A study on newly developed herbal ointment for itching (새로 개발한 한방 피부소양증 치료제에 관한 연구)

  • Objectives : There have been few studies on external treatment for itching. A clinical study was performed to evaluate the effect of a newly developed herbal ointment. Methods : A newly developed herbal ointment made from Rumex japonicus Houtt, Prunus persica(Linne) Batsch, Sesamum indicum L., and Sus scrota domestics Brisson., was given to 49 participants with itching to administer for approximately a month. Demographic characteristics, treatment experiences, symptom change, and other comments were self-reported on a pre-defined questionnaire. Results : Most frequent complaint was itching (24.01%), followed by sensitivity (19.60%), dryness(18.62%), frustration (13.72%) and insomnia (12.25%). Of 41 participants who had been given treatment for itching, around a half of them (n=21) have been treated with Korean Medicine, but all of them tried Western Medicine as well. The effect of the ointment was evaluated as excellent or good/fair (67.32%). Continuity was reported as excellent or good/fair (51.0%) and fragrance (73.44%) and color (75.48%) was reported as excellent or good/fair. Convenience, however, was assessed as good/fair (67.32%) and as inconvenient (22.4%). Conclusion : The newly developed herbal ointment seems promising for relieving itching and further research to standardize and develop this kind of ointment is warranted.

Acute toxicity, Dermal and Ocular Irritation Studies of Taglisodog-eum ointment (탁리소독음 피부외용제형의 급성경피독성시험, 피부자극시험 및 안점막자극시험)

  • Objectives : Taglisodog-eum(TSE), a poly herbal formula, has been widely used to improve carbuncles by removing inflammation of the lymphatic channels in Traditional Korean Medicine. We previousely reported the action mechanism of TSE on experimental atopic dermatitis and the establishment of formulation for TSE ointment. In this study, we examined the toxicity test on skin and eye irritation by TSE ointment to prove the safety of Taglisodog-eum ointment in clinical use. Methods : Acute skin toxicity of the TSE ointment was evaluated in Sprague-Dawley(SD) rats. After dermal administration of TSE ointment(2,000mg/kg), body weight, mortality, and clinical signs of the rats were observed for 14days. Primary skin irritation and ocular irritation tests for TSE ointment were performed in male New Zealand White Rabbits. In dermal and ocular irritation test, body weight, mortality, clinical signs, Primary Irritation Index(P.I.I.), and The Index of Acute Ocular Irritaion(I.A.O.I.) of rabbit were observed after applying at abraded skin and eye balls with Taglisodog-eum ointment. Results : In the results of acute skin toxicity, no significant differences were found in body weight, the clinical sign and the mortality between control and TSE ointment treated group. In primary dermal irritation test, body weight, the clinical sign and the mortality were not significantly changed and Primary Irritation Index(P.I.I.) was 0.8, indicating TSE ointment as weak irritant material. In ocular irritation test, The Index of Acute Ocular Irritaion was 0.0, indicating TSE ointment as non-irritating to the eye of the rabbits. To evaluate toxicity of the TSE ointment in animal test, body weight, the clinical signs, the skin and eye irritation check were conducted; TSE ointment was considered to be weak dermal irritant in test animals. The no response of eye irritation test was observed in this experimental condition. Conclusions : According to the above toxicity test, We consider that this results is helpful for saying about the safety of TSE ointment in clinical use.

  • The purpose of this study is to expand the prescription usage from internal treatment to external treatment by applying the compatibilities of herbal ingredient for ointment massage therapy. For this, specific cases of usage were investigated in Donguibogam, which is now inscribed on the UNESCO Memory of the World Register, has been not only one of the classics of Korean medical asphere, but has compiled all the contents of the Oriental medical traditions including ointment massage therapy that was completed through different Chinese dynasties, such as Song (宋), Jin(金) and Won(元) by the 17th century. Research results of this study is : 1. Ointment Massage Therapy was used to treat the symptoms of diseases such as ACHI-mun(dental range : 牙齒門), PI-mun(dermal range : 皮門), JECHANG-mun (sore range : 諸瘡門), SO A-mun(pediatrics range : 小兒門). 2. Out of all the manipulations, it is a scrubbing manipulation that used most. 3. Out of all the formulations of ointment massage therapy, it is powder types and ointment types that most representative formulation.

A Study of external ointment treatment in "Dongeuibogam" (동의보감(東醫寶鑑) 외용고(外用膏)에 대한 고찰(考察))

  • Objective : The study was performed to investigate prescriptions, herbal drugs, how to make of external ointment treatment in "Dongeuibogam". Method : I will try to find out external ointment treatment in "Dongeuibogam" using a word, Go(膏). Result : 1. The external ointment has been widely used medical diseases as well as sugical diseases. 2. The same prescription name to have different information was written by other people. 3. Various kinds of medicinal herbs, minerals, anlmals were used and type of oil or juice was used to dissolve these. 4. Methods to make the external ointment were to grind medicines or melt medicine. 5. The external ointment was used to put in eyes or to push into wound and rubed the skin. Conclusion : Go(膏) In "Dongeuibogam" are external ointment treatment when they are used at skin. However, they are used as internal medicine in other side so we might be careful to use Go(膏) for medicine.

Effectiveness of herbal ointment Biyeom-go according to cold-heat pattern identification: a subgroup analysis on patients with rhinitis (한열(寒熱) 변증에 따른 한의 외용 치료제 비염고의 치료 효과 : 하위 그룹 분석)

  • Objectives : Hwanglyeonhaedok-tang(黃連解毒湯) is a representative herbal formula with "clear heat(淸熱)" effects. The aim of this study was to evaluate the effects of Hwanglyeonhaedok-tang-based intranasal herbal ointment Biyeom-go based on the cold and heat pattern identification questionnaire(CHPIQ). Methods : We performed a subgroup analysis of the previously published prospective observational study. A total of 58 patients with rhinitis were administered Biyeom-go for 4 weeks, and its effects on the Total Nasal Symptom Score(TNSS), Mini Rhinoconjunctivitis Quality of Life Questionnaire(Mini-RQLQ) score, and nasal endoscopy index score were analyzed based on CHPIQ. Results : Among the 58 patients, the heat and non-heat patterns were shown by 39 and 19 patients, respectively, while the cold and non-cold patterns were shown by 46 and 12 patients, respectively. The change in TNSS from baseline negatively correlated with the heat pattern score(p=0.011). Improvement in TNSS was greater in the heat pattern group than in the non-heat pattern group, with a borderline significant difference(p=0.07). Mini-RQLQ and nasal endoscopy index scores tended to be lower in the heat pattern group than in the non-heat pattern group, but without a statistically significant difference. Conclusion : The findings indicate that CHPIQ is a useful tool for the diagnostic and prognostic evaluation of patients with rhinitis. This study provides fundamental evidence of the close association between the cold-heat pattern in patients with rhinitis and the treatment effects of Biyeom-go.

  • Objectives The purpose of this study is to identify the latest research trends regarding the nonsurgical treatment of rib fractures and suggest the direction for future research. Methods We searched for papers published from January 1, 2015 to July 1, 2022 in PubMed and the China National Knowledge Infrastructure (CNKI) based on keywords 'rib fracture'. 'Korean medicine', 'oriental medicine', 'TCM', 'moxibustion', 'acupunture', 'cupping', 'electroacupuncture', 'pharmacopuncture', 'fire needling'. Results A total of 79 studies were searched at first research. Then the studies were screening according to criteria and Finally 20 studies were selected. The oriental medical interventions analyzed in this study were fumigation method, herbal medicine, external application, acupuncture, ointment, herb fomentation, Tending Diancibo Pu far-infrared therapy. Conclusions Various oriental medical interventions such as herbal medicine, acupuncture, fumigation, herb fomentation, ointment, and physical therapy are being studied abroad for the treatment of rib fractures. It is considered that additional research related to the nonsurgical treatment for rib fractures is needed in the future in korea.

  • Purpose : To observe effects of TKM (Traditional Korean Medicine) treatment for 2 patients who had verruca plana after having laser epilation of leg hair. Methods : We did retrospective chart review for 2 verucca plana patients who had been treated by TKM. TKM treatment included herbal treatment, herbal acupuncture and herbal ointment. Results : After TKM treatment, the patients' verucca plana remarkably disappeared and there was no recurrence. Conclusion : This study suggest that TKM may be an effective treatment for verucca plana.

  • Herbal medicines are popular in Korea communities and used for a variety of diseases, and most herbal medicines has been used an oral administration for treating diseases, but some herbal medicines included Jawoongo has been used by applying on the skin. Jawoongo, a traditional oriental medicine ointment, have been used for treating various dermatitis associated with skin diseases such as tinea manuum, eczema and chilblain, etc. by promoting blood flow(活血), dispersing wind(散風) and moistening(潤燥). Jawoongo used in this study is derived from Yungigo supplemented with Adeps Suillus(豚脂). The aim of this study is to investigate the effect of Jawoongo plus Rehmanniae Radix(生地黃) on the healing of incisional skin wound in rats. After inducing an inflammation on the rats, we observed the amount of Leukocyte, the level of Cortisol and CRP(C-reactive protein), the histological parts and have obtained the healing effect of wound by using Jawoongo plus Rehmanniae Radix(生地黃).

An overview of herbal medicine for atopic dermatitis (아토피성 피부염의 한약치료 효과에 관한 고찰)

  • 목 적 : 한약 또는 한약제제의 아토피성 피부염에 대한 치료효과를 조사하고 향후 연구방향을 제시하기 위하여 고찰연구를 시행하였다. 방 법 : PubMed에 한약과 아토피성 피부염과 관련된 검색어의 조합을 넣어 포함기준에 맞는 무작위배정 대조군 임상연구만 포함하였다. 연구설계, 치료방법, 대조군, 평가지표, 결과, 부작용 관련 정보를 미리 정해놓은 자료 추출 형식에 맞추어 추출하고 방법론적 질 평가는 옥스포드 질 평가 척도와 그룹 할당 은닉(allocation concealment) 여부를 평가하였다. 연구들이 임상적 및 통계적으로 상이하여 메타분석은 이루어지지 않고 기술적 고찰만 실시하였다. 결 과 : 모두 8편의 연구가 고찰기준을 만족시켰다. 다양한 복합한약제제와 한약이 포함된 외용제가 평가되었는데 8편 가운데 5편에서 아토피성 피부염의 증상을 호전시키는 것으로 나타났다. 방법론적 질은 대체로 양호한 것으로 나타났으며 일부 효과적인 것으로 나타난 한약복합제제에서 간손상 등의 부작용도 보고되었다. 결 론 : 한약 또는 한약제제를 이용한 치료는 아토피성 피부염의 증상개선에 도움이 되는 것으로 보이나 현재 근거는 부족하다. 우리나라에서 많이 쓰이는 한약제제들 역시 엄정한 임상연구를 거쳐 그 효과를 평가하고 근거를 구축해야 할 것이다.

A Clinical Case of Tinea Pedis Treated with Korean Medicine (한방치료로 호전된 족부백선 환자 1례)

  • Objectives : This study is to report a case of tinea pedis accompanied with erythema and pruritus, treated with Korean medicine treatment. Methods : A 26-year-old female patient has suffered from erythema and pruritus on her right 3rd toe. She applied desoximetasone ointment on it for a month, but the effect didn't last long and the symptoms got worse. After disappointing application of the corticosteroid, she received Korean medicine treatment including herbal medicine and herbal external medication for 1.5 months. Results : The skin symptoms such as erythema and pruritus were remarkably improved. Also, the Visual Analogue Scale(VAS) of pruritus changed from 4 to 0. Conclusions : This case report suggests that Korean medicine may have a role in treating tinea pedis with some skin symptoms such as erythema and pruritus.
