Acne Treatment Cases with Hwangryeonhaedok-tang (황련해독탕가미방(黃連解毒湯加味方) 발효한약으로 치료한 여드름 환자 증례)

  • Objectives : Acne, one of the most common disorders in dermatology clinic, is a chronic inflammatory disease which has the symptoms of comedones, papules, pustules, cysts, nodules and scars mainly on the face. Although some of pathologic findings are suggested, but the exact causes and mechanisms are not yet known in Western Medicine. Hwangryeonhaedok-Tang(HRHDT) is an antiinflammatory, antipyretic and detoxifying herb decoction. In this report, we would like to share our experience of acne treatment with HRHDT. Methods : We had treated several patients with acne, facial flushing and uprising febrile sensation on face using oral administration and external application of HRHDT. HRHDT is basically made up with Scutellariae Radix, Coptidis Rhizoma, Phellodendri Cortex and Gardeniae Fructus. In this trial, several other herbs were added according to the individual patient's accompanying symptoms. After the decoction of herbs in amount for 10 days, $Yogourmet^{(R)}$ Kefir Starter 10 g was added to the herbal solution that was then fermented in an incubator at $25^{\circ}C$ for 72 hours, and divided into 30 doses. Results : Photographies were taken before and after the treatment. The severities of acne were evaluated with these photos according to the Korean Acne Grading System(KAGS). As results, we observed dramatic clinical improvements and the decreases of KAGS grades after the average treatment period of 8 weeks. The medication was orally administered 3 times a day and the external treatment was applied average twice a week during the whole treatment period. Conclusions : From these results, HRHDT may be considered as a good prescription for the febrile and toxic type of acne patients.

A Case Report of Bogigeopung-san Applied on Trigeminal Neuralgia (삼차신경통에 보기거풍산(補氣祛風散)을 사용한 임상증례 1례)

  • Objectives : This is a clinical report on a 63-year-old female patient with Trigeminal Neuralgia (TN) treated by oriental medicine treatments. Methods : The patient was treated by acupuncture, moxibustion and herb medication. The improvement of the patient was judged by Visual Analog Scale(VAS). Results : The pain had been gradually reduced and VAS became 2 after the treatment for 25 days. Conclusions : Oriental medicine treatment including Bogigeopung-san was very effective to improve the TN patient's symptoms. It is necessary to have more observations and cases on the patients with TN.

A Clinical study of Recurrent Acne in the Oriental Medicine (여드름에 관한 임상적 연구)

  • Acne is a common chronic disease which most frequently affects the face, appearing in both males and females near puberty, and in most cases becomes less active as adolescence ends. The intensity and duration of activity vary for each individual. The disease may be minor, with only a few comedones or papules, or it may occur as the highly inflammatory and diffusely scarring acne conglobation. Permanent scarring of the skin and psyche can result from such inaction. The disease, however, has implications far beyond the few marks that may appear on the face. The lesions cannot be hidden under clothing; each is prominently displayed and detracts significantly from one's personal appearance and self-esteem. Recently many of acne patients are visiting to Oriental Medicine Hospitals. They want to receive the treatment of acne related another disease and causes, or worry about side effects of antibiotics and hormone therapy. This study was performed at Pundang Cha Oriental Medicine Hospital Department of Dermatology from March 1, 1997 to February 29, 2000 in 31 patients with recurrent acne. We investigated the clinical findings, included distribution of age & sex, severity of acne, sites on face, related causes, duration of disease and causes of visiting Oriental Medicine Hospital and so on. The results were as follows : The peak age at onset was 20-29 years old in females with 16 cases reported. The 22 total cases patients had severe signs with most frequently sites being forehead & chin. They visited Oriental Medicine Hospital because they want to take the treatment of acne related with another body problems(12 cases) and worry about side effects of antibiotics. Twenty cases were diagnosed 肺熱血熱(20 cases). And given acupuncture treatment, herb-medication and sometimes we used external applications. Relatively, their satisfaction grade was high. Dermatologists are trying to treat new methods in order to remove scar and decrease sebaceous gland secretion,. And we, Oriental Medicine doctors, have to develop and offer to patients the most effective management and successful treatment of this disease.

  • Vertigo occurs when there is conflict between the signals sent to the brain by various balance- and position-sensing systems of the body. Injury to or diseases of the inner ear can send false signals to the brain indicating that the balance mechanism of the inner ear (labyrinth) detects motion. Inner ear disorders, such as benign paroxysmal positional vertigo (BPPV), Meniere's disease, vestibular neuronitis, or labyrinthitis. Vestibular neuronitis is an inflammation of the vestibular nerve, which is located in your inner ear. In this case, She has vertigo occured suddenly and continued even when the person lies completely still. It may be accompanied by nausea and vomiting. She has taken herbal medication and treated by acupunture. Her symptoms become disappeared after 3 days and All most symptoms were disappeard. In conclusion, It is required to classify vestibulopathy into vestibular neuronitis or labyrinthitis. In this case we diagnosed her case as vestibular neuronitis. Her symptoms of vestibulopathy recovered fast by Korean traditional medical therapy.

The follow-up study on patients of Amyotrophic lateral sclerosis after 1 year (근위축성 측삭경화증 환자 1년 후 추적 조사)

  • Objectives : This study was to investigate the effects of Oriental medical treatment on ALS. Methods : We investigated 12 ALS patients which were admitted to Gwang-Ju O.M.hospital for 3 months and follow-up at 8 months after discharge. All patients were treated by SAAM-acupuncture, herb medication, Bee venom Pharmacopuncture therapy, Needle-embedding therapy, etc and after discharge self-therapy at home. We evaluated patients using the Amyotrophic lateral sclerosis Functional Rating Scale-Revised(ALSFRS-R), Medical Research Council (MRC) Scale. Results : After 1 month, mean ALSFRS-R score of patients was $29.08{\pm}7.99$, 2 months $28.70{\pm}7.17$, 3 months $28.16{\pm}8.23$, 1 year $21.33{\pm}9.93$ and mean MRC Scale of patients was $25.34{\pm}8.45$, 2 months $25.34{\pm}8.45$, 3months $21.56{\pm}9.20$. But in both cases, the variation was not statistically significant. Conclusions : We think that the results of this case be a pilot study that proves the effect of Oriental Medical treatment on ALS.

  • Gastroesophageal reflux disease(GERD) refers to the varied clinical manifestations of reflux of stomach and duodenal contents into the esophagus and is preferable to the 'reflux esophagitis'. Clinical symptoms of reflux esophagitis are heartburn, dysphagia, blood loss, abdominal pain, nocturnal wheezing, coughing, hoarseness. We applied oriental medicines to patient who had reflux esophagitis. we treated them with herb-medication, acupuncture, moxa therapy and aroma therapy for about 2 months. During this period, we observed that symptoms had been improved.

A Clinical Report about Hiccup and Eructation from Emotional Stress (칠정(七情) 자극 후 발생한 해역(咳逆).희기(噫氣) 환자 치혐 1례)

  • This case show that emotional stress and wekness of gastrointestinal function occur a chronic hiccup and eructation(during 3 months), oriental medical therapy can cure completely this symptom. Seven emotional stress(anger, delight, sadness, thinking, worry, fear, horror) occur various pathologic changes. Hiccup is caused by CNS, psychologic, thoracic, abdominal factor etc. In oriental medicine, GI trouble regarded as a very importent factor that occur hiccup and eructation. Besides, emotional stress is a very important factor as well. This patient have an introvert personality ordinary times. One day, she had a miscarriage by hers husband's assault. After that time, she suffered from very serious GI trouble(dyspepsia, epigastric pain, anorexia). And then hiccup, eructation, weight loss(10kg/3months) is occured. This patient diagnosed only gastritis. This symptom did not stop during 3 months, regardless of western medical therapy(some western drugs medication, fluid supply etc). After oriental medical therapy(herb medicine;Yukunjatang, acupuncture;CV12, CV6, S36, CV17, H7, SP6, P6, moxibustion;VI2, CV6, S36, negative therapy;back area and traditional oriental medical psychotherapy;ventilation, support, desensitization and family interview) put in operation during 15 days, patient completely recover from hiccup, eructation and anxiety disorders.

A Case Report of a Essential Tremor Patient Aggravated by Hwa-byung (화병에 의해 악화된 본태성 진전증 환자 치험 1례)

  • Objectives : The essential tremor (ET) is characterized by a tremor of the whole body, including both arms. Recent studies suggest that its psychiatric and cognitive factors are more important for treatment and management than motor symptomsymptoms. This study was performed to report the effects of Korean medicine for essential tremor aggravated by Hwa-byung. Methods : We treated a 42 year- old man with Korean medicine (acupuncture, herb-medication and moxibustion) and mental therapy. The effects of the treatment for tremor and skin symptoms were measured by VAS and Severity score (3 level). We used SCL-90-R, BDI, BAI, PSQI and instrument of oriental medical evaluation for Hwa-Byung score (IOMEHB) for a psychological condition. Results : After treatments, tremor, Hwa-byung and accompanying symptoms have improved and subsided. VAS, SCL-90-R, BDI, BAI, PSQI, and IOMEHB have decreased. Conclusions : This result suggests that Korean medical treatment and mental therapy might be effective for the essential tremor aggravated with Hwa-byung.

The Clinical Study of the Parallel Treatment with Laser Therapy and Combined Essential Oil Inhalation on turbid nasal discharge(鼻淵) (반도체 Laser 요법(療法)과 향기요법(香氣療法)을 병행(竝行)한 비연(鼻淵) 치료(治療)의 임상연구(臨床硏究))

  • Objective: The objective of this study was to assess the ability of Gagamhyunggaeyungyotang(jiajianjinggailianqiao-tang) and the enhancement of the parallel treatment with laser therapy and combined essential oil inhalation on children's turbid nasal discharge. Methods: This clinical study was carried out with the 35 cases(male 29, female 6) of children aged 4 to 14 years old who visited in the Department of Pediatrics, Dongguk University Bundang Oriental Hospital and were diagnosed as turbid nasal discharge through PNS(Paranasal Sinus) CT(Computer Tomography). To assess the impact of dosage of Gagamhyunggaeyungyo -tang(jiajianjinggailianqiao-tang), it was given in different doses. The effects of treatments are assessed with the condition of illness and PNS CT before and after the therapy. The enhancement of the parallel treatment with laser therapy and combined essential oil Inhalation was also observed same as well. Results: The Use of Gagamhyunggaeyungyo-tang(jiajianjinggailianqiao-tang) has remedied rhinorrhea, nasal congestion, sputum, sore throat, cough, postnasal discharge, headache, attention deficit and anorexia above 50%. The relationship between the dosage and the effectiveness was relative high with 28 children among 35 children. The effectiveness of the parallel treatment was above 50% higher than the herb medication only. Conclusion: The efficacy of Gagamhyunggaeyungyo-tang(Jiajianjinggailianqiao-tang) on children's turbid nasal discharge was raised when the laser therapy and the combined essential oil inhalation was paralleled.

Sweet Bee Venom Pharmacopuncture May be Effective for Treating Sexual Dysfunction

  • Sexual dysfunction (SD) is a health problem which occurs during any phase of the sexual response cycle that keeps the individual or couple from experiencing satisfaction from the sexual activity. SD covers a wide variety of symptoms like in men, erectile dysfunction and premature or delayed ejaculation, in women, spasms of the vagina and pain with sexual intercourse, in both sexes, sexual desire and response. And pharmacopuncture, i.e. injection of subclinical doses of drugs, mostly herb medicine, in acupoints, has been adopted with successful results. This case report showed the effect of bee venom on SD. A 51-year-old male patient with SD, who had a past history of taking Western medication to treat his SD and who had previously undergone surgery on his lower back due to a herniated disc, received treatments using pharmacopuncture of sweet bee venom (SBV) at Gwanwon (CV4), Hoeeum (CV1), Sinsu (BL23), and Gihaesu (BL24) for 20 days. Objectively, the patient showed improvement on most items on the International Index for Erectile Dysfunction (IIEF) like 28 to 29 out of perfect score 30 for erectile function, 10 to 10 out of perfect score 10 for orgasmic function, 6 to 8 out of perfect score 10 for sexual desire, 10 to 13 out of perfect score 15 for satisfaction with intercourse, and 6 to 8 out of perfect score 10 for overall satisfaction; subjectively, his words, the tone of his voice and the look of confidence in his eyes all indicated improvement. Among the variety of effects of SBV pharmacopuncture, urogenital problems such as SD may be health problems that pharmacopuncture can treat effectively.
