  • The effect of diluents on the dissolution rate of indomethacin was studied and the dissolution rate for the marketed products was compared to establish the quality standard for indomethacin preparation. The results are as fellows 1. The effect of test methods was not significant in the case of paddle method but was greatly effected in the case of basket method. 2. The effect of diluents was not significant generally, but in the case of starch dissolution rate was greatly effected. 3. The dissolution rate was remarkably decreased in proprotion to increase of capsule size. 4. The marketed products met the KP dissolution requirement but showed a little difference due to manufacturers.

The Inhibitory effect of omeprazole-cholestyramine resin in gastric secretion of rat (Omeprazole-cholestyramine resin 제제의 위산분비에 대한 억제효과)

  • We have examined inhibitory erects on gasritis using omeprazole-cholestyramine resinate, which has been developed to increase the stability of omeprazole, the well-known proton pump inhibitor, in an acidic condition. To test the pharmacological action of this, we investigated the effect of omeprazole-cholestyramine resinate on indomethacin-induced gastritis in rats. Omeprazole was used as a reference drug. Orally administered omeprazole-cholestyramine resinate inhibited the indomethacin-induced gastritis in a dose-dependent manner. The inhibitory effect of omeprazole-cholestyramine resinate on the gastritis was similar to that of reference drug. In addition, rectal adminstration of the omeprazole-cholestyramine resinate inhibited the indomethacin-induced gastritis in a dose-dependent manner. The inhibitory effect of omeprazole-cholestyramine resinate was equipotent to reference drug. The basal gastric acid secretion was decreased when it was administered either orally or rectally. This inhibition of omfprazole-cholestyramine resinate was similar to that of omeprazole. These data suggest that omeprazole-cholestyramine resinate inhibit the gastritis in rats, and are comparable to omeprazole available in market.

  • Near-infrared (NIR) system was used to determine rapidly and simply indomethacin in buffer solution for a dissolution test for tablets and capsules. Indomethacin standards were prepared ranging from 10 to 50ppm using mixture of phosphate buffer(pH 7,2) and water(1:4), The near infrared(NIR) transmittance spectra of indomethacin standard solutions were collected by using a quartz cell in 1 mm and 2mm pathlength, Partial least-square regression (PLSR) was explored to develop calibration models over the spectral range 1100-1700nm.   • The effect of ginseng on gastric ulcer and gastric acid secretion was investigated in pylorus-ligated rats. Methods: Sprague-Dawley strain rats were used after 24 hours fast. Pylorus-ligation was performed under light ether anaesthesia, then gastric mucosal damage was evoked in conscious pylorus-ligated rats by the administration of subcutaneous (s.c.) indomethancin (20mg/kg), s.c. histamine (150mg/kg) or by pylorus-ligation (Shay ulcer). Ginseng was given by intragastric (i.g.) or intraperitoneal (i.p.) route simultaneously with the ulcerogens. Rats were killed after 3h (indomethacin) and histamine models) or after 18h (Shay ulcer), when the gastric secretory responses, the number and severity of gastric mucosal lesions and mucosal mucus content deetermined. the effect of i.p. ginseng on basal gastric acid secretion and on gastric acide secretion in indomethacin (20mg/kg, s.c.)-treated rats was also investigated in urethane anesthetized rats. Gastric acid secretion was measured by flushing of the gastric lumen with saline every 15min through an oesophageal cannula. Results: In conscious pylorus-ligated rats, i.g. ginseng(12.5-50mg/$m\ell$; 50-200mg/kg) protected against gastric mucosal lesions evoked by s.c. indomethacin or s.c. histanmine in the d3-h pylorus-lighted rat, withoutmodifying gastric acid secretory responses. Ginseng given i.p. (150 or 200mg/kg) did not reduce the gastric lesions produced by histamine or by ligating the pylorus (Shay ulcer) Ginseng given orally in 50mg/$m\ell$ (200mg/kg) increased gastric mucus secretion in saline- and indomethacin-treated conscious pylorus-ligated rats. In anaesthetized rats ginseng (50 or 200mg/kg) did not modify basal gastric acid secretion or gastric acid secretion in the indomethacin-treated rats. Conclusions: ginseng given orally exerts gastroprotective effects in the rat stomach. Such anti-ulcer effect does not involve changes in gastric acid secretory responses. In addition, ginseng possesses stimulatory effect on gastric mucus secretion, which could be one mechanism by which the compound exerts its antiulcer effect. Our data are in favor for a beneficial effect for topically applied ginseng on the gastric mucosa.

  • Objective : This study was carried out to systemically investigate the effect of Joongwan(中脘; CV12) acupuncture in cerebrum and cerebellum of indomethacin-induced gastrointestinal disease in SD-rats. Method : 1. We induced gastrointestinal disease by indomethacin oral administration in SD-rats. 2. We selected Joongwan(中脘; CV12) acupuncture point that generally have been used to treat gastrointestinal disease. 3. We categorized SD-rats into three groups as followings. (1) Normal group : The group without any management (2) Control group : The group with indomethacin-induced gastrointestinal disease (3) Treated group : The group that Joongwan(中脘; CV12) acupuncture was performed after inducing gastrointestinal disease 4. We figured out the effect of acupuncture by analyzing staining degree of NADPH-diaphorase in cerebrum and cerebellum. Results : 1. Cerebrum (1) Normal group : The degree of staining was very low. (2) Control group : NADPH-diaphorase was mainly stained in cerebral cortex and the stained region was wider than Normal group. (3) Treated group : The degree and region of staining was higher and wider than the other goups. Sometimes the intensively stained regions were observed. 2. Cerebellum In both cases of Control group and Treated group, the regions in cortex were stained mainly. But, between Control group and Treated group, there was no remarkable difference. Conclusion : In case of cerebellum, there was no remarkable result. On the other hand, in case of cerebrum, there were certain differences among three groups. Through those results, we could conclude that Joongwan(中脘; CV12) acupuncture treatment was able to affect NADPH-diaphorase expression in the cerebrum of SD-rats that have gastrointestinal disease with indomethacin-inducing.

Effect of Prostaglandin $E_2$ and Indomethacin on Electrical Activity of Isolated Cat Stomach (적출한 고양이 위(胃)의 전기활동에 미치는 prostaglandin $E_2$ 및 indomethacin의 영향)

  • This study was conducted to investigate the effect of prostaglandin $E_2(PGE_2)$ and indomethacin upon the electrical activity of the isolated cat stomach muscle strips$(1.5{\times}7.0\;cm)$. Fifty-seven muscle strips, obtained from 57 cat stomachs(including corpus and antrum) were studied in a muscle chamber filled with Krebs solution(pH 7.4, temperature $36{\pm}0.5^{\circ}C$) aerated with 5% $CO_2$ in $O_2$. The electrical activity was recorded by five capillary electrodes (Ag-AgCl), of which two were placed on the corpus and three on the antrum. After recording of the electrical activity in normal Krebs solution, $PGE_2$ in concentrations of 0.25(N=7), 0.5(=7), 1(N=7) and $2{\times}10^{-7}\;M(N=6)$ were administered to 27 muscle strips, while indomethacin was applied in concentretions of 0.25(N=9), 0.5(N=10), 1(N=6) and $2{\times}10^{-3}\;M(N=5)$ to the remaining 30 strips. The mean frequency were minutely measured from each electrogastrogram. 1) By adding $PGE_2$ in all doses, gastric slow wave frequency increased significantly compared with that in resting state. 2) Following $PGE_2$ administration, peak slow wave frequency increased dose-dependently. 3) After indomethacin addition in all doses, the slow wave frequency decreased significantly compared with that in resting state. 4) Following indomethacin administration, incidence of complete abolition of slow wave increased dose-dependently, and its latent period decreased also in a dose-dependent manner. It is inferred from the above results that prostaglandin $E_2$ has a facilitatory role in the development of gastric slow wave in cat.

  • Tooth eruption requires remodeling of surrounding tissues. This study was aimed to investigate the effect of indomethacin on the dental follicle and paradental tissues during tooth eruption by observing the distribution and expression of MMP by the immunohistochemical method. Ten mongrel dogs of ten to twelve weeks old were divided into 5 groups; four experimental groups administered indomethacin 2 mg/Kg/day and 8 mg/Kg/day orally 2 times a day for 14 days and 7 days respectively, and the control group was administered a placebo. Permanent teeth before eruption and their surrounding tissues were selected and excised. H&E staining and immunohistochemical stainings of MMP-3 and -9 were performed and examined under the light microscope. Osteoclasts, osteoblasts, periodontal ligament cells, ameloblasts and odontoblasts of the control group all expressed MMP-3 and -9. In the experimental group, osteoclasts, osteoblasts and periodontal ligament cells showed reduced expression of MMP-3 and -9. Magnitude of MMP reduction In the experimental group showed a time and dose of indomethacin administration dependent manner. These results show that indomethacin inhibited MMP-3 and -9 expression in the dental follicle and surrounding tissues and suggest that when indomethacin is administered for long periods, tooth eruption could be delayed.

Involvement of nitric oxide and prostanoid on the endothelium -dependent vasodilatation by acetylcholine in the isolated rabbit renal artery (토끼 적출 신동맥에 있어서 acetylcholine에 의한 내피세포 의존성 이완작용에 대한 nitric oxide와 prostanoid의 연관성)

  • The effect of the nitric oxide synthase(NOS) inhibitor, $N^{G}$-nitro-L-arginine methyl ester (L-NAME), and the prostanoid synthesis inhibitor, indomethacin, on the vasodilatation produced in response to acetylcholine(Ach) on the isolated rabbit renal artery was examined. The vasodilatory reponses to Ach($10^{-8}-3{\times}10^{-5}M$) were completely absent in thevessel which the endothelium had previous been removed. L-NAME($10^{-4}M$) significantly reduced the vasodilatory reponse to the Ach($10^{-8}-3{\times}10^{-5}M$). When L-arginine ($10^{-3}M$) was also present in the organ bath along with L-NAME($10^{-4}M$), this inhibitory effect of L-NAME ($10^{-4}M$) on the vasodilatory response to Ach ($10^{-8}-3{\times}10^{-5}M$) was significantly attenuated, Indomethacin ($10^{-6}M$) did not significantly affect the vasodilatory responses to Ach ($10^{-8}-3{\times}10^{-5}M$). The inhibition by L-NAME ($10^{-4}M$) and indomethacin ($10^{-6}M$) on vasodilatory response to Ach was significantly greater than the inhibition due to L-NAME ($10^{-4}M$) alone. The present study has established that Ach induce relaxation via and endothelium-dependent mechanism, this relaxation to Ach involves both nitric oxide(NO) and prostanoid in the isolated rabbit renal artery.

A Comparative Study of Pyeongwi-san, Ijin-tang and Pyeongjintang Extracts on Indomethacin-Induced Gastric Mucosal Lesions in Mice (Indomethacin으로 유발된 생쥐의 위점막 손상에 대한 평위산(平胃散), 이진탕(二陳湯) 및 평진탕(平陳湯)의 비교연구)

  • Objectives: This study was performed to investigate the protective and treating efficacy of Pyeongwi-san, Ijin-tang, and Pyeongjin-tang extracts to the mice with gastric mucosal lesions induced from indomethacin. Methods: In order to verify protective and treating efficacy of Pyeongwi-san, Ijin-tang, and Pyeongjin-tang extracts to the mice with gastric mucosal lesions induced from indomethacin, I administered the extracts of these prescriptions to three group, and induced gastric mucosal lesion by indomethacin, and then I observed the gastric mucosal morphology of stomach, changes from stress resulting from HSP70, changes of gastro-protection (mucous barrier, COX-1). After I observed the anti-oxidant effect, and anti-inflammation effect (IKK mRNA, iNOS mRNA, COX-2 mRNA) in vitro, I induced gastric mucosal lesion by indomethacin, and administered the extracts of each prescriptions to three group, and then I observed the gastric mucosal morphology, anti-inflammation effect to mucosa (NF-${\kappa}$B, iNOS, COX-2) in vivo. Results & Conclusions: 1. Hemorrhagic erosion and damaged mucus secreting cell, positive responses to HSP70 were decreased in all the before-gastric-mucosal-lesion-induced groups, compared to non-extract administered group. The effects were good in the order of Pyeongwi-san extracts administered group, Pyeongjin-tang extracts administered group and Ijin-tang extracts administered group. 2. In all the before-gastric-mucosal-lesion-induced groups, gastro- protection functions (mucous barrier, COX-1) were significant. The effects were good in the order of Pyeongwi-san extracts administered group, Pyeongjin-tang extracts administered group and Ijin-tang extracts administered group. 3. Anti-oxidant effect was significant in Pyeongwi-san extracts, Ijin-tang extracts and Pyeongjin-tang extracts. The effects were good in the order of Pyeongjin-tang extracts, Pyeongwi-san extracts and Ijin-tang extracts. 4. The anti-inflammation effects in vitro were good in Pyeongwi-san extracts, Ijin-tang extracts and Pyeongjin-tang extracts. Especially Pyeongjin-tang extracts showed the most prominent results. Damaged mucus secreting cells and the positive reactions of NF-${\kappa}$B, iNOS, COX-2 in vivo were decreased in after-gastric-mucosal-lesion-induced groups compared to non-extract administered group. The effects were good in the order of Pyeongjin-tang extracts administered group, Pyeongwi-san extracts administered group and Ijin-tang extracts administered group. These results show that Pyeongwi-san, Ijin-tang and Pyeongjin-tang are effective on both in protecting and treating the gastric mucosal membrane. Pyeongwi-san is more effective than other prescriptions, in protecting gastric mucosal membrane, and Pyeongjin-tang is more effective in treating gastric mucosal lesion.

Clinical Result of the Patent Ductus Arteriosus in the Premature Infants (미숙아 동맥관 개존증의 치료성적)

  • Background: This study is to suggest the optimal method as a treatment for the patent ductus arteriosus in the premature infants. Material and Method : Between April 1994 and April 1997, 45 premature infants with evidence of a hemodynamically significant patent ductus arteriosus associated with cardiopulmonary compromise underwent indomethacin therapy, surgical treatment, or both. Thirty-nine infants received indomethacin and twelve infants among them were surgically ligated because of indomethacin failure(5) or complications(7). Six infants, who weighed less than 1,500 gm at birth, were referred for primary surgical ligation because of contraindication to indomethacin therapy. Result: The failure rate of indomethacin therapy was 43%(17/39) and the complications(13/39, 33%) to the indomethacin were associated with a high morbidity and mortality. Among the infants who underwent ligation, there were no failures and complications related to the operation. This data suggests that in the premature neonate with a hemodynamically significant PDA, (1) indomethacin therapy is associated with a high failure rate and significant complications, (2) surgical duct closure is associated with minimal morbidity. Conclusion: Although the results of this study cannot suggest the optimal management for PDA in premature infants, primary surgical ligation may be considered. However, long-term studies will be needed to confirm this later.
