Seven Cases Report on the Foot Drop Patients with Herniated Intervertebral Lumbar Disc by Muscle Energy Technique & Korean Traditional Medicine Therapy (근에너지기법 및 한방치료를 적용한 족하수를 동반한 요추추간판탈출증 환자 치험 7례)

  • Objectives : The present study reports seven foot drop patients with herniated intervertebral lumbar disc treated by Muscle Energy Technique and Korean traditional medicine therapy. Methods : Seven foot drop patients with herniated intervertebral lumbar disc were treated by Muscle Energy Rechnique and Korean traditional medicine therapy and were assessed by Manual Muscle Test, numeric rating scale, and oswestry low back pain disability index. Results : Among seven foot drop patients, two cases showed complete recovery, three cases showed recovery over good grade, and two cases showed recovery over good grade after temporary weakness. numeric rating scale and oswestry low back pain disability index of all cases showed improvement. Conclusions : Seven foot drop patients with herniated intervertebral lumbar disc were treated by Muscle Energy Rechnique and Korean traditional medicine therapy and showed improvement. It is anticipated that this report benefits the future in depth study and clinical treatments on the foot drop symptom in korean medicine.

Associated Factors of Clinical sign in Herniated Lumbar Intervertebral Disc (요추간판탈출증의 임상증상에 관련요인)

  • Purpose: This study was undertaken to evaluate the effect analysis of clinical sing in 40 patients with lumbar intervertebral disc. Methods: All patients were assessed by the low back pain protocol which was composed of symptom, clinical sing before and after the treatment. The effect of clinical sing was analyzed by age, occupation, duration of symptom. Results: Occupation by clinical sing of after the treatment was desking job s from 3.6 to 5.2 scores(p<0.05). Duration of symptoms by clinical sing of after the treatment was over 4 months from 3.8 to 5.5 scores(p<0.05). Clinical sing of after the treatment was straight leg raising test from 1.275 to 1.850 scores, sensory disturbances from 1.425 to 2.575 scores, manual muscle testing from 1.525 to 4.625 scores. Conclusion: Herniated disks are most common in middle age, especially between 35 and 45, due to aging-related degeneration of the disks. Significant or increasing pain, numbness or weakness spreading to one or both legs. Progressive loss of sensation in areas that would touch a saddle (inner thighs, back of legs and area around the rectum).

  • The purpose of this study is to research and report the effect of Bee Venom pharmacopuncture therapy at facet joint for the two patients who was diagnosed as herniated intervertebral disc (HIVD) of lumbar spine. Two patients were hospitalized and treated with Complex Korean Medical treatment such as acupuncture, herbal medicine and Essential bee venom (EBV) pharmacopuncture in Dongsuwon hospital of oriental medicine. We measured their pain and function of daily life by numerical rating scale (NRS), straight leg rasing test (SLRT), Roland-morris disability questionnaire (RMDQ). As the result, we found the improvement of gait disturbance, functional disorder, and pain. This study suggested that Bee Venom pharmacopuncture therapy at facet joint is considered to conservative treatment for HIVD of lumbar spine. But further clinical studies are needed to clarify the effect of Bee Venom pharmacopuncture therapy at facet joint for the two patients with HIVD of lumbar spine.

  • Objectives : The Purpose of this study is to investigate the clinical application of conservative treatment including quadratus lumborum MST for lumbar HIVD(Herniated Intervertebral Disc) with sacralization. Methods : We used acupunture, herbal acupuncture, herbal medicine, Quadratus Lumborum MST for this patient. This patient received hospital treatment for 25 days. Results : After conservative treatment, the patient's pain was controlled and NRS score was decreased. ODI and ROM also were improved. Conclusions : Oriental conservative treatment including quadratus lumborum MST has a positive effect to control pain of lumbar HIVD(Herniated Intervertebral Disc) with sacralization.

The Effects of Proprioceptive Neuromuscular Facilitation for Pain, Grip Strength on Cervical Herniated Intervertebral Disc (HIVD) (고유수용성신경근촉진법이 경추추간판탈출증 환자의 통증 및 손의 악력에 미치는 효과)

  • Purpose: The current study aims to identify the effects of proprioceptive neuromuscular facilitation (PNF) on pain and grip strength in patients with cervical herniated intervertebral disc (HIVD) and provide effective interventions for such patients. Methods: Included in this study were 20 subjects (10 men, 10 women) in their 30s-50s who had been diagnosed with cervical HIVD. Intervention methods included PNF neck patterns, dynamic reversal of antagonists, and a combination of isotonics. The visual analogue scale (VAS) was used to measure pain and dynamometers were used to measure grip strength. Results: After four weeks of muscle strengthening exercises, neck pain was significantly reduced, and hand grip strength significantly increased. Conclusion: PNF can be utilized as an effective intervention to reduce the pain of patients with cervical HIVD and to increase grip strength.

Case Report on 4 Patients with Lumbar Disc Herniation Treated with Concurrent Embedding Therapy under Conventional Korean Medical Treatments (매선요법을 병행한 요추 추간판 탈출증 환자 치험 4례)

  • Embedding Therapy keep the stimulation to acupoint. Recently, Embedding Therapy is used for various disease and also studied in many different fields especially in China. In Korea, Embedding Therapy is studied and used for dermatology, face-lift, obesity, facial palsy most frequently. Chinese studies shows us Embedding Therapy could help musculoskeletal system disorder, lumbar herniated intervertebral disc, sprain, spinal canal stenosis. In Korea study concerning lumbar herniated intervertebral disc treatment used Embedding Therapy is few. This study's purpose is to ensure effect of Embedding Therapy at lumbar herniated intervertebral disc treatment. Four lumbar disc herniation patients had been treated with Embedding Therapy were described in this article. The patients(n=4) had been suffering from chronic lower back and leg pain that continued for more than 6 months. All patients had been diagnosed as lumbar disc herniation by MRI or CT, and treated with Embedding Therapy at least 2 times(7 days interval). 2 patients had been treated with concurrent treatment of acupuncture therapy, and the other 2 patients with concurrent treatment of acupuncture and herbal therapy. We observed 4 patients for more than 2 weeks, and their symptoms and quality of daily life had been improved. This suggests that lumbar disc herniation can be successfully recovered with Concurrent Embedding Therapy under conventional Korean Medical Treatment without side effects.

  • Objectives : The Purpose of this study is to investigate the clinical application of Bee venom pharmacopuncture and conservative treatment for lumbar HIVD(Herniated Intervertebral Disc) and Femoroacetabular Impingement. Methods : Patient is hospitalized at Dept. of Oriental Internal Medicine, Jaseng Oriental Medicine Hospital, diagnosed as lumbar HIVD(Herniated Intervertebral Disc) and Femoroacetabular Impingement and treated by Bee venom pharmacopuncture, herbal medicine, acupuncture and moxibustion. This study was measured by NRS(Numerical Rating Scale), ODI and EQ-5D. Results : After Bee venom pharmacopuncture and conservative oriental medical treatment, the patient's pain was controlled and NRS score was decreased. ODI and EQ-5D also were improved. Conclusions : As seen in this one case, Oriental conservative treatment and Bee venom pharmacopuncture have a positive effect to control pain with lumbar HIVD(Herniated Intervertebral Disc) and Femoroacetabular Impingement.

A Controlled Trial on the Effect of Complex Oriental Medical Treatment with or without Balanced Acupuncture on Treatment of Herniated Intervertebral Disc of Lumbar Spine Patients (요추 추간판 탈출증 환자의 침치료와 평형침법 병행치료에 대한 비교 연구)

  • Objectives : The aim of this study was to observe the effect of balanced acupuncture on herniated intervertebral disc of lumbar spine patients. Methods : We investigated 23 cases of in-patients with acute & subacute herniated intervertebral disc of lumbar spine in oriental hospital, and divided patients into two groups. We treated one group(12 people) by complex oriental medical treatment with Balanced acupuncture, and the other group(11 people) by complex oriental medical treatment without Balanced acupuncture. To evaluate the effectiveness of treatment applied for two groups, we used visual analog scale(VAS) and rating scale for low back pain at admission and discharge. Results : 1. In balanced acupuncture group and control group, compared with baseline and final. VAS was significantly improved. 2. In Balanced acupuncture group and control group, compared with baseline and final. Rating scale for low back pain was significantly improved. 3. VAS and rating scale for low back pain improvement rate in balanced acupuncture group were no statistical significance compared with control group. Conclusions : Balanced acupuncture might be used for relieving symptoms related with herniated intervertebral disc of lumbar spine, but it's no statistical significance.

A Case of Cold Hypersensitivity of Hands and Feet Treated with Korean Medicine Including Gyejifabuja-tang-gmibang and Acupuncture in a Patient with Lumbar Spinal Herniated Intervertebral Disc - A Case Report (한방병원에 입원한 수족냉증을 동반한 추간판탈출증 환자에 대한 한방 치료 1례)

  • Objective: The purpose of this study is to report the efficacy of using Korean medicine treatment with cold hypersensitivity of the hands and feet with lumbar spinal herniated intervertebral disc. Case presentation: We used Korean medicine treatment (Gyejigabuja-tang, acupuncture) to treat a hospitalized patient with cold hypersensitivity of the hands and feet with lumbar spinal herniated intervertebral disc. To evaluate the treatment, we used the Numerical Rating Scale (NRS). The degree of cold hypersensitivity of the hands and feet was also evaluated using evaluation paper. The generic health status was measured using the European Quality of Life-5 Dimensions (EQ-5D) scale. The 31-year-old female underwent 13 days of inpatient treatment. Her pattern identification was that both hands and feet are cold (BiYangHeo) types. The treatments were herbal medicine and acupuncture. She took Gyejigabuja-tang for 13 days. She also received acupuncture for 20 minutes twice a day at LI11 (曲池), L14 (合谷), TE5 (外關) on her hands, LR3 (太衝), SP6 (三陰交), SP9 (陰陵泉) ST36 (足三里), GB41 (足臨泣) on Feet, GV4 (命門), GV3 (腰陽關), BL23 (腎兪), BL24 (氣海兪), BL25 (大腸兪), and BL26 (關元兪) on the low back. After 2 weeks of treatment, the NRS scale of her cold hypersensitivity of hands and feet symptoms dropped from NRS 7 to NRS 3, and her low back pain dropped from NRS 6 to NRS 3. Her cold hypersensitivity of hands and feet evaluation paper score decreased from 64 to 32, and her EQ-5D increased from 0.487 to 0.681. After the end of treatment, continued symptom improvements and no significant side effects were confirmed. Results: These findings suggest that Korean medical treatment (Gyejigabuja-tang, acupuncture) may be effective for treating cold hypersensitivity of hands and feet with lumbar spinal herniated intervertebral disc. The NRS, evaluation paper, and EQ-5D scores improved after treatment, with no side effects.

Historical Changes of Extruded Lumbar Intervertebral Disc -A case report- (유출된 요추부 추간판의 시간에 따른 변화 -증례보고-)

  • We studied the historical changes of intervertebral disc displacement using magnetic resonance imaging. The phenomenon of the spontaneous regression of herniated discs is well known. The case of a 40-years-old male presenting with a large disc herniation at L5-S1, experiencing severe sciatic pain, and having the straight leg raising test positive at 25 degrees is presented. The extruded disc was documented by clinical examination. He was treated conservatively with epidural steroid injection (ESI), medication, physical therapy and self-exercise and reevaluated in 10 weeks later, 30 and 1 year. Large extruded disc can be treated successfully by physical therapy with ESI. However, the degeneration and the dehydration of disc result in decrease of disc height. Consequently, the regression of extruded disc might have been due to the resorption and the dehydration.
