젊은 한국인 유방암 환자에서 BRCA1/2 돌연변이와 호르몬 수용체, HER-2 상태에 관한 연구 (A Study on BRCA1/2 Mutations, Hormone Status and HER-2 Status in Korean Women with Early-onset Breast Cancer)

  • 목적: 40세 이하의 젊은 여성 유방암 환자는 나이 든 여성 보다 BRCA1과 BRCA2 배선돌연변이의 빈도가 상대적으로 높다. 유방암 가족력이 있거나 젊은 나이에 유방암을 진단받은 백인 여성들의 BRCA1/2 돌연변이 암에 대한 연구에서 면역조직화학적으로 BRCA1/2 돌연변이 음성인 암과 다른 특성이 발견되었다. 이 연구의 목적은 유방암 가족력과 관계없이 젊은 나이에 유방암이 발생한 한국 여성을 대상으로 BRCA1/2 돌연변이 유뮤에 따라, 그리고 BRCA1과 BRCA2 각각의 돌연변이에 따라 면역조직화학적 특성으로 상호간의 구별이 가능한지, 면역조직화학적 검사로 BRCA1/2 돌연변이의 가능성을 알아보는 것이 가능한지를 조사하는데 있으며 BRCA와 관련된 암의 예후인자로서의 역할도 분석하였다. 대상 및 방법: BRCA1/2 검사를 시행한 40세 이하의 한국인 젊은 여성 유방암 환자의 유방암 수술 후 병리조직을 찾아서 조직미세배열법으로 슬라이드를 만들었다. 이 검체들의 병리조직, 등급, 림프절 전이, T 병기, 에스트로겐 수용체, 프로게스테론 수용체 및 HER-2 상태와 BRCA1/2의 관계를 비교하였다. 그리고 이 환자들의 BRCA 돌연변이 상태와 면역조직학적, 병리학적 상태와 예후 인자로서의 역할도 조사하였다. 결과: BRCA1/2 돌연변이를 조사한 101명 중 14명에서 16개의 돌연변이가 있었으며(13.9%), 유방암-난소암 가족력이 있는 경우(4/14, 28.6%), 양측성 유방암이 있는 경우(3/9, 33.3%)에 BRCA1/2 돌연변이 빈도가 높았다. 에스트로겐 수용체 음성인 경우 BRCA1/2 양성이 19.4%(12/62)로 에스트로겐 수용체 양성 비율 5.1%(2/37)에 비해 유의하게 높았고(p=0.038), HER-2 음성인 경우 BRCA1/2 돌연변이 음성 비율이 16.5%(13/79)로 양성 비율 4.5%(1/22)에 비해 높은 경향을 보였으며(p=0.073), 프로게스테론 수용체는 차이가 없었다. 에스트로겐 수용체, 프로게스테론 수용체 및 HER-2 모두 음성인 경우(triple negative)는 BRCA1/2 돌연변이 비율이 24.2%(8/33)로 매우 높았다. 종양의 크기, 림프절 전이 상태, HER-2 상태는 단변량 변수와 다변량 변수 모두에서 무병 생존에 유의한 영향을 미치는 인자 이었으나 BRCA1/2 돌연변이 상태는 무병생존에 유의한 인자가 아니었다. 결론: 면역조직화학적으로 에스트로겐 수용체나 HER-2 음성을 보이는 젊은 유방암 환자에서 BRCA1/2 돌연변이 발생이 유의하게 높았으며, 프로게스테론 수용체까지 모두 음성인 경우 BRCA1/2 돌연변이가 있을 확률이 24.2%로 높아서 유방암 유전자 돌연변이 가능성을 예측하는데 도움을 줄 수 있다. 한국인 젊은 여성 유방암 환자에서 BRCA1/2 돌연변이 상태가 무병생존에 유의한 인자는 아니었으나 좀 더 많은 환자와 긴 추적관찰 기간의 추가적인 연구가 필요하다.

C57BL/6 마우스의 등에 시행한 자가 전층피부이식편을 이용한 색소침착 동물모델 (Animal Skin Pigmentation Model Using Full Thickness Skin Graft in C57BL/6 Mouse)

  • Purpose: Skin grafting is one of the most commonly used methods in reconstructive plastic surgery field, but complications such as color change, contracture or hypertrophy are common problems. However, pathophysiology of the color change after skin graft is not yet determined and no animal model is established. Methods: Full thickness skin grafts were performed on the dorsum of C57BL/6 mice. Serial chronological gross inspection for color change and pigmentation were examined. Melanin pigments were traced by Fontana-Masson staining and semi-quantitative analysis was performed. In addition, immunohistochemical staining of S-100, Micropthalmia related Transcription Factor (MITF) and Melan-A antibodies were also performed to observe melanocytes and their changes. Results: After skin graft, color change and pigment spots were observed in the graft. Fontana-Masson staining showed melanin pigments in the epidermal and dermal layers in all mice. Immunohistochemistry staining to S-100, MITF, Melan-A antibodies showed melanocytes at the basal layer of epidermis and dermis. Conclusion: In conclusion, we have established an animal model for skin pigmentation after skin graft. We believe this study may be useful in understanding of the behavior of melanocytes after skin graft.

전침(電鍼)자극이 SHR 흰쥐의 Dentate Gyrus에서의 Doublecortin과 PSA-NCAM 양성 신경세포에 미치는 영향 (The Effects of electroacupuncture stimuli to the Doublecortin and PSA-NCAM positive cells in the Dentate Gyrus of spontaneously hypertensive rats)

  • Objective : This experimental in vitro study investigates the effects of electro acupuncture stimuli to the DCX and PSA-NCAM positive cells in the dentate gyrus of spontaneously hypertensive rats. Methods : We used immunohistochemical methods to observe the change of neuron cells, and then counts the immunoactive cells under the $100{\times}$ visual field optical microscope. Results : 1. The counts of DCX-positive cells in the dentate gyrus significantly increased in the 2Hz group versus control group. Otherwise 100Hz group did not have a significant differance. 2. The counts of PSA-NCAM-positive cells in the dentate gyrus rather decreased in the both 2Hz group and 100Hz group versus control group. Conclusions : We find out the electro acupuncture stimuli have some effects upon cerebral neuron cell in the dentate gyrus spontaneously hypertensive rats.

Association of Reduced Immunohistochemical Expression of E-cadherin with a Poor Ovarian Cancer Prognosis - Results of a Meta-analysis

  • Purpose: E-cadherin is a transmemberane protein which is responsible for adhesion of endothelial cells. The aim of our study was to assess existing evidence of associations between reduced expression of E-cadherin and prognosis of ovarian cancer with a discussion of potential approaches to exploiting any prognostic value for improved clinical management. Methods: We conducted a meta-analysis of 9 studies (n=915 patients) focusing on the correlation of reduced expression of E-cadherin with overall survival. Data were synthesized with random or fixed effect hazard ratios. Results: The studies were categorized by author/year, number of patients, FIGO stage, histology, cutoff value for E-cadherin positivity, and methods of hazard rations (HR) estimation, HR and its 95% confidence interval (CI). Combined hazard ratios suggested that reduced expression of E-cadherin positivity was associated with poor overall survival (OS), HR= 2.10, 95% CI:1.13-3.06. Conclusion: The overall survival of the E-cadherin negative group with ovarian cancer was significant poorer than the E-cadherin positive group. Upregulation of E-cadherin is an attractive therapeutic approach that could exert significant effects on clinical outcome of ovarian cancer.

Changes in the Serotonin-, Somatostatin- and Motilin-Immunoreactive Cells in the Gastrointestinal Tract of the Bullfrog, Rana catesbeiana, at Various Developmental Stages

  • In order to investigate the regional distribution and relative frequency of the serotonin-, somatostatin- and motilin-immunoreactive cells in the gastrointestinal tract GIT) of the bullfrog (Rana catesbeiana) with developmental stages, group of bullfrogs subdivided into the tadpole with hindlegs, metamorphosed bullfrog with tail, 2 wk after metamorphosed bullfrog and adult bullfrog, were stained by immunohistochemical methods (PAP methods). Multifarious changes of the regional distribution and relative frequency of the gastrointestinal endocrine cells were observed as developmental stages but motilin-immunoreactive cell ere not detected in this study. Serotonin-immunoreactive cells were detected from 2 wk after metamorphosed bullfrog in the whole GIT. The positive cells were predominant in the pylorus regions. In the adult bullfrog, these cells increased in fundus. On the other hand, somatostatin-immunoreactive cells were observed from the tadpole with hindlegs in the stomach regions. In the metamorphosed bullfrog with tail, they were found in the whole GIT except for the ileum, and increased with developmental stages except for rectum. In the rectum, these cells were not detected after that period. Our results show that most of the immunoreactive cells appear from completed metamorphosed periods when the histological structure of GIT is completely differentiated.

Immunohistochemistry of Janus Kinase 1 (JAK1) Expression in Vitiligo

  • Background: Vitiligo is a chronic autoimmune disease in which the destruction of melanocytes causes white spots on the affected skin. Janus kinase (JAK) is a family of intracellular, non-receptor tyrosine kinases that transduce cytokine-mediated signals via the JAK-signal transducer and activator of transcription pathway. The aim of the present study is to explore the possible role of JAK1 in the pathogenesis of vitiligo using immunohistochemical methods. Methods: The current study was conducted in a sample of 39 patients who presented with vitiligo and 22 healthy individuals who were age and sex matched as a control group. We used immunohistochemistry to evaluate JAK1 status (intensity and distribution) and assess the percentage of residual melanocytes using human melanoma black 45 (HMB45). Results: Intense and diffuse JAK1 expression was significantly more likely to indicate vitiliginous skin compared to normal skin (p<.001). Strong and diffuse JAK1 expression was associated with short disease duration, female sex, and lower percentage of melanocytes (detected by HMB45) (p<.05). Conclusions: JAK1 may be involved in the pathogenesis of vitiligo, as indicated by intense and diffuse expression compared to control and association with lower percentage of melanocytes detected by HMB45 immunostaining.

Immunohistochemical detection of GluA1 subunit of AMPA receptor in the rat nucleus accumbens following cocaine exposure

  • α-amino-3-hydroxy-5-methyl-4-isoxazole propionic acid (AMPA) receptors are differentially regulated in the nucleus accumbens (NAcc) of the brain after cocaine exposure. However, these results are supported only by biochemical and electrophysiological methods, but have not been validated with immunohistochemistry. To overcome the restriction of antigen loss on the postsynaptic target molecules that occurs during perfusion-fixation, we adopted an immersion-fixation method that enabled us to immunohistochemically quantify the expression levels of the AMPA receptor GluA1 subunit in the NAcc. Interestingly, compared to saline exposure, cocaine significantly increased the immunofluorescence intensity of GluA1 in two sub-regions, the core and the shell, of the NAcc on withdrawal day 21 following cocaine exposure, which led to locomotor sensitization. Increases in GluA1 intensity were observed in both the extra-post synaptic density (PSD) and PSD areas in the two sub-regions of the NAcc. These results clearly indicate that AMPA receptor plasticity, as exemplified by GluA1, in the NAcc can be visually detected by immunohistochemistry and confocal imaging. These results expand our understanding of the molecular changes occurring in neuronal synapses by adding a new form of analysis to conventional biochemical and electrophysiological methods.

조직 배양 모형에서 정상 구강 점막 상피에 대한 국소 유전자 요법 (TOPICAL GENE DELIVERY TO NORMAL ORAL EPITHELIUM USING ADENOVIRUS IN ORGAN CULTURE MODEL)

  • Background: Though it is clear that many types of viruses can infect the oral mucosa, its condition for infection is unclear. The purpose of this study was to analyze the conditions for viral infection of normal oral mucosa and explore the possibility of topical gene therapy to oral mucosa using a viral vector. Methods: Freshly taken fragments of the palate and the tongue of mice were used for organ culture. The specimens were exposed to green fluorescent protein (GFP)-adenoviral vector for 1 hour except for the control. Initial viral titer was $6.3{\times}10^{11}\;pfu/ml$ and the virus was diluted to working concentrations. The dilution ratio was 1:1,000 ($6.3{\times}10^8\;pfu/ml$), 1:10,000 ($6.3{\times}10^7\;pfu/ml$), and 1:100,000 ($6.3{\times}10^6\;pfu/ml$). They were then cultured on a stainless steel wire mesh in an organ culture dish. The specimens were stereoscopically examined every 24 hours for 6 days, after which they were fixed and analyzed through immunohistochemical methods Results: There was no visible expression in the control, $6.3{\times}10^6\;pfu/ml$, and $6.3{\times}10^7\;pfu/ml$ groups. Initial expression was observed at 24 hours after infection in both the palate and the tongue in $6.3{\times}10^8\;pfu/ml$ and the expression significantly increased until 3 days in the palate and 2 days in the tongue after infection (P<0.05). In both groups, the expression was mostly observed at the resection margin. Immunohistochemical studies showed that the epithelial cells were positive to GFP. Conclusion: The present study showed that topically applied adenovirus containing specific genetic information of GFP could successfully transduce GFP in normal oral epithelial cells at the resection margin in organ culture in terms of dose and exposure time.

발생단계에 따른 한국재래산양 췌장에서 pancreatic polypeptide 면역반응세포에 대한 면역조직화학적 연구 (Immunohistochemical study of pancreatic poly-peptide-immunoreactive cells in the pancreas of the Korean native goat (Capra hircus) during developmental stages)

  • 태아기, 신생아기, 생후 1개월령, 6개월령 및 성체의 한국재래산양 췌장에서 pancreatic poluypeptide(PP) 면역반응세포의 분포 및 출현빈도를 PAP 법에 의해 검색하였다. 외분비부에서는 모든 연령에서 관찰되었으며 췌장세포 사이에 분포하였다. PP 면역반응세포는 신생아가기에서 생후 1개월에 증가한 후 점차 감소하였다. 췌도관에서는 PP 면역반응세포가 생후 1개월부터 관찰되면 그후 점차 감소하였다. 췌도관에서는 PP 면역반응세포가 생후 1개월부터 관찰되면 그후 점차 감소하였다. 이들 면역반응세포는 생후 1개월령 및 6개월령의 췌도관 상피하 결합조직에서 소수 관찰되었다. 그러나 성체에서는 췌도관 상피세포 사이에서 극소수 관찰되었다. 그러나 성체에서는 췌도관 상피세포 사이에서 극소수 관찰되었다. 내분비부(췌장섬)에서 PP 면역반응세포는 신생아기부터 관찰되며, 생후 1개월령부터의 PP면역세포의 분포는 췌장성 주변부에 중등도 또는 소수가 산재하는 것과 다수의 PP 면역반응세포가 췌도 전체 (생후 1개월령)또는 췌도 중앙부(생후 6개월령)에 산재하는 것 등 두가지 유형을 나타내었다. 결론적으로 한국 재래산양의 췌장에 있어서 PP 면역반응세포의 분포와 출현빈도는 발생단계에 따라 상이하였다. 이는 발생단계 동안의 식이성 및 생리학적 조건변화가 이와같은 상이함을 유발하는 요인이 된 것으로 사료된다.

한우의 임신경과에 따른 황체조직의 광학 및 전자현미경적 변화 (Light and Electron Microscopical Changes of Corpus Luteum during the Course of Pregnancy in Korean Native Cows)

  • Corpus luteum (CL) is the primary productive organ of progesterone in pregnant cows. Progesterone levels in bovine plasma depend on the volume, weight and shape of the CL. Progesterone productions during the late stages of gestation occur both in the CL and placenta, and placentas producted more progesterone than CL on progesterone prcduction. Because division of progesterone production of these two organs is impoxxible, the CL function can not be determined by plasma progesterone levels following gestation stages. This study was carried out to evaluate histological findings on the CL spurium and CL verum, and also on the CL following the pregnant stages by histological and immunohistochemical and electron microscopical methods and then we expect to assume the functions of CL by histological findings. 1. Proliferations of luteal cells occur by day 120 of gestation, vessel hyperplasia occur by day 90 of gestation, and the walls and lumens of vessels developed by day 120 of pregnancy. 2. Sizes of CL cells increased to maximum around day 200 of gestation and similarly maintained by day 240. So these findings indicated that the function of Cl is most active around day 200 of gestation. 3. On parturation day, the number and size of luteal cells were maintained but stain intensity of the luteal cells and vessels are declined or disappeared, and fibrosis of luteal cells increased, and the vessel lumens are emptied. These findings indicate that CL is inactive. 4. In immunohistochemical findings, proliferative positive cells by PCNA antibody appeared more in number during early stages of gestation but appeared less following course of pregnant stages and not nearly appeared on day 120 of gestation. Apoptotic positive cells by TUNEL methods not nearly appeared on the early pregnant stages and a few appeared at late pregnant stages. So developments of CL proceed until day 120 of gestation and regression of CL was occurred by transform of luteal cells into fibrocytes than by luteal cell apoptosis. 5. In electron microscopical findings, the size of luteal cells increased more in CL verum than in CL spurium. During gestation stages, the size of luteal cells increased, mitochondria in the luteal cell cytoplasms densely and abundantly developed and also swelled mitochondria increased. The interspace of luteal cells are also dilated, transformation of luteal cells into fibrocytes are more number. The lumens and walls of peripheral capillaries of large luteal cells more broadened and thickened, and transformation of large and small luteal cells to fibrocytes are increased. The above findings suggest that function of pregnant CL more developed by day 120 of gestation and are most active around day 200 of gestation and similarly maintained by day 240 and are promptly regressed on paturation day.
