Adolescent nutrition and growth (청소년의 영양과 성장)

  • Nutrition is an essential component of total adolescent health care. Two important changes occurring during adolescence can cause a crisis in the teenager's nutritional needs. First, growth in height, weight, and body component is greater and more rapid than at any time since infancy. Second, an adolescent's eating habits may change from regular meals prepared at home to irregular meals, skipped meals, and nutrition-poor snacks and fast-food meals. Adolescents have been found to have the highest prevalence of any age group of an unsatisfactory nutritional status. To understand the nutritional requirements of the adolescent, health practitioners should be aware of the intensity and timing of the adolescent growth spurt, the differences in the growth spurt between males and females, and the individual variation in timing of the growth spurt from teenager to teenager.

Evaluation of Cancer Treatment Using FDG-PET (FDG-PET을 이용한 암 치료 효과의 평가)

  • FDG-PET has potential as an effective, non-invasive tool to measure tumor response to anticancer therapy. The changes in tumor FDG uptake may provide an early, sensitive guide to the clinical and subclinical response of tumors to cancer treatment, as well as functional assessment of residual viable tumor. This may allow the evaluation of subclinical response to anticancer drugs in early clinical trials and improvements in patients management. However, monitoring tumor responses with FDG-PET is still in its infancy. The methods of measurement of FDG uptake are currently diverse and timing with respect to anti cancer therapy variable. Therefore, there is a need for larger-scale trials along with standardized methodology and a collection of reproducibility data. The recent guideline from the European group seems to be the most comprehensive. In future, the combination of morphological and metabolic images may improve the quantitative nature of these measurements by relating tumor viability to total tumor mass. More data on sensitivity and specificity of FDG-PET technique are needed along with continued advancement of PET methodology.

Biliary Atresia (담도폐쇄)

  • Biliary atresia (BA) is an infantile cholestatic disease of progressive obliterative cholangiopathy with varying degrees of damage to both extra and intrahepatic bile ducts due to unknown causes. The diagnostic studies should be done to diagnose or exclude BA without unnecessary delay. Kasai portoenterostomy is the first choice of treatment for bile drainage from microscopic bile ductules present in the portal fibrous mass. The medical management after Kasai portoenterostomy should be done carefully to maintain bile excretion and prevent and treat complications Including cholangitis, hepatic fibrosis, portal hypertension and nutritional problem. The reported five years-survival rates after Kasai portoenterostomy range from 30 to 60 %. About 20 % of all patients undergoing Kasai portoenterostomy during infancy survive into adulthood with their native liver. Even if Kasai portoenterostomy remains as the first line of treatment In BA, liver transplantation serves as a good salvage treatment when portoenterostomy fails or liver function gradually deteriorates after initially successful establishment of bile flow, Overall 5-year survival rate in BA is about 90 % in recent series.

Parenting experiences of mothers of moderate-to-late preterm children in South Korea: a qualitative study

  • Purpose: This study investigated the parenting experiences of mothers of young children born moderate-to-late preterm (MLPT) in South Korea. Methods: In this qualitative study, semi-structured focus group interviews were conducted with 10 mothers of MLPT children from infancy to preschool age. The interviews were video-recorded, transcribed verbatim, and analyzed using qualitative content analysis. Results: Four categories resulted from the analysis of parenting experiences of mothers with young MLPT children, as follows: "becoming a mother of an early-born child", "difficulties as the primary caregiver for a high-risk child", "helpful social support, but still a lack of professional support for parenting a high-risk child", and "mothers and children growing together". Conclusion: Mothers of young MLPT children experienced difficulties due to concerns about their child's health, growth and development, and insufficient child-rearing support. Therefore, social support systems should be strengthened and more aggressive nursing strategies should be adopted for mothers of young MLPT children.

Language Development in Cleft Palate Infants (구개파열 영유아의 언어발달 특성 연구)

  • The purpose of the present study was to investigate the characteristics of receptive and expressive language development of cleft palate infants aged under 3. Twenty-six cleft palate infants and 52 normal infants were grouped into 3 chronological age groups with 1 year intervals: less than 1, 1, and 2 years old. The cleft palate infants were divided into 2 groups: cleft palate only, and cleft lip & palate. Each mother of the infants was asked to complete the questionnaire, Sequenced Language Scale for Infants (SELSI). Receptive and Expressive language scores of SELSI were computed. The scores of receptive and expressive language were respectively analyzed into 4 categories of language: phonology/prosody, semantics, syntax, and pragmatics. The results, concerning the differences of language development between the cleft palate and the normal infants, were as follows: (1) expressive language scores were significantly different at age 2 between the cleft palate and the normal infants; (2) cleft plate groups aged less than 1 and 1 showed lower scores of phonology/prosody of expressive language than the normal groups; (3) cleft palate group aged 2 showed lower scores than the normal group in semantics, syntax, and pragmatics. The results, concerning the characteristics of language development between the cleft palate only and the cleft lip & palate infants, were as follows: (1) the scores of expressive and receptive language were not statistically different between the 2 groups; (2) both groups did not show any difference in .the scores of phonology/ prosody, semantics, syntax, and pragmatics of receptive and expressive language.

Empyema Thoracis: 60 Cases Analysis (농흉의 임상적 고찰: 60례 보고)

  • There appears to be significant problems remained in the treatment of thoracic empyema inspire of the more potent broad spectrum antimicrobial agents available and improved surgical managements. Clinical analysis of 60 patients of thoracic empyema was done who received major and/or minor surgical intervention at the Dept. of Thoracic and Cardiovascular Surgery, Korea University Hospital in the period of 3 years from May 1973 to July 1976. Following was the results: 1. Male was predominent to female with the ratio of 2.16 to 1, and adult to infant was 2.75: 1. 2. Most frequent predisposing factors of thoracic empyema in infancy and childhood was pneumonia [38.3%], and in adult was pulmonary tuberculosis [36.7%]. 3. Cardinal symptoms were dyspnea, fever, chest pain and productive cough etc. 4. Positive result of bacteriological culture study was reported in 26. 7%, and among them, Staph. aureus [10%], Diplo. pneumonia [3.3%], Streptococcal group [1.7%], Pseu. aeruginosa [1.7%], Paracolon bacilli [1.7%] and others [8.3%] in respectively. No growth of pathologic organism was reported in 60.0%, and not requested in 13.3%. Among the negative group, tuberculosis was confirmed on pathological tissue slide in half and other half was not determined bacteriologically and pathologically. 5. Surgical treatment was performed to the almost all of the patients with closed thoracostomy [48.3%], empyemectomy [28.3%], empyemectomy with pulmonary resection [11.7%] and others [I1.6%] in respectively with favorable results. 6. Mortality rate was 8.3% [5 cases], each one with lung cancer, diabetes mellitus, respiratory insufficiency, sepsis and another, two months old infant with asphyxia.

The Effects of Samulsopungeum and Prednisolone on NC/Nga Atopic Mice (사물소풍음(四物消風飮)과 Prednisolone이 NC/Nga mice의 아토피 피부염에 미치는 영향)

  • Objectives : Atopic dermatitis(AD) is a chronic recurrent skin disease which usually developed in infancy or childhood. AD often repeat improvement and relapse. The cause of AD is so indefinite that many methods of therapies(moisturizer, steroid ointment, antihistamine, immunomodulator, immunosuppressant, herbal medicine, alternative medicine, etc.) are tried. Recently, a lot of studies were made. But there is no report about the effect of Samulsopungeum(SM) and Prednisolone(PN) on AD. So, author aimed to investigate the effects of SM and PN on AD of NC/Nga mice. Methods : Thirty two mice(8 Balb/c mice and 24 NC.Nga mice) were divided into four groups; Balb/c mice was normal group. NC/Nga mice were divide into three group : control, PN, SM group. AD was induced in the control, PN, SM group by spreading DNCB. Then normal saline, PN and SM were orally administered three times in a week for 8 weeks to the control, PN, SM group, respectively. We observed changes of clinical skin severity score, serum IgE, IgG1, IFN-$\gamma$, IL-2, IL-4, IL-5, IL-10, IL-13, and so on. We used one-way ANOVA test statistically(p<0.01). Results : The clinical skin severity scores of PN group and SM group in 8th week were decreased compared to the control group. Serum IgE, IgGl levels of PN group and SM group were significantly decreased compared to the control group. Serum IFN-$\gamma$ in SM group was significantly increased compared to the control group. But, Serum IFN-$\gamma$ in PN group was significantly decreased compared to the control group. Serum IL-10 levels of PN group and SM group were significantly decreased compared to the control group. Serum IL-2, IL-4, IL-5, IL-13 levels of PN group and SM group were significantly decreased compared to the control group. mRNA expression levels of IL-2, IL-4, IL-5, IL-13 in the dorsal skin tissues of PN group and SM group were significantly decreased compared to the control group. According to biopsy reports of the ear and skin tissues showed that the tissue damage of PN group and SM group were highly reduced compared to the control group. Creatinine, BUN, ALT, AST levels of PN group and SM group were normal. Conclusion : According to the above results, it is considered that SM is effective treatment for the AD.

Vesicoureteral Reflux and Urinary Tract Infection in Children with Congenital Hydronephrosis (선천성수신증 환아에서 방광요관역류 및 요로감염 발생에 대한 고찰)

  • Purpose : We checked voiding cystourethrography (VCUG) in patients with congenital hydronephrosis (CHN) and followed up the occurrence of urinary tract infection(UTI) in these patients. And we tried to figure out the relationship between CHN and VUR and UTI. Methods : We performed a retrospective study in 122 CHN patients who received VCUG from 2002 to 2008 at Ajou University Hospital. We executed logistic regression, chi-square test and T-test using version 16.0 SPSS package for the statistical analysis. P<0.05 was regarded as significant. Results : Most patients with CHN were male (73%) and there was a tendency of preferences for unilateral (77%) and left (84%) occurrence of hydronephrosis. The incidence of VUR was 18.9% among 122 CHN patients and no laterality difference was found to the severity of VUR or hydronephrosis. The incidences of VUR and UTI was higher in group of higher grade (III-IV) CHN patients, when compared to the lower group, respectively. When VUR was present in CHN patients, the incidence of UTI during infancy was also higher. Conclusion : The incidence of VUR was higher in patients with higher grades of CHN. The incidence of UTI during infancy was higher when VUR was present in the CHN patients.

Risk Factors of Intracranial Hemorrhage in Skull Fracture after Pediatric Head Trauma (두부 외상 후 두개골 골절 환아의 두개내 출혈 위험요인)

  • Purpose: The purpose of this study was to investigate the risk factors of intracranial hemorrhage in children with skull fractures from head trauma. Methods: The retrospective study included 205 patients diagnosed with a skull fracture in a pediatric emergency room. Data were analyzed using 𝓍2-test, Fisher's exact test, t-test, and logistic regression analysis with the SPSS/WIN24.0 program. Results: Intracranial hemorrhage was diagnosed in 71 patients. There were statistically significant differences between the hemorrhagic group and non-hemorrhagic group in age group, places of accident, type of accident, location of the fracture, and symptoms. Intracranial hemorrhage by age group was higher in school-age and adolescence than in infancy. The places of accidents of hemorrhage were higher in street and school than in the home. The types of an accident of bleeding were higher in the case of knock and traffic accident than in fall. Symptoms of nausea, headache, and loss of consciousness were associated with higher intracranial hemorrhage. Multivariable logistic regression analysis showed that knock (OR= 3.29, 95% CI= 1.50-7.22), traffic accident (OR= 4.78, 95% CI= 1.31-17.43), nausea (OR= 4.18, 95% CI= 1.42-12.31), and loss of consciousness (OR= 3.29, 95% CI= 1.41-9.50) were risk factors for intracranial hemorrhage. Conclusion: In this study, the risk factors of intracranial hemorrhage were identified in pediatric patients with skull fractures caused by head trauma. It is recommended that the results of this study be used to manage and educate patients, caregivers, and medical staff after head trauma hemorrhage.

The Longitudianl Study of the Growth by Feeding Practice in Early Infancy (영아의 섭식패턴에 따른 성장발육의 종단적인 비교 연구)

  • The objectives of this study were to examine the growth pattern of infants by anthropometric measurement according to the 5 feeding practices of infants with the subject of two hundred healthy newborn babies from their birth till sixty month of age at intervals of two months. Breast group(BF, n=38), formula group(FF, n=102) and mixed group(ME, n=14) were fed breast milk, formula milk, breast and formula milk, from birth till 6 mo. of age, respectively. Convert 1 group (C1F, n=14) and covert 2 group(CF, n=32) were fed breast milk and mixed milk at 2 mo. of age afterthat switched to formula milk, respectively. From these, the following results were made. All the infants of this study showed superiority to Korean standard growth rate in regards to each growth item for each month age. In the case of males, at their birth, the subscapular skinfold thickness and the total skin fold thickness in the BF group was significantly larger than in MF group and FF group(p<0.05). At 6th month age, the chese circumference of MF group was 45.9cm, and significantly larger than those of BF, FF and C2F groups(p<0.05). In the case of females, at theri birth and 2nd month age, there was no difference among all the feeding groups in regards to each growth rate. At 4th month age, the Kauf index of C1F group was 16.21 and significantly lower than those of four groups(p<0.05). And total skinfold thickness in BF group was larger than in C1F group. The increase rate per month age of all growth items were larger at 2nd month age than at the later months both in males and in females. And until 2nd month age males showed more increase than females in regards to each growth item but after 2nd month age, this sapect did not show up. Multiple linear regression was used to determine predictive factors for infant growth. It was expected that at 6th month age, in the measurement of head circumference and chest circumference and cross-sectional fat area, BF-males were bigger by 22-39% of the explanation index than the infants of other groups. As a result, in spite of the significant lower intakes of energy and nutrients in breast-fed infants than in formula-fed infants, breast-fed infants showed more growth than the average of Korean infant standard growth rate at every month age, and showed no significant growth difference among feeding groups.
