Globular Adiponectin Exerts a Pro-Inflammatory Effect via IκB/NF-κB Pathway Activation and Anti-Inflammatory Effect by IRAK-1 Downregulation

  • Adiponectin, a hormone produced by adipose tissue, is very abundant in plasma, and its anti- and pro-inflammatory effects are reported. However, the mechanisms of these pro- and anti-inflammatory effects are not fully defined. Herein, we evaluated the dual inflammatory response mechanism of adiponectin in macrophages. Short-term globular adiponectin (gAd) treatment induced $I{\kappa}B{\alpha}$ degradation, $NF-{\kappa}B$ nuclear translocation, and $TNF-{\alpha}$ production in RAW 264.7 cells. Polymyxin B pretreatment did not block gAd-induced $I{\kappa}B{\alpha}$ degradation, and heated gAd was unable to degrade $I{\kappa}B{\alpha}$, suggesting that the effects of gAd were not due to endotoxin contamination. gAd activated IKK and Akt, and inhibition of either IKK or Akt by dominant-negative $IKK{\beta}$ ($DN-IKK{\beta}$) or DN-Akt overexpression blocked gAd-induced $I{\kappa}B{\alpha}$ degradation, suggesting that short-term incubation with gAd mediates inflammatory responses by activating the $I{\kappa}B/NF-{\kappa}B$ and PI3K/Akt pathways. Contrastingly, long-term stimulation with gAd induced, upon subsequent stimulation, tolerance to gAd, lipopolysaccharide, and CpG-oligodeoxynucleotide, which is associated with gAd-induced downregulation of IL-receptor-associated kinase-1 (IRAK-1) due to IRAK-1 transcriptional repression. Conclusively, our findings demonstrate that the pro- and anti-inflammatory responses to gAd in innate immune cells are time-dependent, and mediated by the activation of the $I{\kappa}B/NF-{\kappa}B$ pathway, and IRAK-1 downregulation, respectively.

산후조리원의 산모의 신생아 돌보기 교육 요구도와 교육 만족도 (The Educational Needs and Satisfactory about Infant Care of the Mothers in the Post-partum Care Center)

  • This study was designed to compare the educational needs and satisfactory about infant care of the mothers in the Post-partum Care Center for development of the educational program. The subjects for this study were 100 mothers in the Post-partum Care Center in Seoul and Buchon. The data were collected during the period from Feb. to March, 2000. The Educational Need and satisfactory for Infant Care was measured by questionnaire that has developed by researchers. The data were analysed by descriptive statistics, paired t-test using SPSS PC+ WIN. The results were as fellow; 1) The educational needs of the mothers in the Post-partum Care Center were high (mean=4.32). 2) The satisfactory of the mothers in the Post-partum Care Center of educational program of infant care were middle range (mean=3.60). 3) The educational needs of infant cares in the Post-partum Care Center were higher than satisfactory of the mothers in the Post-partum Care Center of educational program of infant care(t=7.71 p=.000). 4) The educational need by content was signs, symptoms and management of disorders(i.g. cyanosis, seizure, fever, etc.) were higher than general care of baby(i.g. feeding, diaper change etc.). In conclusion, when the infant caregiver in the Post-partum Care Center teaching the method of infant care to mother, there should be in consideration of educational need of mother. And its educational contents must be included of mental and emotional development and signs, symptoms and management of disorders.

  • The Purpose of this study was to review the theoretical background of burnout in child care teachers organization. Personal factors, Organizational factors, social factors and the factors affecting burnout I examined. The results of this study were as follows. First, Child care teachers need to be burnout to participate in Prevention programs. Second, child care teacher is required to ensure proper staffing, improved working environment through compliance with statutory working hours. Third, Various institutional arrangements required to control the variables affecting child care teachers. Fourth, Child care teacher's role to prevent excessive strategy is needed.

환자의 신체기능적 능력(Self-Care Status)별 소요되는 간호시간 결정에 관한 연구 (Study on the Determination of Nursing Hours by Self-Care Status of Patients)

  • This study was undertaken to delineate the relationship between numerical score and the amount of nursing hours required in the nursing process. Score was a numerical description of the patients functional nursing needs. Therefore this study focused on standard nursing hours required by patient's self-care status. This study observed the 62 patients and 15 R.N. in H. university hospital from Aug. 7, 1982 to Aug. 13, 1982. 1. For the first time, each head nurse assessed self-care status by Schoening's self-care score-Minimal care patient (self-care score: 23, 24) was placed in Group Ⅰ, intermediate care patient (self-care score: 11∼22) was Group Ⅱ, and special care score: 0∼10) was Group Ⅲ. 2. We observed and recorded the nursing care received from nurses according to patient's group. (8AM∼4PM) 3. And, We observed and recorded the activities of nurses in order to determine standard nursing hours required. (8AM∼4PM) 4. If we apply the content of paragraph 3 to paragraph 2, we will predict the number of patient that nurse can care during day time by self-care status. The following results were obtained: 1) Patient's mean self-care score were Group I : 23.9 score Group Ⅱ:17.8 score Group Ⅲ : 1.6 score 2) Nursing hours required by patient's physical function(self-care status) status were Group I : 35 min. Group Ⅱ: 47.5 min. Group Ⅲ : 104.6 min. 3) Nurse's nursing time and distribution required in nursing activities during day duty were A.D.L. : 84.3min. (17.56%) Functional nursing activities : 279.9min. (58.31 %) Education & Emotional support : 11.3min. (2.35%) Task unrelated patients : 54min. (11.25%) Non Productive nursing care : 50. 5min. (10.52%) 4) Mean nursing hours required by each patient and the number of patient that nurse can rare during day duty by self-care status were Group I : 38.6min. 11.1 patients/1 nurse Group Ⅱ : 51.1min: 8.4 patients/1 nurse Group Ⅲ: 108.2min. 4 patients/1 nurse It seems reasonable that this could be done effectively as each-unit has an established standard for hours required, This not only allows time for planning of staff but helps to avoid the very human inclination to predict excessive staffing requirements by placing the majority of patients in high care group.

간호대학생의 임상실습 전 영적 간호경험 I : 사례연구를 중심으로 (Experiences in Spiritual Nursing Care by Student Nurses before Starting Nursing Practice in Hospital: Part I)

  • The purpose of this study was to describe the spiritual care experiences of student nurses after they have received 2 credits in a spiritual nursing care course at a university in Seoul, Korea. The major finding are as follow: 1 The problems that students reported for their patients were. loss of self- confidence, anxiety about outward appearance, anxiety about his(her) health and illness, maladaptability, guilt, problems with dating, uncertainty about his (her) future. 2. The methods which students used to help solve the patients' problems were: prayer with patients, use of Scripture, conversation, advice, frequent meetings listening, frequent phone call, writing letters, exploring problem solving methods together, and introduction of similar patients. 3. After the students had experienced spiritual nursing care they felt satis-faction, lack of knowledge of spiritual care, understood that spiritual nursing care courses at the university are important for education and experience, and understood the need for experience to increase sensitivity to the spiritual needs of their patients. Conclusion : Maintaining spiritual wellness is a important as maintaining physical fitness and essential for optimal well being. Therefore educating student nurses in developing and maintaining spiritual wellness is essential in order for them to help their patients achieve holism.

전문가 구강위생관리가 장기요양시설 재원노인의 구강상태에 미치는 효과 (Effect of Professional Oral Healthcare Program on the Oral Status of Elderly Residents in Long-Term Care Facilities)

  • 본 연구는 장기요양시설 재원노인을 대상으로 2016년 3월부터 2016년 5월까지 전문가 구강위생관리 프로그램을 적용하고 재원노인의 구강위생상태 개선 효과를 파악하기 위하여 수행되었다. 전문가 구강위생관리와 노인구강보건 교육을 제공받은 요양보호사에 의한 일상 구강관리를 중재한 32명의 실험군 I, 노인구강보건교육을 제공받은 요양보호사에 의한 일상 구강관리를 시행한 30명의 실험군 II, 요양보호사에게 일상구강관리를 제공받은 32명의 대조군으로 구분하여 치면세균막 지수, 구취, 설태, 타액 분비량의 개선 효과를 보고자 하였다. 치면세균막 지수는 반복측정분산 분석 결과 실험군 I, 실험군 II, 대조군 사이에 유의한 차이가 있었으며 중재기간에 따른 변화에도 유의한 차이가 있었다. 또한 중재 방법에 따라서 치면세균막 지수의 변화 양상은 유의한 차이를 보였다. 구취는 실험군 I, 실험군 II, 대조군 사이와 중재기간에 따른 변화에 유의한 차이가 없었으나 중재 방법에 따라서 구취의 변화 양상은 유의한 차이를 보였다. 설태는 실험군 I, 실험군 II, 대조군 사이에 유의한 차이가 없었으나 중재기간과 중재 방법에 따른 설태의 변화 양상은 유의한 차이를 보였다. 타액 분비량은 실험군 I, 실험군 II, 대조군에 따른 변화와 중재기간에 따른 변화에 유의한 차이가 없었으나 중재 방법에 따라서 타액 분비량의 변화 양상은 유의한 차이를 보였다. 전문가 구강위생관리와 요양보호사에 대한 노인구강교육 실시가 재원노인들의 구강위생 상태에 영향을 미치는 것이 확인되었기에, 재원노인의 구강위생 상태를 개선하기 위해 치과 촉탁의 제도와 병행한 전문가 구강위생관리를 주기적으로 수행할 필요가 있으며, 직원을 포함한 간호 인력을 대상으로 노인 구강위생교육을 제공할 필요가 있다.

  • Three coumarin derivatives, which stimulate collagen biosynthesis in human fibroblasts were isolated from Angelica dahurica root which has been used in traditional medicine for cold, headache, discharging blood and women's diseases. Three coumarin derivatives were identified as 5-benzofuranacrylic acid, $6,7-dihydroxy-4-(3-methyl-2-butenyl)-{\delta}-lactone\;(prangenidin)$, 5-benzofuranacrylic acid, $6,7-dihydroxy-4-methoxy-{\delta}-lactone\;(8-hydroxybergapten)$ and 5-benzofuranacrylic acid, 8-hydroxypsoralen(xanthotoxol) from their spectral data.

  • Applying evolved IT technology to increase the satisfaction of the consumer is a typical feature of the rapidly transformed service industry. Smart devices have taken a place as the mainstream of major media, moreover, IT service strategies utilizing smart devices have been constantly developed. Location recognition method of users has been proposed as one of the significant features to the IT service industry. The long-term care home services can be one example of user location recognition methods that real time computerization of service record utilize an attached tag to home of pensioners with RFID reader or an NFC function of mobile when a home carer provides service. And, the Homecare Electronics Management System (HEMS) and the various location recognition methods will be discussed to improve effectiveness of services. In this paper, we propose a home applicable electronic management system which insurer, home care service facilities, home cares, pensioners and guardians which enables to simultaneously check service records based on the improved system by use of iBeacon.

간호업무량 측정 및 간호인력 수요산정 (Measurement of the Nursing Staff Needed for Two Specialized Nursing units in a University Hospital)

  • This study investigated a process to estimate the need for nursing staff on the basis of a patient elassification system and the required care needs and activities. The investigation was carried out in the following four steps. Step 1. Patients were classified according to the amount of nursing care need on each shift as class I (mildly ill), class II (mederately ill), class III (acutely ill), and class IV (critically ill). Step 2. Measurement of the direct nursing care hours needed for each patient class, and measurement of indirect nursing care hourse and personal time of the nursing staff. Step 3. Calculation of he total nursing workload in a nursing unit. Step 4. Estimation of the nursing staff needed. The investigation was carried out from July 17th to 30th, during 24hours every other day. The subjects were the patients and the nursing staff on two units of Seoul National University Hospital, Korea. Some of the results from the investigation are as follows : 1) Distribution of patient classification On the neuro surgical (N.S.), the distribution was class I, 22 patient, 3, class II, 27 patients, class III, 26 patients, and class IV, 25 patients, For the orthopedic surgical unit(0.5.), it was class I, 43 patients, class II, 43 patients, class III, patients, and class IV, 3 patients. 2) Direct nursing care hours per day On the N.S. unit, 3.2 hours of direct nursing care were needed for class I, 3.9 hours for class II, 5.1 hours of class III, and 6.2 hours for class IV patients, while 2.0 hours for class I, 2.5 hours for class II, 3.5 hours for class III, 5.0 hours class IV patients were needed on the 0.5. units. 3) Analysis of direct nursing care activities Activities were classified into assessment and observation(47%), medication(38.7%), communiontion(5.1%), exercise(2.4%), elimination and irrigation(1.3%), treatmemt(1.1%), hygiene(0.8%), nutrition(0.8%), and hot and cold compress(0.1%). 4) Average hours of indirect nursing care per day. On the N.S. unit 4.2 hours, and on the O.S. unit, 3.5 hours of RN indirect care was needed. 5) The average personal time used by the of nursing staff was 17 minutes for both RNs and nursing assistants in the N.S. unit, and 32 minutes for both RNs and nursing assistants in the O.S. unit. 6) Estimation of nursing staff needed on two specialized units of a university hospital For the N.S. nursing unit of 43 beds, 31 nursing staff would be indicated. For the 0.5. nursing unit of the same number of beds, 19 nursing staff would be indicated.
