Obesity from the viewpoint of metabolic rate (대사량의 측면에서 본 비만)

  • The obesity is the matter of the energy balance in essential. The energy balance in human body is energy expenditure subtracted from energy intake. The energy intake is mainly supplied by carbohydrates, proteins and lipids in food, and the energy expenditure is composed of basal metabolic rate or resting energy expenditure, physical activity and thermogenesis including diet-induced thermogenesis. The resting energy expenditure is measured by direct calorimetry and indirect calorimetry. Generally we can simply use predictive equation with the variables of weight, height, age and fat-free mass to yield metabolic rate. But there is discrepancy between the estimate and real metabolic rate because the equations can not reflect individuality and environments. The resting energy expenditure is influenced by many factors but the fundamental factor is fat-free mass. We briefly reviewed the concept and evaluation of the energy balance, intake and expenditure, which are important parts in the study of obesity. Finally, we surveyed the correlation between metabolic rate and obesity and suggested applicable herb medication to increase metabolic rate.

A Case Report of One Patient with Infertility Accompanied by Adenomyosis and Uterine Myoma (자궁근종 및 자궁선근증을 동반한 불임환자 증례보고)

  • Objectives: Adenomyosis is the disease caused by ingrowth of lining tissue into the uterine muscle. Final treatment of the disease is hysterectomy. But if a patient want to get pregnant, there is a tendency to avoid surgical treatment. This paper introduces the potential of oriental medical treatment for infertility caused by adenomyosis through a case study. Methods: A 34-year-old female patient, who wants to get pregnant, was diagnosed with adenomyosis. She wants to take oriental medical treatment in the ${\bigcirc}{\bigcirc}$ Oriental Hospital. We applied herb-medication, acupuncture and moxibustion. Results: Overall condition including the symptoms was relieved and succeeded in pregnancy after oriental medical treatment. Conclusions: Oriental medical treatment has an effect infertility caused by adenomyosis.

  • Purpose: Recently people who want to treatment of cervical dysplasia by Traditional Korean Medicine (TKM) were increased. The purpose of these studies are to confirm the clinical effectiveness of TKM treatment to clinical symptoms of the patients who diagnosed as cervical dysplasia. Methods: The patients who diagnosed as cervical dysplasia, had elytrorrhagia, leukorrhea, dysmenorrhea et al. The patients were treated by TKM such as herb medication and acupuncture therapy. Results: After the oriental medical treatment. the clinical symptoms such as elytrorrhagia, leukorrhea, dysmenorrhea were improved. Conclusion: To treatment and management of cervical dysplasia, we were based on improvement of clinical symptoms. And we used TKM to the clinical symptoms of patients with cervical dysplasia, in result clinical symptoms of cervical dysplasia were improved. TKM is expected to have positive effects on clinical symptoms of cervical dysplasia.

Review of Clinical Researches for Herbal Medicine Treatment on Functional Abdominal Pain in Children. (소아 기능성 복통의 한약치료에 대한 임상연구 동향 - 중의학 논문을 중심으로 -)

  • Objectives The purpose of this study is to evaluate the efficacy and safety of herbal medication for the treatment of functional abdominal pain in children by analyzing randomized controlled trials conducted in China. Methods Literatures from China National Knowledge Infrastructure published up to 23 November, 2020 were searched. Then they were collected and analyzed by their publication year, demographic information, treatment methods used, duration of illnesses, duration of treatments, follow-up periods, outcome measurement and adverse events. Results A total of 27 studies were selected for the analysis. In all studies, the total efficacy of the herbal medicine treatment group was reported to be higher than that of the control group. The most frequently used medical herb was Root of Glycyrrhiza uralensis Fischer (甘草). The most frequently used classes of herbal medications are Qi regulating medicine (理氣藥), Qi tonics (補氣藥), Aromatic wet medicine (芳香化濕藥), and Digestant (消食藥). The adverse events rate for the herbal medicine treatment group were significantly lower than those in the Western medicine treatment group. Conclusions This study showed that herbal medicine treatment can be effective and safe option for treating pediatric functional abdominal pain. However, additional well-designed clinical studies are needed to solidify the findings.

A case of Early Neurological Recovery from Stroke by Recannalization in Early Stage (조기 혈관 재개통으로 뇌졸중 초기에 회복을 보인 환자 1예)

  • Objective: The purpose of this case study is to show a case of early neurological recovery from stroke by recannalization. Methods: The patient was admitted from 12 July, 2004 to 26 July, 2004. The patient received Herb-Medication and Acupuncture Therapy. Brain MRI and Tc-99m HM-PAO brain SPECT was used to diagnose and prognose. Results: The patient fully recovered within a few hours. She was diagnosed as acute infarction in Lt. BG by using brain MRI. Tc-99m HM-PAO brain SPECT showed focal increased perfusion at Lt. BG. Conclusions: We suggest that the important thing to recovery from stroke is recannalization of cerebral arteries in early stage.

Association of Liver Dysfunction with Self-Medication History in Korean Healthy Male Adults (건강한 한국 성인남성의 자가약물복용력에 따른 간기능 장애 발생여부 조사)

  • Background: Korean people could abuse healthy foods as well as medications, which might cause serious side effects. The aim of this study was elucidating liver dysfunction due to the self-medications of hepatotonics, healthy foods and herb medications by nested case-control study. Methods: Study subjects were drawn from male members of Seoul Cohort Study who were recruited by self-administered structured questionnaire survey through mailing to the healthy men between the age of 40 and 59 years through the program of biennial health check-up offered by Korea Medical Insurance Corporation(KMIC). The liver dysfunction was defined as the level of serum AST and ALT above 40 IU/L and increased in more than one hunderd per-cent during the 2 year follow-up period. To estimate the odds ratio between self-medication and liver dysfunction after controlling for potential confounders, logistic regression was performed. Results: During the follow-up period, 30 members were identified to fit into case criteria and 2,625 members were selected as control. In logistic regression analyses, history of healthy foods intake, age under 45 years, obesity, and habit of regular exercise were significantly associated with liver dysfunction. The following factors exhibited no statistical significance: intake of hepatotonics, of herb medicine; history of disease in family, of operation, and of radiologic examination; smoking habits and drinking amounts. Conclusion: The significant association between the intake of healthy foods and the liver dysfunction illustrates that chronically optional overuse of healthy foods might bring to hazards to health. As the increasing trend of the size of purchasing healthy foods in Korea, pharmacoepidemiologic studies evaluating the safety and efficacy of the widely used healthy foods should be performed in the near future.

Effects of Herb Medication on Dry Cough, a Common Side Effect Caused by Angiotensin Converting Enzyme (ACE) Inhibitor (한약치료가 Angiotensin Converting Enzyme Inhibitor를 복용중인 고혈압환자에게 부작용으로 나타나는 건해(乾咳)에 미치는 영향)

  • Objectives: In order to confirm a remedial effect and related influence of the clinic treatment by prescribing herb medicines to hypertensives experiencing angiotensin converting enzyme (ACE) inhibitor dosage and suffering from common side effect generally known as dry cough. Methods: 1. We selected the 19 patients who visited National Oriental Medical Center, from August 21, 2007 to August 16, 2008 and suffering from dry cough caused by taking ACE inhibitor, with no other possible diseases causing dry cough. 2. We separated the 19 patients into two groups (Type 1: Bi-Qi hie (脾氣虛) group prescribed Samchuljojung-tang & Type 2: Qi-hie dam-wul (氣虛痰鬱) group prescribed Samsoumgamibang). 3. We then observed the symptom level and post-treatment effect, and recorded changes of dry cough intensity level for each group. Results: 1. Type 1: In the survey of 12 patients, initial level recorded 16.33 at entry diagnosis, and next level meant changing of symptoms, recorded as 2.75 at Stage 1 and reaching 3.33 at Stage 2. 2. Type 2: 7 patients, with initial level recorded as 18.71 at entry diagnosis, and 1.86 at Stage 1 and reaching to 3.29 at Stage 2. 3. No additional prescriptions were issued at Stage 2 or afterwards, and final result indicates that the mean value ended at 3.95 in the total group. Conclusions: It is concluded that there is a significant remedial effect and related influence of the clinic treatment between the Oriental medicine treatment and one of the common side effects of ACE inhibitor, dry cough.

The Effects of Herb Extracts in Cerebrovascular Accidental Patient (뇌졸중 치료제인 한약의 효과)

  • The several Chinese herbs such as Soon-Ki-Hwal-Hyul-Tang, Bo-Yang-Hwan-O-Tang, Seong-Hyang-Jeong-Ki-San, So-Hap-Hyang-Won and O-Yak-Soon-Ki-San were extracted with water and then lyophilized. For identification of the effect of extracted herbs, they were medicated to 103 patients of cerebrovascular accident for 4 week. They were hospitalized in D-Oriental Medical Hospital from April to August in 1999. The herbs were extracted with water and lyophilized and then, used as samples. The medical history of each patient was detected and analyzed from their medical records. The results were as follows; 1) Each sample (Soon-Ki-Hwal-Hyul-Tang, Bo-Yang-Hwan-O-Tang, Seong-Hyang-Jeong-Ki-San, and O-Yak-Soon-Ki-San) was statistically significant differences of systolic blood pressure(t=4.22, P=0.0004; t=3.44, P=0.0028; t=2.11, P=0.0463; t=3.23, P=0.0052). The statistically significant difference of diastolic blood pressure showed by medicated with Soon-Ki-Hwal-Hyal-Tang, Seong-Hyang-Jeong-Ki-San, and O-Yak-Soon-ki-San (t=2.13, P=0.0459; t=2.68, P=0.0136; t=3.12, P=0.0066). 2) The statistically significant difference of the arm/leg-ROM showed by medicated with Soon-Ki-Hwal-Hyul-Tang, Bo-Yang-Hwan-O Tang, So-Hap-Hyang-Won(t=4.74/4.95, P=0.0002/0.0001; t=2.25/2.44, P=0.0368/0.0248; t=5.85/6.76, P=0.0001/0.0001). 3) In the verbal disorder, Soon-Ki-Hwal-Hyul-Tang, Bo-Yang-Hwan-O-Tang had statistically significant differences(t=4.50, P=0.0002; t=3.32, P=0.0036). 4) In the conscious disorder, Soon-Ki-Hwal-Hyul-Tang, Bo-Yang-Hwan-O-Tang, Seong-Hyang-Jeong-Ki-San, and So-Hap-Hyang-Won had statistically significant differences(t=6.32, P =0.0001; t=8.32, P=0.0001; t=3.74, P=0.0012; t=5.14, P=0.0001). 5) Bovine aortic endothelial cell (BAEC) were cultured in DMEM treating 0.01mg/ml, and 0.1mg/ml of each lyophilized samples for 24 hours. In BAECs were treated by 5 kinds of samples, the effect of So-Hyap-Hyang-Won induced syncytium of adjacent endothelial cells. It may induce the recovering of the damaged blood vessels in cerebrovascular accidental patient by angiogensis of endothelial cells. Therefore, it suggests that the medication of So-Hap-Hyang-Won will help to nursing care for cerebrovascular accidental patients.

Clinical study of Chronic Urticaria (만성두드러기에 대한 임상적 고찰)

  • Introduction Urticaria is widely spread disease. About 15-20 $\%$ of population has experienced at least once in a life time. The etiology of urticaria is uncertain till now, and the affcting factors are various chemical material, physical factors, alcohol, fever, exercise, and hormone, etc. The symptoms of urticaria are small or large wheal-erythema reaction and itching or tingling sense. Cause there is no probe to detect the urticaria clearly, the treatment of urticaria is symptomatic. And mostly urticaria can be chronic and very hard to treat it fundamentally. Nowadays, many trials to treat the urticaria in oriental medical way show good curability. In this paper I'd like to report the treatment rate and degree of urticaria patients mostly using anti-histamine medication. Subjects From the outpatients who visited Korean Hospital of Kyunghee Kangnam during 2000-4 and 2002-1, the 36 chronic urticaria patients, at least more than 4 weeks from onset and 2weeks of treatment period, were chosen. Methods I evaluated the results of treatment with the following scale. Very Good: The all symptoms are clearly disappeared or one third of symptoms remain and sometimes eruption is appeared. Good: Half of the symptoms are improved but most of symptoms remain. No Change: No change appears before and after treatment. Worse: The degree and duration of eruption get worse than pre-treatment state. Results For Acupuncture treatment, I chose the several Acu points like Hapgok(합곡), Taichung(대충), Gokji(곡지), Yanggok(양곡), Yanggea(양계), and lmeup(족임읍), and usually lasted it 15 minutes. With the acupuncture treatment and herb medicine, 2-3 times a week, the patients whose treatment period was 2 to 4 weeks were 17(47.2$\%$), 4 to 8 weeks were 11(30.5$\%$), 8 to 12 weeks were 3(8.3$\%$), 12 to 16 weeks were 3(8.3$\%$), longer than 16 weeks were 2(5.5$\%$) Collecting the statistics of the frequency of prescripted herb medicine, Hyangsosan(향소산) was prescripted 21 times(58.3$\%$), Hyangsapyungwisan(향사평위산) was 15 times(41.6$\%$), Hwapisan(화피산) was 9 times(25$\%$), Yangwitang(양위탕) was 6 times(16.6$\%$), Bojungikkitang(보중익기탕가미) was 4 times(11.1$\%$), Yongdamsagantang (용담사간탕가미) was 4 times(11.1$\%$). The result of the treatments, evaluating with mentioned rate scale, was 11 cases(30.5$\%$) were Very Good degree, 19 cases(52.7$\%$) were Good , 6 cases(16.6$\%$) were No Change. No cases were Worse degree. Conclusion According to this research, we could say that chronic urticaria can be treated with Oriental medical methods. But more precise probes in both Oriental and Western medicine to diagnose the chronic urticaria should be established and we need to make standards for testing and diagnosing the chronic urticaria.

Effect of Mahuang on Heart Rate Variability in Adults : a Double-Blind, Placebo-Controlled, Randomized Trial (마황 복용이 정산인의 심박변이도에 미치는 영향에 대한 무작위배정 이중맹검 임상연구)

  • Objectives : Mahuang (Ephedra sinica), well known as an herbal medicine in the East and West, contains a relatively high percentage of ephedrine known as sympathomimetic alkaloid. We investigated the effects of Mahuang on sympathetic nervous system with heart rate variety (HRV). Time and frequency domain analysis of HRV is a noninvasive technique capable of providing information on autonomic modulation of the sinus node. Methods : We investigated 57 healthy volunteers consisting of 37 subjects in the Mahuang group and 20 subjects in the placebo group. Study form was a randomized, placebo-controlled, double-blind clinical trial. The 37 subjects in the Mahuang group took 3 Mahuang capsules (1 capsule equivalent to 2g herb Mahuang) twice a day at 10 a.m. and 2 p.,., while the 20 subjects in the placebo group took 3 placebo-capsules filled with glutinous rice powder at the same times. Mahuang medicine and placebo medicine were made into opaque capsules. We measured HRV at 3 p.m. 1 or 2 days before medication and at 3 p.m. after medication. Results : Mean-RR and SDNN of the Mahuang group significantly decreased compared with that of the placebo group, but the heart rate of the Mahuang group significantly increased compared with the placebo. HRV-Index, RMSSD and SDSD of the Mahuang group significantly decreased compared with that of the placebo group, but PNN50 of the Mahuang group significantly increased compared with placebo. Ln(TP), Ln(VLF), Ln(LF) and Ln(HF) of the Mahuang group significantly decreased compared with those of the placebo group. There were no significant differences in normalized LF, normalized HF and LF/HF ratio between the Mahuang and placebo groups. Conclusion : The results suggest that Mahuang in healthy adults tends to reduce the autonomic nervous system within the normal range.
