Design of a hospital assignment scheme for ambulances based on minimum cost maximum flow algorithm (최소비용 최대유량 알고리즘에 기반한 구급차의 환자이송 병원 배정 기법의 설계)

  • This paper presents a design and analyzes the performance of a hospital assignment and reassignment scheme for emergency rescue services based on minimum cost maximum flow algorithm. It consists of flow graph building, link capacity updating, and allocation discovering steps. The efficiency of the algorithm makes it possible to reallocate hospitals even in case of dynamic changes in the number of patients or hospitals. The performance measurement result obtained from a prototype implementation shows that the proposed scheme can reduce the transport time requirement miss by up to 24%.

  • Overcrowding in an Emergency Department (ED) of hospital is a common phenomenon. To improve the service quality and system performance of the ED, a task assignment rule for the Registered Nurses (RNs) is proposed in this paper. At each task assignment point, the rule prioritizes all treatment requests based on the urgency which is determined by the multiple attributes such as accomplishment time of treatment task, elapsed time of treatment request, total remain time to patient discharge, and number of remain treatments. The values of partial urgency with a single criterion are determined and then overall urgency is computed to find the most urgent one among current requests with the importance weights assigned to the criteria. Through computer simulation, the performance of the proposed rule is compared with current rule in terms of the length of stay and system throughput in a simplified ED system of the hospital M.

  • The problem of assigning customers to satellite channels is considered in this paper. Finding an optimal allocation of customers to satellite channels is a difficult combinatorial optimization problem and is shown to be NP-complete in an earlier study. We propose a genetic algorithm (GA) approach to search for the best/optimal assignment of customers to satellite channels. Various issues related to genetic algorithms such as solution representation, selection methods, genetic operators and repair of invalid solutions are presented. A comparison of GA with CPLEX8.1 is presented to show the advantages of this approach in terms of computation time and solution quality.

A Study on Role Conflict and Job Satisfaction of Neurosurgery Clinical Nurse Specialist (신경외과 임상전문간호사의 역할갈등과 직무만족에 관한 연구)

  • Purpose: The purpose of this study was to describe role conflict and job satisfaction of Neurosurgery Clinical Nurse Specialist(NCNS) and to identify associated factors. Method: The target populations was 77 NCNSs from 30 general hospitals. Data were collected with self-administered questionnaires and analyzed using t-test and ANOVA. Results: The overall mean scores of role conflict and job satisfaction were $3.60({\pm}0.54)$ and $3.04({\pm}0.46)$ respectively. Significant variables affecting role conflict were the working period as NCNS, the number of colleague NCNSs in working hospital, and assignment of prescription rights. The significant variable affecting job satisfaction was assignment of prescription rights. There was no statistically significant correlation between job and role conflict. Conclusion: The variable effecting both role conflict and job satisfaction was the extent of prescription rights. In order to improve the quality of patient care and to protect Neurosurgery Nurse Clinical Specialist, the enactment of law on prescription practice is needed.

The Effects of the Gender Role Identity and Gender Stereotypes on the Prejudice Against Male Nurses of Hospital Workers (병원 종사자의 성 역할 정체감과 성 고정관념이 남자간호사에 대한 편견에 미치는 영향)

  • As more and more males are participating in the female dominated nursing profession in Korea, they are facing a number of discriminations such as department assignment on the job because of male gender stereotype. The purpose of this study is to examine hospital workers's gender role identity, gender stereotype, and the relationship between those attitudes and gender prejudice against male nurses. The results showed that the androgynous group had lower gender stereotypes and lower prejudice against male nurses than those of the other groups. Hospital workers had a strong tendency to believe that male nurses should work mainly in special departments such as emergency rooms, intensive care units, operating rooms and they think that the male nurses are not fit for caring patients because of the patients' unfamiliarity with male nurses. Gender stereotypes were positively correlated to prejudice against male nurses. Therefore, hospitals should consider developing a detailed plan to encourage a culture of gender equality by tackling the existing prejudice against male nurses so that male nurses can get the same opportunities as female counterparts in terms of department assignment and job promotion.

  • In "Maekyojeongmiron(脈要精微論)", Jangbu(臟腑) is assigned to Chon.Gwan.Cheok(寸關尺) in according to the position. Kidney[腎] is assigned to the backward area of Cheok. Abdominal cavity[腹中] is assigned to the middle area of Cheok. Hypochondrium[季脇] is assigned to the forward area of Cheok. Liver[肝] is assigned to the backward area of left Gwan. Diaphragm[鬲] is assigned to the forward area of left Gwan. Stomach[胃] is assigned to the backward area of right Gwan. Spleen[脾] is assigned to the forward area of right Gwan. Heart[心] is assigned to the backward area of left Chon. Central part of the chest[膻中] is assigned to the forward area of left Chon. Lung[肺] is assigned to the backward area of right Chon. Thoracic cavity[胸中] is assigned to the forward area of right Chon. This method of assignment is simple and useful in clinic.

  • The purpose of this study is to promote efficiency of hospital management with customer satisfaction by understanding qualitative system about more systemic hospital service coordinator's job under analyzing the satisfactory degree and coherence of hospital service coordinator's job because of increase of customer need level by medical environment improvement and the variety of offering method of medical service. I allocated a questionnaire per each hospital around dentistry, Oriental medicine clinic, plastic surgery in medical examination department and withdrew questionnaire on total 662 hospitals. According to the study results stated above, generally necessity on employment of a hospital service coordinator appeared high and To the necessity, I considered that a hospital service coordinator's job must strengthen the role for going still more to customer with discriminated mind and service not simple kind service to medical consumer, with establishing more systematic and realistic educational culture in qualification and educational department of a hospital service coordinator. Also a hospital service coordinator must be changed to promote customer satisfaction and efficiency of hospital management by more systematic business assignment and role establishment.

A Study on Job Stress and Emotional Burnout of Clinical Nurses

  • This study attempts to provide basic data for the development of manpower maintenance programs by checking the degree of job stress and emotional burnout for nurses working in a university hospital and identifying factors affecting emotional burnout. Data were obtained through a structured questionnaire survey conducted on 187 nurses. The average score for job stress of nurses was 2.50 (range 1 to 4) and emotional burnout was 3.29 (range 1 to 5). The factors affecting emotional burnout were occupational climate, job demand, job insecurity, and lack of reward, which accounted for 44% of explanatory power. In order to reduce the emotional burnout of nurses, the management of medical institutions needs administrative and financial support. Further, it is necessary to improve the organizational culture regarding job assignment through job analysis, employment security, and a performance-based reward system.

Optimal Density Assignment to 2D Diode Array Detector for Different Dose Calculation Algorithms in Patient Specific VMAT QA

  • Background: The purpose of this study is to assign an appropriate density to virtual phantom for 2D diode array detector with different dose calculation algorithms to guarantee the accuracy of patient-specific QA. Materials and Methods: Ten VMAT plans with 6 MV photon beam and ten VMAT plans with 15 MV photon beam were selected retrospectively. The computed tomography (CT) images of MapCHECK2 with MapPHAN were acquired to design the virtual phantom images. For all plans, dose distributions were calculated for the virtual phantoms with four different materials by AAA and AXB algorithms. The four materials were polystyrene, 455 HU, Jursinic phantom, and PVC. Passing rates for several gamma criteria were calculated by comparing the measured dose distribution with calculated dose distributions of four materials. Results and Discussion: For validation of AXB modeling in clinic, the mean percentages of agreement in the cases of dose difference criteria of 1.0% and 2.0% for 6 MV were $97.2%{\pm}2.3%$, and $99.4%{\pm}1.1%$, respectively while those for 15 MV were $98.5%{\pm}0.85%$ and $99.8%{\pm}0.2%$, respectively. In the case of 2%/2 mm, all mean passing rates were more than 96.0% and 97.2% for 6 MV and 15 MV, respectively, regardless of the virtual phantoms of different materials and dose calculation algorithms. The passing rates in all criteria slightly increased for AXB as well as AAA when using 455 HU rather than polystyrene. Conclusion: The virtual phantom which had a 455 HU values showed high passing rates for all gamma criteria. To guarantee the accuracy of patent-specific VMAT QA, each institution should fine-tune the mass density or HU values of this device.

Determinants of Quality in Outpatient Medical Service (외래 의료서비스 질적 수준의 결정요소)

  • This study was conducted to evaluate the subjective ideas about the determinants of quality in ambulatory care unit among outpatients and medical staff of a university hospital, and to compare the differences of the ideas, between patients themselves and hospital staff. A self-administered questionnaire survey was conducted covering 799 outpatients and 190 hospital staff in March, 1998. The questionnaire included general characteristics and 26 determinants of ambulatory care quality. The following are summaries of the findings: 1. Both of outpatients and hospital staff perceived, "Physician's knowledge" as the most important determinant of medical care quality. 2. In respect of 7 determinants related to physician's knowledge and skill, both outpatients and hospital staff perceived "physician's knowledge and skill" as important determinants. The scores of determinants such as, "Not doing unnecessary examinations", and "Assignment of adequate number of patients and duty schedule for the physician" were significantly different between outpatients and hospital staff. 3. In respect of 4 determinants related to doctor-patient relationship, both outpatients and hospital staff perceived "attention to patient's complaints" as the most important determinant. The scores related to the determinants such as "kindness of physician" and "explanation of treatment outcome" were significantly different between outpatients and hospital staff. 4. Among the amenities related determinants, "Modern facilities and equipments" were perceived as the most important determinant in both group. 5. In respect of 8 determinants related to non-financial accommodation, outpatients perceived, "Waiting hours for treatment" as the most important determinant, and hospital staff perceived, "Kindness of hospital staff". 6. In respect of 4 determinants related to financial accommodation, outpatients perceived, "Fare account of medical cost" as the most important determinant, and hospital staff perceived, "Increasing reimbursements". Further comprehensive research should be made on the evaluation of perceptions of medical care quality, both of outpatient and inpatient care, among patients and hospital staff. So good quality in medical care will be achieved based on clients' needs.
