First Outcome of MDR-TB among Co-Infected HIV/TB Patients from South-West Iran

  • Background: Tuberculosis (TB) is the leading cause of mortality among human immunodeficiency virus (HIV) patients and the majority of them occur in developing countries. The aims of the present study were to determine the frequency of HIV/TB co-infection and other probable associated factors. Methods: This 10 year retrospective study was conducted on 824 HIV patients in the south-west of Iran. HIV infection was diagnosed by the enzyme linked immunosorbent assay and confirmed by Western blot. TB diagnosis was based on consistency of the clinical manifestations, chest X-ray, and microscopic examination. Drug susceptibility testing was done by the proportional method on $L{\ddot{o}}wenstein$-Jensen media. Results: Of 824 HIV patients, 59 (7.2%) were identified as TB co-infected and the majority (86.4%) of them were male. Of the overall TB infected patients, 6 cases (10.2%) showed multidrug-resistant with the mean CD4+ lymphocyte count of $163{\pm}166cells/mm^3$. The main clinical forms of TB were pulmonary (73%). There was a significant (p<0.05) correlation between TB infection and CD4+ lymphocyte counts ${\leq}200cells/mm^3$, gender, prison history, addiction history, and highly active anti-retroviral therapy. Conclusion: We reported novel information on frequency of HIV/TB co-infection and multidrug resistant-TB outcome among co-infected patients that could facilitate better management of such infections on a global scale.

  • In irradiated human lymphocytes, translocation of chromosome has been more frequently observed than dicentric chromosome. Differences in the misrepair process leading to translocation and dicentric chromosomes may explain the above observations. In order to find out whether dicentric and translocation are originated from different mechanism, the frequencies of radiation induced translocation and dicentric in lymphocytes were examined following treatment of irradiated lymphocytes with two DNA repair inhibitors, 3AB for inhibition of poly(ADP-ribose) synthesis and Ara C for inhibition of DNA-polymerase $\alpha$. Ara C potentiated the frequencies of radiation induced dicentric and translocation. 3AB also potentiated the frequencies of radiation induced dicentric, but not translocation. These results suggest the potential differences in the mechanisms in the formation of translocation and dicentric chromosomes.

  • Fluorescence in situ hybridization (FISH) with the DNA probe for human chromosome 4 was used to analyse in vitro radiation induced chromosome rearrangement in peripheral lymphocyte. Translocations, dicentrics, acentrics and color junctions involving the painted chromosome were scored according to the Protocol for Aberration Identification and Nomenclature Terminology (PAINT) system. The frequency of chromosome rearrangements including reciprocal translocation, dicentric, acentric fragment and color junction increased with radiation dose. The frequency of dicentric chromosome reduced by the fixation time following irradiation, whereas that of translocation was relatively persistent. The applicability of FISH for scoring stable translocation for biological dosimetry was demonstrated.

Unusual chromosomal features in a child with gradual disappearance of right ulna (mono ostolic osteolysis)

  • A nine month old male child presenting degenerating right ulna (massive osteolysis) has been followed up for two years. The bone completely disappeared due to abscesses on the right forearm and without orthopedic or haematological complications. Repeated lymphocyte cultures showed somatic pairing (mostly chromosome pair 5), end to end association involving chromosome 14, 21, 21 and 16, and satellite enlargement in a high proportion of cells with an otherwise normal 46,XY karyotype. These observations are compared with 13 other types of orthopaedic patients, and we opine that cumulative picture of chromosomal aberrations appears to correspond with the present rare anomaly "Mono Ostolic Osteolysis" involving right ulna. None of the controls or any other orthopaedic anomaly studied hereunder exhibits this chromosomal picture.

혈액세포를 이용한 염색체 분리 분석에 관한 방법적 고찰 (A Study on the Methodology of Chromosome Preparation from Blood Culture)

  • This study was carried out to develop the methodology of chromosome preparation from blood cultures in mammals which included human, mouse, cattle and pig. For karyotyping, 0.5-5.0ml of peripheral blood were collected and cultured. The satisfactory results were obtained from macroculture and microculture in all species. In culture, the patterns of cell growth were no difference among media except serum concentration and mitogen supplement. The presence of mitogen and fetal bovine serum in medium significantly affected the mitotic index. The optimal culture condition was 37$^{\circ}C$ for 3 days. And the concentration of colcemid and reincubation time also affected the chromosome morphology. In harvest, chromosome patterns were mainly affected on hypotonic treatment which included treated time and temperature, dropwise of fixative solution, and drying after slide preparation.

Suppressive Effects of Various Antioxidants on Melamine-induced Oxidative DNA Damage in Human Lymphocytes

  • Melamine, which is used to produce melamine resin for various industrial applications, has a high nitrogen content by mass. For this reason, it has been illegally added to foods to increase their apparent protein content. In the present investigation, melamine-induced oxidative damage of human lymphocyte DNA was evaluated by Comet assay. The in vitro oxidative DNA damage caused by melamine increased in a dose-dependent manner. This DNA damage was significantly inhibited by treatment with ascorbate. Moreover, the traditional Korean medicinal herb, named Acanthopanax, red ginseng and green tea markedly reduced the DNA damage. Various edible plant extracts also inhibited melamine-induced oxidative DNA damage in vitro. Melamine enhanced intracellular ROS generation, and this effect was suppressed by treatment with various antioxidants.

Xylene Induces Oxidative Stress and Mitochondria Damage in Isolated Human Lymphocytes

  • Xylene is a cyclic hydrocarbon and an environmental pollutant. It is also used in medical technology, paints, dyes, polishes and in many industries as a solvent; therefore, an understanding of the interaction between xylene and human lymphocytes is of significant interest. Biochemical assessment was used to demonstrate that exposure of lymphocytes to xylene induces cytotoxicity (at 6 hr), generates intracellular reactive oxygen species, collapse of mitochondrial membrane potential, lysosomal injury, lipid peroxidation and depletion of glutathione (at 3 hr). The findings show that xylene triggers oxidative stress and organelle damage in lymphocytes. The results of our study suggest that the use of antioxidant, mitochondrial and lysosomal protective agents can be helpful for individuals subject to chronic exposure to xylene.

  • Reciprocal exchanges of DNA in sister chromatids (SCEs) are induced by various carcinogens and mutagens, although the quantitative relationship between the number of mutations and SCEs induced varies among chemicals. Nevertheless, the analysis of SCEs production by various agents often proposed as a sensitive and quantitative assay for mutagenicity and cytotoxicity. Mercury, even if which has no evidences for mutagenicity and carcinogenicity, is reported to exert some cytotoxic effects, such as chromosomal aberrations or bad influences to ovulation and reproduction in experimental animals, etc.. In this study, tests for sister chromatid exchanges have been carried out on normal human lymphocytes in whole blood culture to add mercury chloride ($HgCl_2$) or methylmercury chloride ($CH_3\;HgCl$) for 72 hr. The results indicate the dose-dependent relationship between the frequencies of SCEs and the concentrations of $HgCl_2,\;CH_{3}HgCl$ and 5-bromo-2'-deoxyuridine (BrdU). Lymphocyte proliferation has depressed in the higher concentration of mercury.

  • Fluorscence in situ hybridization with chromosome-specific probe has been shown to be a valid and rapid method for detection of chromosome rearrangements induced by chemical and physical agents. This method is useful for quantifying structural aberrations, expecially for stable ones, such as translocation and insertion, which are difficult to detect with conventional method in human lymphocyte. In order to use the FISH method as a biodosimeter for monitoring human population exposed to various chemical and physical agent, baseline level of chromosome rearragement was established. Blood from forty four healthy adults were collected and analysed with whole chromosome-specific probes by human chromosome 1,2 and 4. The frequencies of stable translocation were 2.45 per 100 cell equivalent and those of insertion, color juction, acentric and dicentric were 0.32, 3.28, 0.23 and 0.27 per 100 cell equivalent respectively. The frequencies of chromosome rearragements increased with age in both sexes except for dicenrics. From above result, stable aberrations accumulate with age and it may reflect integrated lifetime exposure of adverse environment.

Exploiting cDNA Microarray-Based Approach Combined with RT-PCR Analysis to Monitor the Radiation Effect: Antioxidant Gene Response of ex vivo Irradiated Human Peripheral Blood Lymphocyte

  • Although ionizing radiation (IR) has been used to treat the various human cancers, IR is cytotoxic not only to cancer cells but to the adjacent normal tissue. Since normal tissue complications are the limiting factor of cancer radiotherapy, one of the major concerns of IR therapy is to maximize the cancer cell killing and to minimize the toxic side effects on the adjacent normal tissue. As an attempt to develop a method to monitor the degree of radiation exposure to normal tissues during radiotherapy, we investigated the transcriptional responses of human peripheral blood lymphocytes (PBL) following IR using cDNA microarray chip containing 1,221 (1.2 K) known genes. Since conventional radiotherapy is delivered at about 24 h intervals at 180 to 300 cGy/day, we analyzed the transcriptional responses ex-vivo irradiated human PBL at 200 cGy for 24 h-period. We observed and report on 1) a group of genes transiently induced early after IR at 2 h, 2) of genes induced after IR at 6 h, 3) of genes induced after IR at 24 h and on 4) a group of genes whose expression patters were not changed after IR. Since Biological consequences of IR involve generation of various reactive oxygen species (ROS) and thus oxidative stress induced by the ROS is known to damage normal tissues during radiotherapy, we further tested the temporal expression profiles of genes involved in ROS modulation by RT-PCR. Specific changes of 6 antioxidant genes were identified in irradiated PBL among 9 genes tested. Our results suggest the potential of monitoring post-radiotherapy changes in temporal expression profiles of a specific set of genes as a measure of radiation effects on normal tissues. This type of approach should yield more useful information when validated in in vivo irradiated PBL from the cancer patients.
