  • 역행성 인공관절 전치환술은 가성마비가 동반된 복원 불가능한 회전근개 파열, 회전근개 파열 관절증, 어깨 관절의 골관절염 등에서 최근 널리 사용되고 있는 수술 방법이다. 역행성 인공관절 전치환술에 대해 서술할 수 있는 주제는 매우 다양하나, 본 종설에서는 역행성 인공관절 전치환술 시행 시 임플란트의 선택 기준에 대한 의견을 제시하고자 한다.

  • Implant treatment has become the treatment of choice to replace missing teeth in partially edentulous areas. Dental implants present different biological and biomechanical characteristics than natural teeth. Occlusion is considered to be one of the most important factors contributing to implant success. Most literature on implant occlusal concepts is based on expert opinion, anecdotal experiences, in vitro and animal studies, and only limited clinical research. Furthermore, scientific literature regarding implant occlusion, particularly in implant-supported fixed dental prostheses remains controversial. In this study, the current status of implant occlusion was reviewed and discussed. Further randomized clinical research to investigate the correlation between implant occlusion, the implant success rate, and its risk factors is warranted to determine best clinical practices.

  • "Dynamic stabilization" systems have been developed in recent years to treat degenerative disorders of the spinal column. In contrast to arthrodesis (fusion), the aim here is to conserve intervertebral mobility to maximize comfort. When developing innovative concepts, many mechanical tests need to be carried out in order to validate the different technological solutions. The present study focuses on the B Dyn$^{(R)}$ "dynamic stabilization" device (S14$^{(R)}$ Implants, Pessac, France), the aim being to optimize the choice of polymer material used for one of the implant's components. The device allows mobility but also limit the range of movement. The stiffness of the ring remains a key design factor, which has to be optimized. Phase one consisted of static tests on the implant, as a result of which a polyurethane (PU) was selected, material no.2 of the five elastomers tested. In phase two, dynamic tests were carried out. The fatigue resistance of the B Dyn$^{(R)}$ system was tested over five million cycles with the properties of the polymer elements being measured using dynamic mechanical analysis (DMA) after every million cycles. This analysis demonstrated changes in stiffness and in the damping factor which guided the choice of elastomer for the B Dyn$^{(R)}$ implant.

FEM을 이용한 cement type hip implant의 Stem 단면형상에 따른 응력분석 (The Stress analysis for a sectional stem shape of cement type hip implant using the finite element method)

  • The choice of suitable hip implant is one of important factors in total hip replacement (THR). In clinical view points, improper adaptation of hip implant might cause abnormal stress distribution to the bone, which can shorten the lifespan of replaced hip implant. Currently, interest in custom-designed hip implants has increased as studies reveals the importance of geometric shape of patient's femur in modeling and designing custom hip implants. In this study, we have developed the custom-designed hip implant models with various sizes in hip implant, and the stress distribution in the bone was analyzed using Finite Elements methods. It was found that minimizing the gap between implant stem and femoral cavity is crucial to minimize stress concentration in the bone.

  • For a missing teeth, orthodontic treatment may be a better choice of treatment in comparison to a conventional prosthetic replacement such as FPD, resin bonded prosthesis in view of aesthetics, periodontal health and function. Occasionally after an orthodontic treatment, an insufficient space may occur. The mini-implant could be an alternative in situations of narrow ridge dimension, where conventional root form implant could be compromised. The aim of this clinical report is to describe how a space that could not be restored with a traditional root form endosteal implant was managed and to present a technique to achieve optimal anterior esthetics in single implant restoration.

하악 부분 무치악 환자에서 Milled Bar와 부착장치를 이용한 임플란트 피개의치 수복 증례 (Implant overdenture using milled bar and attachment in partially edentulous mandible: a case report)

  • 부분 무치악 환자에 있어 심한 골소실으로 인해 치관부 수직높이가 과도해질 우려가 있는 경우에는 임플란트 식립 후 고정성 보철물로 수복시 임플란트 주변 골의 응력 증가 및 나사 풀림이 발생할 가능성이 증가할 수 있다. 이러한 경우에 milled bar 및 가철성 보철물로 수복할 수 있는데, milled bar는 단일 임플란트 간의 일차적인 스플린팅 효과와 안정성을 부여할 수 있다. 또한 milled bar 사용 시 Advanced Dental Device-Treatment Of Choice(ADD-TOC), 자석 부착장치 같은 부가적인 부착장치 사용을 통해 임플란트 피개의치의 추가 유지력을 부여할 수 있다. 본 증례의 환자는 하악 좌측 구치부 평활근육종으로 인한 절제술 후 심한 골소실 및 다수 치아를 상실한 부분 무치악 환자로 장기간의 하악 구치부 상실로 인한 대합치는 정출된 상태였다. 상악 좌측 대구치는 교정적 압하를 통해 교합평면을 회복하였고, 하악 좌측 대구치 부위는 임플란트 식립 후 milled bar에 부가적인 부착장치인 ADD-TOC, 자석 부가장치를 이용하여 임플란트 피개의치로 수복하였다. 이를 통해 장기적으로 기능적, 심미적인 만족할 만한 결과를 얻었기에 보고하는 바이다.

  • The evolution of breast reconstructive surgery keeps pace with the evolution of breast oncologic surgery. The reconstructive choice should always balance the optimal local control of disease and the best cosmetic result, reflecting an informed decision that is shared with the patient, who is always at the center of the decision-making process. Implant-based breast reconstruction following mastectomy represents a complex choice. In order to obtain optimal results, the following considerations are mandatory: thorough preoperative planning, a complete knowledge of the devices that are used, accurately performed surgery, and appropriate follow-up.

Segmental osteotomy for mobilization of dental implant

  • Purpose: The aim of this work is to evaluate a surgical technique for mobilization of mal posed dental implant in anterior area. Methods: A 38-year-old patient consulted our unit for esthetic dissatisfaction with the implant treatment of a central incisor. An implant was observed in 11 and 21, where 11 was 3 mm above the ideal limit, with excessive vestibular angulation. The choice was made to perform a segmental osteotomy and mobilize the bone block and the implant down and forward; a bone block extracted from the mandibular ramus was installed between the implant block and the bed to stabilize the segment. Results: After 4 months, a conventional fixed prosthesis was created and the esthetic result achieved was close to what the patient wanted, with no need for further surgery. The surgical condition was stabilized and maintained for the long-time and no complications how necrosis, infection or bone defects was present. Conclusions: It was concluded that the procedure is efficient, and the biological arguments in favor of the procedure are discussed.

하악 4전치 상실 시 임플란트 수복 (Mandibular 4 incisors implant restoration)

  • 하악 4전치 수복은 치아가 작고 공간도 좁아서 지대주 선택에 어려움이 따른다. 치주적으로 좋지 못한 상황에서 치아를 발거하는 경우가 대부분이고, 이러한 경우 치간공극을 크게 열어주는 것이 구강위생 관리 측면에서 유리하다1. 커스텀 지대주로 아무리 작은 직경으로 포스트 부위를 만든다 해도, 스크류가 들어가는 공간 때문에 작은 직경으로 지대주의 포스트 부위를 만드는 것에는 한계가 있다. One body implant2의 경우, post 부위를 넓은 정면으로 설정하는 것 보다는, 보다 d 얇은 측면(90도 회전면)으로 설정하는 것이 치간공극 형성을 위해 유리하다. Internal bone level implant 활용 시 two piece abutment를 활용하는 것 보다, one-piece abutment를 활용한다면 더 효과적으로 치간공극을 위한 공간을 확보할 수 있다.
