The Relationship between Antiviral Drugs for Chronic Hepatitis B and Diagnostic Markers

  • In this study, we investigated the correlation between the administration of various antiviral agents and the alternation of specific biomarkers induced by the hepatitis B virus (HBV). Eligible subjects diagnosed with chronic hepatitis B were prescribed with antiviral drugs at the Gastroenterology Internal Medicine Department of E University Hospital in Daejeon between May 2004 and September 2009. Lamivudine was prescribed to 66 out of 100 patients. Of the 12 patients, 6 (50.0%) showed a change from being HBe-antigen-positive to being HBe-antigen-negative. Of the 39 patients, 23 (59.0%) showed higher than 40 IU/L alanine aminotransferase (ALT). Of the 65 patients, 41 (63.1%) showed HBV DNA decrease of 1 log, and were prescribed with Lamivudine. Adefovir was prescribed to 3 out of 100 patients. Of the 12 patients, 1 (8.3%) showed a change from being HBe-antigen-positive to being HBe-antigen-negative, and was prescribed with Adefovir. Entecavir was prescribed to 19 (19.0%) out of 100 patients. Of the 12 patients, 3 (25.0%) showed a change from being HBe-antigen-positive to being HBe-antigen-negative. Of the 12 patients, 3 (125.8%) showed higher than 40 IU/L ALT. Of the 65 patients, 14 (21.5%) showed HBV DNA decrease of 1 log, and were prescribed with Entecavir. Clavudine was prescribed to 7 out of 100 patients. Of the 12 patients, 1 (8.3%) showed a change from positive HBe antigen to negative HBe antigen. Of the 39 patients, 5 (12.8%) showed higher than 40 IU/L ALT. Of the 65 patients, 6 (9.2%) showed HBV decrease of 1 log, and were prescribed with Clavudine. These results do not show a statistically significant correlation between drugs and biomarkers. Data on combination therapy using Lamivudine and Adefovir show no statistically significant difference between drugs and biomarkers. Medications for periodic inspection was not correlated to HBe-antigen-negative conversion, ALT, and HBV DNA. HBV DNA was significantly reduced in patients with high levels of AST(aspartic acid aminotransferase) and ALT before treatment. In addition, the decrease of HBV DNA after 12 months of treatment was less frequently observed in patients treated with Lamivudine compared with other drugs. This result is associated with Lamivudine resistance. Although the association of drugs with diagnostic markers and the correct choice of treatment is difficult to determine, these results may be useful for further research on diagnosis and treatment of the hepatitis B virus.

한방병원의 건강검진 분석을 통한 대학생들의 B형간염 유병률과 간효소 이상의 유병률 특성 (Prevalence of HBsAg Positive and Abnormal Hepatic Enzyme among College Students by Health Examination in a Korean Medicine Hospital)

  • Objectives: Given current vaccinations against the hepatitis B virus and changes in dietary lifestyles, the status of liver disorders, especially in the younger population, is different than it was in the past. This study aimed to discover the prevalence of the HBsAg positive and abnormal hepatic enzyme among college students.Methods: The study analyzed the medical examinations of 2,646 college students that were performed at Semyung University from March to September 2013.Results: A total of 11 out of 2,646 students (0.4%) showed as positive for the hepatitis B antigen, while 23.8% of students had the serum HBs antibody. A total of 212 students (8.0%) showed abnormally elevated serum levels of ALT, with significant differences between men and women. The prevalence of the abnormal serum ALT and its average value significantly increased according to an increase of BMI score, especially in male students.Conclusions: This study showed the importance of obesity as a causative factor in liver disorders, especially in the young generation. This finding will be useful for the development of therapeutics using Korean medicine in the future.

Lamivudine Therapy Exacerbates Bilirubinemia in Patients Underlying Severely Advanced Hepatitis

  • Lamivudine belongs to the set of antiviral agents effective against hepatitis B virus infection. Given case reports on liver injuries after certain antiviral agent treatments, this study examined the effects of lamivudine on alanine aminotransferase (ALT) and total bilirubin (TB) using a medical system database. A total of 1,321 patients taking lamivudine alone or with others were evaluated using laboratory hits in an electronic medical system at Seoul National University Hospital from 2005 through 2011. The patients were grouped according to prior ALT results: G#1, ALT < 40 IU/L; G#2, 40 IU/L ${\leq}$ ALT < 120 IU/L; G#3, 120 IU/L ${\leq}$ ALT < 240 IU/L; and G#4, ALT ${\geq}$ 240 IU/L. In G#1 and G#2 patients, lamivudine or adefovir treatment decreased ALT and TB compared to prior values. In G#3 and G#4 patients with three times the upper limit of normal (ULN) ${\leq}$ ALT < 15 times the ULN, both ALT and TB were decreased after treatment with lamivudine alone, or adefovir following lamivudine therapy, indicating that lamivudine therapy ameliorated liver functions. However, in G#4 patients who experienced severely advanced hepatitis (ALT ${\geq}$ 15 times the ULN, or ${\geq}$ 600 IU/L), lamivudine augmented TBmax ($6.3{\rightarrow}13.3mg/dL$) despite a slight improvement in ALT ($839{\rightarrow}783IU/L$), indicative of exacerbation of bilirubinemia. Patients who used adefovir after lamivudine also showed a high incidence of hyperbilirubinemia when they experienced severely advanced hepatitis. Treatment with adefovir alone did not show the effect. In conclusion, lamivudine may increase the risk of hyperbilirubinemia in patients with severely advanced hepatitis, implying that caution should be exercised when using lamivudine therapy in certain patient populations.

전북 지역 건강 검진자들의 Anti-HCV 양성률 조사 (Prevalence of Anti-HCV among the Health-checkup Adults in Jeonbuk Province)

  • The author was performed to investigation of current status of prevalence for anti-hepatitis C virus (HCV) among the health-checkup adults in Jeonbuk province. A toal of 1,553 (male 1,046, female 507) serum samples were diagnosed by 3rd generation enzyme immunoassay (EIA) for anti-HCV. Total prevalence of anti-HCV was 0.9%, and prevalence of male and female were 0.8% and 1.2%, respectively. The prevalence of female was higher than male. According to ages group, prevalence of anti-HCV was highest in 60 age group, but it was not found in 20 age group. 14 samples with anti-HCV positive were diagnosed by EIA for hepatitis B virus surface antigen (HBs Ag), by chemiluminescence immunoassay (CLIA) for serum albumin, alanine transaminase (ALT) and asparagine transaminase (AST). Positive for HBs Ag was not found. The mean of serum albumin levels was 4.5 g/dL, and mean of ALT and AST were 34.3 IU and 31.9 IU, respectively. Through this study, I know that the prevalence of anti-HCV among adults in Jeonbuk, and suggest that the positive of anti-HCV persons who have lower serum albumin, normal to mild elevations in serum enzymes are chronic hepatitis.

Correlation of Chronic Hepatitis B Virus Infection with Diabetes Mellitus Indicators

  • Hepatitis B virus (HBV) infection has recently shown to be associated with diabetes mellitus. The objective of this study was to investigate the relationship between chronic hepatitis B and diabetes mellitus indicators. We evaluated anthropometry, metabolic syndrome risk factors, fasting glucose, HbA1c, and C-peptide among the normal and HBV subjects. The partial correlation and average comparison analysis were used to assess the independent association between chronic hepatitis B and diabetes mellitus indicators. Average comparisons of normal and HBV subjects were significantly different in fasting glucose (p<0.000), HbA1c (p<0.000), C-peptide (p<0.000), alanine transaminase (ALT) (p<0.000) and aspartate transaminase (AST) (p<0.000). We may suggest that HBV infection is related to diabetes mellitus indicators such as fasting glucose, HbA1c and C-peptide.

Current Role of Lamivudine Regarding Therapeutic Response and Resistance in Children with Chronic Hepatitis B

  • Purpose: To identify the predictive factors of long-term therapeutic response or resistance to lamivudine treatment in children and adolescents with chronic hepatitis B. Methods: Eighty one children and adolescents with chronic hepatitis B were included, who received lamivudine treatment for at least 6 months. Their condition was monitored for at least 12 months (12-88 months) thereafter. Twenty one (25.9%) were preschool children ($age{\leq}6$). For patients who had developed HBeAg seroconversion or breakthrough, univariate and multivariate analyses were used to identify the effects of age, gender, pretreatment alanine aminotransferase (ALT) and hepatitis B virus DNA levels. Results: HBeAg seroconversion occurred in 49 (60.5%) of the 81 patients after the initiation of the lamivudine therapy. In 65 patients whom were monitored for over 24 months, the seroconversion rate was significantly higher in younger patients (p=0.040), especially in those patients of preschool age ($age{\leq}6$, p=0.031). The seroconversion rate was significantly higher in higher pretreatment ALT (p=0.003). The breakthrough occurred in 21 (25.9%) of the 81. The breakthrough rate was lower in younger aged patients ($age{\leq}6$), and with higher pretreatment ALT levels, but no significant difference. Conclusion: Younger age is a good predictor of HBeAg seroconversion in children with long-term lamivudine treatment as well as high pretreatment ALT levels.

소아에서 발생한 비-A, B, C형 바이러스성 간염의 임상 고찰 (Clinical Features of Non-A, B, C Viral Hepatitis in Children)

  • 목 적: 소아에서 발생하는 급성 간염 중 많은 빈도를 차지하는 비-A, B, C 바이러스성 간염의 원인과 간염의 임상적 특징 및 경과에 대한 임상 양상에 대해 알아보고자 하였다. 방 법: 2001년 1월부터 2004년 6월까지 부산대학교병원 소아과에서 급성 간염으로 진단된 환자 중 독성, 대사성, 자가 면역성 간염과 신생아 간염은 제외하고 A형, B형, C형의 간염 바이러스 표지자 검사에서 음성 판정을 받은 총 45명의 환아를 대상으로 하여 후향적으로 원인 및 임상적 특징을 분석하였다. 결과: 1) 45명 중 원인 불명의 바이러스성 간염이 26명(57.8%)이었고, CMV 간염이 14명(31.1%), EBV와 HSV 간염이 각각 2명(4.4%), RV 간염이 1명(2.2%)이었다. 2) 남녀비는 1.8 : 1 이었고, 1세 이하가 27명(60.0%)이었다. 3) 여름에 환자 발생이 18명(40.4%)으로 가장 많았고, 동반 증상으로는 발열 13명(28.9%), 오심 또는 구토 12명(26.7%), 황달 10명(22.2%), 설사 8명(17.8%)의 순으로 많았다. 4) 신체 검사에서 간종대가 15명(33.3%), 공막 및 피부 황달 10명(22.2%), 비종대 9명(20.0%)이었다. 5) 임상 경과는 전형적인 급성 간염이 43명(95.6%), 전격성 간염이 2명(4.4%)이었으며, 한 명은 간부전으로 사망했다. 6) 혈청 ALT의 최고치는 $488.7{\pm}771.9IU/L$였고, 원인 불명의 간염이 $606.2{\pm}962.2IU/L$로 비교적 높았다. 7) 혈청 ALT는 2년 동안 경과 추적이 가능했던 38명 중 31명(81.6%)이 6개월 이내에, 4명(10.5%)이 1년 이내에 정상화되었다. 3명(7.9%)이 1년 이상 증가되었지만, 1년 6개월 이내에 모두 정상화되었다. 8) 혈액학적 이상으로 백혈구 증가증이 17명(47.2%)이었고, 재생 불량성 빈혈, 비정형적 림프구증가증, 혈소판 감소증이 각각 1명(2.8%) 있었다. 결 론: 비-A, B, C형 바이러스성 간염은 대부분 예후가 양호하나, 임상적으로 만성 간염의 경과를 보인 경우도 있고 전격성 간염으로 진행한 경우가 있으므로 면밀한 관찰이 필요하다.

만성 B형 바이러스성 간염환자에서 lamivudine 52주 투여의 치료효과에 대한 연구 (The Effect of 52 week Lamivudine Therapy in Patients with Chronic Hepatitis B)

  • Lamivudine, an oral nucleoside analogue, effectively inhibits hepatitis B virus replication and reduces hepatic necroinflammation in patients with chronic hepatitis B. Although lamivudine has shown a promise in patients with chronic hepatitis B, a long-term data on Korean patients with hepatitis B are lacking. The purpose of this study is to evaluate the effects and safety of 52-week lamivudine therapy in Korean patients with chronic hepatitis B, A total of twenty-nine patients (27 male and 2 female) who had received 100 mg of oral lamivudine daily for 52 weeks were evaluated, retrospectively. The mean age of 29 patients in the study group was 37.7 $\pm$ 8.9 years (range 19-54). Pretreatment HBV PCR and HBsAg were positive in all 29 patients, and HBeAg were positive in 25 patients (86%). The serum HBV DNA of 28 patients (97%) significantly fell to undetectable levels (<5 pg/ml) within 12 weeks of therapy and it remained undetectable in 24 patients (83%) by the end of 52-week therapy (p<0.001). Mean serum ALT levels of 29 patients declined to the normal range within 12 weeks and remained within the normal range during the evaluative period (p<0.05). The proportions of patients with HBeAg seroconversion (loss of HBeAg, development of antibody to HBeAg, and undetectable HBV DNA) were 42% after 52-week therapy. The differences of response to lamivudine therapy in HBeAg- positive and HBeAg-negative patients were negligible (p>0.05). Furthermore, the study showed that pretreatment serum HBV DNA and ALT levels have no effect to the efficacy of lamivudine therapy (p>0.05). Further comparison of lamivudine's efficacy showed that lamivudine is just as efficacious in patients with cirrhosis as without cirrhosis (p>0.05). In conclusion, lamivudine is an effective and safe therapy for the treatment of chronic hepatitis B in Korean patients.

Use of ALLGIO Probe Assays for Detection of HBV Resistance to Adefovir in Patients with Chronic Hepatitis B, Kerman, Iran

  • Hepatitis B virus (HBV) infection is contagious with transmissiobn vertically or horizontally by blood products and body secretions. Over 50% of Iranian carriers contracted the infection prenatally, making this the most likely route of transmission of HBV in Iran. To evaluate the resistance to adefovir (ADV) therapy in patients with chronic hepatitis B infection, a study was conducted on 70 patients (63 males and 7 females), who had received in first line lamivudine and second line adefovir. All were tested for the presence of hepatitis B surface antigen (HBsAg), hepatitis B envelope antigen (HBeAg), serum alanine amino transferase (ALT) level and HBV DNA load before and after treatment with ADV. In all samples, resistance to lamivudine and ADV was tested with real time PCR. Among seventy patients with chronic hepatitis B infection, 18 (25.7%) were resistant to LAM and 8 (11.4%) were resistant to ADV. Only one patient was negative for the presence of HBS-Ag (5.6%) and two were negative for HBe-Ag (11.1%). In this study we used a new method (ALLGIO probe assay) that has high sensitivity in detection of adefovir resistance mutants, which we recommend to other researchers. Mutant strains of the YMDD motif of HBV polymerase can be found in some patients under treatment with lamivudine and ADV. ADV has been demonstrated to be efficient in patients with lamivudine resistant HBV.

Clearance of Hepatitis B virus from Chronic Carrier by Oriental Medicine Treatments

  • Objective : To study the Oriental Medicine-based strategies or therapeutics for chronic HBV infection. Methods : A chronic HBV carrier was treated with only oriental therapies. Then, serum biochemical parameters were serially chased, and change of HBV-DNA level was evaluated. Result : The biochemical indicators (AST, ALT, gamma-GTP, bilirubin) fluctuated during the treatment period. After one episode of drastic elevation of serum aminotransferase, HBV-DNA disappeared from the blood along with normalization of biochemical parameters within two years of beginning treatment. Conclusion : Oriental Medicine-based therapeutics could be an alternative strategy against chronic infection of HBV.
