This study aimed at investigating correlation of nutrients intake and serum lipids in middle-aged korean male and female workers. One hundred sixty eight(male ;105, female ;63) for adult aged 30-59yr(average age male ;36.5yr, female ;44.5yr) were selected as subjects during 2 months, from April to May, 1996. Nutrients intake status was investigated by questionnaire, 24-hour recall method. Antropometric assessments, serum lipids and blood pressure of the subjects were investigated. The results as follows: 1. Nutrients intake status ; male was superior to female. Vitamin A intakes of 40-49yr group were inferior to other group but amounts of intake were above Korean RDA(recommended dietary allowances), Intakes of calcium and vitamin C of 30-39yr and over 50yr group in female were below Korean RDA. In overweights group of male and underweight group of female, most nutrients intake were increased than other groups. 2. Antropometric assessments ; female was higher than male in BMI (body mass index). All subjects increasingly with age were increased in BMI. 3. Blood components were above normal range in triglyceride in both sexes and all subjects increasingly with age were increased In level of serum lipids and blood pressure, especially in male's forty and in female's fifty were higher than other groups In serum lipids and blood pressure. In overweights group, serum lipids and blood pressure had a significantly($\alpha$=0.05) increasing tendency in both sexes. 4. Serum lipids and connected factors; atherogenic index correlated positively with triglyceride, total-chol(total cholesterol), LDL-chol(low density-lipoprotein cholesterol) and negatively with HDL-chol(high-density-lipoprotein cholesterol) significantly(P<0.001). Systolic blood pressure correlated positively with total-chol and diastolic blood pressure correlated positively with total-chol, LDL-chol significantly(P<0.05). 5. Serum lipids and nutrients intake status: energy correlated positively(P<0.05) with BMI, dietary fiber correlated positively with LDL-chol(P<0.05), calcium correlated positively with triglyceride, atherogenic index and dietary fiber correlated negatively with BMI in male significantly(P<0.05). Triglyceride correlated positively with vitamin C and phosphorus correlated negatively with BMI in female significantly(P<0.05). Above results, in male's forty and in female's fifty were liable to variation in terms of serum lipids and blood pressure. As increasing triglyceride, total-chol, LDL-chol and decreasing HDL-chol Increased atherogenic index that suggest increasing risk of atherosclerosis. But, this point will be considered more subdivided study.

Purpose: The objective of this study was to find the predictors and generate a prediction scoring model of nonresponse to intravenous immunoglobulin in patients with Kawasaki disease. Methods: We examined 573 children diagnosed with KD at the Severance Children's Hospital between January 2009 and december 2012. We retrospectively reviewed their medical records. These patients were divided into 2 groups; the experimental group (N=433) and the validation group (N=140). Each group were divided into 2 groups the intravenous immunoglobulin nonresponders and the responders. Multivariate logistic regression analysis identified predictive factors of intravenous immunoglobulin nonresponders which make predictive scoring model. We practice internal validation and external validation. Results: Multivariate logistic regression analysis identified male, cervical lymphadenopathy, changes of the extremities, platelet, total bilirubin, alkaline phophatase, lactate dehydrogenase, C-reactive protein as significant predictors for nonresponse to intravenous immunoglobulin. We generated prediction score assigning 1 point for (1) male, (2) cervical lymphadenopathy, (3) changes of the extremities, (4) platelet (${\leq}368,000/mm^3$), (5) total bilirubin (${\geq}0.4mg/dL$), (6) alkaline phophatase (${\geq}227IU/L$), (7) lactate dehydrogenase (${\geq}268IU/L$), (8) C-reactive protein (>77.1 mg/dL). Using a cut-off point of 4 and more with this prediction score, we could identify the intravenous immunoglobulin nonresponder group. Sensitivity and specificity were 52.5% and 82.4% in experimental group and 37.8% and 81.8% in validation group, respectively. Conclusion: Our predictive scoring models had high specificity and low sensitivity in Korean patients. Therefore it is useful in predicting nonresponse to intravenous immunoglobulin with Kawasaki disease.

A study was carried out to determine the effect of a very low-carbohydrate diet on weight gain, body composition, and serum levels of cholesterol, triacylglycerol, glucose, ketone bodies and insulin. Twenty rats (mean initial weight, 212 g) were divided into two groups and each group was assigned a conventional high-carbohydrate diet (control) or a very low-carbohydrate diet containing 59.8% ground pork rind snack and fed the diet for four weeks. Average daily body weight gain was not different between the two groups during the first two weeks, but was significantly lower in rats fed the very low-carbohydrate diet than in those in the control group during third (p<0.05) and fourth weeks (p<0.01). Feedintake as well as energy intake was lower in rats fed the very low-carbohydrate diet than in those in the control group. The very low-carbohydrate diet reduced (p<0.01) serum triacylglycerol (34$\pm$83 vs 82$\pm$8 mg/l00 mL) and insulin (3.90$\pm$0.53 vs 7.60$\pm$0.61 $\mu$IU/mL) levels, while increasing (p<0.01) ketone body level (368$\pm$25 vs 236$\pm$24 $\mu$mol/L), compared with the control. Serum glucose and total cholesterol levels were not different (p>0.05) between the two dietary treatments. Proximate analysis of carcasses showed that the very low-carbohydrate diet decreased (p<0.01) body fat (26.1$\pm$1.04 vs 30.5$\pm$0.86%), while increasing (p<0.01) body protein (63.1$\pm$0.94 vs 59.4$\pm$0.70%) contents. Results indicate that short-term feeding of a very low-carbohydrate diet is beneficial for alleviating risk factors known to involve cardiovascular diseases or artherosclerosis. However, more studies with model animals as well as humans are recommended to examine the long-term health benefits of low-carbohydrate diets.

Background: Distance from anal verge and abdominoperineal resection are risk factors for circumferential resection margin (CRM) positivity in rectal cancer. Induction chemotherapy (IC) before concurrent chemoradiation (CRT) has emerged as a new treatment modification. Impact of IC before concurrent CRT on CRM positivity in low rectal cancer remains to be independently studied. The objective of this study was to determine CRM positivity in low rectal cancer, with and without prior IC, and to identify predictors of disease free and overall survival. Materials and Methods: Patients who underwent surgery for rectal cancer between 2005 and 2011 were retrospectively reviewed and divided into two groups. Group 1 received IC before CRT and Group 2 did not. Demographics, clinicopathological variables and CRM status were compared. Actuarial 5 year disease free survival (DFS), overall survival (OS) and independent predictors of survival were determined. Results: Patients in the IC group presented with advanced stage (Stage 3=89.2% versus 75.4%) (P=0.02) but a high rate of total mesorectal excision (TME) (100% versus 93.4%) (P=0.01) and sphincter preservation surgery (54.9 % versus 22.9%) (P=0.001). Patients with low rectal cancer who received IC had a significantly low positive CRM rate (9.2% versus 34%) (P=0.002). Actuarial 5 year DFS in IC and no IC groups were 39% and 43% (P=0.9) and 5 year OS were 70% and 47% (P=0.003). Pathological tumor size [HR: 2.2, CI: 1.1-4.5, P=0.01] and nodal involvement [HR: 2, CI: 1.08-4, P=0.02] were independent predictors of relapse while pathological nodal involvement [HR: 2.6, CI: 1.3-4.9, P=0.003] and IC [HR: 0.7, CI: 0.5-0.9, P=0.02] were independent predictors of death. Conclusions: In low rectal cancer, induction chemotherapy before CRT may significantly decrease CRM positivity and improve 5 year overall survival.
Background: To explore the safety, efficacy, and oncological outcome of 3-port laparoscopic radical cystectomy (LRC) compared to open radical cystectomy (ORC) in patients older than 75 years. Materials and Methods: From June 2010 to July 2014, we analyzed 16 radical cystectomies in patients older than 75 years (LRC group=8; ORC group=8). Demographic parameters, operative variables, and perioperative outcome in the 2 groups were retrospectively collected, analyzed, and compared. Results: Patients in both groups had comparable preoperative characteristics. A significantly longer operating time (476 vs. 303 min, P=0.0002) and less estimated blood loss (627 vs. 2,106 mL, P=0.021) were observed in the LRC group compared to the ORC group. Infection and ileus were the most common early complications after surgery. Patients who underwent ORC suffered from more postoperative infection (22.2% vs. 0.0%, P=0.054) and ileus (25.0% vs. 12.5%, P=0.521) than the LRC group, but the difference was not significant. Conclusions: Judging from this initial trial, 3-port LRC can be safely carried out in elderly patients. We suggest 3-port LRC as the primary intervention to treat muscle-invasive or high-risk nonmuscle-invasive bladder cancer in elderly patients with an otherwise relatively long life expectancy.
This study was conducted to examine the effects of Cheonggukjang powder made using black foods on liver function and lipid composition in streptozotocin(STZ)-induced diabetic rats. The experimental animals were divided into 5 groups and fed the following for 7 weeks; normal diet(control), STZ+normal diet(Diabetic), STZ+50% soybean Cheonggukjang supplementation(DSC), STZ+44.5% yakkong Cheonggukjang supplementation(DYC), and STZ+supplementation with 50% yakkong black food(black rice, black sesame seeds, and sea tangle) Cheonggukjang(DYCB). The results showed that the body weight gain and food efficiency ratio of the STZ-induced diabetic groups were significantly lower than those of the control group. In the Diabetic group, glutamic oxaloacetic transaminase(GOT) and glutamic pyruvic transaminase(GPT) activities and total bilirubin content in serum were significantly greater than those in the control group. However, supplementation with Cheonggukjang reduced these values. In the Diabetic group, the triglyceride, total cholesterol, and low-density lipoprotein(LDL)-cholesterol contents in the serum and liver tissue, as well as the atherogenic index(AI) and cardiac risk factors(CRF) were significantly higher than the corresponding values in the control group, although the high-density lipoprotein(HDL)-cholesterol and phospholipid contents were significantly lower than those in the control group. However, supplementation with Cheonggukjang normalized the changed lipid composition in the STZ-induced diabetic rats. Further, yakkong Cheonggukjang and black food contaning yakkong Cheonggukjang normalized AI and CRF.

Background: Adding taxanes to adjuvant antracycline and cyclophosphamide (AC) in combination may provide significant improvement in node-positive and high risk node-negative breast cancer (BC) patients. However, the optimal dose and the role of dose-dense (DD) chemotherapy have yet to be determined. The aim of this study was to compare the efficacy of a DD paclitaxel (P)-AC combination with conventional weekly P-AC or docetaxel D-AC combinations in patients with node-positive breast cancer. Materials and Methods: Newly diagnosed 280 node-positive BC patients diagnosed from 1998 to 2013 in three clinics were retrospectively analyzed. Demographic and medical data were collected from the medical charts. Patients were categorized to 3 groups according to treatment arms: arm A, ddAC-P; arm B, weekly P and AC combination; and arm C; T and AC combination. Adjuvant trastuzumab was added for HER2-positive patients. Kaplan-Meier survival analysis was carried out for disease free survival (DFS) and overall survival (OS). The log-rank test was used to examine the statistical significance of the differences observed between the groups. Two-sided P values <0.05 were considered statistically significant. Results: Of the total of 280 patients, 101 were in arm A, 114 in arm B and 65 in arm C.The median ages were 49, 50 and 46, respectively (p=0.11). Median follow-up was 39 (3-193) months. Stage, lymphovascular and perineural invasion, receptor patern, and menopausal status were similar in the 3 treatment arms, but HER2 positivity was significantly lower in arm A, compared to arms B and C (25.7%, 53.1%, 41.5% in arms A, B and C, respectively; p<0.001). Also grade 3 tumors were significantly less frequent in treatment arm A compared to arm B and C (27.3%, 56.8% and 49.2%, respectively, p=0.01). Afterunivariate and multivariate analysis were performed, 3-year DFS rates were 89%, 81%, and 75%, respectively (p=0.12) and three year OS rates were 96.6%, 89%, and 75% (p=0.62). Conclusions: In this study, no significant difference was found between adjuvant dose dense and conventional taxane treatment regimens.

The antihypertensive and cholesterol- lowering effects of Monascus koji (M. koji) prepared with Monascus ruber IFO32318 were examined in spontaneously hypertensive rats (SHR). Control was fed a normal diet and groups I, II or III were fed diets containing 0.03%, 0.1% or 0.3% M. koji for 8 weeks, respectively. After 8 weeks, all animals were fed normal diets in the following 2 weeks. The blood pressure of rats fed M. koji added diets were significantly attenuated as compared with control and the diet containing high concentration of M. koji had a tendency of stronger antihypertensive effect. These differences lasted throughout the experimental period when they were fed experimental diet. For 2 weeks after the 8 weeks of experimental diet all groups were fed the same normal diet and the differences of blood pressure caused by M. koji disappeared. In rats fed the M. koji added diet, the serum total cholesterol (TC), HDL cholesterol (HDL-C), LDL cholesterol (LDL-C) were not differed compared with control, but VLDL cholesterol (VLDL-C) was significantly lowered. M koji also significantly decreased serum risk factors, both TC/HDL-C ratio and non HDL-C/HDL-C ratio. Consequently, it is suggested that M. koji may play an important role to attenuate hypertension and to improve serum lipid Profiles.

Purpose : The prevalence of hepatitis A virus (HAV) in a certain community reflects that community's living standards and hygienic conditions. And the pattern of HAV infection differs over time and geography. Recently, a shift in prevalence has been observed in cases from chilhood to adulthood. We studied the HAV antibody prevalence in the general population in Busan. Methods : From October 2004 to March 2005, total 472 subjects were tested for HAV antibodies. All samples were collected from patients in Maryknol Hospital. Results : The overall seropositive rate was 22.8% (108/472). The seropositive rates were 1.7% in subjects aged 2-5 years, 1.7% in 6-10 years, 0% in 11-20 years, 40.5% in 21-30 years, 82.1% in 31-40 years, 94.7% in 41-50 years, and 100% in subjects aged over 50 years. There was no significant gap between gender groups. Conclusion : As the socioeconomic conditions in Korea have improved, the HAV seropositive rate in school-aged children has dramatically decreased in the last 20 years. But, the seropositive rate of HAV didn't differ according to gender. The seropositive rate of HAV in the pediatric group was very low, which suggests the increasing possibility of clinical HAV infection in adults in the near future. Therefore, we should actively prevent the spread of hepatits A virus. In order to do that, we need to reorganize our lifestyle and personel hygiene and carry out active and passive immunization to high risk groups.
Objective: This study was carried out to evaluate the distribution and clinical features of patients with degenerative change of the mandibular condyle. Methods: Six thousand and seventy patients with TMD (temporomandibular cisorder) were selected for this study, who had complete initial clinical records and radiographs. Panorama and TMJ panorama radiographs were used to screen the degenerative change in the condyle, and the patients were divided into DJD (degenerative joint disease) and non-DJD groups. Results: The distribution. and clinical features of the two groups were compared. Out of the total number of patients, 31.7% were in the DJD group, and 68.3% were in the non-DJD group. The portion of females was larger in the DJD group (80.8%) than in the non-DJD group (67.5%), and the DJD group had high prevalence in the second and third decades. Lack of incisal contact, retrusive chin, facial asymmetry, and mouth opening limitation were the chief complaints of the patients who had positive relation to DJD. Conclusion: Patients with an orthodontic treatment history, CO-CR discrepancy and crepitation were at possible risk of having DJD.

② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
