특발정상압수두증에서 해마 및 외측 뇌실의 부피와 뇌척수액배액검사 (Hippocampal and Ventricular Volumes of Idiopathic Normal-pressure Hydrocephalus and the Cerebrospinal Fluid Tap Test)

  • We investigated differences in ventricular and hippocampal volumes between CSF tap test (CSFTT) responders and non-responders in idiopathic normal-pressure hydrocephalus (INPH) patients and compared these parameters in INPH patients with that of age- and gender-matched healthy controls. We also evaluated relationships between ventricular and hippocampal volumes and clinical profiles in INPH patients. We enrolled 48 patients with INPH and 29 healthy controls. Ventricular and hippocampal volumes were measured on MRI, including 3-dimensional volumetric images. INPH patients, when compared to healthy controls, had significantly larger ventricular and smaller hippocampal volumes. No difference in ventricular and hippocampal volumes was found between CSFTT responders and non-responders in INPH patients. And hippocampal volumes showed significant negative correlations with Clinical Dementia Rating Scale scores, INPH grading scale cognitive scores, Timed Up and Go Test scores, and Unified Parkinson's Disease Rating Scale motor scores in INPH patients. Volumetric assessment of ventricular and hippocampal regions may have no predictive value in differentiating between CSFTT responders and non-responders in INPH patients. Our findings may help us understand the potential pathophysiology of unique symptoms associated with INPH.

정상압수두증에 의한 보행장애 환자 치험 1례 (A case study of normal pressure hydrocephalus patient with gait disturbance using conservative Korean medical treatment)

  • A case of a 75-year-old Korean female with gait disturbance due to Normal pressure hydrocephalus (NPH) is presented. She was treated with acupuncture, electroacupuncture and herbal medicine - 柴苓湯(Shirhyung-Tang, Chai-ling-tang, Sairei-to) We used iNPH grading scale, and specified further the grade of gait disturbance category. After Korean medical treatment, there was notable improvement in gait disturbance on our specified scale. Cognitive impairment, tremor and rigidity were improved on each scale alongside. Korean medical treatment may be effective in treating NPH patients.

Retrospective Analysis of Cerebrospinal Fluid Profiles in 228 Patients with Leptomeningeal Carcinomatosis : Differences According to the Sampling Site, Symptoms, and Systemic Factors

  • Objective : Elevated cell counts and protein levels in cerebrospinal fluid (CSF) result from disease activity in patients with leptomeningeal carcinomatosis (LMC). Previous studies evaluated the use of CSF profiles to monitor a treatment response or predict prognosis. CSF profiles vary, however, according to the sampling site and the patient's systemic condition. We compared lumbar and ventricular CSF profiles collected before intraventricular chemotherapy for LMC and evaluated the association of these profiles with patients' systemic factors and LMC disease activity. Methods : CSF profiles were retrospectively collected from 228 patients who underwent Ommaya reservoir insertion for intraventricular chemotherapy after a diagnosis of LMC. Lumbar samples taken via lumbar puncture were used for the diagnosis, and ventricular samples were obtained later at the time of Ommaya reservoir insertion. LMC disease activity was defined as the presence of LMC-related symptoms such as increased intracranial pressure, hydrocephalus, cranial neuropathy, and cauda equina syndrome. Results : Cell counts (median : 8 vs. 1 cells/mL) and protein levels (median : 68 vs. 17 mg/dL) significantly higher in lumbar CSF than in ventricular CSF (p<0.001). Among the evaluated systemic factors, concomitant brain metastasis and previous radiation were significantly correlated with higher protein levels in the lumbar CSF (p=0.01 and <0.001, respectively). Among the LMC disease activity, patients presenting with hydrocephalus or cauda equina syndrome showed higher lumbar CSF protein level compared with that in patients without those symptoms (p=0.049 and p<0.001, respectively). The lumbar CSF cell count was significantly lower in patients with cranial neuropathy (p=0.046). The ventricular CSF cell counts and protein levels showed no correlation with LMC symptoms. Carcinoembryonic antigen (CEA), which was measured from ventricular CSF after the diagnosis in 109 patients, showed a significant association with the presence of hydrocephalus (p=0.01). Conclusion : The protein level in lumbar CSF indicated the localized disease activity of hydrocephalus and cauda equina syndrome. In the ventricular CSF, only the CEA level reflected the presence of hydrocephalus. We suggest using more specific biomarkers for the evaluation of ventricular CSF to monitor disease activity and treatment response.

뇌실 복강간 단락 부전의 기여인자 (The Contributory Factors of CSF Shunt Failure)

  • 목 적 : 뇌실 복강간 단락술 부전의 기여인자를 조사 및 분석하여 뇌실 복강간 단락술의 합병증 및 재수술의 가능성을 줄이는 데에 목적이 있다. 대상 및 방법 : 1995년 1월부터 1998년 12월까지 본원에서 뇌실 복강간 단락술을 시행 받은 237명의 수두증 환자를 후향적 방법으로 고찰하여 뇌실 복강간 단락 부전의 기여인자 및 단락기 생존율을 통계학적으로 분석하였다. 결 과 : 수두증의 원인은 종양, 출혈, 감염, 선천성 기형, 정상 뇌압 수두증, 외상 등이었다. 68명의 환자에서 109회의 재수술이 이루어졌다. 재수술의 이유는 폐색, 근위부 카세타의 위치이상, 감염 등이었다. 단락기 생존율은 1년, 2년, 3년에 각각 77.1%, 75.4%, 74.1%이었다. 10세 이하의 환자에서 재수술의 빈도가 통계학적으로 의의 있게 높았다. 그리고 수두증의 원인에 따라서 통계학적으로 의의 있게 재수술의 빈도 차이가 있었다. 결 론 : 대부분의 뇌실 복강간 단락 부전은 술후 1년내에 발생하였다. 환자의 나이 및 수두증의 원인이 뇌실 복 강간 단락 부전의 주요 기여인자였다.

  • This study was applied using vojta therapy in the patient with hydrocephalus occurred by on traumatic brain injury. Vojta treatment was a recently developed of the brain damage patient treatment which can be applied eariler than the other traditional methods. The results were as follows. 1. Hip joint flexion contracture from $100^{\circ}$ to $15^{\circ}$ was improved on prone position. 2. Left convexity curve on thracolumbar region due to functional scoliosis the normal aligment. 3. The thumb finger was changed from thumb-in to thumb-out. 4. Right tilted pelvis on prone position became the normal symmetry

뇌실-복강 단락 원위도관의 심장내전위 - 증례보고 - (A Case of Intracardiac Migration of Distal Ventriculo-Peritonal(V-P) Shunt Catheter - Case Report -)

  • Ventriculoperitoneal(V-P) shunt has been used as a popular method for surgical treatment of hydrocephalus. But complications such as infection, mechanical obstruction and failure of flow rate sometimes make painful stress to neurosurgeons and patients. Of particular, migration of distal V-P shunt catheter to extraperitoneal space has rarely been reported. Even rarer is intracardiac migration of distal V-P shunt catheter. Authors report a such case and discuss the possible mechanism and preventive method.

Endoscopic Coagulation of Choroid Plexus in Hydranencephaly

  • Hydranencephaly is a rare congenital disease defined as an absence of cerebral hemispheres with an intact thalamus, brain stem, and cerebellum. Generally, patients with hydranencephaly require cerebro-spinal fluid diversion due to progressive hydrocephalus. An excellent view of the choroid plexus and advancement of the neurosurgical endoscope were possible, leading to easy coagulation of the choroid plexus to balance cerebro-spinal fluid production without the use of a shunt device. We present this rare case and good treatment outcome from endoscopic coagulation of the choroid plexus.

Anal Extrusion of Distal V-P Shunt Catheter after Double Perforation of Large Intestine

  • We describe the extrusion of a ventriculoperitoneal shunt catheter from the anus after double perforation of the large bowel in a 3-year-old girl with hydrocephalus. She was admitted because the tip of the peritoneal catheter protruded 10 cm from the anus and clear cerebrospinal fluid dripped from the tip. Emergency laparotomy was performed. The distal peritoneal catheter perforated and penetrated the sigmoid colon and re-perforated into the rectal cavity. The distal peritoneal catheter was removed, the proximal catheter was exposed for external drainage, and intravenous broad-spectrum antibiotics were administered for 2 weeks. After control of infection, the shunt system was completely removed. Bowel perforation by a peritoneal catheter is a rare complication. Diagnosis is often difficult, delayed, and its incidence is likely underestimated. Most bowel perforation is the result of infection as opposed to technical errors.
