Forefoot disorders and conservative treatment

  • Forefoot disorders are often seen in clinical practice. Forefoot deformity and pain can deteriorate gait function and decrease quality of life. This review presents common forefoot disorders and conservative treatment using an insole or orthosis. Metatarsalgia is a painful foot condition affecting the metatarsal (MT) region of the foot. A MT pad, MT bar, or forefoot cushion can be used to alleviate MT pain. Hallux valgus is a deformity characterized by medial deviation of the first MT and lateral deviation of the hallux. A toe spreader, valgus splint, and bunion shield are commonly applied to patients with hallux valgus. Hallux limitus and hallux rigidus refer to painful limitations of dorsiflexion of the first metatarsophalangeal joint. A kinetic wedge foot orthosis or rocker sole can help relieve symptoms from hallux limitus or rigidus. Hammer, claw, and mallet toes are sagittal plane deformities of the lesser toes. Toe sleeve or padding can be applied over high-pressure areas in the proximal or distal interphalangeal joints or under the MT heads. An MT off-loading insole can also be used to alleviate symptoms following lesser toe deformities. Morton's neuroma is a benign neuroma of an intermetatarsal plantar nerve that leads to a painful condition affecting the MT area. The MT bar, the plantar pad, or a more cushioned insole would be useful. In addition, patients with any of the above various forefoot disorders should avoid tight-fitting or high-heeled shoes. Applying an insole or orthosis and wearing proper shoes can be beneficial for managing forefoot disorders.

A Study on Effects of Revised High Heeled Shoes on Foot Pressure and Static Balance during Standing in Young Female (젊은 여성에서 수정보완된 하이힐이 서 있는 자세에서 족부압력과 정적균형에 미치는 영향에 관한 연구)

  • 본 연구는 전통적 하이힐과 수정보완된 하이힐 착용 시 간에 서있는 자세에서 족부압력과 정적균형을 비교하여 수정보완된 하이힐이 서 있는 자세에 미치는 영향을 알아보고자 하였다. 여대생 15명을 대상으로 하루 간격을 두고 동일한 대상에게 전통적 하이힐과 수정보완된 하이힐을 각각 착용하게 한 후 족부압력과 정적균형을 측정하여 비교 분석하였다. 그 결과, 수정보완된 하이힐의 족부압력은 전통적 하이힐보다 전족부는 유의하게 감소하고 후족부는 유의하게 증가하였다. 그리고 정적균형 능력은 전통적 하이힐보다 개선되었으나 유의하지 않았다. 따라서 수정보완된 하이힐이 전통적 하이힐보다 좀 더 정상적인 족부 압력을 가지게하고 그에 따라 균형능력에도 긍정적인 영향을 보일 것으로 생각된다.

Brand as determinant of evaluation of product personality - A cross-cultural study - (브랜드 개성이 제품 개성에 미치는 영향에 대한 연구 - 한국과 독일의 실험연구를 중심으로 -)

  • A cross-cultural study was carried out in Germany and in South Korea in order to investigate the relationship between brand personality and product personality facilitating the three dimensions of personality agreeableness, excitement, and extroversion. Two pairs of shoes were prepared across categories of product function symbolic (a pair of high-heeled shoes) versus utilitarian (a pair of sport shoes). In experiments, each pair of shoes was labeled as a luxury brand ("Versace") or a casual brand ("C&A", Germany; "Migliore", South Korea) or left unlabeled. Prior to the experiments, an expert group in each country evaluated the brand personality in terms of "cheerful" (agreeableness), "honest"(conscientiousness), and "provocative" (extroversion) and the results were considered as a baseline. In Experiment I and II, subjects were exposed to two pairs of shoes labeled in one of the three ways and assessed the personality of both pairs of shoes using the personality traits, cheerful, honest, and provocative. Identical versions of the experiment were conveyed in Germany (N=56), an individualist culture, and in South Korea (N=72), a collectivist culture, and we purposed to find cultural differences in evaluating product personalities influenced by brands. The empirical results do not show any significant influence of brand personality on product personality in either cultural group (p>.05). Nevertheless, the subjects estimated the retail price of the shoes to be significantly different depending on the brands in both cultural groups (p<.001).

  • The purpose of this study was to establish objective data for marketing of industries and product development design by researching popular singer fashion of Korea, investigating the preference for popular singer design in street fashion among adolescents on the basis of the research, and drawing an conclusion. To analyze street fashion, total 190 young men and women in their 20s, including teenagers, were surveyed mainly in downtowns around Daehakro, Gangnam and Hongik University. First, in relation to the upper garment, men were found to wear T-shirts and hood T-shirts the most, while women were found to wear T-shirts and one-piece dress the most. Second, in relation to the lower garment, the ratio of those who wore skinny clothes was high both among men and women. Third, in relation to the outer apparel, the ratio of men who wore casual jacket and hood zip-up was high, while the ratio of women who wore tailored jacket and leather jacket was high. Fourth, in relation to shoes, the ratio of men who wore sports shoes was high, while the ratio of women who wore heeled-shoe was high. Fifth, it was found that plane pattern was preferred the most. As it was stated above, the findings of this study on the effect of entertainers' fashion on adolescents through street fashion indicated that the number of adolescents who imitate the fashion of entertainers increased.

The Influence of Shoes with Different Heels on Movement of Lower Limb Joints during Sit-to-stand (일어서기 동작 시 발 뒷굽의 형태가 하지 관절 움직임에 미치는 영향)

  • Purpose: The purpose of this study was to investigate changes in movement strategies of lower limb joints depending on the type of heel during sit-to-stand. Methods: Twenty healthy females participated in this study. All subjects performed sit-to-stand three times each with three different types of heels - bare feet, 9 cm high-heeled shoes, and unstable shoes. Trails were conducted in random order. Three-dimensional motion analysis systems were used for collection and analysis of the kinematic data of lower limb movements. Results: Results of this study showed kinematic differences in pelvis, hip joints, knee joints, and ankle joints during sit-to-stand based on the type of heels. At the initial sit-to-stand, hip joint flexion, knee joint flexion, ankle joint flexion, and ankle joint inversion showed significant differences. The maximal angles of hip joint flexion, hip joint adduction, knee joint flexion, ankle joint flexion, and ankle joint inversion were significantly different, while hip joint adduction, pelvic forward tilt, hip joint rotation, knee joint flexion, ankle joint flexion, and ankle joint inversion differed significantly during the terminal of sit-to-stand. Conclusion: Therefore, the type of heel played an important role in selection of lower limb movements during sit-to-stand which were essential parts of daily life movements.

A Study on Fashion Style Expressed in Women Magazine Advertisements (여성 잡지 광고에 표현된 패션스타일 연구)

  • The purpose of this study was to analyse the fashion style expressed in women magazine advertisements. The method of this study was used content analysis with 692 samples in women magazines ('Woman Sense', 'Yeosung Dong-A', and 'Jubu Saeng-hwal') which were issued in March, June, September, and December from 1998 to 2002. The data analysis were divided nine elements: 1. silhouette, 2. color, 3. pattern, 4. length of skirt & slacks, 5. adjustment, 6. breadth of collar lapel, 7. shoes, 8. make-up, 9. hair style. The results of this study were as follows: 1. silhouette : The four seasons were expressed in square silhouette. 2. Color : The spring, summer, and winter seasons were expressed in white color, while the fall season was expressed in neutral color. 3. Pattern : The four seasons were expressed in plain pattern. 4. Length of skirt & slacks : The four seasons were expressed in various length. 5. Adjustment : The four seasons were expressed in single button. 6. Breadth of collar lapel : The spring, summer, and winter seasons were expressed in small breadth, while the fall season was expressed in middle breadth. 7. Shoes : The four seasons were expressed in high-heeled shoes. 8. Make-up : The four seasons were expressed in light tone. 9. Hair style : The four seasons were expressed in up-style.

Effects of Height of the Shoe Heel in a Static Posture on Muscle Activity of the Rectus Abdominis, Erector Spinae, Rectus Femoris and Hamstring (구두 굽의 높이가 한다리 서기 자세에서 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도에 미치는 영향)

  • The purpose of this study was to examine the effects of height of the shoe heel in a one leg standing posture on the muscle activity of the rectus abdominis, erector spinae, hamstring, and rectus femoris. Twenty healthy female subjects were asked to perform a one leg standing posture with three types of shoes, which had different heights of shoe heel, 0cm, 3cm, and 7cm. Surface electromyography was used to evaluate the activities of the right rectus abdominis, erector spinae, hamstring, and rectus femoris muscles. The data was analyzed by repeated one-way ANOVA and the muscle activities among three heights of shoe heel were compared. The results showed that the muscle activities of the erector spinae and rectus femoris increased significantly according to increase height of shoe heel (p<0.05). The activity of the rectus abdominis and hamstring were not significantly different among the three height of shoe heel (p>0.05). These results suggest that the height of shoe heel affects the lumbo-pelvic alignment and center of mass; therefore, high-heeled shoes lead to musculoskeletal pain in the lumbopelvic, hip, and knee areas.

Changes in the Plain Radiographic Parameters of the Forefoot with Heal Elevation - A Preliminary Report - (뒤꿈치 높임에 의한 전족부 단순 방사선 지표상의 변화 - 예비보고 -)

  • Purpose: To investigate the effect of heel elevation on the radiographic parameters of the forefoot. Materials and Methods: Forty feet in twenty-one adults were studied. Weight bearing dorsoplantar radiograph was taken with the foot on a flat surface and with the heel of the foot elevated by 5cm. Various parameters were measured and compared between the results with and without heel elevation. Result: The hallux valgus angle was increased by $5.0{\pm}3.5$ degrees with heel elevation and the change was statistically significant(p<0.01). There was no statistically significant changes in the other parameters. Conclusion: The results of this study suggest that high-heeled shoes might contribute in causing or aggravating the degrees of the hallux valgus.

Comparison Analysis of Lower Extremities Activity while Walking Downhill according to the Height of Heel for Women in 20's

  • The purpose of this study is to measure the effect of change in heel height on lower extremities activity of young women on high-heeled shoes that young women prefer from more kinetic and realistic perspective as this study changes the degree of slope on a treadmill. The study subjects are 15 young and healthy women who do not have any external injuries or problem with walking and understand the purpose of this study clearly. They wore three different height of heels(1cm, 7cm, 12cm) and walked on a treadmill at a constant speed of 3km/h. EMG value of four muscles (anterior tibial muscle, gastrocnemius muscle, straight muscle of thigh, and biceps muscle of thigh) were collected when walking and the change according to the height of heels were analyzed using one-way ANOVA. Multiple comparison analysis on anterior tibial muscle and heel height showed that the group with 12cm heel showed significantly high muscle activation compared to the groups with 1cm and 7cm heels. The result of this study can be used for various perspectives from inferring and mediating problems caused by wearing high heels on different ground slopes for a long time.

  • Purpose : The purpose of this study was to investigate the effect of different heel height on the electromyographic (EMG) activity in Rectus abdominis (RA) and Erector spinae (ES) during gait activity. Methods : A repeated measures design was used. 33 healthy females carried out a standardized gait activity under 3 conditions; with heel wedges of 1, 7, and 12cm in height. EMG activity was recorded from RA and ES during the activity. Data was analysed using the repeated one-way ANOVA. Results : First, as the shoe-heel height was getting higher, change of muscle activation indicated the statistically significant difference in rectus abdominis(p<0.05). Second, as the shoe-heel height was getting higher, change of muscle activation indicated the statistically significant difference in erector spinae(p<0.05). Conclusion : We found that as heel height increased, there was an increase in EMG activity in both RA and ES. This indicated that no RA : ES imbalance was elicited. This study provides information that will inform future research on how heel height affects muscle activity around the trunk.
