한국청년의 구강상태에 대한 방사선학적 연구 (THE STUDY OF ORAL CONDITIONS BY THE FULL MOUTH ROENTGENOGRAMS IN YOUNG ADULTS.)

  • The author examined 515 full mouth roentgenograms stored in the Dept. of Oral Radiology, College of Dentistry, Seoul National University. For evaluating the efficiency of the routine full mouth roentgenogram, each of abnormal conditions such as impacted teeth, missing, caries, crown, filling and apical lesions was observed. The results obtained were as follows; 1. Among 14.420 teeth examined, missing teeth were 174, impacted 16, caries 161 and treated 1,162. 2. The incidence of impacted third molar was close to 18.7 percent and the incidence of missing third molar was about 33.4 percent. 3. Among carious and treated teeth, 178 teeth (1.2%) were needed observation of root apex. And of these teeth, 119 teeth 0.8% were observed with apical lesion. 4. There is a considerable relation between the incidence or size of apical lesion and the accuracy of endodontical treat. 5. Among the teeth with apical lesion, upper and lower first molars were most frequent (about 41%), upper and lower canine were rare (about 1.7%).

매복지치 발치 시 발생한 합병증 양상에 대한 후향적 연구 (Complications of impacted third molar extraction: retrospective study)

  • Introduction: Surgical extraction of third molar is one of the most frequently performed procedures in oral and maxillofacial surgery unit as the impacted wisdom teeth could cause various complications. Even though, however, extraction of the impacted wisdom teeth is an obligation for the reason of possible complication even for general practitioners, it has been avoided. Various factors concerning surgical extraction of impacted third molar are considered: general condition of patients, relationship with relative anatomies, aspects of impaction, surgeon's skill. Materials and Methods: The consideration and crossing analysis of these factors with 2,463 patients who visit Dankook University dental hospital to extract those impacted third molar. Results: 1. Gender doesn't affect. 2. Medical problems have more complications. 3. There are more complications in high difficulty index (DI) impacted teeth. 4. When inferior alveolar canal overlap mandibular third molar, complication rate is 26.92%. 5. The most common complication was swelling and pain. 6. There is no statistical difference between the incidence of complication and surgeon's experience. Conclusion: In regard to these results, it seems that clinical or radiological examination can predict potential complications of wisdom teeth, and it is helpful to bear in mind the fact.

골성 고정원을 이용한 영구 대구치의 교정적 견인 : 증례 보고 (Orthodontic Traction of the Permanent Molar Using Skeletal Anchorage: A Case Report)

  • 매복된 영구 대구치의 치료 방법으로는 교정적 견인, 외과적 재위치, 자가치아이식, 매복치아의 발치 등이 있으며, 이 중 가장 보존적인 방법인 교정적 견인이 권장된다. 그러나 매복된 영구 대구치의 교정적 견인 시 치아를 이용한 고정원의 확보는 고정원의 상실을 동반할 수 있으며, 이러한 한계를 극복하기 위한 노력의 일환으로 골성 고정원 장치가 개발되었다. 본 증례 보고에서는 영구 대구치가 맹출하지 않는다는 주소로 본원에 내원한 3명의 환자에서 매복치아를 외과적으로 노출하여 교정 장치를 부착하고 골성 고정원 장치를 식립하여 성공적으로 견인하였음을 보고하는 바이다.

The effects of impacted premaxillary supernumerary teeth on permanent incisors

  • Purpose: The aim of this study was to examine the radiographic features associated with impacted premaxillary supernumerary teeth, to determine the relationship between their characteristics and their effects on permanent incisors, and to investigate the types of orthodontic treatment that patients received after the extraction of impacted supernumerary teeth. Materials and Methods: The clinical records and radiographs of 193 patients whose impacted premaxillary supernumerary teeth were removed were retrospectively reviewed, and 241 impacted supernumerary teeth were examined. Cone-beam computed tomographic images and panoramic radiographs were examined to determine the number, location, sagittal position, orientation, and morphology of the supernumerary teeth. Their effects on permanent incisors and the orthodontic treatment received by patients after the extraction of the supernumeraries were also investigated. Results: Supernumerary teeth were most frequently observed in the central incisor region, in the palatal position, in the inverted orientation, and were most commonly conical in shape. The most common complication was median diastema, followed by displacement and delayed eruption of the adjacent incisors. Ten (71.4%) of the 14 odontomas showed delayed eruption of the adjacent incisors. Displacement of the incisors was more frequently observed in association with supernumerary teeth with tuberculate or supplemental shapes. Orthodontic traction was most frequently performed after the removal of odontomas. In 32 cases (13.3%), permanent incisors erupted after the orthodontic creation of sufficient space. Conclusion: Median diastema was most common complication. The delayed eruption of incisors was common in supernumerary teeth with a vertical orientation and an odontoma shape.

  • Computer-aided navigation system is helpful in maxillofacial surgery with real time instrument positioning and clear anatomic identification. Generally, completely impacted tooth extraction surgery have e high risk by iatrogenic injury such as, adjacent tooth injury, normal anatomical structure injury. This case report describes performing extraction of impacted supernumerary teeth on anterior maxilla by using the navigation system in a 15 years old male patient.

매복치의 자가치아이식과 교정치료를 통한 심미적 치험례 (Orthodontic Treatment with Autotransplantation of Impacted Tooth)

  • Autotransplantation of tooth has recently become a combined method with orthodontic therapy. With the use of autotransplantation, it has become possible for orthodontists to solve complicated problems in the dental arches. Autotransplanted teeth showed the same development as their contralaterals with respect to both eruption and root growth. This indicates that autotransplantation and orthodontic treatment is a method that should be considered in case of impacted, missing, or malformed teeth. In this report two cases are presented to demonstrate that orthodontic treatment with autotransplantation can contribute to more satisfactory solution to the problem for impacted teeth.

  • 유착, 부적절한 교정치료, 과잉치, 낭종, 비정상적인 맹출경로등의 국소적인 원인으로 인하여 구치부의 매복이 발생할 수가 있다. 매복치는 적절한 치료를 하지 않으면, 교합기능의 상실, 영구치의 상실, 대합치의 정출, 인접치의 치근흡수등을 야기 할 수가 있다. 매복치를 성공적으로 맹출유도하기 위해서는 매복치의 치근형성여부가 중요한 요소가 되며, 매복치의 치근형태, 주위환경등을 유의한 적절한 교정력이 가해져야 한다.

  • Impacted teeth are a frequent phenomenon encountered by every clinician. The artificial eruption of embedded teeth is the process of directing an impacted tooth into normal occlusion. This procedure is currently attracting attention, with the aim of finding the best technique to use according to each case. This article presents key information regarding impacted incisors, canines, and premolars. In addition, we describe the most common techniques to use for artificial eruption, the open and closed techniques. We review the literature concerning these techniques and outline how clinicians can manage every type of impacted tooth.

  • Supernumerary teeth are extra teeth or tooth-like structures which may have either erupted or unerupted in addition to the 20 deciduous teeth and the 32 permanent teeth. Clinicians sometimes confront unerupted supernumerary teeth where implants would be placed. Many clinicians consider immediate placement of implant into an extracted socket as an effective technique compared to the conventional method of waiting several months for bone healing, because of the advantages such as reduction of treatment period, preservation of alveolar bone and soft tissue, etc. We could hardly find the case of immediate placement of implant after extraction of impacted supernumerary teeth and permanent teeth together. Therefore we report the case of immediate implantation following extraction of impacted supernumerary teeth and poor periodontal conditioned permanent teeth.

다수 영구치의 매복 및 맹출 지연의 교정치험례 (ORTHODONTIC TREATMENT OF MULTIPLE TEETH IMPACTION)

  • This report presents a case of adolescent patient who had multiple impacted teeth and no systemic disease, and was treated with removable and fixed orthodontic appliances. The results obtained through these cases were summarized as follows : 1. Even in the case without systemic disease, there can be the delayed eruption of multiple teeth 2. If the root formation is not completed, root have no excessive curvature, axial inclination of the tooth is not excessive, and there is eruption space, the tooth can be induced to normal occlusion. 3. Since esthetically proper occlusion and dentition was acquired by recovery of the impacted teeth to normal occlusion, the prosthodontic treatment after extraction of the impacted teeth was less needed. 4. This treatment brought psychological stability to pubertal patient who was interested in his facial profile.
