The Analysis of Risk Exposure and Ill Health Symptom by Difference Depending on the Special Type of Employment (특수형태근로 종사 여부에 따른 작업위험요인과 건강상 문제의 차이 분석)

  • Objectives: The object of this paper is to analyze difference of general and occupational characters, risk exposure and ill health symptom depending on the special type of employment. Methods: 50,007 participants from Fourth Korean Working Condition Survey conducted in 2014 was analyzed. Cross-tabulation analysis was conducted for deriving difference in general and occupational characters, mulitvariate logistic regression analysis was conducted after controlling general and occupational characters for deriving difference in risk exposure and ill health symptom depending on the special type of employment. Results: In the special type of employment, female, ages of 50, education levels of high school, monthly income of 2,000-3,000 thousand won, job types of sales person, enterprises of less than 100 workers, working duration of 2-10 years and long working hours worker's proportion was higher than general employment. Physical(OR: 0.84, 95% CI: 0.76-0.94) and ergonomic (OR: 0.48, 95% CI: 0.42-0.55) risk exposure was lower, psychological(OR: 1.77, 95% CI: 1.54-2.04) risk exposure was higher than general employment, and there was no significant difference between the special type of employment and general employment in chemical/biological risk exposure(OR: 1.06, 95% CI: 0.94-1.20) and ill health symptom(OR: 0.98, 95% CI: 0.89-1.09). Conclusions: This article offers the analysing difference between the special type of employment and general employment using huge size cross sectional data represent Korean employees.

Nursing Need of Patients with Chronic Illness - A Primary Study for Development for the Nursing Center of Chronic Illness - (만성질환자의 간호요구 사정 : 만성질환자 간호 센터 모형 개발을 위한 1차 연구)

  • The purpose of this descriptive study was to identify the nursing needs of patients with chronic illness. Method: The subjects of this study were 636 patients with chronic illness, 323 general nurses, and 106 public health nurses. The instruments used for this study were questionnaires including perceived functions of nursing from the center for chronic illness, preference to placement, intention to use, needs to receive services from the center for chronic illness, strategies management for nursing, and nursing needs of chronically ill patients. Results: The mean of perceived functions for nursing from the center were 3.1(0.5) in public health nurses, 2.9 (0.59) in general nurses, and 2.4(1.33) in chronically ill patients. Regards of needs to receive on services of the nursing centers were, the regularly physical examination, for health educational services which was perceived highest request amongst chronically ill patients. We found the means of each specific need as 2.2(0.6), for physical health, 2.1(0.7), for psychosocial health, and 1.8(0.6) for spiritual health. Conclusion: From the results of this study, it is suggested that establishing a nursing center for chronically ill patients consider physical, psychosocial, for spiritual health needs of chronically ill patients. It is also a consideration that direct care for symptom management and health education in the nursing center be implemented.

The effect of computer using work hours on work-related symptom experiences (업무상 컴퓨터 사용 정도가 작업관련 불건강 증상 경험 여부 및 회수에 미치는 영향 분석)

  • Employees used computer significantly(IT employee) is increased by technological development in workplace. This study tried to find the magnitude of IT employees and the profile of their working conditions and ill health experiences in South Korea. Korean Working Conditions Survey was used with descriptive and mean difference test in order to find the difference working conditions and ill health status among the groups classified by the degree of using computer during work. Some of working conditions was positively correlated with the density of computer use such as work autonomy, the exposure of repetitive movement of hand and arms. The pain of upper extreme was also more frequently found in the highest user group of computer than lower user groups. Based on the result of hierarchical regression analysis, the effect of computer using on the experience of work-related symptoms after controlling other factors was shown as U shape. This study as heuristic one suggest that some of depth analysis with hypothesis will be done to identify the causes and pathway of impact of computer use.

  • Flexible production system has been expanded to all sectors of economy with the change of labor market and diversification of employment. The unstable employment with irregular work has replaced stable employment with regular work. This study has investigate the impact pathway of the type of employment on health status especially ill-health symptoms experiences. Among the first Korean Working Conditions Survey data, the employee's response data was used to analyze the path way with multiple regression analysis. The result has shown the direct effect of the type of employment on ill-health symptoms experience. Indirect effect of the type of employment was found the pathway via the exposure to noise, high temperature, low temperature, dust, skin contact to chemicals, painful posture, heavy material handling, standing position, repetitive movement of hands. However the exposure to the other hazardous factors such as organic solvent, wholebody vibration, radiation, lifting people, infectious materials were not influenced by the type of employment.

Review of Nursing Research on Psychotropic Drugs in Korea (정신약물 복용과 관련된 국내 간호연구의 고찰)

  • Purpose: This study critically reviewed nursing research psychotropic drugs that has been published in Korean journals. Another aim of this study was to identify trends in nursing research on psychotropic drugs and make suggestions for further study in Korea. Methods: Data were collected from degree theses and original articles on psychotropic drugs published in Korean journals from 1992 to 2013. Thirty-four articles were analyzed of which at least one nursing author participated in the study. Search keywords were "psychotropic drug" and "mentally ill patient & medication". Results: For the research design, quasi-experimental study was 58.8%, descriptive study was 17.7%, descriptive correlational study was 8.8%, qualitative study was 8.8% and model development research was 5.9%. Variables measured were knowledge of medication & symptom management, knowledge of disease, side effects, drug attitude, medication pattern, diet & activity, quality of life, and self-care. Conclusion: Despite recent increased interest in psychiatric medication, research on psychotropic drugs remains very limited, particularly regarding findings from a nurse's perspective. More research project should be designed to develop programs for the treatment of side effects from a nursing view-point.

  • Tubercular peritonitis become ill suddenly or insidiously. Common clinical symptom is an abdominal pain which is spread out to all abdomen or extention of abdomen by a localized pain and ascites. Besides, there is fever, nausea, vomiting, diarrhea and general whole body sign by tuburclosis. We watched a tubercular peritonitis patient which is a woman in poor health after childbirth and investigated treatment process, every disease after childbirth and tubercular peritonitis. We treated her with Herb-Medicine for 30 days according to chief symptom's changes. The patient improved in clinical symptoms.

Factors Influencing Health Risk Behaviors of the Chronic Mental Illness in the Community (지역사회 만성정신질환자의 건강위험행위 영향요인)

  • The study was done to identify factors influencing the health risk behavior of the chronic mental illness in the community. A cross-sectional study design was used in this study. The sample was 255 chronic mentally ill persons from D city and C province and who agreed to participate in the study. Data were collected on August-september, 2011 and analyzed using the SPSS/WIN 20. Stress event, insight and depression had significant correlations with health risk behavior. Hierarchial regression analysis showed gender (men), diagnosis (schizophrenia), stress event, insight and depression together explained 24% of variance in health risk behavior. Findings of this study allow a comprehensive understanding of health risk behavior of the chronic mentally ill persons in community. It is necessary to integrated health promotion programs designed for this population should focus on these factors for effective behavioral modification.

A Cluster of Health Symptoms After a Law Enforcement Operation: A Case Study

  • Law enforcement officers (LEOs) often encounter rapidly changing and uncontrolled situations that expose them to various hazards. A law enforcement agency requested an evaluation by the National Institute for Occupational Safety and Health (NIOSH) when multiple LEOs reported illness after executing a search warrant and taking a suspect into custody. NIOSH investigators interviewed LEOs and reviewed medical records, forensic laboratory results for collected evidence, and environmental testing results of samples taken after the operation. Two-thirds (25 of 38) of LEOs who participated in the operation reported ≥1 symptom. Eleven LEOs met a case definition for influenza-like illness (ILI). Members of one unit were more likely to have ILI than non-members (prevalence ratio (PR), 4.1; 95% confidence interval (CI): 1.3-13.0; p = 0.01). Influenza vaccination was associated with a lower prevalence of ILI (PR, 0.2; 95% CI, 0.1-0.9; p = 0.02). Preventing employees from working while ill and annual influenza vaccination might prevent similar occurrences.

Loneliness and Life Satisfaction in Pneumoconiosis Patients Hospitalized in Long-term Care Facilities (요양기관에 입원중인 진폐증 환자에서 고독감과 삶의 만족도와의 연관성)

  • Purpose: Loneliness may be a typical psychological consideration in chronically ill patients, linking with low life satisfaction. We aimed to determine if loneliness would be independently associated with life satisfaction in pneumoconiosis patients hospitalized in long-term care facilities. Methods: We recruited 164 patients from 3 pneumoconiosis-specialized hospitals in M and S cities, South Korea, and measured loneliness and life satisfaction by using the University of California Los Angeles loneliness and life satisfaction index-Z scales. The Multiple regression analysis was performed to examine an independent association between loneliness and life satisfaction after adjustment for age, the levels of dyspnea and bronchitis-emphysema symptom, length of institutionalization, marital status, previous coal work experience, perceived socioeconomic status, and presence of caregivers. Results: The mean of loneliness was 46.4. We found a significant association between loneliness and life satisfaction (standardized beta=-0.16, p=0.049), independent of all the covariates. Conclusions: Loneliness was an independent associate of life satisfaction in patients with pneumoconiosis who were institutionalized in long-term care facilities. Thus, a nursing intervention strategy for relieving loneliness is needed to increase life satisfaction in these patients.

Factors Influencing Pain with Terminally Ill Cancer Patients in Hospice Units (호스피스 병동에 입원한 말기 암환자의 통증에 영향을 미치는 요인 : 입원 경과 시점에 따른 분석)

  • The purpose of this study was to investigate the impact of depression, discomfort, spirituality, physical care, and opioid use on pain with terminally ill cancer patients residing in hospice units. The convenient sample of this study consisted of 41 terminally ill cancer patients at three hospice units in university affiliated hospitals. Patients were interviewed with structured questionnaires three times at predetermined intervals: admission to the hospice unit (Time 1), one week later (Time 2), and two weeks later (Time 3). The data was collected from January 1998 to January 1999 and was analyzed using ANOVA, Pearson correlation coefficient, and multivariate multiple regression. 1. The mean age of the participants was approximately 55 years old. In terms of diagnosis, lung cancer showed the highest frequency (19.5%), followed by stomach cancer and rectal cancer (17.1%). The motive of seeking hospice unit admission was control (72. 2%), followed by spiritual care (50%), and symptom relief (38.9%). 2. Regarding the type of pain felt, the highest pain frequency the participants experienced was deep pain (55%), followed by multiple pain (25%), intestinal pain (10%), then superficial (5%) and neurogenic pain (5%). For the level of pain measured by VAS, there was no significant difference among the three time points; Time 1 (5.04$\pm$2.21), Time 2 (4.82$\pm$2.58) and Time 3(4.73$\pm$2.51). 3. There was significant change seen in spirituality and physical care in each time interval. Namely, the longer the length of admission at the hospice unit, the higher the importance of spirituality (p=0.0001) and the more the physical care the participants received (p=0.01). The opioid use at the three time points showed the following frequencies : Time 1 (75.6%), Time 2 (85.4%) and Time 3 (75.6%). 4. Regarding factors influencing pain, the pain level was significantly affected by the depression level (p〈0.01) and the opioid use (p〈0.1). These results were the most significant at the two time points (Time 1 and Time 2). At Time 3 (two weeks later), the pain level was significantly affected by the depression level (p〈0.05) and the amount of physical care the participants received (p〈0.1). In conclusion, the terminally ill cancer patients had moderate pain, were generally depressed, and were treated with opioid analgesics. As approaching death, the patients received more physical care due to increased physical symptoms experienced and they had a higher perception of the importance of spirituality. Thus, health care professionals need to provide continuous care for each of them to die comfortably physically, psycho- logically, and spiritually.
