Usage of informed consent form for Bee-venom pharmacopuncture Therapy at korean medicine hospitals and Proposal for development of a standard informed consent form. (한방병원의 봉약침 시술 동의서의 사용 현황과 표준 시술 동의서 개발에 대한 제안)

  • Objectives: We investigated the current status of the consent form for bee-venom pharmacopuncture therapy, which is using in Korean medicine hospitals. We suggest the development of a standard informed consent form. Method: Through the questionnaire survey, status of using informed consent form was identified at 24 Korean medicine hospitals. We analyze different types of informed consent form, which was developed by each hospitals. We investigated the types of informed consent forms for various medical procedures through electronic searches. A standard informed consent form for bee-venom pharmacopuncture therapy was developed based on the medical law and the standard informed consent form for medical procedures developed by Korea Fair Trade Mediation Agency. Result: In our survey, 65% of the hospitals do not use consent well, only 35% of the hospitals use informed consent form, and the most hospitals use self-developed informed consent form. As a result of analyzing the contents of informed consent form used in each hospitals, the explanation of diagnosis, treatment precautions, suggestions for other treatments, consequences of not performing the scheduled procedure, possibility of treatment change was insufficient. 48% of hospitals manage consent in recording on a chart, 39% in scanned documents, and 13% in digital electronic consent form. Conclusion: A standard informed consent form for Bee-venom pharmacopuncture therapy include diagnosis, effectivness, necessity, indications, method, skin reaction test, hypersensitivity questionnaire, treatment precautions, possible hypersensitivity reactions and countermeasures, suggestions for other treatments, consequences of not performing the scheduled procedure, possibility of treatment change and the name of doctor.

  • Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a rare and severe drug-induced hypersensitivity syndrome characterized by hematological abnormalities and multiorgan involvement. Liver involvement is the most common visceral manifestation. However, renal failure has been rarely described. The common culprit drugs are anticonvulsants and allopurinol. We experienced a patient with DRESS syndrome with acute interstitial nephritis caused by concomitant administration of quinolone and non-steroidal anti-inflammatory drugs (NSAIDs). A 41-year-old man presented with a diffuse erythematous rash and fever which developed after administration of quinolone and NSAIDs for a month due to prostatitis. He was diagnosed with DRESS syndrome. Skin rash, fever, eosinophilia, and elevations of liver enzymes improved with conservative treatment and discontinuation of the causative drugs. However, deterioration of his renal function occurred on day 8 of admission. The levels of blood urea nitrogen and serum creatinine increased and oliguria, proteinuria and urinary eosinophils were observed. Ultrasonography showed diffuse renal enlargement. The clinical features were compatible with acute interstitial nephritis. Despite intravenous rehydration and diuretics, renal function did not improve. After hemodialysis, his renal function recovered completely within 2 weeks without administration of systemic corticosteroid.

Contact Sensitivity to Dinitrochlorobenzene as a Marker Trait in the Indirect Selection for Body Mange and Coccidiosis Resistance in Broiler Rabbits

  • To determine the effects of genetic and environmental influences on cell mediated immune (CMI) responses in broiler rabbits, contact sensitivity to 2,4-Dinitrochlorobenzene (DNCB) was assessed in three temperate broiler breeds of rabbits, namely Soviet Chinchilla, New Zealand White and Grey Giant. The feasibility of using the contact sensitivity to DNCB as a marker trait in selection for disease resistance was examined. There were highly significant differences between breeds (p<0.01) in initial skin thickness and contact sensitivities to DNCB at 24, 48 and 72 hours. Initial skin thickness was greatest in the Soviet Chinchilla breed (mean 2.2484 mm), and was significantly greater (p<0.01) in males (2.4963 mm) than in females (1.7846 mm) (p<0.01). Highest contact sensitivity to DNCB was in the New Zealand White breed with mean increase in skin thickness of 1.1884, 0.9072 and 0.5879 mm at 24, 48 and 72 hours post challenge respectively. Delayed type hypersensitivity (DTH) reaction to DNCB at 24 hours post challenge had a highly significant association (p<0.01) with the incidence of body mange in rabbits. The results indicated a lowered contact sensitivity to DNCB at 24 hours post challenge was associated significantly (p<0.01) with an increase in incidence and severity of body mange, suggesting its potential value as a marker. The correlation s among contact sensitivities at 24, 48 and 72 hours were positive and highly significant (p<0.01); correlations between initial skin thickness and contact sensitivities were negative and highly significant (p<0.01). Another notable significant correlation was between body weight and delayed type hypersensitivity at 24 hours indicating that an enhanced CMI might be associated with better growth rate and general wellbeing.

Immunological Assessment of Respiratory Allergy Status for the Swine Farm Workers in Gyeonggi Province of Korea (경기 일부지역 양돈 축산인들의 호흡기 알레르기 관련 면역학적 지표 분석)

  • Objectives: Animal husbandry workers could be exposed to various work hazards including toxic gases, chemicals such as pesticides or organic dust. Immunological evaluation focusing on respiratory allergic hypersensitivity occurrence was under-taken for swine farm workers as a part of the study on immunologic status of dairy barn, swine confinement, and poultry farm workers. Materials and Methods: Peripheral bloods were collected from 25 workers at the year of 2001 and 12 workers at the year of 2012 from swine farms located at Gyeonggi province, Korea. Seven adults not involved with animal husbandry were recruited at the year of 2001 from the same residential area as the swine farm workers'. Level of plasma IgE and 20 respiratory allergen-specific IgE were evaluated using commercially available ELISA kit. Results: Plasma IgE level was approximately five-fold higher in the swine farm workers regardless of the sampling year than the control subjects. Plant allergens from outdoor environments such as golden rod, pigweed, Russian thistle, or ragweed were the major allergens with positive reaction(allergen specific IgE${\geq}$0.7 IU/mL) for the swine farm workers at 2001 year. Meanwhile, house dust mite(Dermatophagoides farinae, D. pteronyssinus) and cockroach, typical indoor allergens in Korea, were the major respiratory allergens for the swine farm workers at 2012 year. Conclusions: Overall, even though our results are primitive, the results suggest that immunological function of swine farm workers could be modulated toward type-2 reactivity.

The Effect of Rancid perilla oil diet on the Immune Response in Mice (마우스에 있어서 부패들기름 식이가 면역반응에 미치는 영향)

  • The effect of rancid perilla oil on the immune response in mice was studied. ICR male mice were divided into 5 groups and were fed on the experimental diets for 4 weeks. Mice were sensitized and challenged with sheep red blood cell. Immune responses were evaluated by antibody production, Arthus reaction, delayed type hypersensitivity (DTH), Rosette forming cell and macrophage activity. Biochemical items were measured by serum protein and serum albumin. The weight of spleen, thymus and liver were measured. The rancid perilla oil diets decreased humoral and cellular immune responses, the number of peripheral circulating white blood cells and total protein and serum albumin. These results showed that the high rancid perilla oil diet decreased more humoral and cellular immune response, the number of peripheral circulating white blood cells, and total protein and serum albumin than the low rancid perilla oil diet did.

Effect of Sound Stress on Immune Response (소음 스트레스가 면역반응에 미치는 영향에 관한 실험적 연구)

  • This study was undertaken to assess the effect of sound stress on humoral and cellular immune responses to thymus-dependent and independent antigens in mice. After mice were exposed to 4 hr daily sound stessors(83㏈) for 4 days before or after immunization, the primary and / or secondary immune response to sheep red blood cells(SRBC), polyvinylpyrroridone(PVP) or picry1 chloride(TNCB) were assayed. When mice were exposed to sound stressor before or after immunization, delayed-type hypersensitivity reaction and contact sensitivity to TNCB was remarkably depressed compared with those of the unstressed control mice. However, the primary and secondary hemagglutinin response of the stresed mice to SRBC showed a pronounced increase compared with that of the unstressed mice, In contrast to antibody response to SRBC, the primary antibody response of the stressed mic to PVP was almost not detected. surprisingly, the secondary antibody response to PVP of the mice receiving the secondary sound stress was markedly increased when the immune-depressed mice received the secondary immunization with PVP at 46 days after the primary immunization. The susceptibility of mice to intraven-oulsy infected Candida albicans was not changed by the sound stress.

  • Experiments were performed to investigate the effect of Panax ginseng petroleum ether fraction on delayed type hypersensitivity, rosette formation, phagocytic activity and histophathological influence in lead acetate treated mice. Lead acetate was administered in the drinking water and ginseng pet. ether fraction was injected i.p.. Mice were sensitized and challenged with sheep red blood cells. Erythrocyte(I) rosette formation and DTH reaction were significantly depressed in lead acetate treated mice, and those were restored administration of ginseng fraction. Ginseng pet. ether fraction administration did not have any effect on decreased phagocytic activity. Follicular and parafollicular areal destruction of spleen, and destruction of thymus were finded in lead acetate exposed-mice. Small dose of ginseng pet. ether fraction (5 mg/kg, 10 mg/kg), administraction inhibited those histopathological changes, but large dose (20 mg/kg) didn't.

  • Experiments were performed on mice to investigate the effect of panax ginseng extracts on the immunotoxicity of ethanol. Immune response were evaluated by antibody production, Arthus reaction, delayed type hypersensitivity (DTH), Rosette froming cell (RFC) and macrophage activity in mice, sensitized and challenged with sheep red blood cells. The weight of liver, spleen and thymus were measured. Following results obtained in this experiment. The exposure of ethanol decreased humoral and cellular immune response, the body weight and macrophage activity. Ginseng extracts such as ethanol extract, petroleum ether extract and n-butanol fraction were significantly increased the body weight. The administration of ginseng ethanol extract and ginseng petroleum ether extract were restored or increased humoral and cellular immune response. Macrophage activity was decreased by ethanol, but restored by the ginseng extracts.

  • Acute vanishing bile syndrome is a cause of progressive cholestasis. It is most often drug or toxin related. It's pathogenesis is unknown. Stevens-Johnson syndorme is a well-recognized immune complex-mediated hypersensitivity reaction. It is induced by drug or infection. It has classic systemic, mucosal, and dermatologic manifestations. We report a case of a 14 years old female suffering from Stevens-Johnson syndrome plus vanishing bile duct syndrome associated with ibuprofen use. We report the case with a brief review of its related literature.

  • The functional dyspepsia is a complex disease reaction that ocurs due to the presence of various factors and its pathophysiology has not been clearly verified until now. This disease is diagnosed after the organic diseases are ruled out. Therefore, the main treatments are limited down to compensating for revealed symptoms through exercising the supportive care. The international prevalence of the functional dyspepsia under the Rome I criterior is approximately 25%. For the cases of Koreans, there are very omnifarious symptoms in gastrointestinal disorders and the medical vocabularies which describe the diagnostic standard are not closely associated with the daily vocabularies, thus there would be a lot of restrictions to directly apply the Rome II criterior. However, the Korean Society of Gastrointestinal motility had decided to promote the use of the international standardized Rome II criterior in order for Korea to be comparable to the clinical studies from the other nations. There should be more extensive studies to differentiate the definitions of symptom descriptions from Korea to foreign countries. The extended researches concerning the pathophysiology are highly recommended which in return, would help enabling the reliable treatment techniques for this disease.
