstruction images, but not between other subgroups. Conclusion: AL was a relatively common finding. The voxel size and fields of view of CBCT devices were adequate for assessing AL; however, a 2-mm safety margin from anatomical structures(such as the AL) could be recommended to be considered when using CBCT imaging.

The purpose of this study was to evaluate the accuracy and usefulness of spiral tomography through the comparison and analysis of SCANORA cross-sectional tomographs and DentaScan computed tomographic images of dry mandibles taken by a SCANORA spiral tomographic machine and a computed tomographic machine. Thirty-one dry mandibles with full or partial edentulous areas were used. To evaluate the possible effect of location in the edentulous area, it was divided into 4 regions of Me (region of mental foramen), MI (the midportion between Me and M2), M2 (the midportion between mental foramen and mandibular foramen) and S (the midportion of the mandibular symphysis). A ZPC column (sized 4 mm x 5 mm) was seated on the edentulous regions of Me, MI, M2 and S using the acrylic stent. Then SCANORA spiral tomography and computed tomography were taken on the edentulous regions which contained the ZPC column. The ZPC columns and cross-sectional images of the mandible were measured in the radiographs by three observers and the differences between the two imaging modalities were analysed. The results were as follows: 1. In comparing the actual measurements of the ZPC column and measurements in the radiographs, the mean error of the DentaScan computed tomography was 0.07 mm in vertical direction and -0.06 mm in horiwntal direction, while the mean error of the SCANORA spiral tomography was 0.06 mm in vertical direction and -0.12 mm in horizontal direction. There was a significant difference between the two radiographic techniques in the horizontal measurement of the ZPC column of the symphysis region (p<0.05). But there was no significant difference in the measurements of other regions (p>0.05). 2. In measurements of the distance from the alveolar crest to the inferior border of the mandible (H), and of the distance from the alveolar crest to the superior border of the mandibular canal (Y), there was no significant difference between the two radiographic techniques (p>0.05). 3. In measurements of the distance from the lingual border of the mandible to the buccal border of the mandible (W), and of the distance from the lingual border of the mandible to the lingual border of the mandibular canal (X), there was a significant difference between the two radiographic techniques in measurements of the midportion between the mental foramen and the mandibular foramen (M2) (p<0.05). But there were no significant differences in measurements of the other regions of symphysis (S), mental foramen (Me), the first one-fourth portion between the mental foramen and the mandibular foramen (M1) (p>0.05). 4. Considering the mean range of measurements between observers, the measurements of SCANORA spiral tomography showed higher value than those of DentaScan computed tomography, except in measurements of symphysis (S). 5. On the detectability of the mandibular canal, there was no significant difference between the two radiographic techniques (p>0.05). In conclusion, SCANORA spiral tomography demonstrated a higher interobserver variance than that of DentaScan computed tomography for implant site measurements in the posterior edentulous area of the mandible. These differences were mainly the result of difficulty in the detection of the border of the mandible in SCANORA spiral tomography. But considering the cost and the radiation exposure, SCANORA spiral tomography can be said to be a relatively good radiographic technique for implant site measurement.
최근 임플란트를 원하고, 필요로 하는 환자가 증가하면서 술전의 정확한 진단이 보다 중요해졌고 다양한 진단방법이 소개되고 이용되어져 왔다. 그 중 보편적으로 가장 널리 사용되어지는 방법이 방사선 촬영법이고 그 중에서도 파노라마방사선사진이다. 하지만 파노라마방사선사진의 용이함과 간편성에도 불구하고 그 방법 자체가 가지고 있는 한계점 때문에 CT/MPR(Computer Tomography/Multiplanar Reformatting)이 일부 환자에서는 반드시 필요하다는 주장이 대다수이다. 본 연구의 목적은 하악 구치부에 임플란트 시술을 받은 환자를 대상으로 파노라마방사선사진의 확대율을 알아보고 파노라마방사선사진과 CT/MPR에서 치조정-하악관간 거리가 실제 어느 정도 차이를 보이는지, 그리고 CT/MPR에 대한 파노라마방사선사진의 확대율에 환자의 하악골의 경사도와 하악관의 해부학적 위치가 어떤 영향을 미치는지를 알아보는 것이다. 본 연구는 서울대학 치과병원 치주과를 내원하여 하악구치부에 Dental Implant 시술을 받은 15명의 환자, 32개 부위를 대상으로, 임플란트 시술을 위한 술전 파노라마방사선사진과 CT/MPR, 술후의 파노라마방사선사진, CT/MPR상을 자료로, 임플란트가 식립될 위치에서 치조정에서 하악관의 길이, 하악골의 협설폭경, 하악골의 설측면에서 하악관의 수평길이, 임플란트 매식체의 설측에서 하악관까지의 수평거리를 계측하였고 이 해부학적 계측치들이 CT/MPR에 대한 파노라마방사선사진의 확대율과 어떤 상관관계에 있는지 Pearson's correlation coefficient test를 이용해 살펴보았다. 1. CT/MPR에 대한 파노라마방사선사진의 확대율은 CT/MPR상에서 하악골 단면의 협설폭경(Pearson test에서의 상관관계 분석시, 상관계수 r=-0.604), 임플란트 매식체의 설측에서 하악관까지의 수평거리(r=-0.515), 하악골의 설측면에서 하악관까지의 수평거리(r=-0.640)와 뚜렷한 음적 선형관계가 있었다. 2. CT/MPR에 대한 파노라마방사선사진의 확대율은 CT/MPR상에서 하악골의 장축의 각도와는 뚜렷한 양적 선형관계(Pearson test에서의 상관관계 분석시, 상관계수 r=-0.446)가 있었다. 3. 여러 요소중 CT/MPR에 대한 파노라마방사선사진의 확대율과 가장 유의성있는 연관성을 보이는 것은 CT/MPR상에서 하악골의 설측면에서 하악관까지의 수평거리였다(r=-0.640) 4. CT/MPR에 대한 파노라마방사선사진의 확대율은 일반적으로 생각되어지는 파노라마방사선사진의 확대율 125%보다 큰, 약 135%로 나왔다. 이상의 결과로 볼 때, 하악골의 설측면에서 하악관까지의 수평거리가 CT/MPR에 대한 파노라마방사선사진의 확대율에 가장 영향을 미치는 요소로 추정되며 확대율은 125%보다 높은 것으로 나타났다.
