Effect of Majoon Idraare Haiz in Polycystic Ovarian Syndrome - A pilot study

  • Background & Objectives: Polycystic ovarian syndrome (PCOS) is one of the commonest endocrine abnormality in women of reproductive age affecting from 4% - 21% of the reproductive women and is characterized by chronic anovulation and hyperandrogenism. The aim of the study was to evaluate the effect of majoon idraare haiz in menstrual regulation and morphological changes in ovaries in poly cystic ovarian syndrome. Methods: A Pilot study was carried out in the department of Ilmul qabalat wa amraze niswan, National institute of unani medicine, hospital, Bengaluru. Fifteen Patients of PCOS aged 18-35 diagnosed using Rotterdam criteria were included in the study. Patients with insulin sensitizing treatment within 3 months, hormonal treatment and those with h/o diabetes mellitus, hypertension, pregnant and lactating women were excluded.Majoon idraare haiz was administered orally at a dose of 10 g with 20 ml arqbed mushk once daily from fifth day of cycle for 21 days for three consecutive cycles. Primary outcome measure was menstrual regularity while changes in USG pelvis(normal ovarian morphology) was considered as secondary outcome measure. In addition, duration of flow and changes in basal metabolic index (BMI), modified Ferriman Gallwey (mFG) score, acanthosis nigricanswere observed. Data were analyzed using, ANOVA, paired student 't' test, fisher exact test. Results: Changes in duration of cycle, duration and amount of flow was achieved in 93.3% patients with p<0.0001 and 46.6% patients showed normal findings on pelvic ultrasonography with p=0.006. In addition, significant changes were also observed in BMI, hirsutism and acanthosis nigricans with p value of 0.0001, p=0.003 and p=0.009 respectively Conclusion: Majoonidraare haiz can be used as an effective alternative in management of PCOS patients. It has significant effect on menstrual regulation and changes in polycystic ovarian morphology to normal.

안면마비 환자 802례의 임상적 특성: 족양명위경(足陽明胃經) 관련 문헌에 근거한 고찰 (Clinical Characteristics of 802 Patients with Facial Palsy: Based on Literature Related to Stomach Meridian)

  • Objectives: The purpose of this study was to suggest the clinical characteristics and risk factors of facial palsy from the perspective of Korean Medicine. Methods: Medical records of 856 patients, who visited the Korean Medicine hospital with facial palsy from 2004 to 2019, were retrospectively analyzed. The clinical characteristics of facial palsy were suggested by figuring out the distribution of age, sex, occupation, onset season, obesity, modes, signs, symptoms, past and family history. Results: By gender, there were more males (54.6%), and the most common age groups were in their 50s (27.4%) and 40s (23.6%). As for the occupation, unemployed (27.7%) and service and sales workers (14.6%) were the most common, and the onset season was the most common in winter (28.9%). The most common clinical characteristics were overwork (33.0%), mental stress (24.8%), and exposure to cold (19.3%) in modes, 'none' (64.3%), postauricular pain (33.2%) in signs, postauricular pain (26.9%), parageusia (14.8%) in symptoms. Hypertension and diabetes were the most common in both past and family history. In particular, the obesity rate of the subjects (59.8%) was much higher than that of Koreans (32.8%). Conclusions: Based on the historical Korean Medicine literature and the results of this study, it is suggested that overwork, stress, exposure to cold, and obesity are identified as risk factors for facial palsy. Furthermore, dysfunction of the stomach meridian is thought to contribute to the cause of facial palsy.

Association between dietary fiber intake and kidney stones: results from the National Health and Nutrition Examination Survey (2011-2018)

  • BACKGROUND/OBJECTIVES: Studies on the impact of dietary fiber intake on kidney stones are few, and their results were controversial. This study aimed to explore the association between dietary fiber intake and kidney stones in the nationally representative population of the USA. SUBJECTS/METHODS: This cross-sectional research included 8,588 participants from the National Health and Nutrition Examination Survey, 2011 to 2018. Information regarding dietary fiber intake was obtained from a 24-h recall survey. Participants were categorized into different dietary fiber intake tertiles according to the average of 2 days of dietary recall data. The outcome was self-reported kidney stones. After adjusting for the traditional risk factors, a multivariate logistic regression model was used to examine the association between dietary fiber intake and kidney stones. RESULTS: Eight hundred seventy-two participants had kidney stones. The weighted prevalence (SE) of kidney stones in the lowest tertile, medium tertile, and highest tertile of dietary fiber intake was 11.8% (0.8%), 10.3% (0.8%), and 9.1% (0.8%), respectively. After adjusting for age, sex, race and ethnicity, education level, smoking status, alcohol consumption, physical activity, body mass index, hypertension, diabetes, dyslipidemia, daily water intake, chronic kidney disease stage 3-5, and total energy intake, participants with the highest tertile of fiber intake had a significantly lower risk of kidney stones (odds ratio [OR], 0.68; 95% confidence interval [CI], 0.48-0.95) compared to those in the lowest tertile. Every 5 g/day increment in dietary fiber intake was associated with a significant decrease in risk of kidney stones (OR, 0.90; 95% CI, 0.83-0.98). CONCLUSION: An increase in dietary fiber intake was associated with a lower risk of kidney stones, suggesting adults should be encouraged to maintain an adequate dietary fiber intake to prevent the development of kidney stones. Our results provide evidence to formulate nutrition management strategies for the prevention of kidney stones.

Internal maxillary artery (IMax) - middle cerebral artery bypass in a patient with bilateral atherosclerotic carotid occlusion: A technical case report

  • Since the first description of the possible utilization of the internal maxillary artery for bypass surgery, there are some reports of its use in aneurysm cases; however, there is no information about the possible advantages of this type of bypass for cerebral ischemic disease. We present a 77-year-old man with a history of diabetes, hypertension, systemic atherosclerosis, and two acute myocardial infarctions with left hemiparesis. Imaging studies reported total occlusion of the right internal carotid artery and 75% occlusion on the left side, with an old opercular infarction and repeated transient ischemic attacks in the right middle cerebral artery territory despite medical treatment. After a consensus, we decided to perform a bypass from the internal maxillary artery to the M2 segment of the middle cerebral artery using a radial artery graft. After performing the proximal anastomosis, the calculated graft's free flow was 216 ml/min. Subsequently, after completing the bypass, the patency was confirmed with fluorescein videoangiography and intraoperative Doppler. Postoperatively, imaging studies showed improvement in the perfusion values and the hemiparesis from 3/5 to 4+/5. The patient was discharged one week after the operation, with a modified Rankin scale of 1, without added deficits. The use of revascularization techniques in steno-occlusive disease indicates a select group of patients that may benefit from this procedure. In addition, internal maxillary artery bypass has provided a safe option for large areas of ischemia that cannot be supplied with a superficial temporal artery - middle cerebral artery bypass.

Analysis of health behavior changes among residents in depopulation areas in Korea: a cross-sectional study based on Community Health Survey data from 2010 to 2019

  • Objectives: The total population of Korea began to decline in 2019; in particular, the population in rural areas has been rapidly decreasing and is aging. Therefore, the government has designated depopulation areas and is seeking ways to support them. To assess whether health disparities exist between areas with population decline and those without, this study used community health survey data to observe temporal changes in health behaviors between the two types of areas. Methods: The analysis used Community Health Survey data from 2010 to 2019, and regional classification was divided by depopulation areas designated by the Ministry of the Interior and Safety. Trends in health behavior and chronic disease prevalence between depopulation and non-depopulation areas were analyzed. All analyses were conducted using complex sample analysis procedures in SAS 9.4 software. Results: The smoking rate steadily decreased in both depopulation and non-depopulation areas, whereas the high-risk drinking rate increased slightly. The walking practice rate did not improve in depopulation areas compared to non-depopulation areas. Furthermore, nutritional labeling usage rate was consistently lower in depopulation areas than in non-depopulation areas, with the gap being the largest. The prevalence of obesity, diabetes, and hypertension showed that the gap between depopulation and non-depopulation areas is continuously increasing. Conclusions: Health behaviors in depopulation areas have not improved, and the prevalence of chronic diseases is increasing rapidly. Therefore, the demand for health care services that support healthy lifestyle practices and chronic disease management in these areas is expected to increase.

혈관 재개통술 후 하퇴절단술을 시행한 당뇨병성 족부 괴저 환자들에서 2차 재개통술의 예측인자 (Predictive Factors for Secondary Revasculation Procedures in Patients with Diabetic Foot Gangrene Undergoing Transtibial Amputation Following Revascularization)

  • Purpose: Diabetic foot ulcers and gangrene are major complications of diabetes, often accompanied by peripheral vascular occlusion. Revascularization is performed to restore blood flow and reduce complications such as amputation surgery. Nevertheless, reocclusion, a frequently reported complication after revascularization, often necessitates further lower limb amputations to facilitate rehabilitation and ambulation. This study examined the factors influencing the performance of secondary revascularization procedures in patients with diabetic foot gangrene who even underwent transtibial amputation (TTA) following revascularization. Materials and Methods: A retrospective study was conducted on 36 patients with diabetic foot gangrene who underwent TTA after revascularization from March 2005 to March 2022. The factors influencing restenosis were classified into three categories: revascularization factors, preoperative factors, and intraoperative factors. The revascularization factors were categorized based on whether percutaneous transluminal angioplasty (PTA) or bypass surgery had been performed. Preoperative factors included the patient's age, gender, body mass index (BMI), hypertension, and other relevant factors. Intraoperative factors included surgery duration, blood loss, and transfusion. The study examined the factors influencing secondary revascularization in these three categories. Results: Among the 36 patients in the study, 27.8% (11 patients) underwent secondary revascularization procedures. There was no significant correlation between the performance of secondary revascularization and the type of revascularization procedure, whether PTA or bypass surgery (p>0.05). Similarly, no significant differences were observed in preoperative factors (including age, BMI, smoking status, HbA1c, and underlying diseases) and intraoperative factors (surgery duration, blood loss, and transfusion). On the other hand, regarding gender, all patients who underwent revascularization procedures were male, indicating a statistically significant result (p=0.039). Conclusion: This study suggests that while most clinical variables showed no association with reocclusion, the fact that all patients who underwent secondary revascularization procedures were male indicates that gender may be a significant predictive factor of revascularization.

일부 한국인 여성에서 뇌졸중 선행인자(先行因子)로서의 마목(麻木) (Paresthesia as the Prodrome of Stroke in Korean Women)

  • Although Traditional Korean Medicine had always referred to Mamok(痲木: Paresthesia) as the prodrome of stroke for a long time, yet there were only few research papers on it. Hence, the purpose of this research is to explore the relationship between Mamok(痲木; Paresthesia) as the prodrome and an attack of stroke. Setting 509 Korean impatient women as the subject of research, 264 of the whole are stroke patients and 245 are non-stroke patients. Blood tests, questionnaires, and body measurements (height, weight, waist, hip) are performed to the subjects. Basic population traits, life-style, and past history are included in the contents of questionnaires. whether or not they have experience the numbness, description about the clinical characteristics of the symptom, the body parts which the symptom occurred, the onset of the symptom, and the incidences are examined in the category inquiring the numbness of the hand and foot. After adjusting for age, unilateral numbness was associated with significantly increased risk of stroke. When adjusted for other factors(Half of WHR, History of Hypertension and Diabetes mellitus, Cigarette smoking, Alcohol drinking, Education, Marital status, Regular exercise), the risk of stroke was positively associated with unilateral numbness.(Age Adjusted Odds Ratios=2.282(95%CI=1.107-4.705), Multivariate Odds Ratios=4.105(95% CI=1.233-13.671)) Whereas no significant association was observed in the onset and the incidence of unilateral numbness. This study suggest that preceding Mamok(痲木; paresthesia) before stroke attack is may be common prodrome symptoms of stroke. Prospective cohort study on the association between Mamok(痲木;Paresthesia) as the prodrome and an attack of stroke in Korean population will be necessary.

뇌졸중 위험지표로서의 전해질 수치에 대한 환자;대조군 연구 (Case-Control Study on Electrolytes as a Risk Factor of Stroke)

  • This study was aimed to investigate the electrolytes and its relationship with acute stroke patients in each stroke type by case-control study. 217 patients with first-ever acute stroke within 2 weeks as the case group(Cases), 146 people without four major risk factors(hypertension, diabetes mellitus, hyperlipidema and ischemic heart disease) as the healthy control group(Normals), and 160 people as the general control group(Controls) were recruited at the Stroke Medical Center in Daejeon University Oriental Medicine Hospital from july 2005 to march 2007 for this case-control study. We analyzed the odds ratio of electrolytes in binary logistic analysis and evaluated each stroke type and general characteristics such as age, sex etc. The level of sodium(Na+) and potassium(K+) were significantly lower in LAA, SVO type of ischemic stroke and hemorrhagic stroke. But the level of chloride(Cl-) had no significant relation with stroke occurance. In this study we demonstrated that low sodium(Na+) and potassium(K+) have an influence on stoke occurance than chloride(Cl-). And we think that the electrolytes must be considered in risk factors of ischemic stroke in Korean and more prospective studies are needed.

외상 후 반흔에 생긴 안면부 대상포진의 경험 (Clinical Experience of Herpes Zoster Developing within Recent Surgical Scar Area)

  • Purpose: Herpes zoster is a common dermatologic disease characterized by unilateral pain and vesicular lesions over the unilateral sensory dermatomes being caused by the reactivation of Varicella zoster virus and its incidence seems to be increasing recently. In case of involving the ganglion of the fifth cranial nerve(trigeminal nerve), it can descend down the affected nerve into skin, then producing an eruption in the dermatome. Among the patients, about 40 - 50% had associated conditions such as diabetes mellitus, hypertension, pulmonary tuberculosis, liver diseases, peptic ulcer, hypothyroidism, pharyngitis but rare facial trauma. Methods: Retrospective study was done for 3 cases of Herpes zoster from May 2000 to May 2007, which had been treated with acyclovir and steroid. Results: The clinical course was uneventful. Follow-up length was about 3 months. After treatment, the patients became stable and there was no complications. Conclusion: Herpes zoster was commonly associated with systemic disorders and the treatment duration was prolonged in associated diseases. But herpes zoster occurring specifically at the site of previously traumatized facial skin has not yet been reported. We experienced the treatment of herpes zoster developing within recent operative facial scar and three cases are presented with the review of literatures. Finally, facial trauma might be a risk factor for herpes zoster in traumatized patients.

근로자의 근무유형별 건강상태와 영양섭취상태 비교 연구 (Health and Nutritional Status of Industrial Workers)

  • The study was curried out to collect information to establish a framework for nutrition education for the prevention of chronic degenerative disease. We analyzed differences in diet quality, food habits and health status of workers by work condition. Anthrometric parameters of height, weight and body fat were measured and biochemical parameters including glucose, total cholesterol, GOT, GPT and hemoglobin were determinded for 194 subjects. To assess the nutrient intake and diet quality of workers, dietary intake was measured by the day 24-hour recall method, Average daily nutrient intake, except for phos-phorous and vitamin C was lower than Korean RDA. The obesity related behavior score was significantly better in laborers than in office workers, while chronic degenerative diseases related to food habit score was significantly better in laborers than in office workers, while chronic degenerative diseases related to the food habit score was beet in offices workers than in laborers. Blood pressure, blood glucose levels were significantly higher in laborer than in office workers. Dietary variety score (DVS) food composition group score(FCGS), mean adequacy ratio(MAR) of office worker were better than those of labor workers. When diet quality was evaluated by FCGS(food composition group score) 16.0% of the subjects acquired 5 points and 14.4% of the subjects acquired 2 points. MAR and INQ showed a significantly positive correlation with DVS and FCGS . This results indicated that the onset possibility of hypertension and diabetes mellitus among chronic degenerative disease was higher in laborers than in office workers, while the onset possibility of obesity was higher in office workers than in laborers. In conclusion the overall diet quality of office workers is betters than that of laborers, therefore, nutrition education for prevention of chronic degenerative disease of industrial workers needs to be more focused on the improvement of the health status of laborers.
