Implant Placement Using Palatal Bone in Patients with Severe Maxillary Alveolar Bone Defect: Case Series Study

  • Purpose: The purpose of this case study series was to introduce successful implant cases that used the palatal bone in patients with severe maxillary alveolar bone atrophy or defects. Case Presentation: In this case series study, a total of four patients underwent implant placement in the palatal bone of the maxilla. A total of 6 implants were installed using the palatine bone. The patients' ages ranged from 40 to 73 years with an average age of 63.5 years. The patients had maxillary sinus-related diseases, such as maxillary sinusitis, oroantral fistula, and dentigerous cysts, prior to implantation. To achieve initial stability, the implants were placed on the palatal side, and buccally tilted. The average postoperative recovery period was 8 months. No postoperative complications occurred in any of the cases, and the approach was used without reported issues. Conclusion: Implant treatment by securing the initial fixation in the palatal bone is a good alternative when an implant must be installed in a patient who requires extensive and invasive bone graft.

골유착성 치과 임플란트의 생존율에 관한 임상적 연구 (CLINICAL STUDY ON SURVIVAL RATE OF OSSEOINTEGRATED IMPLANTS)

  • Objectives: The aim of the present review was to evaluate survival rate and various factors associated with survival of osseointegrated implants. Patients and methods: The clinical comparisons were performed to evaluate survival rate of 794 endosseous implants that had been inserted between 2004 through 2008 in relation to sex and age of patients, position of implant, implant system and surface characteristics, length and diameter of implant, and bone graft technique. Results: The survival rate of implant was 94.3% in posterior area of maxilla and 98.6% in posterior area of mandible by position of implant, a statistically significant difference. As to diameter of implant, survival rate was 98.4% between the 4.0 and 4.5 mm and 75.0% in larger than 5.0 mm, that was statistically significant difference. There was a statistically significant difference regard to bone graft and surgical technique. The implant survival rate was 89.0% in a placement site which performed sinus lifting, and in case of implant placement with guided bone regeneration technique and without bone grafting was 97.6% and 100% each. Conclusion: According to these findings, this study establishes a relationship between survival rate of implant and position, surface characteristics, diameter of implant and bone graft technique.

  • Purpose: Although the autogenous tissue transfer has been the mainstay of the breast reconstruction, concern for the donor site morbidity can lead to the superseded method using tissue expander with implant or permanent expander-implant. However, the additional procedure of tissue expansion possibly cause discomfort and raise the cost. We tried to verify the efficacy of using the saline-filled breast implant by itself for the safe and convenient immediate breast reconstruction modality if the patients have small, round and non-ptotic breasts and the sufficient breast skin can be saved with mastectomy. Methods: From July 2002 to July 2005, 29 breasts of 26 patients were restored only with the saline-filled breast implant immediately after the skin sparing or nipple-areolar skin sparing mastectomy in Asan Medical Center. A pocket with pectoralis major and serratus anterior muscle was created and the implant was covered with this muscle pocket. Simultaneous contralateral augmentation was performed in patients whose mastectomy specimen weighed less than 100g. Results: Using only the saline-filled breast implant resulted in the successful reconstruction with few complications including partial necrosis of nipple areolar skin (five cases, 17.2%), capsular contracture (three cases, 10.3%), hematoma (one case, 3.4%), depigmentation of areolar skin (one case, 3.4%), hypertrophic scar (one case, 3.4%), which were all healed by conservative management. There were no significant complications such as implant exposure and subsequent removal. Conclusion: Immediate breast reconstruction only with the saline-filled breast implant can be a satisfactory alternative option for the patients whose breast is small, round and non-ptotic, especially when the nipple-areolar skin of the breast is preserved in the mastectomy.

임프란트 지지 국소의치의 임상적 연구 (Clinical study of implant supported removable partial dentures)

  • OBJECTIVE The purpose of clinical study was to evaluate the survival of dental implants used in restoring patients with implant supported removable partial dentures (ISRPDs) of different configurations. MATERIALS AND METHODS The sample consisted of 20 consecutively treated partially edentulous patients, who, between 2003 and 2008, had a total of 84implants placed in different arch sites and who were treated with ISRPDs. The mean age was 57 years. Mean follow up time from delivery of ISRPDs was 3 years 2 months (range, 1 to 6 years). Osseointegration failure, postoperative complication of dental prosthesis, and the success rate of ISRPDs were retrospectively evaluated using clinical and radiographic examination. RESULT The overall implant survival rate was 100%. During follow up, the one clasp of removable partial denture was broken in 1 year11 months after prosthesis delivery. All patients were satisfied with their prosthesis. CONCLUSIONS Implant supported removable partial dentures could serve as favorable prognosis. Careful patient selection, with an appropriate maintenance and recall system, is recommended to obtain satisfactory results.

임플란트 환자의 치료 만족도 및 구강건강관련 삶의 질(OHIP-14)에 관한 연구 (A study on treatment satisfaction and oral health-related quality of life (OHIP-14) among implant patients)

  • Objectives: This study aimed to investigate factors affecting treatment satisfaction of patients with dental implants and quality of life. Methods: A questionnaire survey was conducted for adults who underwent dental implant treatment. at dental departments of general hospitals or dental clinics in Seoul and Gyeonggi-do. Data was analyzed to get descriptive statistics of variables, and was under independent t-test, one way ANOVA, and multiple regression analysis. Results: The mean of total satisfaction with implants was $3.73{\pm}0.41$ in the 5-point scale, and that of general satisfaction was $3.97{\pm}0.53$, followed by mean values of psychological satisfaction of $3.68{\pm}0.51$, aesthetic satisfaction of $3.67{\pm}0.49$, and masticatory function of $3.65{\pm}0.53$. Factors that affect implant treatment satisfaction among patients were monthly income (more than 4 million won), hesitation of the procedure (treatment period), regular checkup (yes) and concerns during treatment (work). Factors that affect the quality of life of patients who underwent implant treatments were satisfaction of aesthetic function and overall satisfaction. Conclusions: The above results showed that the higher the aesthetic satisfaction and overall satisfaction with the implant are, the higher the quality of life of patients get. Therefore, it is necessary for dental specialists to perform dental implants well to improve quality of life of patients.

Clinical study on screw loosening in dental implant prostheses: a 6-year retrospective study

  • Objectives: In this study, we determined the incidence and pattern of screw loosening in patients who received dental implants. Materials and Methods: Patients who received implants between January 2008 and October 2013 and completed their prosthetic rehabilitation were evaluated for the incidence, frequency, and onset of screw loosening using dental charts and radiographs. The association between each factor and screw loosening was analyzed using the chi-square test and a multivariate analysis with binary logistic regression models (P<0.05). Results: Total 1,928 implants were placed in 837 patients (448 males, 389 females), whose follow-up period after loading varied from 0.25 to 70 months (mean period, 31.5 months). Screw loosening occurred in 7.2% of implants. Most cases occurred less than six months after loading. Among those, 22.3% experienced recurrent screw loosening. Screw loosening was most common in the molar region (8.5%) and frequently associated with an implant diameter of ≥5 mm (14.2%). External implant-abutment connections (8.9%) and screw-retained implant prostheses (10.1%) showed higher incidence of problems than internal implant-abutment connections and cement-retained implants, respectively. Screw loosening was most common in implant prostheses with single crowns (14.0%). Conclusion: Within the limits of the current study, we conclude that the incidence of screw loosening differs significantly according to the position of implant placement, the type of implant and manufacturer, implant diameter, the type of implant-abutment connection, the type of retention in the implant prosthesis, and the type of implant prosthesis.

임프란트 수술 및 보철물에 따른 합병증 연구 (Study about the complications associated with implant surgery and prosthetic treatment)

  • Purpose : The aim of this study was to compare surgical complications between simple implant placement and implant placement combined with complicated surgical procedures. We also evaluated prosthetic complications according to the specific types of prosthesis. Material and Method : A retrospective analysis of dental chart of patients who was performed implant therapy during the period from June 2003 to December 2005 was carried out. This study was performed on 408 patients (208 male, 200 female). In addition, 1671 implants were performed. Based on their medical record and radiographs, the authors evaluated surgical and prosthetic complications, surgical procedures accompanied at the time of implant, risk factors of implant failure etc. Result : Surgical complications were developed on 358 implants(21.4% on total placed implants) and wound dehiscence was most prevalent complication. On maxillary posterior area, surgical complications developed more frequently on implants with major surgery and showed a significant difference. And complication rate of implants accompanied with GBR was higher than that of simple implants placement and also showed significant difference. The implant supported prosthesis showed no statistical difference in the occurrence of complications according to the types of prosthesis, and food retention was the most common post-prosthetic complication. Also we speculated that length and width of implant showed significant correlation to the failure of implant primary osseointegration. Conclusion : Based on the result, clinician should provide more careful maintenance for patients with implant placement accompanied by complicated surgical procedure. And periodic maintenance for the patient is requested for long-term survival of implant therapy.

  • Transmandibular implant system은 하악골이 심하게 흡수된 환자에게 특히 유용하게 사용할 수 있다. 또한 여러 문헌에서도 높은 성공률을 보이고 있다. 저자가 시행한 증례에서도 환자는 높은 만족감과 의치의 안정성 및 저작효율의 증대를 보였다. 하지만 성공적인 보철물의 제작을 위해서는 신중한 진단과 치료계획 및 위에서 열거한 여러 가지 고려사항들을 참고해야 한다.

  • During the past decade, the use of osseointegrated implants as a foundation for prosthetic replacement of missing teeth has become highly predictable and successful. SPT(Supportive Periodontal Therapy) identified as regular visits to the therapist for periodontal control and maintenance in a well-organized scheme, the number of appointments per year following a pre-designed subject-tooth/implant-site risk assessment method. Peri-implant disease was a frequent finding in subjects having natural healthy dentition and in subjects without periodontitis. Supportive periodontal program were found to be strongly related to implant survival. This study demonstrates that regular maintenance reduces the risk for peri-implant inflammation significantly as compared with irregular maintenance. This underlines the value of the SPT in enhancing the long-term outcomes of implant therapy, particularly in subjects affected by periodontitis, in order to control reinfection and limit biological complications. It is highly recommended to maintain implant patients under a strict supportive periodontal treatment protocol that might contribute to implant survival, and regular maintenance reduces the risk for periimplant inflammation significantly as compared with irregular maintenance. Ideally, patients may be informed on the beneficial effect of a regular patient-related post-therapy care before implant insertion.

A TWO-YEAR STUDY OF IMPLANT RETAINED OVERDENTURES IN THE TREATMENT OF TOTALLY EDENTULOUS JAWS

  • Statement of problem. Conventional denture treatment for totally edentulous patients is associated with a variety of functional and psychosocial problems. The placement of implants in the anterior region of the maxilla and mandible and the fabrication of an implant-retained overdenture might solve these problems. Purpose. This study compared the marginal bone loss around the implant and evaluated the implant survival rate and complications in patients treated with overdentures retained by implants for 2 years. Material and methods. Patients who had received implant-retained overdentures using a Dolder bar at Samsung Medical Center from January 1999 to June 2005 and had participated in the annual recall programs for two years were selected for this study. A total of 18 patients and 56 $Br{\aa}ne-mark\;system^{(R)}$ implants were used, and their data were reviewed. Evaluations of the survival rate, bone quality, marginal bone loss, and complications were performed. The data on the Dolder bar length and clip length were measured. The change in marginal bone loss and the correlation between the marginal bone loss and bar length, clip length, or bone quality were investigated. Results. Implants placed in this study showed a 100% survival rate. The average annual bone loss was 1.12mm in the first year and 0.27mm in the second year in the maxilla, and 0.58mm in the first year and 0.22mm in the second year in the mandible. The marginal bone loss in the maxilla showed no significant association with those in the mandible. (P>.05). There was no significant difference in marginal bone loss around implants between the first and second year. (P>.05) There was no statistically significant relationship (P>.05) between the marginal bone loss and bone quality, clip length, or Dolder bar length. The Dolder bar length showed a high correlation with the clip length. (P<.05) Various complications were noted. Conclusion. These results confirmed the favorable outcome for patients treated with implant-retained overdentures.
