  • The experimental study have showed that the Polygonatum officinale All. contains diosgenin and ${\beta}-sitosterol$ as active components. Even though many experimental studies were reported, still there are many unknown actions of components from the plant. The purpose of this study was to elucidate the mechanism of hypoglycemic action of ether extract of Polygonatum officinale. The hypoglycemic effect of the ether extract was tested in hyperglycemic rats induced experimentally, The effect on plasma free fatty acid and the acute toxicity were also examined by using mouse. The hypoglycemic effect of the ether extract was compared with other oral hypoglycemic agents such as chlorpropamide and buformin. The ether extract of Polyonatum officinale showed a marked hypoglycemic effect by 1 hour after oral administration in hyperglycemic rats and rabbits but the ethanol extract did not exhibit the hypoglycemic effect. The hypoglycemic effect of the ether extract was potentiated by chlorpropamide while buformin was weak.

  • Metformin which is an oral hypoglycemic agents, acts by enhancing insulin sensitivity, decreasing hepatic glucose production and increasing peripheral utilization of glucose. Deliberate self poisoning with oral hypoglycemic agents is rare. The lactic acidosis associated with metformin toxicity is well described in the medical literature. Metformin overdose even in otherwise healthy patients may produce a profound and life threatening lactic acidosis. We report a case of massive metformin ingestion(75g) in a patient presenting with lactic acidosis and hypotension. She died 24h after presenting to our emergency department despite bicarbonate treatment and hemofiltration therapy.

Biochemical Studies on Hypoglycemic Agents (I) Effect of Azadirachta indica leaf extract

  • It was confirmed that extracts of some plants were used in folklore medicine as hypoglycemic agents. Of these plants is Azadirachta indica ("neem") which grows in tropical regions. The present study deals with biochemical effects of the "neem" leaf water extract given orally to experimental aminals, especially the hypoglycemic characteristics. Normals as well as alloxan diabetic rats have been used in this work. The results showed that the "neem" leaf extract produced some hypoglycemia in normal rats when given in two doses, while in diabetic rats there was a decrease in blood sugar, but it could not alleviate the diabetic in body weight loss and high percentage mortality, especially with a high dose. It was observed increased with concomitant decrease in the liver fat as compared to normal levels. There was also a drop in liver proteins which was dose-related. The results were compared wiht those obtained with an oral hypoglycemic drug (Glibenclamide).glycemic drug (Glibenclamide).

Biochemical Studies on Some Hypoglycemic Agents (II) Effect of Calligonum comosum extract

  • People in some tropical countries used to use some plant extracts in folklore medicine as treatment of diabetes melitus. Of these plants is Calligonum comsum ("orta"). The present work deals with the biochemical effects of the whole plant water extract given orally to the albino rats, normals and alloxan diabetics, as to fulfil its hypoglycemic effects on rats. The results showed that "orta" extract produced a hypoglycemic effect indicated by the decrease in blood sugar level. It was observed that the diabetic state in rats treated with 300 mg/kg body weight of "orta" extract, was alleviated, showeing normal levels of blood glucose, liver fat and cholesterol contents. Liver proteins were still below the normal level in the rats. Gibenclamide, a hypoglycemic drug, was used for comparison with "ora" extract. Although it did alleviate the diabetic state, yet the liver fat and cholesterol contents were still higher than those of normal. Also the liver proteins were lower than the control levels.

기존의 경구용 혈당강하제로 혈당조절이 불량한 제2형 당뇨병 환자에 대한 한양방 병용 치료 1례 (A Case Report of Add-On Traditional Korean Medical Treatments in a Patient with Uncontrolled Type 2 Diabetes Mellitus with Conventional Oral Hypoglycemic Agents)

  • This study presents a case of add-on traditional Korean medical treatments in a 77-year-old male patient with uncontrolled type 2 diabetes mellitus with conventional oral hypoglycemic agents. Chief complaints were uncontrolled glucose level and weight loss. The patient was treated with electroacupuncture (on ST36, CV4, and CV12), herbal medication, and lifestyle advice. The conventional oral hypoglycemic medications were maintained. After 12 weeks, the glycated hemoglobin A1c (HbA1c) decreased from 11.0% to 8.4%. There were significant abnormalities in the liver function and renal function tests, but no adverse event was reported. Further studies are needed to confirm the findings of this case.

오적산 병용투여가 혈당강하 치료를 받고 있는 제2형 당뇨병 환자의 혈당에 미치는 영향 (A Retrospective Study on the Effect of the Co-Administration of Ojeok-san and Hypoglycemic Agents on Blood Glucose Levels in Type 2 Diabetes Mellitus)

  • Objective: This study was conducted to investigate the hypoglycemic effect and safety of Ojeok-san in patients with type 2 Diabetes Mellitus. Methods: We investigated type 2 diabetes mellitus patients at Kyung-Hee University Korean Medical Hospital who were administered Ojeok-san for at least one day between January 2012 and September 2020, basal characteristics and laboratory tests were reviewed retrospectively. The hypoglycemic effect of Ojeok-san was assessed by comparing fasting blood sugar (FBS) and two hours post-prandial plasma glucose (PP2) levels from before and after taking Ojeok-san. Subgroup analyses were conducted according to baseline hypoglycemic treatments and glycated hemoglobin levels (< or ≥6.5%). The safety of Ojeok-san was assessed by comparing levels of aspartate aminotransferase, alanine aminotransferase, γ-glutamyl transferase, blood urea nitrogen, and creatinine. Results: After Ojeok-san administration, FBS and PP2 were significantly reduced to an average of 14.33 mg/dL and 27.67 mg/dL respectively. In the subgroup analysis, PP2 in patients receiving metformin mono therapy was significantly reduced to 28.67 mg/dL, and those receiving a dual therapy of metformin and DPP-4 inhibitor, it was significantly reduced to 32.33 mg/dL. In patients with glycated hemoglobin of lower than 6.5%, FBS was significantly reduced to 12.20 mg/dL, and both FBS and PP2 were significantly reduced, to 15.50 mg/dL and 40.00 mg/dL, respectively, in those with glycated hemoglobin levels of more than 6.5%. The safety profile showed no significant difference after Ojeok-san administration. Conclusions: Ojeok-san has significant hypoglycemic effects in patients with type 2 diabetes mellitus who are also taking hypoglycemic agents.

  • 3, 4-Dihydro-3-oxo-2H-1, 2, 4-benzothiadiazine-1, 1-dioxides were synthesized as conformationally rigid analogues of sulfonylurea hypoglycemic agents. All the compounds prepared showed hyperglycemic activity. The hyperglycemic activity was enhanced for the diabetic rats in which the blood glucose level was increased by 30-70%.

술포닐 우레아 유도체들의 형태분석 (Conformational Analysis of Sulfonylureas)

  • To determine the optimal conformation of sulfonylureas, the correlation between conformation and hypoglycemic activity of the two sulfonylureas of tolbutamide and chlorpropamide as hypoglycemic agent was studied using an empirical potential function (ECEPP/2) and the hydration shell model in the unhydrated and hydrated states. The conformational energy was minimized from several starting conformations with possible torsion angles in each molecule. The conformational entropy change of each conformation was computed using a harmonic approximation. To understand the hydration effect on the conformation of the molecules in aqueous solution, the contribution of water-accessible volume of each group or atom in the lowest-free-energy conformation was calculated and compared each other. From comparison of the computed lowest-free-energy conformations of two sulfonylureas, it could be suggested that the hydration of sulfonylurea moiety is related to increase the hypoglycemic activity. From the calculation results, it was known that the conformational entropy is the major contribution to stabilize the low-free-energy conformations of two sulfonylureas in unhydrated state. Whereas, in hydrated state, the hydration free energy largely contributes to the total free energies of low-free-energy conformations of tolbutamide and conformational entropy contributes to stabilize the low-free-energy conformations of chlorpropamide. The torsion angles from phenyl ring to urea moiety of the low-free-energy conformations of the two sulfonylureas were shown the nearly regular trend. On the basis of these results, the conformation exhibiting the optimal hypoglycemic activity of sulfonylureas and the binding direction to pancreatic receptor site A could be predicted. Also, according to the side chain lengthening of urea moiety, tolbutamide showed various conformational change. Therefore, steric effect may be important factor in the interaction between sulfonylureas and the putative pancreatic receptor.

  • A series of 2-oxiranecarboxylate derivatives was prepared as carnitine palmitoyl transferase I (CPT- -I) inhibitors for the development of new antidiabetic agents. The syntheses and biological activities were reported. The most promising derivative, 15b showed 2.5 times more hypoglycemic activity and 2 times lower acute toxicity compared to Etomoxir.
