코골이를 주소로 내원한 환자의 부비동염과 아데노이드 비후와의 관계 (Sinusitis and Adenoid size is related to Snoring in children)

  • Background: Snoring in children, is associated Kith adenotonsillar hypertrophy and sinusitis which contribute to upper airway obstruction, so we studied the sinusitis and adenoid size of snoring children with plain radiography Materials and method: Fifty patients having snoring, nasal obstruction in our hospital between November 2001 and November 2002 were studied using plain radiology PNS(water's view) and Neck lateral view(adenoid size with A/N ratio) Results: 1. In oriental medicine, Hu-Bi means laryngopharyngeal edema and obstruction, laryngopharyngeal disease are due to Hwa-Yeoul( fire and hot), sinus disease are due to Fung-Han(wind and cold) and Fung-Yeoul(wind and hot). 2. Age ranged from 2 to 17 year-old ( mean age: 6 years), 5-7 year-old were 18 patients (36%). Age of on set, 25 patients were 1-4 year-old (50%). 3. Of the 50 snoring patients, 37 patients were sinusitis(74%), 20 patients had enlarged adenoid(40%). Of the 20 large adenoid patients, 19 patients were sinusitis(95%). 4. Of the 50 snoring patients, size of tonsil were flowed. Fifteen were severe(3+), 17 patients were moderate(2+) and 15 patients were mild(1+). Of the 20 enlarged adenoid patients, size of tonsil flowed. Five were severe(3+), 11 patients were moderate(2+) and 4 patients were mild(1+). Conclusion : Of the 50 Snoring patients, 37 patients showed sinusitis(74%), 32 patients showed large tonsil(64%), 20 patients showed large adenoid(40%).

교통사고 환자의 요통후유증에 대한 고찰 (Study of Sequale of Low Back Pain in Traffic Accident Patients)

  • Objective: We have various ways in evaluating the level of low back pain as sequelae, general approaches such as neurologic examination, MRI, Radiologic examination and evaluating the effect of psychological stress on the low back pain. Besides We can find another approach to evaluating the sequale of low back pain in TA patients. So, I intend to analyze how much relationship the patients that got low back pain by TA have with Roland Morris Disability Scale(RMS) in 2 months after discharge. Methods: In this article, I will compare two results of TA inpatients and non-TA inpatients, which obtained with the RMS. This study was carried out about 22 TA patients and 18 non-TA patients, who had low back pain and were hospitalized between March 2002 and July 2002. Results & conclusions : 1. RMS point is related with the post-discharge term to a point of time of answering the questionnaire in both TA patients and non-TA patients. 2. In distribution of RMS point, Gr II take most possesion as 41% in TA patients while Gr I take most possesion as 56% in non-TA patients. 3. In distribution of RMS point, patients that correspond to more than Gr III take 27% in TA patients, 16% in non-TA patients. 4. TA patients show higher level of distribution than non-TA patients in RMS point in verifying them by mean value and T-test. 5. Degree of pain score change(${\Delta}$P.S), using mean value and T-test, showed lower level of distribution in TA patients than non-TA patients. 6. We can see that TA patients have more restriction in their life for low back pain.

Radiofrequency catheter ablation of atrioventricular nodal reentry tachycardia in children and adolescents: a single center experience

  • Purpose: Atrioventricular nodal reentry tachycardia (AVNRT) is less common in pediatric patients than in adult patients. Thus, data for pediatric AVNRT patients are insufficient. Hence, we aimed to analyze the patient characteristics, treatment, and any recurrences in pediatric AVNRT patients. Methods: We reviewed the records of 50 pediatric AVNRT patients who had undergone radiofrequency catheter ablation (RFCA) between January 1998 and December 2016 at a single regional center. The patients were aged ${\leq}18years$. Results: Among 190 pediatric patients who underwent RFCA for tachyarrhythmia, 50 (26.3%; mean age, $13.4{\pm}2.6years$) were diagnosed as having AVNRT by electrophysiological study. Twenty-five patients (25 of 50, 50%) were male. Twenty patients (20 of 50, 40%) used beta-blockers before RFCA. All patients had no structural heart disease except 1 patient with valvular aortic stenosis and coarctation of the aorta. RFCA was performed using the anatomic approach under fluoroscopic guidance. The most common successfully ablated region was the midseptal region (25 of 50, 50%). Slow pathway (SP) ablation and SP modulation were performed in 43 and 6 patients, respectively. Complication occurred in 1 patient with complete atrioventricular block. During follow-up, 6 patients had recurrence of supraventricular tachycardia, as confirmed by electrocardiography. Among them, 5 underwent successful ablation at the first procedure. In 1 patient, induction failed during the first procedure. Conclusion: RFCA is safe and effective in pediatric AVNRT patients. However, further research is needed for establishing the endpoints of ablation in pediatric AVNRT patients and for identifying risk factors by evaluating data on AVNRT recurrence after RFCA.

A Survival Analysis of Gastric or Colorectal Cancer Patients Treated With Surgery: Comparison of Capital and a Non-capital City

  • Objectives: The objective of the present study was to compare prognosis of patients with gastric or colorectal cancer according to places where they received surgeries. Methods: The cancer patients underwent surgeries in sampled hospitals located in Daegu were matched 1:1 to the patients who visited sampled hospitals in Seoul using propensity score method. After the occurrences of death were examined, Kaplan-Meier method was used for survival analysis and the log-rank test was performed to compare the survival curves. Results: A total of six out of 291 gastric cancer patients who had surgeries in Daegu died (2.1%) and ten deaths (3.4%) occurred from patients went Seoul hospitals. Out of 84 gastric cancer patients who had chemotherapy after surgeries in Daegu, 13 (15.5%) patients died while 18 (21.4%) deaths occurred among patients underwent surgeries in Seoul. Six deaths (6.9%) out of 87 colorectal cancer patients who had surgeries in Daegu were reported. Five patients (5.7%) died among the patients underwent surgeries in Seoul. Among the colorectal cancer patients with chemotherapy after surgeries, 13 patients (12.4%) who visited hospitals in Daegu and 14 (13.3%) patients who used medical centers in Seoul died. There were no significant differences according to places where patients used medical services. Conclusions: The result of this study is expected to be used as basic data for policy making to resolve centralization problem of cancer patients and to help patients to make rational choices in selection of medical centers.

일 지역 119구급대를 통한 약물중독 사례 고찰 - 서울시 서초지역 중심 - (A Survey of Drug Intoxication Patients Transferred by 119 Ambulance Service - Seocho area in Seoul -)

  • This study aims to provide information about the acute poisoning patients transferred by 119 ambulance service for nurses and emergency medical technicians. The data is based on 119 ambulance services patients charts of Seocho area in Seoul. The results were as follows: 1. Total number of emergency patients all over the country has been nearly fourtimes in 1995 compared to 1991. Total number of emergency patients were 322.051 in 1994. 2. About $35\%$ of total E.R. patients were transferred by 119 ambulance services in Seoul in the last 5years. Acute drug intoxication patients occupied $2-3\%$ of E.R. patients in Seoul. 3. About $4\%$ of E.R. patients in Seoul were transferred by 119 E.R. services of Seocho area in Seoul in 1995. Among them $2.4\%$ patients were drug intoxication patients. 4. Data were collected from available patients of 119 ambulance services from Jan. 1993 to Dec. 1995 in Seocho area. Total poisoning patients were 184. - The female/male ratio was 69.7:30.4 and most of patients$(69.7\%)$ were in the age of 20th and 30th. - The busy time of calling 119 ambulance service was from 6P.M. to midnight$(37.5\%)$ - It took within 10 minutes for patients$(62.8\%)$ from notification to arrival in hospital. - Regarding poisoning substances. hypnotics $(22.8\%)$, tranquilizer$(14.7\%)$, agricultural agents$(6.0\%)$ rodenticide$(27\%)$ and others $(10.3\%)$ were in order and unknown were $43.5\%$. - Most of the patients or protector $(72.8\%)$ chose hospitals. Among them $(87.3\%)$ were general hospital. - The most predominant symptoms were coma$(22.3\%)$. dizziness$(6.5\%)$ and then allergy. vomiting. gastrointestinal cramps etc. - Airway management and oxygen administration together was the main medical control of prehospital emergency medical services$(33.7\%)$. It is proposed that first, a systematic survey of drug intoxication patients must be conducted to give an appropriate prehospital emergency care for the emergency medical technician and second, a wide and regular public education to improve understanding of first aids should be undertaken.

An Experience of Management of Homeless Neurosurgical Patients

  • Objective : Homeless patients probably have epidemiologic features that are different from those of general population. However, there have been no published articles about clinical characteristics of neurosurgical homeless patients. The authors tried to assess the clinical characteristics and treatment outcome of homeless neurosurgical patients. Methods : We retrospectively reviewed the medical records and radiological films of 76 homeless patients and 72 non-homeless patients following head trauma who were admitted to our neurosurgical department between June 2001 and June 2005. We compared two groups of the patients with the demographics, the clinical and laboratory characteristics. Also, 3D-day mortality was determined according to Glasgow Coma Scale (GCS) score. Results : Age of homeless patients was younger than that of non-homeless patients. Homeless patients had previous craniotomy evidences in skull x-rays more frequently (10.5% vs. 1.4%). Acute subdural hematoma was the most common type of head injury in the two groups. Moderate and severe head injury, based on GCS score on admission was more frequent in homeless patients (64% vs. 39%). Fifty percent of homeless patients underwent operation for traumatic head injury. However, 3D-day mortality according to GCS score was not significantly higher in homeless patients. Conclusion : Most homeless neurological patients were relatively young men. Also, moderate or severe brain injuries were observed more frequently. However, mortality rate of homeless patients in neurosurgical field is not significantly higher in the present study.

노인 입원환자들의 영양섭취 실태에 관한 조사연구 (A Studies on Nutririon of Aged patients in hospital)

  • This survey was conducted to investigated the status of nutritional consumption and relationship of nutrition of patients in hospitals from 1998.9.1 to 1998.9.10. Calorie intake of male patients was lower than RDA but that of female patients were higher than RDA except female younger than 64 years old. Protein intake of male patients was 130% higher than RDA, and that of female patients was 150% more than RDA. Calcium intake was lower than RDA in male patients but higher than RDA in female patients except older than 75 years. Iron intake was higher than RDA in all patients except in male patients older than 75 years. while vitamin B$_1$and vitamin C intake was higher than RDA in all patients but vitamin A and niacin intake was lower than RDA. The rich patients intake enough iron through animal foods and stabilized patients intake vitamin B$_2$ more than RDA. All patients were recognized the importance of protein and vegetable for health and they were recognized also careful choosing of foods for health. Positive relationships were showed between intake of nutritional tablet and nutritional status, between less vegetable intake and less intake of calorie, fat, sugar and fiber and between appetite and vitamin B$_1$ and vitamin C. The result showed that the shorter period staying in hospital, the more intake of sugar, vitamin A and vitamin C.

내ㆍ외과계 환자의 입원스트레스 특성 비교 (Differences in the Characteristics of Hospital Stress between Medical and- Surgical Patients.)

  • This study explored differences in how medical and surgical patients compare on the degree of hospital stress and their subjective physical status. Subjects were 343 medical and surgical patients in five university hospitals in Seoul and Taegu. They responded to the Hospital Stress Rating Scale and a self-report on physical status. The controlled variables were age, education, number of previous hospitalizations and seriousness of the illness. Medical and surgical patient differences on nine factors of the hospital stres scale and nine areas of physical conditions were reported as follows edplored : 1. 1) There was not a statistically significant difference at the .05 level in the total mean score for hospital stress between medical patients and surgical patients. 2) The mean score of the factor lack of information (M=2.308) for medical patients was higher than the mean score (M=2.064) of the surgical patients. 3) The mean scores of the factor of discomfort (M=2.130), loss of independence (M=1.889) for surgical patients were higher than for medical patients. 2. 1) There was a statistically significant difference at the .05 level in the total mean score for physical status between medical patients and surgical patients. 2) The mean scores were lower in subjective physical status for surgical patients(S) than for medical patients (M) ; stomach condition (S : M=2.8433, M : M=3.0-000), self-assistance(S : M=3.0373, M : M=3.4498), movement (S : M=2.6716, M : M=3.2392), interest in your surroundings (S : M=3.0522, M : M=3.2632). 3. Patients scoring high on the subjective physical status such as sleep, appetite, stomach condition bowel condition and urination states had higher scores in hospital stress than with patients scoring low on those subjective Physical status. The results suggest that subjective physical status might be on expression of hospital stress. Also patients with high scores in subjective physical statas might be predicted have a high level of stress on admission. And surgical patients had a higher level of hospital stress than medical patients.

관상 동정맥루의 외과적 치험 (Surgical Treatment of 25 Patients with Congenital Coronary Arteriovenous Fistula)

  • From Jan. 1981 to Dec. 1991, we had treated 25 patients with congenital coronary art-eriovenous fistulas [CAVF] in Seoul National University Hospital. A retrospective review was made to delineate the course and the management of CAVF and to clarify the role of surgical treatment. Fifteen patients were male and 10 were female with The mean age of 17.4 years[from 3 months to 58 years]. The most frequent symptom was dyspnea on exertion[56%]. Other symptoms were angina and palpitation. Sixty-eight percent of the patients were symptomatic. Fifty-three percent of patients less than 20 years old were symptomatic and 100% of patients over 20 years old were symptomatic. Three patients had multiple CAVFs. The fistula drained into the right ventricle in 13, pulmonary artery in 9, left ventricle in 4, right atrium in 2, and left atrium in 1. Thirteen patients had other associated cardiac lesions. The mean pulmonary-to-systemic blood flow[Qp /Qs] in the isolate CAVF group was 2.19. All patients were operated on to correct the fistulas and other associated cardiac lesions. All patients were followed from 1 month to 11 years without late death. Postoperative complication rate was 24% -significant arrhythmia [3], recurred CAVF[1], psychosis[1], pneumonia [1]. Symptomatic improvement was evident postoperatively. Below 20 years old, 94% of patients were asymptomatic, but above 20 years old, symptoms persisted in 25%. In summary, early elective repair of CAVF is indicated in all patients because of higher complication rate and frequent persistent symptoms in older patients.

뇌졸중 후 편마비 환자의 가족지지와 일상생활동작 수행과의 관계분석 (The Relationship between Family Support and Activities of Daily Living Abilities for the Hemiplegic Patients(after stroke))

  • Stroke patients have increased with the overall aging of our population, 60 years and older. Recently the number of stroke patients has been on the increase even among younger people in their thirties. The family support is frequently mentioned as a major variable which plays an important role in helping the patients adapt themselves to their hemiplegic situations. The purpose of the study was to examine between family support and activities of daily living(ADLs)abilities for the hemiplegic patients, and to provide the basic data to help the patients carry out their ADLs on their own. The sample for the study included 106 hemiplegic patients and their family members. The data were collected using a focused, structured interviews. The major findings of the study were as follows: 1. The hemiplegic patients perceived that their family supports were very high(Mean, 49.00). 2. The degree of family support was significantly higher in female. high economic status, and non-use of brace patients than in male, poor economic status, use of brace patients. 3. The ADL abilities of the hemiplegic patients were significantly higher in ambulatory, younger, and longer-period-of-illness patients and patients who used a cane as a assistive device. 4. The family support for the hemiplegic patients showed a statistically significant Cor relationship with their performance of ADL(r=.30809. p=0.0013). 5. The performance of ADL for the ambulatory patients were mainly affected by the degree of family support, the period of the hemiplegic illness, the use of brace, informal care giving, and the age. These variables explained 50% of variance. 6. The length of illness was a statistically significant affecting variable for ADL performance in OPD and assistive device in IPD. In conclusion, the higher the hemiplegic patients perceived the degree of family support, the better they performed ADL. We should develop nursing methods which enhance the family support for the hemiplegic patients in order to increase their performance of ADL.
