Acceptance Measure of Quality Improvement Information System among Long-term Care Workers: A Psychometric Assessment (장기요양인력의 질 향상 정보시스템 수용 측정도구: 신뢰타당도 평가)

  • Purpose: We evaluated the psychometric properties of a questionnaire on the acceptance of the quality improvement information system (QIIS) among long-term care workers (mostly nurses). Methods: The questionnaire composes of 21 preliminary questions with 5 domains based on the Technology Acceptance Model and related literature reviews. We developed a prototype web-based comprehensive resident assessment system, and collected data from 126 subjects at 75 long-term care facilities and hospitals, who used the system and responded to the questionnaire. A priori factor structure was developed using an exploratory factor analysis and validated by a confirmatory factor analysis; its reliability was also evaluated. Results: A total of 16 items were yielded, and 5 factors were extracted from the explanatory factor analysis: Usage Intention, Perceived Usefulness, Perceived Ease of Use, Social Influence, and Innovative Characteristics. The five-factor structure model had a good fit (Tucker-Lewis index [TLI]=.976; comparative fit index [CFI]=.969; standardized root mean squared residual [SRMR]=.052; root mean square error of approximation [RMSEA]=.048), and the items were internally consistent(Cronbach's ${\alpha}=.91$). Conclusion: The questionnaire was valid and reliable to measure the technology acceptance of QIIS among long-term care workers, using the prototype.

The Effects of P-A Mobilization on The Cervical Range of Motion and Pain for Patients with Chronic Neck Pain (만성 경부 통증환자에 대한 후-전방 가동기법이 경부 가동범위와 통증에 미치는 영향)

  • The main purpose of this study is to evaluate the effects of manual therapies which are Posterior Anterior Central Vertebral Pressure (PACVP) and Posterior Anterior Unilateral Vertebral Pressure(PAUVP} in patients having a chronic pain in the neck. This study focused on sixty patients having a chronic pain in the neck from 19 years old and 65years old. There are 33 females and 27 males each. This study randomize two groups from all subjects that have been mentioned above. Group 1 received a cervical spine manual therapy, Group 2 received a conservative physical therapy. Each therapy was held for 3 times a week, totally, 6 times for two weeks. The tape was used to measure cervical range of motion(ROM) in six areas-flexion, extension, left lateral flexion, right lateral flexion, left rotation, right rotation. 100mm Visual Analogue Scale(100mm VAS) was used to measure the subjective pain level. All measurements of each patient were measured at pre-treatment and 2 weeks post-treatment. The results of this study would be summarized as follow: 1. The manual treatment group has much more recovery than that of conservative physical therapy group in terms of the degree of the ROM improvement showing a significant difference between two groups(p<0.05) 2. The manual treatment group has more recovery than that of the conservative physical therapy group in the improvement of pain(p<0.05).

NFlex Dynamic Stabilization System : Two-Year Clinical Outcomes of Multi-Center Study

  • Objective : Pedicle-based dynamic stabilization systems, in which semi-rigid rods or cords are used to restrict or control spinal segmental motion, aim to reduce or eliminate the drawbacks associated with rigid fusion. In this study, we analyzed the two-year clinical outcomes of patients treated with the NFlex (Synthes Spine, Inc.), a pedicle-based dynamic stabilization system. Methods : Five sites participated in a retrospective study of 72 consecutive patients who underwent NFlex stabilization. Of these 72 patients, 65 were available for 2-year follow-up. Patients were included based on the presence of degenerative disc disease (29 patients), degenerative spondylolisthesis (16 patients), lumbar stenosis (9 patients), adjacent segment degeneration (6 patients), and degenerative lumbar scoliosis (5 patients). The clinical outcome measures at each assessment were Visual Analogue Scale (VAS) to measure back pain, and Oswestry Disability Index (ODI) to measure functional status. Radiographic assessments included evidence of instrumentation failure or screw loosening. Results : Sixty-five patients (26 men and 39 women) with a mean age of 54.5 years were included. Mean follow-up was 25.6 months. The mean VAS score improved from 8.1 preoperatively to 3.8 postoperatively, representing a 53% improvement, and the ODI score from 44.5 to 21.8, representing a 51% improvement. Improvements in pain and disability scores were statistically significant. Three implant-related complications were observed. Conclusion : Posterior pedicle-based dynamic stabilization using the NFlex system seems effective in improving pain and functional scores, with sustained clinical improvement after two years. With appropriate patient selection, it may be considered an effective alternative to rigid fusion.

Performance Improvement of the SVM by Improving Accuracy of Estimating Vanishing Points (소실점 추정 정확도 개선을 통한 SVM 성능 향상)

  • In this paper, we propose an improved single view metrology (SVM) algorithm to accurately measure the height of objects. In order to accurately measure the size of objects, vanishing points have to be correctly estimated. There are two methods to estimate vanishing points. First, the user has to choose some horizontal and vertical lines in real world. Then, the user finds the cross points of the lines. Second, the user can obtain the vanishing points by using software algorithm such as [6-9]. In the former method, the user has to choose the lines manually to obtain accurate vanishing points. On the other hand, the latter method uses software algorithm to automatically obtain vanishing points. In this paper, we apply image resizing and edge sharpening as a pre-processing to the algorithm in order to improve performance. The estimated vanishing points algorithm create four vanishing point candidates: two points are horizontal candidates and the other two points are vertical candidates. However, a common image has two horizontal vanishing points and one vertical vanishing point. Thus, we eliminate a vertical vanishing point candidate by analyzing the histogram of angle distribution of vanishing point candidates. Experimental results show that the proposed algorithm outperforms conventional methods, [6] and [7]. In addition, the algorithm obtains similar performance with manual method with less than 5% of the measurement error.

Performance Analysis of Similarity Reflecting Jaccard Index for Solving Data Sparsity in Collaborative Filtering (협력필터링의 데이터 희소성 해결을 위한 자카드 지수 반영의 유사도 성능 분석)

  • It has been studied to reflect the number of co-rated items for solving data sparsity problem in collaborative filtering systems. A well-known method of Jaccard index allowed performance improvement, when combined with previous similarity measures. However, the degree of performance improvement when combined with existing similarity measures in various data environments are seldom analyzed, which is the objective of this study. Jaccard index as a sole similarity measure yielded much higher prediction quality than traditional measures and very high recommendation quality in a sparse dataset. In general, previous similarity measures combined with Jaccard index improved performance regardless of dataset characteristics. Especially, cosine similarity achieved the highest improvement in sparse datasets, while similarity of Mean Squared Difference degraded prediction quality in denser sets. Therefore, one needs to consider characteristics of data environment and similarity measures before combining Jaccard index for similarity use.

Analysis of the Macroscopic Traffic Flow Changes using the Two-Fluid Model by the Improvements of the Traffic Signal Control System (Two-Fluid Model을 이용한 교통신호제어시스템 개선에 따른 거시적 교통류 변화 분석)

  • The operational effect of traffic signal control improvement was evaluated using the Two-Fluid Model. The parameters engaged in the Two-Fluid Model becomes food indicators to measure the quality of traffic flow due to the improvement of traffic signal operation. A series of experiment were conduced for the 31 signalized intersections in Uijeongbu City. To estimate the parameters in the Two-Fluid Model the trajectory informations of individual vehicles were collected using the CORSIM and Run Time Extension. The test results showed 35 percent decrease of average minimum trip time per unit distance. One of the parameters in the Two-Fluid Model is a measure of the resistance of the network to the degraded operation with the increased demand. The test result showed 28 percent decrease of this parameter. In spite of the simulation results of the arterial flow, it was concluded that the Two-Fluid Model is useful tool to evaluate the improvement of the traffic signal control system from the macroscopic aspect.

Repeat Auditing of Primary Health-care Facilities Against Standards for Occupational Health and Infection Control: A Study of Compliance and Reliability

  • Background: The elevated risk of occupational infection such as tuberculosis among health workers in many countries raises the question of whether the quality of occupational health and safety (OHS) and infection prevention and control (IPC) can be improved by auditing. The objectives of this study were to measure (1) audited compliance of primary health-care facilities in South Africa with national standards for OHS and IPC, (2) change in compliance at reaudit three years after baseline, and (3) the inter-rater reliability of the audit. Methods: The study analyzed audits of 60 primary health-care facilities in the Western Cape Province of South Africa. Baseline external audits in the time period 2011-2012 were compared with follow-up internal audits in 2014-2015. Audits at 25 facilities that had both internal and external audits conducted in 2014/2015 were used to measure reliability. Results: At baseline, 25% of 60 facilities were "noncompliant" (audit score<50%), 48% "conditionally compliant" (score >50 < 80%), and only 27% "compliant" (score >80%). Overall, there was no significant improvement in compliance three years after baseline. Percentage agreement on specific items between internal and external audits ranged from 28% to 92% and kappa from -0.8 to 0.41 (poor to moderate). Conclusion: Low baseline compliance with OHS-IPC measures and lack of improvement over three years reflect the difficulties of quality improvement in these domains. Low inter-rater reliability of the audit instrument undermines the audit process. Evidence-based investment of effort is required if repeat auditing is to contribute to occupational risk reduction for health workers.

Measurement of Group Average for the Evaluation of Attention Improvement for a Tangible Game (체감형 게임에서 주의력 측정 변인에 대한 집단 평균 측정)

  • It has been reported that more than 10 percent of Korean elementary students are classified into ADHD (Attention Deficit / Hyperactivity Disorder). Major treatment of ADHD is medication. However, medication brings various kinds of side effects. Therefore, we need a way to improve attention leve, and lead children to voluntarily participate in it. With this purpose, we tried to develop a 3D tangible game to improve the children's attention level. To measure the level of attention enhancement, firstly we measured both the group average and the standard deviation of the measurement variables from the raw data gathered during the game play time. The measured value can be effectively used to measure the level of attention improvement of the children who might need treatment.

The Effect of Public PMO Service Quality on Customer Satisfaction and Reuse Intention (공공 PMO 서비스 품질이 고객만족과 고객충성도에 미치는 영향)

  • After the revision of the Software Promotion Act, small and medium-sized enterprises (SMEs) with little experience in e-government projects began to participate in public projects. Accordingly, the government introduced the public Project Management Office (PMO) in July 2013 to reduce the risks and secure the quality of informatization business, and the public PMO is still spreading. However, the public PMO service currently experiences sluggish business, such as low-priced orders, due to cut-throat competition, and thus needs improvement plans. In this research, we establish a research model for quality factors of public PMO services that can be considered for the improvement plans, measure and evaluate quality factors based on the model, and then analyze the impact of service quality on customer satisfaction and customer loyalty. For that, we measure quality factors using the SERVPERF model for those who have used public PMO services, establish a structural equation research model, and empirically analyze and evaluate the impact of quality factors. Lastly, we suggest improvement plans and implications for public PMO services.

Improvement of Keyword Spotting Performance Using Normalized Confidence Measure (정규화 신뢰도를 이용한 핵심어 검출 성능향상)

  • Conventional post-processing as like confidence measure (CM) proposed by Rahim calculates phones' CM using the likelihood between phoneme model and anti-model, and then word's CM is obtained by averaging phone-level CMs[1]. In conventional method, CMs of some specific keywords are tory low and they are usually rejected. The reason is that statistics of phone-level CMs are not consistent. In other words, phone-level CMs have different probability density functions (pdf) for each phone, especially sri-phone. To overcome this problem, in this paper, we propose normalized confidence measure. Our approach is to transform CM pdf of each tri-phone to the same pdf under the assumption that CM pdfs are Gaussian. For evaluating our method we use common keyword spotting system. In that system context-dependent HMM models are used for modeling keyword utterance and contort-independent HMM models are applied to non-keyword utterance. The experiment results show that the proposed NCM reduced FAR (false alarm rate) from 0.44 to 0.33 FA/KW/HR (false alarm/keyword/hour) when MDR is about 8%. It achieves 25% improvement of FAR.
