성인 남녀의 호모시스테인, 엽산 및 비타민 $B_{12}$ 상태와 건강관련 생활습관과의 상관관계 (The Relationships of Health-Related Lifestyles with Homocysteine, Folate, and Vitamin $B_{12}$ Status in Korean Adults)

  • The elevation of plasma total homocysteine(tHcy) is now established as a risk factro for cardiovascular disease. It is also well known that plasma levels of folate and vitamin $B_{12}$ influences homocysteine metabolism as cofactors. Recently, the effects of health-related lifestyle factors, such as smoking, alcohol drinking coffee consumption, regular exercise, and etc, on plasma tHcy have been determined. The Hordalane Homocysteine Study revealed that smoking and coffee consumption are major deter minants of plasma tHcy as well as folate levels; however, the influence of alcohol intake is still controversial. In Koreans, the effects of lifestyle factors of plasma tHcy have not yet been determined. Thus, we investigated the relationships of various lifestyle determinants with plasma tHcy, folate, and vitamin $B_{12}$ levels and the erythrocyte folate concentrations in Korean adults (99 males and 96 fermales). Plasma tHcy levels were significantly hight in male subjects. On the contrary, plasma levels of folate and vitamin $B_{12}$ and erythrocyte folate concentration of the females were significantly higher than those of the males. Among the five lifestyle factors determined in the study, regular exercise significantly affects plasma tHcy levels only in the females, Contrary to the expectation, there were on significant differences in plasma tHcy levels between alcohol drinkers and non-alcohol drinkers as well as smokers and non-smokers. And also, plasma tHcy leverls were not different between coffee consumers and non-coffee consumer and between green tea consumers and non-green tea consumers. Although alcohol intake did not influence plasma tHcy levels, the duration, frequency, and amount of alcohol drinking showed significant negative relationships with plasma folate levers. These results indicate the regular exercise and alcohol intake might influence plasma levels of tHcy and folate in Koreans, although the results were not reveled in both sexes.

조릿대 잎 추출물이 고지방식이 유도 비만 마우스 (C57/BL6J)의 혈장 Adiponectin, Resistin, C-reactive Protein 및 Homocysteine 농도에 미치는 영향 (The Effects of the Sasa Borealis Leaves Extract on Plasma Adiponectin, Resistin, C-Reactive Protein and Homocysteine Levels in High Fat Diet-Induced Obese C57/BL6J Mice)

  • As obesity is known to be related to hyperlipidemia, diabetes and coronary heart disease, and other chronic diseases, many researches have focused on functional food materials showing anti-obesity activity. The adipokines secreted by adipose tissue, resistin and adiponectin are known to play an important role in the pathogenesis of chronic diseases directly. C-reactive protein and homocysteine are molecules regulated by adipose tissue indirectly also relate to the chronic diseases. This study was performed to study of the anti-obesity effects of Sasa borealis in diet-induced obese mice (C57/BL6J). The mice were divided into four group: NFD (Normal fat diet), HFD (High fat diet), BSE (High fat diet containing 5% of 70% ethanol extract of Sasa borealis leaves), BLW (High fat diet containing 5% of water extract of Sasa borealis leaves). The experimental diets were fed for 11 weeks. The final body weight of the mice in the groups of BSE and BLW groups were significantly lower than the HFD group. The effects of weight reduction were due to reduced body fat accumulation. The adiponectin levels are significantly decreased in HFD group compared than NFD group and increased taken by Sasa borealis containing diet. The resistin levels are not significantly different between experimental groups. The CRP and homocyteine levels are significantly higher in HFD group than NFD group and significantly decreased by Sasa borealis containing diet, especially BLW group. These results indicate that orally administered Sasa borealis not only has the effect of reducing the body weight and total fat weight, but preferable effect in adiponectin levels and related molecules as CRP and homocysteine. Therefore we expect the Sasa borealis may have an anti-obesity function and anti-metabolic syndrome effect in diet-induced obese mice.

MTHFR Polymorphism and Folate Status of Korean Women of Childbearing Age

  • It should be concerned to the women with mutated genotype of methylenetetrahydrofolate reductase (MTHFR), C677T or A1298C, since they need more folate than those with wild genotypes. In this study, we evaluated the folate status of Korean women of childbearing age according to their MTHFR polymorpiysm. Dietary folate intakes, plasma and erythrocyte folate concentrations, plasma homocysteine concentrations, and urinary excretions of para-aminobenzoylglutamate (pABG) and para-acetoamidobenzoylglutamate (ApABG) of twenty-five subjects aged between 19 and 35 years old were determined Folate intakes seemed to be inadequate, being only three-quarters of the Korean RDA of folate. More than one-quarter of the subjects was exposed to folate deficiency risk as determined by erythrocyte folate concentration and almost one-quarter of the subjects showed hyperhomocysteinemia, although they had normal plasma folate concentrations. Urinary excretions of pABG and ApABG seemed to be low and ApABG constituted more than $85\%$ of total folate catabolites. There were no significant differences in dietary folate intakes, plasma concentrations of folate and homocysteine, and urinary excretions of pABG and ApABG among the geneotypes of both C677T and A1298C. However, the subjects with 1298AC genotype had significantly lower erythrocyte folate concentration than those with 1298AA. Erythrocyte folate concentration showed an inverse relationship with plasma homocysteine concentration and positive relationships with urinary excretions of pABG and ApABG. The results of this study imply that mutations of 677C$\rightarrow$T and 1298A$\rightarrow$C in the study were not associated with decreased plasma folate and raised plasma homocysteine concentrations. A1298C polymorphism night be, however, more influential on erythrocyte folate concentration than C677T polymorphism, and urinary excretions of folate catabolites, pABG and ApABG, might be reliable indexes of folate nutritional status like plasma homocysteine concentrations.

한국 성인 남녀의 엽산 영양상태와 혈장 호모시스테인 농도 (Folate Status and Plasma Homocysteine Concentration of Korean Adults)

  • We examined the relationship between plasma folate and total homocysteine(Hcy) levels and the distribution of plasma folate and Hcy levels from 204 Korean adults(113 men and 91 women aged between 20yr and 69yr). Plasma folate levels were significantly lower in men(12.2nmol/L) than in women(14.6nmol/L) after controlling for smoking and drinking(p<0.05). Plasma Hcy levels were significantly higher in men(13.9$\mu$mol/L) than in women(11.8$\mu$mol/L) after controlling and drinking. Plasma Hcy levels were more more strongly correlated with plasma folate in women(${\gamma}$=-0.321, p<0.05) than in men(${\gamma}$=-0.202, p<0.05), but the difference between men and women was no longer statistically significant controlling for plasma folate concentration. Prevalence of mild homocysteinemia(plama Hcy>15$\mu$mol/L) was greatest among subjects with lowest folate status. These results indicate a strong association between plasma Hcy concentration and folate status and the poor folate status is the strong causative factor of mild homocysteinemia. Ginseng Total Saponin Attenuate Cardiac Hypertrophy Induced by Homocysteine in Rats

  • Recent studies have shown that Panax ginseng has a variety of beneficial effects on the cardiovascular system. Homocysteine (Hcy), which is derived from methionine, has been closely associated with the increased risk of cardiovascular diseases. In the present study, whether the in-vivo long-term co-administration of ginseng total saponins (GTS), active ingredients of Panax ginseng, with L-methionine (Met) inhibits methionine-induced hyperhomocysteine (HHcy) and H-Hcy-induced cardiovascular dysfunctions was investigated, and it was found that the plasma Hcy level, which was measured after 30 and 60 days, in the GTS+Met co-administration group was more significantly reduced than in the Metalone-treatment group. The left-ventricle (LV) wall thickness of the heart was likewise examined in each treatment group, and it was found that the co-administration of GTS with Met significantly reduced the Met-induced LV wall thickness. The results of the study indicate that the in-vivo long-term co-administration of GTS with Met not only inhibited H-Hcy induced by long-term Met-alone administration but also attenuated the H-Hcy-induced cardiovascular dysfunctions in rats.

기체크로마토그래피-전자포획검출기를 이용한 혈장 중의 Homocysteine, Cysteine 및 Methionine의 동시 분석법 (Determination of the Homocysteine, Cysteine and Methionine in Human Plasma by Gas Chromatography with Electron Capture Detector)

  • 기체 크로마토그래피-전자포획검출기 (gas chromatograph-electron capture detector : GC-ECD)를 이용하여 사람의 혈액 내에 존재하는 homocysteine (Hcy), 관련 화합물인 cysteine (Cys)과 methionine (Met)에 대해 선택적이고 감도 좋은 분석방법에 대하여 연구하였다. 시료는 혈장 내에서 환원과정을 거친 후 ethyl chloroformate (ECF)와 pentafluoropropyl alcohol (PEP-OH) 유도체화 시약을 사용하여 N(O,S)-ethylearhonyl pentafluoropropyl (PEP) ester 유도체를 만들어 추출의 효율을 높이면서 동시에 GC-ECD에 대한 크로마토그래피 성질을 향상시켰다. 생성된 유도체들은 chloroform으로 추출한 후 잔류물을 완전히 건조시키고 hexane으로 추출하여 GC-ECD로 분석하였다. Hcy와 Met는 $5-50{\mu}mol/L$, Cys는 $40-400{\mu}mol/L$ 범위에서 $R^2=0.990$ 이상의 직선상의 상관관계를 나타내었다. 이 작성한 질량곡선을 통해서 5명의 자원자들의 혈액 중에 존재하는 Hcy, Cys, met의 함량을 분석하였다.

임신부의 혈청 비타민 B 수준과 혈중 호모시스테인 수준 및 MTHFR 유전자형이 임신 결과에 미치는 영향 (Effects of Maternal Serum B vitamins, Homocysteine Concentrations and the 5, 10-Methylenetetrahydrofolate Reductase (MTHFR) Polymorphism on Pregnancy Outcomes)

  • Maternal nutritional status has been shown to influence pregnancy outcomes. And the elevated maternal plasma homocysteine concentrations have been associated with adverse pregnancy outcomes. We investigated the effects of maternal serum levels of B vitamins and homocysteine, and the C677T MTHFR (5, 10-methylenetetrahydrofolate reductase) polymorphism on pregnancy outcomes. In 177 pregnant women of 24-28 wks of gestation, the MTHFR gene mutation, serum B vitamins and homocysteine concentrations were measured, and their pregnancy outcomes were investigated from medical records. The birth length, and 1- and 5-min Apgar scores of neonates in the T/T mothers were 45.4 $\pm$ 9.3 cm, 7.6 $\pm$ 3.2 and 8.5 $\pm$ 3.8, respectively, which were significantly lower than those in the C/T (48.6 $\pm$ 3.3 cm, 9.0 $\pm$ 0.2, 10.0 $\pm$ 0.2) or the C/C mothers (49.4 $\pm$ 1.9 cm, 9.0 $\pm$ 0.2, 10.0 $\pm$ 0.0). The birth weight, birth length and the gestational age of neonates at delivery from hyperhomocysteinemic mothers whose homocysteine levels higher than 15 $\mu$ mol were 2.5 $\pm$ 1.3 kg, 43.9 $\pm$ 9.0 cm, 35.4 $\pm$ 6.3 wk, respectively, which were significant lower than those from normohomocysteinemic mothers (3.1 $\pm$ 0.6 kg, 48.8 $\pm$ 3.6 cm, 38.5 $\pm$ 2.5 wk). The birth weight and birth length of neonates in mothers whose PLP levels were below the median were significantly lower than those from mothers with the PLP levels above the median. The 1- and 5-min Apgar scores of neonates were lower in mothers with the T/T MTHFR genotype than those with the C/T or C/C only when the serum PLP levels were below the median. The 1-, 5 min Apgar scores and birth length of neonates were lower in mothers with the T/T MTHFR genotype than those with the C/T or C/C only when the serum FMN levels were below the median. In conclusion, maternal B vitamin status, homocysteine and the C677T MTHFR genotype seem to have played an important role on pregnancy outcomes.

건강한 성인에서 대뇌 백질 변성과 호모시스테인 농도의 연관성 (Relationship between White Matter Changes and Homocysteine Concentration in Healthy Adults)

  • 본 연구는 뇌졸중, 치매 병력이 없는 50-75세 건강한 성인을 대상으로 뇌 CT를 통해 대뇌 백질 변성과 뇌혈관 위험인자들이 상관성이 있는지 분석하고자 하였다. 2016년과 2017년에 일개 병원에서 건강 검진을 받은 900명 중 722명을 대상으로 연구를 진행하였으며 의무기록을 토대로 후향적으로 조사하였고, SPPS를 이용하여 카이제곱검정, 독립표본 t검정, 단변량 로지스틱 회귀분석을 이용하여 분석하였다. 대뇌 백질 변성이 있는 집단과 없는 집단으로 나누어 연구 대상자 특성을 확인하였는데, 대뇌 백질 변성이 있는 군이 없는 군보다 평균 연령과 고혈압, 당뇨 유병률이 높았고, 교육 기간은 짧았으며, 호모시스테인 농도가 높았다. 대뇌 백질 변성이 있는 군에서는 호모시스테인 농도가 증가함에 따라 백질 변성군 내에서 차지하는 비율도 증가했다. 호모시스테인 농도를 4분위하여 가장 낮은 집단(Q1)을 기준으로 상대적 위험도를 비교하였을 때, 나이[p<0.001], 고혈압[p<0.001], 고호모시스테인혈증(Q4)[p=0.021]이 대뇌 백질 변성의 위험인자임을 확인하였다. 이는 건강한 사람에게서 고혈압, 고호모시스테인혈증과 같은 대뇌 백질 변성의 조절 가능한 위험 인자를 확인했다는데 임상적으로 의의가 있다. 하지만 지금까지 고호모시스테인혈증의 일으키는 원인별 위험성에 대한 보고가 없고, 고호모시스테인혈증과 대뇌 백질 변성에 대해 한국인을 대상으로 한 연구가 알려지지 않아 대규모 전향적 연구가 필요할 것으로 사료된다.

공복혈당장애 성인에서 엽산 또는 아스코르브산의 보충급여가 혈장 호모시스테인 수준과 산화 스트레스에 미치는 영향 (Effects of Folic Acid or Ascorbate Supplementation on Plasma Homocysteine Levels and Oxidative Stress in Korean Adults with Impaired Fasting Glucose)

  • Impaired fasting glucose (IFG) is one of significant risk factors of developing diabetes. The persons with IFG are, thus, an important target group for primary prevention of diabetes. It is well known that plasma homocysteine concentration may be increased in poor folate nutritional status. Elevated level of plasma homocysteine is considered as a marker of enhanced oxidative stress. In addition, the protective effect against oxidative stress may be diminished in poor antioxidative nutrient status as vitamin C. It is, therefore, important to maintain adequate nutritional status of folate and vitamin C in the patients with type 2 diabetes or IFG. This study was performed to determine the effects of supplementation of folic acid or vitamin C on plasma concentrations of homocysteine, oxidized LDL, and lipids and on the activity of plasma anti-oxidative enzyme in patients with IFG. A total of 97 patients with IFG were participated voluntarily with written consents. They were divided into one of the four experimental groups; Control (C), Folatesupplemented (F), Ascorbate-supplemented (A), and Folate plus ascorbate-supplemented (FA). The subjects in C were taken placebo, those in F were supplemented 1 mg of folate, those in A were received 1,000 mg of vitamin C, and those in FA were given 1 mg of folate plus 1,000 mg of vitamin C daily for 4 weeks. No change in plasma concentrations of vitamin C, lipids, and oxidized LDL and the activity of GSH-Px were observed in vitamin C-supplemented group (A + FA) and folate-supplemented group (F + FA) compared to the placebo group (C + A). Only the folate-supplemented group (F + FA) had significantly increased average serum folate concentration and lowered plasma homocysteine concentration compared to the placebo group (C + A). Thus, it should be recommended the patients with IFG to increase folate intake through diets and, if it is not sufficient, to take folic acid supplements to prevent the development of complications induced by hyperhomocysteinemia as well as oxidative stress.
