노인 방문구강건강관리를 위한 치위생 전문교육과정 요구도 (Demand for dental hygiene specialization training courses for visiting oral health care for older individuals)

  • Objectives: This study aimed to examine the opinions of registered dental hygienists (RDH) and dental hygiene students (DHS), professionals who can practice visiting oral health care (VOHC), and the demand for specialized training courses of RDH for VOHC. Methods: Using a Google online survey, 125 RDH and 106 DHS personnel were surveyed on their perception of VOHC and the operation of special training courses. Results: RDH (97.6%) and DHS (96.1%) agreed that they needed VOHC for older people. Moreover, the qualification course for specialized training for VOHC was considered necessary. Regarding the appropriate curriculum, RDH preferred training programs comprising 2-hours classes over 6 months and 15 weeks and so on, and DHS preferred 3 hours classes over 6 months and 15 weeks and so on. Regarding the desired working type, the RDH preferred the half-day system, whereas the DHS preferred the alternate-day system. The Korean Dental Hygienists Association was intended for human resource development. They agreed that continuing education was necessary when working as a visiting RDH, and many responded that they would participate if a specialized training course was opened. Conclusions: RDH and DHS have a high level of awareness of the importance and necessity of VOHC, and consider a specialized training qualification course necessary for efficient VOHC.

Searching for Ways to Improve Visiting Oral Health Care Services in Korea through Comparison with Japanese System in Long-Term Care Insurance

  • Background: Legal regulations and fees have been established in Korea to provide visiting oral health care services to individuals with long-term care insurance (LTCI). However, beneficiaries of this service are very limited. Therefore, to improve the Korean system we propose a comparative analysis with the Japanese system. Methods: This study is a descriptive analysis based on secondary data, such as statistics, laws, and service record forms from Korea and Japan. The most recent institutional documents were obtained through a Google search. The variables investigated were financial resources of LTCI, co-payment structure, monthly limit of LTCI benefits, care levels of LTCI, service providers, service costs, contents of service, and the number of cases of service. Results: In both Korea and Japan, LTCI is financed through a combination of taxes and insurance premiums. However, the monthly limit for receiving LTCI services in Japan is about 2.4 times higher than in Korea. Visiting medical and dental treatment is also possible in Japan. Furthermore, nursing staff can provide daily oral health care services according to dental hygienists' instruction unlike Korea. Oral health care services in Korea are focused on oral hygiene and prevention of oral diseases, while Japan additionally provides oral function screening, patient education for oral health management, and training for nursing staff to enhance oral function, eating, and swallowing of the patients. Conclusion: We concluded that the possibility of visiting dental treatment, differences in monthly limit of LTCI benefits, oral function assessment and guidance, as well as collaboration with other healthcare professionals contributed to the difference in the frequency of utilization of visiting oral health care services between Korea and Japan.

ATP bioluminescence assay를 이용한 경북 일부 어린이 급식시설에 대한 위생방문교육의 효과 평가 (Application of ATP bioluminescence assay for effect assessment of hygiene visiting education on children's foodservice facilities in the local small town)

  • 본 연구는 경상북도 예천군에 소재하는 100인 이하의 어린이급식소 32개소를 대상으로 급식 위생·안전 순회지도 및 위생교육 지원에 따른 교육효과를 조사·분석함으로써 어린이급식소의 문제점을 파악하고, 올바른 개선방안을 제시하고자 하였으며, 위생·안전 체크리스트 분석 결과와 ATP 측정 결과의 상관관계를 비교·분석함으로써 위생교육의 효과를 평가하기 위한 ATP bioluminescence assay의 적용성을 검토하였다. 조사결과 설립 유형은 법인 9.4%, 국공립 18.7%, 기타 18.7%, 가정 21.9%, 민간 31.3% 순으로 확인되었고, 규모별로 집단급식소 25.0%, 소규모 급식소 34.4%, 비집단급식소 40.6% 순으로 조사되었다. 위생순회지도의 결과들은 '조리실, 식품 등의 원료·제품 보관실 등을 정기적으로 소독하는지 여부' 항목을 제외한 모든 항목에서 유의적인 개선을 보였으며, 위생순회지원에 따른 교육효과의 평가를 위해 칼, 도마, 고무장갑, 냉장고 손잡이, 정수기 꼭지 등 5가지 항목에 대한 ATP bioluminescence analysis가 수행되었다. 칼과 도마의 경우 전 개소 모두 순회 지원 후 20 RLU/cm2 이하의 수치를 보였으며, 특히 국공립시설, 법인시설, 민간시설에서 칼과 도마 모두 10 RLU/cm2 이하의 수치를 보였다. 조리기구 중 식품위생의 주요 기준이 되고 있는 칼과 도마를 중심으로 이와 관련된 항목인 '자외선 또는 전기 살균 소독기 설치 여부', '주방용구의 살균·소독제 또는 열탕의 방법으로 소독한 것을 사용하는지 여부', '어류·육류·채소류를 취급하는 칼·도마 구분 사용 여부' 항목들이 상관관계 분석을 통해 검증되었으며, 칼과 도마 모두에서 음(-)의 상관관계를 가지며 높은 상관성을 나타내었다. 이러한 결과는 위생교육의 효과에 대한 간편하면서도 객관적인 평가수단으로서 ATP bioluminescence assay의 가능성을 제시하는 것으로 사료된다. 전반적으로 제한적인 점검횟수로 인해 전 항목 모두 뚜렷한 개선을 확인할 수는 없었으나, 지속적인 교육지원을 통한 개선의 효과를 기대할 수 있을 것으로 판단된다.

오마하체계에 따른 보건소 방문간호사의 간호업무 수행도와 필요도 (Nursing Performance and Requirements of Visiting Nurses at Public Health Centers according to Omaha system)

  • 본 연구는 오마하체계를 이용하여 보건소 방문간호사의 간호업무 수행도와 필요도를 조사한 서술적 조사연구이다. 방문간호사의 간호업무는 '개인위생', '통증', '약물요법', '영양', '신체활동', '위생', '수면과 휴식양상', '구강건강', '정신건강', '인지'순으로 수행도가 높았다. '성생활', '산후', '수입', '가족계획', '임신', '영성', '학대', '생식기능', '방임', '말과 언어' 순으로 간호업무 수행도가 낮았다. '성생활', '학대' '방임', '생식기능' 문제는 문제의 사정 및 적극적 개입을 위한 교육적 지원이 필요하다. '산후', '임신', '가족계획' 등의 문제는 출산지원을 위한 지역의 정책방향에 따라 방문간호사의 역량 강화가 필요하다. '수입', '주거', '지역사회자원과의 의사소통'은 보건복지 연계 및 정부와 지방자치단체의 정책적 지원이 필요하겠다. 본 연구결과는 방문간호사의 역할규명 및 역할확대를 위한 기초자료로 활용할 수 있을 것으로 본다.

어머니의 구강보건지식과 행동에 관한 연구 (Study on Dental Health Knowledge and Behavior of Mothers)

  • Purpose: The purpose of this study was to analyze level of dental health knowledge and behavior of mothers, health educators should encourage mother to have continuous dental care habits for lifelong dental health with practical education. Methods: The subjects in this study were 490 mothers of four-year, fifth-year and sixth-year from elementary school in Daegu areas. The data was collected through a structured questionnaire from 26, June to 27, June 2006. Collected data were analyzed into frequency and Chi-square test using SPSS 10.0 program. Result: First, no statistically significant relations were found between mothers, knowledge with general characteristics of subject(age, degree of education, yes or no of occupation, subjective economy). Second, mothers, number of toothbrushing were statistically significant relations with degree of education, yes or no of occupation as over three times a day. Third, no statistically significant relations were between visiting frequency of dental clinic for one years with general characteristics of subject(age, degree of education, yes or no of occupation, subjective economy). Fourth, yes or no of use in auxiliary oral hygiene device were statistically significant relations with degree of education, yes or no of occupation, subjective economy. Fifth, mothers, knowledge were statistically significant relations with auxiliary oral hygiene device.

모친의 구강보건교육경험이 유아의 구강건강관리에 미치는 영향 (The influence on infant oral health care from mother's experience of oral health education)

  • Objectives : This study is for suggesting the data base for help mother and child improve the oral health enhancement action from understanding child's oral health enhancement action from mother's oral health education experience and researching the effects of mother's oral health enhancement action on child's oral health enhancement action. Methods : The subject was 220 children and mothers of some kindergarten in Jeollabuk-do, Korea out of convenience'sampling. The results were collected by carrying out a survey out of self record method from visiting the kindergarten from June 24th to July 12th. and were analyzed. Results : 1. The mother's and child's oral health enhancement action was satisfactory in the factor of 'toothbrushing', 'oral hygiene care item' and 'periodic visits' in case the subjects have experienced oral health education, and that data was meaningful statistically. 2. In the oral health enhancement action practiced by mother to child from existence or absence of the mother's oral health education experience, the data was meaningful statistically in the factor of 'directly brushing its teeth more than once a day', 'washing its tongue' and 'coaching that child could keep the toothbrush not to overlap with other toothbrushes'. 3. In the factor that affect to oral health enhancement action practiced by mother to child, 'toothbrushing' and 'oral hygiene care item' were high level of the mother's oral health enhancement action, and that data was meaningful'statistically. Conclusions : The children's oral health condition could be influenced by mother's oral health knowledges and attention. Accordingly, oral health education for the mother and child has to be enforced'systematically'suited for the characteristic change of oral condition with children age groups. Also, utilizing human resources who have expertise and development of the oral health education program are needed.

커뮤니티케어 기반의 방문구강건강관리 중재 수혜자의 프로그램 운영 관련 인식 및 태도: 근거이론적 접근 (Awareness and attitudes regarding oral care intervention program based on community care for older adults at home : focusing on the grounded theory)

  • Objectives: This study is based on a visiting oral health care intervention program in the community care. This qualitative study was conducted through in-depth interviews to identify awareness and attitudes regarding intervention program among older adults. Methods: The research team visited the homes of the target older adults and conducted in-depth interviews for approximately an hour using a semi-structured questionnaire. The collected voice recordings were transcribed using Clova Note, and AI program by Naver. Using the 'Word Cloud Generator 3.7' program, words of high importance and interest from interview answers were extracted, visualized, and analyzed. Results: Participating older adults acknowledged that their quality of life related to oral health could be improved by increasing the level of oral health awareness and oral health knowledge through the intervention program. In addition, the older adults indicated that their oral hygiene management ability improved compared to before the intervention through expert oral hygiene management and oral health education. Further, as the level of oral health knowledge increased, so too did satisfaction with the intervention program increase. Conclusions: The intervention program for visiting oral health care showed a positive effect on the awareness and attitude of older adults. Thus, it is suggested that education for continuous competency enhancement of dental hygienists and multidisciplinary education for the improvement of general health and quality of life of older adults should be promoted.

중학생들의 치아우식증 예방법에 대한 인지도 및 행태 조사 (A survey on awareness and behavior on preventive method of dental caries in middle school students)

  • Objective : This study came to be carried out in order to develop oral health education program for maintaining adolescents' oral health and to be helpful for qualitative improvement in prevention program by surveying the awareness and behavior on preventive method of dental caries in adolescents. Method : The collected materials of totally 1,100 people who responded were carried out frequency analysis by using SPSS 12.0(Statistical Package for the Social Science). Results : 1. As for the results of awareness on fluoride effect and sealant effect, the students with response to 'effect of preventing decayed tooth' were indicated to be the highest. It was indicated to be the highest in girls as for gender and in the 2nd grade as for school year. (p<0.05). 2. As for the results of being created when eating sweet food and carbonated drink, the students with response as saying that 'a decayed tooth occurs well' were indicated to be the highest. 4. As for the results on the aim of visiting dental clinic, the students with response as saying of 'visiting for having treatment' when tooth is painful were indicated to be the highest. Conclusion : Synthesizing this study, the students' awareness on preventive method of dental caries was high. But the practice on regular check-up or diet was indicated to be low. Through oral health education, the adolescents should be not only delivered information or knowledge, but also changed habit and behavior.

치과환자의 치과기피요인에 대한 분석 (Analysis of dental avoidance factors of dental patients)

  • Objectives: This study was conducted to investigate the factors that prevent dental patients from visiting the dental clinic. Methods: From June 22, 2020 to July 17, 2020, among patients 10 years or older who had visited the dental clinic, 314 individuals who agreed to the study were asked to fill out a self-administered questionnaire. The data were analyzed using the SPSS Statistics 22.0 program, and the significance level was set at 0.05. Results: The dental avoidance factors according to the general characteristics were high in women and housewives, and among the dental-related characteristics, the most common reason for visiting the dental clinic was experiencing pain. The factors influencing anxiety were fear, environment, stimulus, age, and exercise, and the factors influencing the fear were anxiety, stimulus, subjective oral health status, education, and exercise. The factors influencing the stimulus were environmental factors, fear, anxiety, and age, and the factors that influenced environmental factors were stimulus and anxiety. Conclusions: There is a need to develop measures and policies to overcome fear of dental procedures, such as developing a dental experience program and allowing patients to experience dental fear and anxiety in advance.

일부지역 치위생(학)과 학생들의 구강건강신념과 구강보건행태 관련요인 (Oral health beliefs and oral health behaviors related factors of dental hygiene students)

  • Objectives: The study was conducted to evaluate the effects of the students majoring in dental hygiene on their oral health beliefs and oral health behavior. Methods: The survey was conducted using selfadministered questionnaires targeting 619 female students majoring in dental hygiene at three Universities in k and J province. The stronger health oral belief means higher score in susceptibility, severity, benefit and salience, but the higher score in barrier means stronger recognition in obstacle. Results: There was significant correlation between susceptibility and experience of not-treated oral disease (OR [odds ratio] 2.40; 95% CI [confidence interval] 1.73-3.34)' and 'dental caries (OR 2.36; 95% CI 1.25-4.45)'. Benefit had significant correlation with 'visiting dental clinic (OR 1.58; 95% CI 1.14-2.19)'. Salience had significant correlation with 'experience of not-treated oral disease (OR 0.70; 95% CI 0.52-0.94)'. Barrier had significant correlation with 'visiting dental clinic (OR 0.73; 95% CI 0.56-0.95)' and 'removing oral plaque (OR 0.71; 95% CI 0.52-0.95)'. There was no significant correlation between oral health belief and 'using of oral hygiene device' or 'regular tooth brushing'. Conclusions: Diverse oral health behaviors were affected by susceptibility, benefit, salience and barrier in oral health belief. The programs for oral health education and preventing oral disease should be prepared to change oral health belief to promote the oral health systematically based on the results of this study.
