Combined Trial of Fish Oil and Exercise Training Prevents Impairment in Insulin Action on Glucose Transport of Skeletal Muscle Induced by High-Fat Diet in Rats

  • The purpose of the present study was to determine the preventive effects of combined interventional trial of fish oil treatment and exercise training on insulin resistance of skeletal muscle in high-fat fed rats. Male Wistar rats were randomly divided into chow diet (CD), high-fat diet (HF), high-fat diet with fish oil (FO), high-fat diet with exercise training (EX), and FO+EX groups. The rats in control group were fed chow diet containing, as percents of calories, 58.9% carbohydrate, 12.4% fat, and 28.7% protein. High-fat diet provided 32% energy as lard, 18% as corn oil, 27% as carbohydrate and 23% as casein. The fish oil diet had the same composition as the high fat diet except that 100 g menhaden oil was substituted for corn oil. Insulin sensitivity was assessed by in vitro glucose transport in the soleus muscle after diet treatment and treadmill running for 4 weeks. While the FO or EX only partially prevented insulin resistance on glucose transport and visceral obesity induced by high-fat diet, these interventions completely corrected hyperinsulinemia and hyperglycemia from the high-fat diet. The rats in the FO+EX showed normalized insulin action on glucose transport, plasma chemicals and visceral fat mass. Insulin-mediated glucose transport was negatively associated with total visceral fat mass (r=-0.734; p<0.000), plasma triglyceride (r=-0.403; p<0.05) and lepin (r=-0.583; p<0.001) concentrations with significance. Multiple stepwise regression analysis showed that only total visceral fat mass was independently associated with insulin-mediated glucose transport (r=-0.668; p<0.000). In conclusion, combined interventional trial of FO+EX recovered insulin resistance on glucose transport of skeletal muscle induced by high-fat diet. Visceral fat mass might be more important factor than plasma TG and leptin to induce insulin resistance on glucose transport of skeletal muscle in high-fat fed rats.

한국인 젊은 여성에서 고당질, 고지방 및 고단백질 식사가 식후 열생성에 미치는 영향 (Effects of High Carbohydrate, High Fat and Protein Meal on Postprandial Thermogenesis in Young Women)

  • 일반식사, 고당질식사, 고단백식사 및 고지방식사가 DIT에 미치는 영향을 관찰하기 위하여 건강한 성인 여성에게 각각 동량의 에너지를 급식시켰을 때 에너지 소모량과 혈액중 포도당, 지질 및 인슐린 함량변동을 3시간 동안 관찰한 결과 다음과 같은 성적을 얻었다. 고당질식사, 고단백식사, 고지방식사 및 일반식사의 섭취에 따른 혈당의 $\delta$-AUC 값은 각각 301.9$\pm$64.4 mg/dL, 63.8$\pm$ 14.4 mg/dL, 22.3$\pm$8.3 mg/dL, 153.4$\pm$ 19.8 mg/dL로 고당질식사가 가장 높았고, 고지방식사에서 가장 낮았다. 혈중 인슐린의 $\delta$-AUC 값은 고당질식사와 고단백식사에서 314.1$\pm$45.3 $\mu$IU/mL 및 165.3$\pm$ 23.8$mu$IU/mL이었고, 고지방식사와 일반식사에서 각각 102.4$\pm$11.4 $\mu$IU/mL 및 244.5$\pm$24.4$mu$IU/mL이었으며 고당질식사가 가장 높았고, 고지방식사에서 가장 낮았다. 혈중 중성지방의 $\delta$-AUC 값은 고당질식사, 고단백식사, 고지방식사 및 일반식사에서 각각 -4.7$\pm$21.5 mg/dL, 44.3$\pm$13.9 mg/dL, 120.4$\pm$44.7 mg/dL, 50.5$\pm$48.5 mg/dL로 고지방식사가 가장 높았고, 고당질식사에서 가장 낮았다. 섭취한 열량에 대한 DIT의 비율은 일반식사의 경우8.7$\pm$2.0$\%$였으며, 고당질식사와 고단백식사 및 고지방식사에서 각각 10.4$\pm$1.3$\%$, 12.9$\pm$0.5$\%$, 6.9$\pm$1.2$\%$로, 고단백식사가 고지방식사에 비해 유의적으로 높은 결과를 보였다. 그러나 DIT가 인슐린 및 제지방량과 상관성을 보이지 않았다.

Nutritional Evaluation of Young Bulls on Tropical Pasture Receiving Supplements with Different Protein:Carbohydrate Ratios

  • The objective of this work was to evaluate the nutritional parameters of young bulls supplemented with different ratios of protein: carbohydrate on tropical pastures from 4 until 18 months old. Fifty-five non-castrated beef calves ($138.3{\pm}3.4kg$, 90 to 150 d of age) were used. The calves (young bulls) were subjected to a 430-d experimental period encompassing 4 seasons. The treatments were as follows: control, only mineral mixture; HPHC, high protein and high carbohydrate supplement; HPLC, high protein and low carbohydrate supplement; LPHC, low protein and high carbohydrate supplement; and LPLC, low protein and low carbohydrate supplement. The amount of supplement was adjusted every 28 d. Dry matter (DM) intake was higher in the dry-to-rainy transition and rainy seasons for all nutritional plans. Non-supplemented animals had lower intakes of DM and total digestible nutrients (TDN) than supplemented young bulls in all seasons. Although differences in DM intake were not observed between supplemented animals, the supplements with high carbohydrate (HPHC and LPHC) had lower forage intake during suckling (rainy-to-dry transition season) and in the rainy season. However, the HPHC treatment animals had higher intake and digestibility of neutral detergent fiber. It can be concluded that supplementation with high protein levels (supplying 50% of the crude protein requirement) provide the best nutritional parameters for grazing young bulls in most seasons, increasing intake and digestibility of diet, and these effects are more intense when associated with high carbohydrate levels level (supplying 30% TDN requirement).

  • To test the effect of different carbohydrate sources, wheat flour, used as the carbohydrate source in the control diet, was replaced with 30% glucose, 30% maltose, 30% cellulose or 30% $\alpha$-starch. After feeding trial for 8 weeks, no significant differences were found in growth and proximate composition of edible fraction of the snail fed diets containing different carbohydrate sources. All the tested carbohydrates are shown as good dietary carbohydrate source.

청소년기 여자의 칼슘과 인 평형 연구 (Ca and P Balance in Korean Female Adolescents)

  • Intakes and excretions of calcium and phosphorus were determined for 8 female adolescents(aged 16.3$\pm$0.5y; body mass index 20.4$\pm$1.3kg/$m^2$; body fat 33.3$\pm$2.5%; bone mineral density of lumbar spine in L2-L4; 0.96$\pm$0.08g/$\textrm{cm}^2$) when they consumed diets basal and high in calcium for 6 days each. All subjects consumed a basal Ca diet containing 800mg, Korean RDA level of the subjects, and a high Ca diet containing 1200mg, RDA plus 2 SDs of calcium intake. The diets provided 58% of energy intake as carbohydrate, 25% as fat, and 17% as protein. Food, urine, and fecal composites were collected during the last 3 days of each feeding period. Fecal excretion of calcium was significantly greater on the high Ca than on the basal Ca diet. Hence, apparent absorption rate was significantly lowered from 40.9% on the basal Ca diet to 33.1% on the high Ca diet. There was no significant difference in calcium retention between the two diet periods but it tended to be greater on the high Ca diet. However, excretions and retention of phosphorus did not show any significant difference even though intake was significantly increased from 1,253mg on the basal Ca diet to 1,583mg on the high Ca diet. The results indicate that higher calcium intake than the Korean RDA level is recommended for adolescents to meet peak bone mineral accretion and attain a desirable level of calcium retention. Association between Vegetable, Fruit and Carbohydrate Intake and Breast Cancer Risk in Relation to Physical Activity

  • Background: Although the nutritional may exert effect on the breast cancer risk, it is not clear whether the role diet is the same in sedentary and physically active women. The aim of this study was to evaluate the association between fruit, vegetable and carbohydrate intake and the risk of breast cancer among Polish women considering their physical activity level. Materials and Methods: A case-control study was conducted that included 858 women with histological confirmed breast cancer and 1,085 controls, free of any cancer diagnosis, aged 28-78 years. The study was based on a self-administered questionnaire to ascertain physical activity, dietary intake, sociodemographic characteristics, reproductive factors, family history of breast cancer, current weight and high, and other lifestyle factors. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated in unconditional logistic regression analyses including a broad range of potential confounders. Results: With comparison of the highest vs lowest quartile of intake, strong significant associations were observed for total vegetables (OR=0.37, 95%CI=0.20-0.69 P for trend <0.01 and OR=0.53, 95%CI=0.29-0.96, P for trend <0.02), and total fruits (OR=0.47, 95%CI=0.25-0.87, P for trend <0.05 and OR=0.47, 95%CI=0.24-0.90, P for trend <0.02) among women characterized by the lowest and the highest quartile of physical activity. No associations were observed for total carbohydrate intake. Additional analysis showed a positive association for sweets and desert intake among women in the lowest quartile of physical activity (OR=3.49, 95%CI=1.67-7.30, P for trend <0.009) for extreme quartiles of intake comparing to the referent group. Conclusions: The results suggest that a higher consumption of vegetable and fruit may be associated with a decreased risk of breast cancer, especially among women who were low or most physically active throughout their lifetimes. These findings do not support an association between diet high in carbohydrate and breast cancer. However, a higher intake of sweets and deserts may by associated with an increased risk of breast cancer among women who were less physically active.

영양소 함량이 다른 부상 배합사료의 공급횟수가 육성기 넙치의 영양소 소화율에 미치는 영향 (Effect of Feeding Frequency of Extruded Diets Containing Different Macro-nutrient Levels on Apparent Nutrient Digestibility in Grower Flounder Paralichthys olivaceus)

  • 평균체중 280 g의 넙치를 대상으로 탄수화물, 단백질 및 지질 함량이 다른 3종류의 EP사료(HC, HP및 HL)를 제조하여, 사료 공급율과 공급휫수를 달리하여 영양소 소화율을 조사하였다. 사료의 공급율에 따른 실험(실험 1)은 파 사료별로 1일 1회 만복 공급구와 만복의 80% 공급구로 설정하였다. 사료 공급횟수에 따른 실험(실험 2)에서는 사료의 공급휫수를 각 사료마다 2일 1회, 1일 1회, 1일 2회 및 1일 3회로 설정하였다. 사료 공급율을 달리한 실험 1에서 넙치의 건물, 단백질, 지질, 에너지 및 NFE 소화율은 사료 조성과 공급율에 영향을 받지 않았다. (P>0.05).사료 공급횟수를 달리한 실험 2리 건물 소화율은 동일한 공급횟수에서 HP 공급구가 HC와 HL 공급구와 비교하여 높은 소화율을 보였다. 단백질 소화율은 사료 조성에 영향을 받아 HP 공급구가 수치상으로 높은 값을 보였으나 실험구간에 유의차는 없었다(P>0.05).지질 소화율은 사료 공급횟수에 영향을 받아 1일 2회와 3회 공급구에서 감소하는 경향을 보였다. NFE 소화율은 사료 조성과 공급횟수에 경향을 받았으며, 동일한 공급횟수에서 1일 3회 공급구를 제외한 실험 구에서 HP 공급구의 소화율이 높은 경향을 보였다. 그리고 동일한 사료에서는 2일 1회에서 1일 2회까지 공급횟수가 증가하면서 NFE 소화율이 낮아지는 경향을 보였다. 에너지 소화율은 동일한 공급 횟수에서 1일 1회 공급구를 제외하고 HP를 공급한 실험구가 다른 실험구에 비해 높았고, HP 공급구들 간에는 사료공급횟수에 영향을 받지 않았다.

The effect of high fat dietary modification and nutritional status on the outcome of critically ill ventilated children: single-center study

  • Background: Ventilator dependency constitutes a major problem in the intensive care setting. Malnutrition is considered a major determinant of extubation failure, however, attention has been attracted to modulating carbon dioxide production through decreasing carbohydrate loading and increasing the percent of fat in enteral feeds. The detected interrelation between substrate oxidation and ventilation outcome became the base of several research to determine the appropriate composition of the nonprotein calories of diet in ventilated patients. Purpose: We aimed to assess the effect of high-fat dietary modification and nutritional status on ventilatory and final outcomes of pediatric intensive care. Methods: Fifty-one ventilated children (1 month to 12 years of age) with pulmonary disease who could be enterally fed, in the Cairo University Pediatric intensive care unit, were divided into 2 groups: group A included 25 patients who received isocaloric high-fat, low-carbohydrate diet; group B included 26 patients who received standard isocaloric diet. Comprehensive nutritional assessment was done for all patients. Results: Group A had a significant reduction in carbon dioxide tension, but no similar reduction in the duration or level of ventilatory support. Assisted minute ventilation was predicted by weight-for-age and caloric intake rather than the type of diet. Poor nutritional status was associated with higher mortality and lower extubation rates. Mild hypertriglyceridemia and some gastrointestinal intolerance were significant in group A, with no impact on the adequacy of energy or protein delivery. Conclusion: The high-fat enteral feeding protocol may contribute to reducing carbon dioxide tension, with mild hypertriglyceridemia and negligible gastrointestinal intolerance as potential adverse effects. Optimization of nutritional status rather than dietary modification may improve ventilatory and survival outcomes in critically ill-ventilated children.

전통 한국 식이의 비교 영양학 (Comparative Nutrition of Traditional Korean Diet)

  • Rice is the primary main dish of Traditional Korean diet. Although there have been changes in food consumption and nutrient intake among Koreans, traditional dietary pattern is stil dominant among Koreans. Traditional Korean diet has emphasized breakfast, which is the most frequently missed meals in Korea today but important for daily work performance and health. Compared to diets of the U.S. and Greece, Korean diet is high in carbohydrate and low in fat and cholesterol due to low intake of meat. Koreans also consume large amount of plant food, which makes fiber content of diet to be high. However fruit and milk consumption tends to be low in Korea. Koreans use fermented food, including kimchi, very frequently as well as foods cooked and consumed at high temperature and over direct fire. Traditional cooking methods are time consuming which limits the usage among modern city dwellers with working women. Despite the strengths of traditional Korean diets in reducing risk factors of chronic diseases, preservation of the tradition in modern Korean society requires special attention and efforts to make them more adaptable to contemporary life styles.

한국 노인 식사의 탄수화물 에너지비에 따른 만성질환 위험성 비교: 2007~2009년 국민건강영양조사 자료 이용 (Comparison of chronic disease risk by dietary carbohydrate energy ratio in Korean elderly: Using the 2007-2009 Korea National Health and Nutrition Examination Survey)

  • 우리나라 대다수 노인이 고 탄수화물 식사를 하고 있고, 고 탄수화물 식사가 만성질환의 위험과 관련될 수 있다고 보고되었다. 이에 본 연구는 한국 노인에서 탄수화물 에너지비 수준에 따른 만성질환 위험성을 파악하고자 2007~2009년 제4기 국민건강영양조사 자료를 이용하여 65세 이상 노인 3,917명을 대상으로 1일 에너지 섭취량이 500~5,000 kcal에 속하면서, 고혈압, 당뇨, 고지혈증, 뇌졸중, 심근경색 또는 협심증, 빈혈로 치료받는 자를 제외한 1,535명에 대한 자료를 분석하였다. 식사의 탄수화물 에너지비가 55~70%인 적정군과 70%를 초과한 과다군의 두 군으로 대상자를 분류하여 영양섭취상태와 만성질환의 위험 비율을 비교하였다. 모든 자료는 제4기 국민건강영양조사의 가중치를 적용한 후 성, 거주지역, 소득수준, 교육수준 또는 에너지 섭취량을 보정하여 일반선형모델을 사용하여 분석하였다. 만성질환 위험도는 로지스틱 회귀모형을 적용하여 만성질환 위험요인의 판별기준치에 따라 위험군의 백분율과 교차비를 구하고 ${\alpha}$ = 0.05 수준에서 유의성을 검토하였다. 1) 전체 대상자 중 탄수화물 과다군이 81.4%로 적정군에 비해 4배 이상 많았다. 과다군에 남자보다는 여자가 많았고, 읍면에 거주하고, 소득수준과 교육수준이 낮은 사람이 많았다. 2) 체질량 지수에서는 두 군 간에 차이가 없었으나, 체중, 허리둘레, 이완기 혈압이 과다군에 비해 적정군에서 높았다. 3) 탄수화물비 과다군에서 육류 및 난류, 우유 및 유제품, 주류의 섭취 빈도가 낮았고, 반면에 서류의 섭취 빈도는 높았다. 남자에서는 탄수화물 적정군에서 육류 및 난류의 섭취빈도가 높았고, 여자에서는 육류 및 난류 외에도, 우유 및 유제품, 스낵류, 해조류의 섭취 빈도가 적정군에서 높았다. 4) 탄수화물: 단백질: 지방 에너지비는 과다군 79.87: 11.66: 8.46, 적정군 64.88: 16.18: 18.94로 두 군 간에 탄수화물비 외에도 단백질비와 지방비에 큰 차이가 있었고, 에너지 섭취량은 과다군의 1,492 kcal에 비해 적정군이 1,722 kcal로 높았다. 조섬유, 칼륨, 비타민 A, 카로틴, 비타민 C섭취량은 두군 간에 차이를 보이지 않았으나, 나머지 영양소-단백질, 지방, 칼슘, 인, 철분, 레티놀, 티아민, 리보플라빈, 나이아신, 수분-의 섭취량이 탄수화물 적정군에서 높았다. 5) 영양소적정섭취비 (NAR)와 영양밀도지수 (INQ) 역시 비타민 C를 제외한 모든 영양소에서 탄수화물 적정군이 높았다. 성별로 분석하였을 때에도 전체 대상자에서와 비슷한 경향이었으나, 특히 칼슘, 리보플라빈의 영양소적정섭취비와 영양밀도지수가 여자 탄수화물 과다군에서 매우 낮았다. 6) 탄수화물 적정군에서 과다군에 비해 여성은 복부비만의 위험이 1.719배, 남자는 이상지질혈증의 위험이 2.094배 높았다. 이를 요약하면 탄수화물 과다군에서 동물성 식품의 섭취빈도가 낮고 에너지 및 다수 영양소의 섭취량이 낮아 영양부족의 우려가 있으나 빈혈 위험도에는 차이가 없었고, 남자에서는 이상지질혈증, 여자에서는 복부비만의 위험이 탄수화물 적정군에 비해 낮게 나타나서 탄수화물 과다군에서 만성질환의 위험이 높고 빈혈 위험이 높으리라는 연구 가설과는 일치하지 않는 결과를 나타내었다. 추후 이에 대한 확인과 함께, 노인 연령층을 일반 성인층에서 분리하여 만성질환 위험을 낮추는데 도움을 줄 수 있는 노인의 적정 탄수화물 에너지비 설정을 위한 연구가 필요하다.
