A Case of Renovascular Hypertension Controlled by Renal Artery Embolization (신동맥 색전술로 치료한 신혈관 고혈압증 1례)

  • Renal artery stenosis is a major cause of renovascular hypertension and the most common cause of treatable secondary hypertension. There are several methods to treat renal artery stenosis, including surgery, percutaneous transluminal renal angioplasty(PTRA), and renal artery stenting(RAS). But, renal artery embolization can be tried in atherosclerotic stenosis, multiple stenosis, microaneurysm, and stenosis difficult to try PTRA or RAS. We report a case of renovascular hypertension in a 14-year-old female who had multiple segmental renal artery stenosis. Hypertension was controlled by renal ablation therapy with renal artery embolization.

Factors Affecting the Intention to Quit Smoking due to the Prevalence of Hypertension (고혈압 유병 여부에 따른 금연의사에 영향을 미치는 요인)

  • The purpose of this study is to identify factors related to the intent of quit smoking in adult hypertension and normal group. Data are based on the National Health and Nutrition Survey from 2010 to 2018. The survey was performed on 1,981 male adults(Hypertension group 407, Normal group 1,574). In the hypertension group, single was 0.226 times lower in the will to intention to quit smoking than the group spouses. Also, high school graduation was 2.102 times higher than middle school graduation, and the worse the subjective health status was, the higher the intention was to quit smoking. It was also 5.427 times higher than when people did not walk if the day they walked for more than 10 minutes a day was one day of the week. The characteristics of hypertensive patients should be understood. In quit smoking education, it is necessary to recommend health exercises and proper exercise to patients. It should be used as the basis data for a quit smoking policy for efficient personalized management of hypertensive patients.

  • Hypertension has a steadily increasing incidence rate as well as represents a risk factors for secondary diseases such as cardiovascular disease. Therefore, it is important to predict the incidence rate of the disease. In this study, we constructed nomograms that can predict the incidence rate of hypertension. We use data from the Korean National Health and Nutrition Examination Survey (KNHANES) for 2013-2016. The complex sampling data required the use of a Rao-Scott chi-squared test to identify 10 risk factors for hypertension. Smoking and exercise variables were not statistically significant in the Logistic regression; therefore, eight effects were selected as risk factors for hypertension. Logistic and Bayesian nomograms constructed from the selected risk factors were proposed and compared. The constructed nomograms were then verified using a receiver operating characteristics curve and calibration plot.

Incidence of Hypertension in a Cohort of an Adult Population (성인코호트에서 고혈압 발생률)

  • Objectives : This study was peformed in order to assess the incidence of hypertension based on two-years follow-up of a rural hypertension-free cohort in Korea. Methods : The study cohen comprised 2,580 subjects aged above 20 (1,107 men and 1,473 women) of Chung-Song County in Kyungpook Province judged to be hypertensive-free at the baseline examination in 1996. For each of two examinations in the two-year follow-up, those subjects free of hypertension were followed for the development of hypertension to the next examination one year (1997) and two years later (1998). The drop-out rate was 24.7% in men and 19.6% in women. Hypertension was defined as follows 1) above mild hypertension as a SBP above 140 mmHg or a DBP above 90 mmMg,2) above moderate hypertension as a SBP above 160 mmHg or a DBP above 100 mmHg or when the participant reported having used antihypertensive medication after beginning this survey. Results : The age-standardized incidence of above mild hypertension was 6 per 100 person years (PYS) in men and that of above moderate hypertension was 1.2. In women, the age-standardized rate for above mild hypertension was 5.7 and 1.5 for above mild and moderate hypertension, respectively. However, the rates of incidence as calculated by the risk method were 4.8% and 1.0% in men and 4.6%, 1.2% in women, respectively. In both genders, incidence was significantly associated with advancing age(p<0.01), In men, the incidences of above moderate hypertension by age group were 0.5 per 100 PYS aged 20-39, 0.7 aged 40-49, 1.7 aged 50-59, 3.6 aged 60-69, and 5.8 aged above 70(p<0.01). In women, those the incidence measured 0.6 per 100 PYS aged 20-39, 1.8 aged 40-49, 1.3 aged 50-59, 3.3 aged 60-69, and 5.6 aged above 70(p<0.01). After age 60, the incidence of hypertension increased rapidly. Conclusions : The incidence data of hypertension reported in this study may serve as a reference data for evaluating the impact of future public efforts in the primary prevention of hypertension in Korea.

Statistical Analysis for Risk Factors and Prediction of Hypertension based on Health Behavior Information (건강행위정보기반 고혈압 위험인자 및 예측을 위한 통계분석)

  • The purpose of this study is to develop a prediction model of hypertension in middle-aged adults using Statistical analysis. Statistical analysis and prediction models were developed using the National Health and Nutrition Survey (2013-2016).Binary logistic regression analysis showed statistically significant risk factors for hypertension, and a predictive model was developed using logistic regression and the Naive Bayes algorithm using Wrapper approach technique. In the statistical analysis, WHtR(p<0.0001, OR = 2.0242) in men and AGE (p<0.0001, OR = 3.9185) in women were the most related factors to hypertension. In the performance evaluation of the prediction model, the logistic regression model showed the best predictive power in men (AUC = 0.782) and women (AUC = 0.858). Our findings provide important information for developing large-scale screening tools for hypertension and can be used as the basis for hypertension research.

Convergence Effect Factors of Hypertension and Diabetes Mellitus on Depression and Suicidal Impulse (고혈압과 당뇨병이 우울과 자살생각에 미치는 융합적인 영향 요인)

  • The purpose of this study is to investigate the effects of hypertension and diabetes mellitus on depression and suicidal impulse through the total inspection on specific rural areas in Gangwon-do. This survey selected adults over the age of 30 and conducted from May 19 to Sep. 30 in 2014. Of the 1,200 questionnaires distributed, 970 were completed and returned to be used for the study. To figure out the actual condition of hypertension and diabetes among the participants, the research examined the degree of their depression and suicidal impulse. It examined their smoking and exercise habits to check their health conditions. The research showed that patients diagnosed with hypertension and diabetes had a higher tendency of depression and suicidal impulse. The significant variable affecting depression is suicidal impulse(p<0.001>). Also, the significant variables affecting suicidal impulse are hypertension and depression. Based on these results, local health institutions need to include mental health services for the patients with hypertension and diabetes and coordinate them comprehensively and effectively.

Factors Affecting Subjective Health Status in Middle-aged Adults with Hypertension (고혈압이 있는 중년의 주관적 건강상태에 대한 영향요인)

  • This study was conducted to identify factors that affect subjective health in middle-aged with hypertension. Hypertension, a representative chronic disease, is a disease of increasing prevalence from middle age. To effectively manage hypertension from middle age, it is necessary to revise lifestyle based on subjective health awareness. The subjects of this study were 2,254 middle-age with hypertension who were 30-64 years of age among the 2014-17 national health and nutrition survey. Study variables consisted of general, physical, and psychological factors. For data analysis, compound statistics were made using SPSS 25.0 program and then complex statistics. The results of this study showed that the factors that significantly influenced the subjective health of the subjects were sex, age, economic level, number of households, home ownership, stress, quality of life, and their explanatory power was 20.4% (F = 12.58, p<.001). The results of this study can be used as a basis for the intervention to improve the subjective health of middle-aged with hypertension. Appropriate lifestyle modifications are needed to effectively manage hypertension based on good subjective health awareness.

  • The purpose of this study was to identify the factors related to awareness, treatment, and control of hypertension in Korea using data from the 4th KNHANES. The analytical technique employed multivariate logistic regression analysis and the major findings of this research are as follows. First, awareness of hypertension was positively associated with age, occupation, self-rated health, Body Mass Index, and health examination since 2 years ago, in both males and females. Second, treatment of hypertension was positively associated with age, self-rated health, Body Mass Index, and health examination since 2 years ago, in both males and females. Third, control of hypertension was positively associated with age, self-rated health, Body Mass Index, and health examination since 2 years ago, in both males and females. Finally, the results of analysis suggest that the government should especially focus on the vulnerable group and continue to strengthen effective efforts for improving awareness, treatment, and control of hypertension.

A Study on the Effect of the Dendropanax Mobifera Extract on Anti-Hypertensive (황칠나무 추출물이 고혈압에 미치는 영향에 관한 연구)

  • This study examined the effects of an extract of Dendropanax morbifera on blood pressure, Angiotensin II, Angiotensin Converting Enzyme, Aldosterone, and lipid levels of spontaneously hypertensive rats. The groups were as follows: Control group, Hypertension control group, Water extract treated group, Ethanol extract treated group, n- hexane fraction treated group, Ethyl acetate fraction treated group, n- butanol fraction treated group, and Water fraction treated group. The blood pressure, and Angiotensin II, and Angiotensin Converting Enzyme, and Aldosterone levels were lower in the Ethyl acetate fraction treated group than in the hypertension control group. The change in blood pressure was lower in the Water extract treated group, Ethyl acetate fraction treated group, Ethanol extract treated group, n- hexane fraction treated group, and n- butanol fraction treated group than the hypertension control group. The concentration of Angiotensin II was lower in the Ethyl acetate fraction treated group, Ethanol extract treated group, n- hexane fraction treated group, and n- butanol fraction treated group than the hypertension control group(p<0.05). The level of Angiotensin Converting Enzyme was lower in the Ethyl acetate fraction treated group, Ethanol extract treated group, n- hexane fraction treated group, and n- butanol fraction treated group than the hypertension control group. The concentration of Aldosterone was lower in the Ethyl acetate fraction treated group, and n- butanol fraction treated group than the hypertension control group(p<0.05). In addition, the concentration lower in the Water extract treated group, Ethanol extract treated group, n- hexane fraction treated group than the hypertension control group. Overall, the effect of the Dendropanax morbifera extracts might be useful as anti-hypertensive, and functional food agents.

Effects of Dietary Approaches to Stop Hypertension Program on Blood Pressure among the Pre-hypertension Group in the Community (지역사회 주민의 고혈압 예방 식이프로그램의 효과 평가)

  • Objectives: DASH (dietary approaches to stop hypertension) diet are recommended as first-line therapy for prevention of hypertension individuals with high normal blood pressure. We studied the effects of DASH program on blood pressure among the pre-hypertension group. Methods: To determine the impact of dietary patterns on the control of hypertension we studied the subgroup of 141 participants with systolic blood pressure of 120 to 139 mm Hg or diastolic BP of 80 to 89mmHg, body mass index(BMI) ${\geq}25kg/m^3$ or waist to hip ratio(WHR) ${\geq}0.95$(for man). 0.85(for woman) enrolled in DASH program. Participants were received of education and consulting about DASH every week for 8 weeks. Results: The level of diet to prevention of hypertension and compliance of DASH increased after education. Also, The DASH program significantly reduced systolic BP (from $136.03{\pm}12.40mmHg$ to $126.09{\pm}11.25mmHg$, p< .01) and diastolic BP (from $81.80{\pm}6.32mmHg$ to $76.44{\pm}10.61mmHg$, p< .01). Conclusion: The DASH program effectively lowers BP and may be useful in achieving prevention of hypertension.
