Development of animal experimental periodontitis models

  • Purpose: An animal periodontitis model is essential for research on the pathogenesis and treatment of periodontal disease. In this study, we have introduced a lipopolysaccharide (LPS) of a periodontal pathogen to the alveolar bone defect of experimental animals and investigated its suitability as a periodontitis model. Methods: Alveolar bone defects were made in both sides of the mandibular third premolar region of nine beagle dogs. Then, the animals were divided into the following groups: silk ligature tied on the cervical region of tooth group, Porphyromonas gingivalis LPS (P.g. LPS)-saturated collagen with silk ligature group, and no ligature or P.g. LPS application group as the control. The plaque index and gingival index were measured at 0 and 4 weeks postoperatively. The animals were then euthanized and prepared for histologic evaluation. Results: The silk ligature group and P.g. LPS with silk ligature group showed a significantly higher plaque index at 4 weeks compared to the control (P<0.05). No significant difference was found in the plaque index between the silk ligature group and P.g. LPS with silk ligature group. The P.g. LPS with silk ligature group showed a significantly higher gingival index compared to the silk ligature group or the control at 4 weeks (P<0.05). Histologic examination presented increased inflammatory cell infiltration in the gingival tissue and alveolar bone of the P.g. LPS with silk ligature group. Conclusions: An additional P.g. LPS-saturated collagen with silk ligature ensured periodontal inflammation at 4 weeks. Therefore, P.g. LPS with silk ligature application to surgically created alveolar bone defects may be a candidate model for experimental periodontitis.

Temporal changes of periodontal tissue pathology in a periodontitis animal model

  • Purpose: This study aimed to characterize the early stages of periodontal disease and determine the optimal period for its evaluation in a mouse model. The association between the duration of ligation and its effect on the dentogingival area in mice was evaluated using micro-computed tomography (CT) and histological analysis. Methods: Ninety mice were allocated to an untreated control group or a ligation group in which periodontitis was induced by a 6-0 silk ligation around the left second maxillary molar. Mice were sacrificed at 1, 2, 3, 4, 5, 8, 11, and 14 days after ligature placement. Alveolar bone destruction was evaluated using micro-CT. Histological analysis was performed to assess the immune-inflammatory processes in the periodontal tissue. Results: No significant difference in alveolar bone loss was found compared to the control group until day 3 after ligature placement, and a gradual increase in alveolar bone loss was observed from 4 to 8 days following ligature placement. No significant between-group differences were observed after 8 days. The histological analysis demonstrated that the inflammatory response was evident from day 4. Conclusions: Our findings in a mouse model provide experimental evidence that ligature-induced periodontitis models offer a consistent progression of disease with marginal attachment down-growth, inflammatory infiltration, and alveolar bone loss.

Topical application of herbal formula for the treatment of ligature-induced periodontitis

  • Purpose: The aim of this study was to investigate the therapeutic effects of a herbal formula, PerioH-035, containing Angelica sinensis, steamed Rehmannia glutinosa, Angelica dahurica, Cimicifuga heracleifolia, and Zanthoxylum piperitum on the periodontal breakdown in a well-established ligature-induced periodontitis model in rats. Methods: Sprague-Dawley rats were randomly assigned to 1 of 4 groups: NL (non-ligatured), L (ligatured), P1 (ligatured and treated with 1 mg/mL PerioH-035), P100 (ligatured and treated with 100 mg/mL PerioH-035). Periodontitis was induced by placing a ligature around the mandibular first molars. PerioH-035 was topically applied to both sides of the first molar for 2 weeks. The right side of the mandibles was retrieved for micro-computed tomography (CT) and methylene blue staining to analyze alveolar bone loss. The left side of the mandibles was histologically analyzed by TRAP and H&E staining. The MMP-9 mRNA level in gingival tissue was investigated by RT-PCR. Results: Alveolar bone resorption was significantly reduced in the PerioH-035-treated groups. The number of dense multi-nucleated cells found to be TRAP-positive by staining in the ligatured rats was markedly decreased by PerioH-035 application. In addition, periodontal tissue destruction, especially cementum demineralization, was ameliorated in the P1 and P100 groups. Moreover, gingival tissue from the PerioH-035-treated group showed a decrease in the MMP-9 mRNA level, resulting in recovery of collagen degradation. Conclusions: These results suggest that PerioH-035 has therapeutic effects on periodontitis, and thus, PerioH-035 shows promise as a treatment for periodontitis.

  • The goal of periodontal therapy is the regeneration of the periodontium lost by periodontal disease. The purpose of this study was to evaluate the regenerative potential of the autogenous bone graft and guided tissue regeneration in the treatment of periodontal bony defect in dogs. Experimental periodontitis were induced in the mandibular left 3rd premolar and right 3rd and 4th premolars of 5 dogs using orthodontic ligature wire. After 6 weeks, the ligature wire removed, surgical procedure were performed as follows. 1) control group : Flap operation(Mn.Lt 3rd premolar) 2) experimental group I : Flap operation + autogenous bone graft (Mn.Rt. 3rd premolar) 3) experimental group II : Flap operation + Gore-Tex membrane (Mn.Rt. 4th premoalr) Thereafter, dogs were sacrificed on the 1,2,4,8,16th week and the specimens were prepared and stained with hematoxyline-eosin stain for the light microscopic examination. The results of this study were as follows. 1. The apical migration of junctional epithelium was most remarkable in the flap operation and the experimental group II was less than the experimental group I. 2. In the formation of new alveolar bone, it was found in experimental group I,II and experimental group I is more than II. In the control group, few bone formation was found. 3. In the formation of new cementum, it was found in experimental group I,II and experimental group II is more than I. So, the periodontal therapy combined with autogenous and guided tissue regeneration will be produce the periodontal regeneration.

임플란트 주위염 치료 효과의 프랙탈 분석 (Fractal analysis of the surgical treatment of ligature-induced peri-implantitis in dogs)

  • Purpose : To evaluate the effect of surgical treatment of ligature-induced peri-implantitis in dogs using fractal analysis. Also, the capabilities of fractal analysis as bone analysis techniques were compared with those of histomorphometric analysis. Materials and Methods : A total of 24 implants were inserted in 6 dogs. After a 3-months, experimental periimplantitis characterized by a bone loss of about 3 mm was established by inducing with wires. Surgical treatment involving flap procedure, debridement of implants surface with chlorhexidine and saline (group 1), guided bone regeneration (GBR) with absorbable collagen membrane and mineralized bone graft (group 2), and $CO_2$ laser application with GBR (group 3) were performed. After animals were sacrificed in 8 and 16 weeks respectively, bone sections including implants were made. Fractal dimensions were calculated by box-counting method on the skeletonized images, made from each region of interest, including five screws at medial and distal aspects of implant, were selected. Results : Statistically significant differences in the fractal dimensions between the group 1($0.9340{\pm}0.0126$) and group 3($0.9783{\pm}0.0118$) at 16 weeks were found (P<0.05). The fractal dimension was statistically significant different between 8($0.9395{\pm}0.0283$) and 16 weeks in group 3 (P<0.05). These results were similar with the result of the evaluation of new bone formation in histomorphometric analysis. Conclusions : Treatment of experimental peri-implantitis by using $CO_2$ laser with GBR is more useful than other treatments in the formation of new bone and also the tendency of fractal dimension to increase relative to healing time may be a useful means of evaluating.

16S rDNA 클론 library 제작 및 핵산염기서열 결정을 통한 교정용 미니임플랜트 주위 열구의 세균 동정 (Identification of bacteria from the peri-implant sulcus of orthodontic mini-implants using 16S rDNA clone library)

  • 교정치료 시 고정원으로 사용되는 교정용 미니임플랜트는 가동성 점막인 치조점막 부위에 식립해야 하는 경우가 많은데, 이때 미니임플랜트 주위 연조직의 증식을 동반한 염증이 빈번히 발생한다. 본 연구에서는 치조점막에 식립하여 연조직 증식이 발생된 미니임플랜트 주위의 세균총과 동일 환자의 인접한 건강한 치은 열구의 세균총을 비교하고자 하였다. 이를 위해 7명의 환자를 대상으로 하악 구치부의 치조점막에 식립하여 연조직 증식이 발생한 미니임플랜트 및 이에 연결된 결찰선 주위 열구의 세균막과 미니임플랜트에 인접한, 치은 염증이 없는 제2대구치의 치은열구의 세균막을 멸균된 paper point로 채취한 후, 16S rDNA 클론 library 제작 및 핵산염기서열 분석법을 이용하여 세균을 동정하여 비교하였다. 실험 결과 미니임플랜트 주위 열구로부터 304개의 16S rDNA 클론을 얻었으며, 치은열구로부터 238개의 16S rDNA 클론을 얻었다. 클론의 9.2%에 해당하는 24종의 세균들은 미니임플랜트 주위 열구에서만 검출되었고, 이들은 Haemophilus aphrophilus, Sphingomonas species, Capnocytophaga species, Prevotella melaninogenica, Lachnospiraceae species, Porphyromonas species, Neisseria flava 등이었다. 전체 클론의 80.4%에 해당하는 29종의 세균들은 미니임플랜트 주위 열구 및 건강한 치은 열구 모두에서 검출되었다. 이들 중 특히 미니임플랜트 주위에서 더 많은 클론이 분리된 세균들은 Prevotella species, Atopobium rimae, Veillonella species, Streptococcus intermedius/constellatus, Streptococcus salivarius 등이었다. 향후 연구에서는 본 연구에서 치조점막에 식립한 미니임플랜트 주위에서 검출된 세균들이 염증을 일으키거나 악화시킬 수 있는지를 밝히는 것이 필요하며, 이를 바탕으로 치조점막에 식립한 미니임플랜트 주위의 연조직 염증을 줄일 수 있는 방법을 찾는 것이 바람직하다.

Adjuvant therapy with 1% alendronate gel for experimental periodontitis treatment in rats

  • Purpose: The aim of this study was to evaluate the effects of locally delivered 1% alendronate (ALN) gel used as an adjunct to non-invasive periodontal therapy. Methods: Ligature-induced periodontitis was performed in 96 rats. The ligature was tied in the cervical area of the mandibular left first molar. The animals were randomly divided into 4 groups: 1) NT, no treatment; 2) SRP, scaling and root planning; 3) SRP/PLA, SRP followed by filling the periodontal pocket with placebo gel (PLA); and 4) SRP/ALN, SRP followed by filling the periodontal pockets with 1% ALN gel. Histomorphometric (percentage of bone in the furcation region [PBF]) and immunohistochemical (receptor activator of nuclear factor-κB ligand, osteoprotegerin, and tartrate-resistant acid phosphatase) analyses were performed. Data were statistically analyzed, with the threshold of statistical significance set at P≤0.05. Results: The SRP, SRP/PLA, and SRP/ALN groups presented a higher PBF than the NT group (P≤0.01) at 7, 15, and 30 days. The SRP/ALN group presented a higher PBF than the SRP/PLA group in all experimental periods, as well as a higher PBF than the SRP group at 15 and 30 days. No differences were observed in the immunohistochemical analyses (P>0.05 for all). Conclusions: Locally delivered 1% ALN gel used as an adjunct to SRP enhanced bone regeneration in the furcation region in a rat model of experimental periodontitis.
