Among pre-harvest environmental factors, increasing attention has been paid to the effects of chemical and microbiological factors on fresh produce. The occurrence and prevalence of these factors have been usually studied with regard to the final products at the post-harvesting stage and/or when they are sold in the market. However, the origin and routes of transmission of both factors remain to be clarified. In the present study, we examined the contamination levels of food-borne pathogens and chemical factors such as pesticide residues and heavy metals in 83 and 43 samples, respectively, including various soil, water, and fertilizer samples, as well as post-harvested and processed samples. Among the organic farming samples, only one pesticide, dimethomorph, was detected in the soil sample, however no pesticides were observed from any other samples in organic farming system. Thus, it was thought that might be contaminated from conventional farm land in the vicinity. Whereas many pesticide residues were detected in conventional farming systems such as soil, fertilizer, water, and fresh produce as expected. Furthermore, heavy metals detected from all tested samples did not shown contamination levels higher than the standard limit. We comparatively assessed the levels of contamination by food-borne pathogens on the samples from organic and conventional farming systems, and found aerobic bacteria at approximately 7 log CFU/g, with no significant differences observed between the two systems. Coliforms were present at lower levels than aerobic bacteria. No human pathogens were present among the coliforms detected, indicating that these bacteria are saprophytes without the ability to cause food-borne illnesses. In contrast, among the high-risk food-borne pathogens, only sporadic cells of Bacillus cereus were found on samples of organic farming system. These data extend previous findings that the most prevalent food-borne pathogen is B. cereus and demonstrate that it spreads to whole living plants via soil.

Purpose : The incidence of neonatal hyperbilirubinemia is twice as high in East Asians as in Caucasians. However, its metabolic basis has not been clearly explained. The UDP-glucuronosyltransferase gene(UGT1A1) mutation was found to be a risk factor of neonatal hyperbilirubinemia. We studied whether neonatal hyperbilirubinemia is associated with the 1828G>A(rs 10929303) polymorphism of the UGT1A1 gene, which encodes for a key enzyme of bilirubin metabolism. Methods : The genomic DNA was isolated from 80 Korean full term neonates who had greater than a 12 mg/dL level of serum bilirubin with no obvious cause, and the genomic DNA was also isolated from 164 Korean neonates of the control population. We studied a single nucleotide polymorphism (SNP) of 1828G>A in the untranslated region of the UGT1A1 gene by direct sequencing. Results : Three of the 80 neonates with a serum bilirubin level above 12 mg/dL had homozygous mutations and 10 of the 80 neonates with a serum bilirubin level above 12 mg/dL had heterozygous mutations. Thirteen of the 164 neonates of the control group had homozygous mutations and 16 neonates of the control group had heterozygous mutations. The allele frequency of 1828G>A polymorphism of UGT1A1 in the hyperbilirubinemia group was 10.0 percent, which was not significantly different from the allele frequency of 12.8 percent in the control group. Conclusion : In this study, the 1828G>A polymorphism of the UGT1A1 gene was detected in the Korean neonates with neonatal hyperbilirubinemia. Our results indicated that this SNP is not associated with the prevalence of hyperbilirubinemia in Koreans.

Purpose : To investigate the feature of the range of measles in 2000, we studied epidemics of the measles in western Kyungnam District. Methods : Among 489 patients under 15 years of age who visited or were admitted to in our hospital from January to December in 2000, we selected 344 patients whose measles IgM antibody was positive. We investigated age, monthly incidence, clinical features, vaccination history, and measles IgG antibody. Results : Patients under 12 months of age occupied the largest proportion(36.9%). Patients firstly occurred in May and geometrically increased from October. Fever, cough and rashes were observed in all patients but conjunctivitis in 54.9%, Koplik spot in 23.8%. The first vaccination ratio was 41.8%. Compared with the number of patients and the first vaccination ratio, the first vaccination ratio was the lowest under 12 months of age. After that age group, the number of patients was decreased as the first vaccination ratio was increased. Among 152 patients sampled for measles IgG antibody, 35 patients had received the first vaccination and 6 patients had received the second vaccination. Among 35 patients who received the first vaccination, 22(62.9%) patients were negative of measles IgG antibody. Among 6 patients who received the second vaccination, 3(50%) patients were negative. Conclusions : Measles patients under 12 months still dominated. Therefore, routine vaccination of single measles vaccine, is currently done at 6 month when measles are prevalent, should be considered. High first vaccination failure suggests problems of vaccines itself, transport and storage rather than vaccination methods. Therefore a thorough investigation should be made.

Purpose : Every year, there is a remarkable increase in the prevalence of children with excess body fat. The aim of this study is to find a useful screening method in assessing adolescent, obesity and to specify the predictable risk factors that are related to adult cardiovascular disease according to BMI and obesity index. Methods : From July to November in 2001, a total of 2,814(male : female=2,011 : 803) mid to high school students who were in the obesity range according to obesity index(>20%) were evaluated according to height, weight, body mass index(BMI), obesity index, and serum lipid levels. Results : Among the obese students over 20% by obesity index, 86.6% were over the range of 95 percentile by BMI. Among students defined as obese by BMI, 21.0% of males and 21.4% of females students had hypercholesterolemia; by obesity index, the ratio was 20.7% male and 19.0% female. The frequency of hypertriglyceremia in male students was 15.0% in group I(overweight group, 85P97p); in female students 11.8%, 20.7% and 28.2% respectively. Conclusion : In this study, using BMI alone to test the serum lipid level in adolescent obesity had a limit of 10.0-17.0% omission. Therefore using obesity index as a screening method to find the adult cardiovascular disease would rather reduce the omission rate. The risk factor of cardiovascular disease according to BMI was the increasing level of triglyceride in both male and female students. We think that using the obesity index has more rationale rather than BMI in assessing lipid profiles.
Dialysis patients are at risk of malnutrition not only because of losses of nutrients during peritoneal dialysis but also because of anorexia that results in inadequate nutrient intakes. The aim of this study was to estimate the nutritional status of 154 patients receiving continuous ambulatory peritoneal dialysis (CAPD), especially focused on protein-energy malnutrition and vitamin and mineral status. The mean age of the subjects was $5.12\;{\pm}\;12.4\;y$ with educational years of $12.3\;{\pm}\;0.4\;y$ for male and $9.6\;{\pm}\;0.4\;y$ for female. The mean duration of dialysis was $22.7\;{\pm}\;21.7\;mo$. The causes of renal failure included diabetes (32.7), chronic glomerulonephritis (15.0%), and hypertension (8.5%). The main complications associated with chronic renal failure were hypertension (86.1%), diabetes (35.4%) and liver disease (9.0%). The mean daily energy intake was $1216.8\;{\pm}\;457.3\;kcal$ and increased to $1509.2\;{\pm}\;457.2\;kcal$ when added with the energy from dextrose in dialysate. The latter was still much lower than estimated energy requirement but energy intake per kg of body weight (28.1 kcal/1 g) was within the range of that recommended for CAPD patients' diet therapy (25 - 30 kcal/kg). The average daily intake of protein was $49.2\;{\pm}\;25.1\;g$ with 37.6% of the patients showing their intakes less than Estimated Average Requirement. The average protein intake per kg of weight was 0.9 g/kg, which is less than that recommended for CAPD patients (1.2-1.5g/kg) with mean serum albumin level $3.2\;{\pm}\;0.5\;g/dl$. The proportion of the patients with dietary calcium intake less than EAR was 90.9%, but when added with supplementary calcium (phosphorus binder), most patients showed their total calcium intake between EAR and UL. Fifty percent of the patients were observed with dietary iron intake less than EAR, however most patients revealed their total iron intake with supplementation above UL. The addition of folic acid with supplementation increased mean total folic intake to $1126.0\;{\pm}\;152.4\;{\mu}g$ and ninety eight percent of the subjects showed their total folic acid intake above UL. The prevalence of anemia was 83.1 % assessed with hemoglobin level, even with high intakes of iron with supplementation. Thirty four percent of the patients showed their fasting blood glucose was not under control $(\geq\;126\;mg/dl)$ even with medication or insulin probably due to dextrose from dialysate. The mean blood lipid levels were within the reference levels of hyperlipidemia, but with 72.1 % of the patients showing lower HDL-C. In conclusion, Fairly large proportion of the patients were observed with protein malnutrition with low intake of protein and serum albumin level. Few patients showed their vitamins and minerals intake less than EAR with supplementation. For iron and folic acid, their intakes were increased to above UL for large proportion of he patients. However, more than eighty percent of the patients were still anemic associated with decreased renal function. The serum blood glucose and lipid level were not under control for some patients with medication. It seems that supplementation and medications that patients are taking should be considered for dietary consulting of CAPD patients.
Coastal ferry routes are very important because they connect the mainland to islands, offer transportation for islanders, and bring tourism to islands. In particular, the recent increase in national income, prevalence of a culture that emphasizes work-life balance, and introduction of the five-day workweek have stoked interest in travel as a leisure pursuit, and demand for tourism from Korean people in islands has increased. As the number of passengers using coastal passenger terminals increases due to changes in the coastal tourism environment, the importance and need for coastal passenger terminal facilities and services is growing. Under these circumstances, it is necessary for the management organizations of coastal passenger terminals to manage and efficiently operate the terminals in such a way that budgets can be reasonably implemented and the convenience of passengers using the terminals is enhanced. This study primarily analyzed the management efficiency of coastal passenger terminals in terms of profitability and public interest using data envelopment analysis. The eight passenger terminals achieved low management efficiency in terms of profitability. These terminals should improve profitability by increasing income from terminal office rental and ancillary businesses and revenue from terminal fees by boosting island tourism and expanding overseas passenger transportation. The eight terminals with low management efficiency for public interest should increase the number of passengers by promoting island tourism, developing tour packages to Japan and China's coastal areas, developing new routes, and introducing super-high-speed ships.

The purposes of this study were to investigate the factors affection the Oral Impacts on Daily Performances for Children (C-OIDP) in elementary and middle school students, and identify the association between oral health-related behaviors, oral health condition and C-OIDP. A cross-sectional study was conducted in three schools in Incheon, Asan, Korea. A total of 175 selected children were interviewed by a trained examiner using a questionnaire. Oral Health Related Quality of Life was assessed by the Korean version of C-OIDP. Socio-economic characteristics, oral health-related behaviors, oral health condition and C-OIDP were verified using the questionnaire. ANOVA analysis was performed to determine the oral health and C-OIDP, and multiple regression analysis was performed to determine the factors affecting the C-OIDP. The activities with the greatest effect were eating (28.0%), cleaning teeth (22.9%), and smiling (18.9%). In the logistic regression model, the high item score of C-OIDP was associated with experiencing dental caries and gum pain in the past month. The more the C-OIDP prevalence item, the more the fillng deciduous tooth surface (fs) (p=0.024), caries experienced deciduous tooth surface (dfs) (p=0.049), total caries tooth surface (ds+DS) (p=0.021), and total caries experienced tooth surface (dfs+DMFS) (p=0.047). It can be concluded that the factors affecting C-OIDP are fs, dfs, dfs+DMFS, and gingival pain. Based on these results, we can improve C-OIDP to advance preventive practice.
Background: Pleural effusion is one of the most common clinical problems in pulmonology because of high prevalence of pulmonary tuberculosis and bronchogenic carcinoma in Korea. The differential diagnosis between pleural transudate and exudate is very important, but it is very difficult in some cases. Methods: In order to assess the clinical usefulness of cholesterol levels for the differential diagnosis of pleural transudate and exudate, we measured pleural fluid cholesterol levels by enzymatic method in 45 patients who were admitted due to pleural effusion. Results: The mean cholesterol level of transudate was $33.1{\pm}12.9\;mg%$, tuberculous exudate was $97.3{\pm}28.2\;mg%$ and malignant exudate was $97.3{\pm}28.2mg%$. When the cut-off value of pleural cholesterol level was 60 mg%, one case (6.7%) of tuberculous exudate and two cases (13.3%) of malignant exudate were incorrectly classified, but all cases of transudate were classified correctly. When the cut-off value of pleural/serum cholesterol ratio was 0.3, one case (6.7%) of transudate and two cases (13.3%) of malignant exudate were incorrectly classified, but all cases of tuberculous exudate were classified correctly. When the cut-off value of pleural cholesterol level to differentiate pleural transudate from exudate was 60 mg%, sensitivity was 90% and specificity was 100%. When the cut-off value of pleural/serum cholesterol level to differentiate pleural transudate form exuidate was 0.3, sensitivity was 93% and specifiity was 93%. Conclusions: From the above results, it can be concluded that measurement of pleural fluid cholesterol levels is useful for the differential diagnosis between pleural transudate and exudate.

The number of the disabled person had been increased for the industrial accident and the environmental pollution. Especially, developmental disability has the high prevalence rate between 5% and 10% of the whole children. The children with a developmental disability can be treated by the physical therapy, the occupational therapy, the psychology therapy, speech therapy, and art therapy. Visual preception which is function to recognize the external environment through the optic organ could be related to most behaviors on the everyday life. But because the children with disability could not develop the visual-preception enough, they came to have difficulties in executing daily life project. For this reason, it is most important to understand the estimation and the cure on the visual-preception in the pediatric occupational therapy. To improve the visual-preception power, we have many kind of methods including sensory integration, training program for the visual-perception and art-craft program. Particularly, the art-craft which is the representative activity for making something by hands, can be applied to anyone. As the study on the brain has been activated, it was proved that handicraft actives could have an good effect on the brain function and using brain. When the fine motor exercise and more delicate and accurate motion were carried, these motions need the essential help of the visual-perception. So it could be expected that using the repetitive hand function by art-craft makes the brain function improve, when a activity that needs a fine motor exercise and more delicate, accurate motion was carried, It also indicates that the art-craft program has a clear treatment value. Though the intervention between visual-perception development and visual-perception disability have a majority in the field of occupational therapy, there is a few study yet. Therefore, this study tried to look back on the necessity of applying the art-craft program to the children with disability as the prestudy for preliminary validity of the master's thesis.

Objectives: The purpose of this study was to identify the health promotion behavior, health problem, perceived health status and farmers' syndrome of rural residents. Methods: The subjects of this study were 637 adults of 19 primary health care post extracted from 1.875 primary health care post in Korea by quota sampling method. The collected data were analyzed by descriptive statistics, Pearson's correlation. Results: The average score of health promotion behavior was 2.23. In the sub-scales, the highest degree of performance was 'nutrition', followed by 'spiritual growth' and the lowest degree of performance was 'physical activity'. The most prevalent health problem was ailments in 'musculoskeletal system', followed by 'fatigue', 'eyes and ears', 'cardiovascular system'. The mean score of perceived health status was 8.54. The prevalence of farmers' syndrome was 98.4%. Health promotion behavior showed a significant positive correlation health status and perceived health status showed a significant negative farmers' syndrome. Conclusions: With the above findings, rural residents' physical activity is less than urban residents', and the rural resident's score of perceived health status, musculoskeletal system and fatigue are lower than urban residents'. The farmers' syndrome of the rural people is high. A tailored health promotion program for rural community is needed to develop in the consideration of these findings.
