사망수준과 사망 원인관련 지표들 간의 관계에 대한 자료탐색 분석 (An Explanatory Data Analysis about the Relationship between Mortality Level and Four Indicators Relating to the Causes Mortality Decline)

  • 이 연구의 목적은 사망수준의 저하에 영향을 미치는 세 요소-사회경제적 발전, 공공 보건의 발달, 사회경제적 발달의 균등상태-의 상대적 중요성을 분석하는 것이다. 종속변수인 사망 수준의 지표로는 영아사망률과 출생시 기대수명 등 두 변수가 사용되었다. 국민총생산(GNP)은 사회 경제적 발달지표로 여성의 초등학교 취학률과 기니계수(GINI index)는 사회경제적 균등상태 지표로 병원침대당 인구수는 공공보건 지표로 간주되었다. 변수들에 대한 자료는 두 시점에 걸쳐 수집되었다. 하나는 1970년 이전 53개국에서. 다른 하나는 1970-80년대 55개국에서 수집되었다. 탐색적 자료 분석 방법이 통계 분석 방법으로 사용되었다. 이 기법은 종속변수와 독립변수와의 관계가 선형인지 아닌지, 그리고 우리 모형에서 어느 것이 유력 사례인지를 파악할 수 있는 장점이 있다. 분석결과에 따르면, 첫째로 영아 사망률과 세 요소의 관계가 선형이 아니라 비선형임이 밝혀졌다. 영아 사망률 저하에 국민총생산이 가장 많이, 여성의 초등학교 취학률이 두 번째, 기니계수가 그 다음으로 영향을 미치는 것으로 나타났다. 반면 병원침대당 인구수는 통계적으로 유의미한 영향을 보여주지 않았다. 둘째, 출생시 기대수명은 여성의 취학률, 기니계수 등과 같은 변수와는 선형 관계를 가지는 반면 국민총생산 변수와는 비선형 관계를 가진다. 영아사망률 변수와는 달리 출생시 기대수명의 변이에는 여성의 초등학교 취학률이 국민총생산보다 더 커다란 영향을 미쳤다.

산모 연령과 영아 사망과의 관련성 연구 (Maternal Age and Infant Mortality in Korea)

  • 본 연구는 우리나라 출생아 전수(n=617,867)를 대상으로 산모 연령과 영아 사망과의 관련성을 파악하고자 시행하였다. 연구자료는 인구동태통계자료와 영아사망조사자료를 연계하여 사용하였고, 산모 연령에 따른 영아 사망 위험을 파악해 보기 위해 출생아의 성별과 다태 여부, 출생순위, 저체중아 여부, 선천성기형 여부를 보정한 다중 로지스틱 회귀분석을 시행하였다. 여러 혼란요인을 보정한 분석 결과, 25-29세 산모에 비해 20세 미만 산모(Odds ratio [OR], 5.29, 95% confidence interval [CI], 3.51-7.98)와 20-24세 산모(OR, 1.44, 95% CI, 1.23-1.69), 35-39세 산모(OR, 1.28, 95% CI, 1.11-1.46), 40세 이상 산모(OR, 1.94, 95% CI, 1.53-2.45)에서 영아 사망 오즈가 통계적으로 유의하게 높았다. 결론적으로, 이번 연구를 통해 35세 이상 고 연령 산모와 25세 미만의 저 연령 산모에서 영아 사망 위험이 높은 것을 확인할 수 있었고, 이들에서 영아 사망 위험이 높은 이유는 저체중아나 미숙아의 출산이 많기 때문으로 보인다. 산전관리 시 고령 산모의 경우 생물학적인 문제를 극복할 수 있는 관리정책이 만들어져야 할 것으로 보이고, 저 연령 산모의 경우 사회경제적인 부분과 사회적 편견 문제를 해결할 수 있는 관리정책이 만들어져야 할 것으로 보인다.

군위 및 합천군과 대구시 남구 모자보건 센터에서 관찰한 코호트 영아 사망률 (Cohort Infant Mortality Rate of Gunwee and Hapchun Counties and an MCH Center in Taegu)

  • 1985년 4월 1일부터 1987년 3월 31일 사이에 경상북도 군위군에 거주하는 49세이하의 가임여성들과 1987년 3월 1일부터 1988년 2월 28일 사이에 경상남도 합천군에 거주하는 49세이하의 가임여성들 그리고 1985년 4월 1일부터 1987년 3월 31일 사이에 대구시 남구 모자보건센터에 분만을 위해 찾아온 모든 임산부들을 대상으로 이들에게서 태어난 출생아들을 두 군지역에서는 생후 1년간 그리고 모자보건센터에서 출생한 경우는 생후 27일간 추적하여 영아사망률과 신생아 사망률을 각각 조사하였다. 조사기간중에 군위군과 합천군에서 출생한 1,359명의 영아들 가운데 생후 1년 동안에 17명이 사망하여 농촌지역의 영아사망률은 출생아 1,000명당 12.5이었다. 총 영아사망중 신생아사망이 82.4% 그리고 신생아 후기 사망이 17.6%를 차지하였다. 대구시 남구 모자보건센터에 내원한 산모들에게 태어난 6,001명의 출생아들 가운데 4,834명(80.6%)만이 생후 27일까지 추적되었는데 이들 중 36명이 사망하여 신생아 사망률은 출생아 1,000명당 7,4명이었다. 추적된 산모와 추적 안된 산모의 특성 및 신생아 체중을 비교한 결과 현저한 차이를 나타내지 않았다. 신생아 후기 사망률의 비를 6 : 4로 가정했을 때 모자보건센터에서 출생한 영아의 영아사망률은 출생아 1,000명당 12.3명으로 추정되었다. 군위군과 합천군의 영아사망률과 대구시 남구 모자보건센터에서 관찰된 신생아사망률, 영아사망의 사망시기별 분포와 영아사망원인 그리고 저체중아출생률을 고려할 때 1985년부터 1988년 사이의 한국의 영아사망률은 출생아 1,000명당 12에서 15명일 것으로 추정되었다.

Changes in the neonatal and infant mortality rate and the causes of death in Korea

  • Neonatal mortality rate (NMR) or infant mortality rate (IMR) are the rate of deaths per 1,000 live births at which babies of either less than four weeks or of one year of age die, respectively. The NMR and IMR are commonly accepted as a measure of the general health and well-being of a population. Korea's NMR and IMR fell significantly between 1993 and 2009 from 6.6 and 9.9 to 1.7 and 3.2, respectively. Common causes of infantile death in 2008 had decreased compared with those in 1996 such as other disorders originating in the perinatal period, congenital malformation of the heart, bacterial sepsis of newborns, disorders related to length of gestation and fetal growth, intra-uterine hypoxia, birth asphyxia. However, some other causes are on the increase, such as respiratory distress of newborn, other respiratory conditions originating in the perinatal period, other congenital malformation, diseases of the blood and blood-forming organs and certain disorders involving the immune mechanism. In this study, we provide basic data about changes of NMR and IMR and the causes of neonatal and infantile death from 1983 to 2009 in Korea.

Short-term Associations of Air Pollution with Postneonatal Infant Death in Seoul, Korea, 1999-2003

  • Objective to assess whether exposure to air pollutants is associated with postneonatal infant death, using a timeseries methodology, between 1999 and 2003 in Seoul, Korea.. Methods We investigated the short-term effects of air pollution for 548,725 live births during the study period. The daily count of postneonatal infant deaths from all causes and from SIDS (sudden infant death syndrome) by birth order was analyzed by a Generalized Additive Poisson model, with controlling for the effects of seasonal trends, air temperature, relative humidity, barometric pressure, and day of the week as covariates. Results During the study period, we observed 699 deaths from all causes and 47 deaths from SIDS. We did not find any significant associations between daily mortality and ambient levels of air pollutants except for CO and $NO_2$. The estimated relative risk of postneonatal infant death from all causes was 1.17 (95% CI=1.04-1.32) and 1.16 (95% CI=1.03-1.29) by IQR (interquartile range) for CO and $NO_2$ respectively. Also, we observed no clear trend of the mortality effects of air pollution by birth orders. Conclusion In conclusion, our findings suggest that air pollution, in general, influenced adversely postneonatal infant death from all-cause and SIDS although it was not statistically significant. This study may support that the rationale.

저출생 체중아 분마에 대한 임상적 고찰 (Clinical Observation on Delivery of Low Birth Weight Unfant)

  • A clinical study was made on 365 low birth weight infant and 406 normal birth weight infant who had been born at Kangnam St. mary's Hospital during past 3 years from Jan. 1, 1995 to Dec. 31, 1997. the data of this study were gathered through reviewing of medical records. 1. Comparison of general characteristic with of obstetric characteristic 1) Old maternal age, previous abortion and previous LBWI delivery in the group of low birth weight infant(LBWI) mother were more prevalent than those in the group of normal birth weight infant(NBWI)mother 2) Cesarean section, abnormal presentation and multiple pregnancy in the group of LBWI mother were prevalent than those in the group of NBWI mother. 3) regular antenartal care and visiting rate of tertiary hospital in the group of LBWI mother were more prevalent than those in the group of NBWI mother. 2. Frequency of low birth weight infant 1) Anmual average frequency of LBWI was 6.5% and monthly frequency was the highest in January and december. 2) The frequency of LBWI was the highest in 37-40wks of gestational age and was the highest in 2251-2500 gm of birth weight. 3) The frequency of congenital anomaly in the group of LBWI was more prevalent than that of NBWI. 3. Mortality rate of LBWI The mortality rate of LBWI was 9.2%. The highest mortality rate was noted before 27wks of gestational age, less than 1000gm of birth weight and within 12hrs of delivery. 4. The most common complication of pregnant women was pre-term labor, the most complication relating to placenta was premature rupture of membrane(PROM) and the most fetal complication was fetal distress in delivered LBWI. 5. Significant relating factors of low birth weight infant delivery were associated with maternal age, previous delivery, previous low birth weight delivery, pre-eclampsia, anemia, oligohydramnios, PROM, placenta previa, abruptio placenta, fetal sex, fetal distress and congenital anomaly.

Comparison of Parameter Estimation Methods in the Analysis of Multivariate Categorical Data with Logit Models

  • In fitting models to data, selection of the most desirable estimation method and determination of the adequacy of fitted model are the central issues. This paper compares the maximum likelihood estimators and the minimum logit chi-square estimators, both being best asymptotically normal, when logit models are fitted to infant mortality data. Chi-square goodness-of-fit test and likelihood ratio one are also compared. The analysis infant mortality data shows that the outlying observations do not necessarily result in the same impact on goodness-of-fit measures.

한국인의 사망구조 변화에 대한 고찰 (A Study on Changes of Korean Mortality Pattern, 1930~1980)

  • Death is one of the population movement phenomena used as an important health index in most society. Especially it is regarded as group phenomenon in a specific group rather than individual one and considered important in public health field because the level and cause of death is related to health of public. The auther examined the changes of the Korean mortality pattern to evaluate the status of the Korean public health by studying mortality indicator using the population census and other materials from 1930 to 1980. The results are as follows: First, the Korean crude death rate was reduced to one third in 1980 compared to that in 1930, but the crude birth rate was constant from 1930 to 1960 causing the increase of population. So the population pattern is changing from the classic pyramic shape to bell shape and the dependency ratio was reduced from 78 in 1930 to 61 in 1980. Second, the infant mortality rate decrease rapidly. In 1980 it was one seventh of that in 1930 which was proved by the change of the age-specific death rate curve from U to J shape. Third, the male mortality reduction after the age of forty was much less than that of female, which explained the specific death pattern of high mortality in Korean middle and oldaged males. Fourth, the main cause of death was changed from infectious, parasitic, respiratory and digestive system disease to circulatory of tumorous diseases. Considering the above results, Korean health problem is now changing from the infant infection to geriatric chronic regressive disease. That naturally the direction of health service should be turned from the infant stage maternal and child health to the health problems of old people.

Trends and Characteristics of Mortality Associated with Congenital Anomalies in Korean Children under 5 Years of Age

  • Purpose: Studies have been conducted on the prevalence and infant mortality rate of congenital anomalies; however, studies on child mortality are rare. Therefore, we evaluated the characteristics of deaths associated with congenital anomalies among children born in Korea who died within 5 years of age. Methods: Birth-to-death cohort linked data of children under the age of 5 years from 2010 to 2013, and statistical data on the cause of death by age from 1999 to 2019, both provided by the Korea National Statistical Office's Microdata Integrated Service, were retrospectively investigated. We investigated the trends and characteristics of mortality associated with congenital anomalies. Results: Among 1,858,945 children, 6,510 children who died were under 5 years of age, and among them, 1,229 deaths were associated with congenital anomalies, while 5,281 deaths were due to other causes. Deaths associated with congenital anomalies accounted for 18.9% of all deaths. When comparing congenital anomalies by systems, anomalies of the cardiovascular system (52.6%) were the most common. The mortality rate associated with congenital anomalies and those of other causes showed similar declining trends in 21 years. Conclusion: The mortality rate of congenital anomalies during the first 5 years of life did not increase differently from the prevalence of congenital anomalies but rather decreased. Deaths associated with congenital anomalies accounted for 20.5% of all infant deaths and 12.1% of child deaths, since the major causes of death in infants and children are slightly different, continuous and careful monitoring is required.
