A Case Report of Patient with Type 2 Diabetes Mellitus Treated with Galgeun-tang (갈근탕을 투여하여 호전을 보인 제 2형 당뇨병 증례 1례)

  • Objectives: The aim of this study was to explore and describe how fasting blood sugar (FBS), postprandial 2 h Glucose (PP2h), and glycated hemoglobin (HbA1c) of a patient diagnosed with type 2 diabetes mellitus can be reduced by treatment with Galgeun-tang herbal medicine. Methods: The patient was administered herbal medicine to reduce serum glucose levels. The prescribed herbal medicines included Galgeun-tang and Galgeun-tang-gami. Results: The therapeutic outcomes were control of blood sugar and glycated hemoglobin (HbA1c) levels and decreased insulin administration. Conclusion: The herbal medicine, Galgeun-tang, appears to be a valid treatment for type 2 diabetes mellitus. Serum glucose (FBS/PP2hrs) and HbA1c were well controlled and insulin administration was decreased. Galgeun-tang was effective in controlling the daily glucose levels in a patient with type 2 diabetes mellitus.

  • A survey was carried out in order to figure out of phenomenon by school feeding from June 1973 to May 1974. Hematological values and plasma protein of 94 boys and 91 girls (total 185 students)were studied. As the results of this survey, the following conclusions were obtained. 1. Hemoglobin, hematocrit, mean corpuscular hemoglobin concentration of boys and girls were increased after school lunch. 2. Plasma protein of boys and girls were clearly increased after school lunch.

Blood Analysis for Indirect Doping Control of Erythropoietin in Sports (운동선수들의 혈액분석을 통한 Etrythropoietin 간접도핑검사)

  • The use of recombinant human erythropoietin (rhEPO), a stimulator of erythropoiesis, banned in sports because of the medical risk associated with thrombosis. Due to analytical difficulties to differentiate between natural human EPO (hEPO) and rhEPO, blood parameters of erythropoiesis such as contents of hemoglobin (cut-off value <17.5 g/d l for man, and < 16.0 g/dl for women), hematocrit and reticulocytes (cut-off value <2.0%) were measured to focus the misuse of rhEPO. We conducted anti-doping test for 122 blood samples of the World Cup athletes. The mean values of key parameters are as follows; 14.5$\pm$1.0 g/dl for hemoglobin, 41.7$\pm$2.8% for hematocrit, and 1.3$\pm$0.4% for reticulocyte. Blood sample was found to be stable up to 8 hours for the reticulocyte measurement. In addition, the soluble transferrin receptor and ferritin levels were measured by immunoassay methods using plasma samples (n=28) in which the mean value was 0.8$\pm$0.5 $\mu\textrm{g}$/$m\ell$ and 54.6$\pm$33.7 ng/$m\ell$, respectively. The results indicate that all samples tested were negative for the blood parameters of indirect anti-doping test for hEPO misuse. The statistical evaluation suggest that several other parameters such as red blood cell, mean corpuscular hemoglobin concentration, mean corpuscular volume, mean corpuscular hemoglobin and white blood cell could be considered as factors influencing hEPO function in addition to five parameters mentioned.

Association between Chronic Obstructive Pulmonary Disease and Hemoglobin Concentration in the Elderly: Based on National Health and Nutrition Survey (국민건강영양조사 자료를 이용한 노인의 만성폐쇄성폐질환과 헤모글로빈 농도의 상관관계)

  • This study determines the difference in prevalence of anemia and hemoglobin concentration between chronic obstructive pulmonary disease (COPD) and control groups in the elderly. Moreover, this study also examines the prevalence of anemia and hemoglobin concentration according to the severity of COPD in the elderly. Medical records of 8150 participants who answered the 7th National Health and Nutritional Survey conducted in 2016 were reviewed, and a total of 694 participants was included in the analysis. Participants were classified into two groups: COPD and control groups. Laboratory studies on hemoglobin, hematocrit, red blood cell concentration and prevalence of anemia were collected. Subgroup analysis was also conducted according to the severity of COPD. No significant differences were determined for the prevalence of anemia and hemoglobin concentration between both groups. Hemoglobin concentration was significantly higher in the COPD group. Although not statistically significant, there was a tendency for decrease in the prevalence of anemia and increase in the hemoglobin concentration as the severity of COPD increased. Our findings present specific data about the prevalence of anemia and hemoglobin concentration in elderly patients with COPD. Hence, we propose that occurrence of anemia in elderly patients with COPD requires further evaluation of the causes.

The Influence of Self-care Behaviors, Empowerment and Social Support on Glycosylated Hemoglobin in Patients with Type 2 Diabetes (제2형 당뇨병 환자의 자가간호행위, 임파워먼트 및 사회적 지지가 당화혈색소에 미치는 영향)

  • Purpose: The purpose of the study was to analyze the effects of self-care behavior, empowerment, and social support on glycosylated hemoglobin in patients with type 2 diabetes. Methods: The data were collected during the period of July 1 to July 31, 2016. In total, 172 participants were recruited from outpatients who had been diagnosed with type 2 diabetes at a health care center, a health promotion center at National Health Insurance Corporation, and a tertiary hospital. Statistical data were analyzed with SPSS 20.0 using frequency analysis, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients, and hierarchical regression analysis. Results: The study results showed that self-care behavior (${\beta}=-.34$, p<.001), empowerment (${\beta}=-.34$, p<.001), and social support (${\beta}=-.20$, p=.018) were found to be influential factors affecting glycosylated hemoglobin, with an overall descriptive power of 69%. Conclusion: Self-care behavior, empowerment, and social support are considered to be important factors in blood glucose management for the patients with type 2 diabetes. Therefore, self-caring blood glucose programs and internal synchronizing education through social support and empowerment need to be improved.

A study on the treatment of soil contaminated by pentachlorophenol with hydrogen peroxide and hemoglobin catalytic reaction (과산화수소와 헤모글로빈 촉매에 의한 펜타클로로페놀(PCP) 오염토양 처리에 관한 연구)

  • 400ppm의 pentachlorophenol(PCP) 오염토양을 5$m\ell$ scintillation vial의 multi reactor와 1L 크기의 one reactor를 써서 Hydrogen peroxide와 Hemoglobin 촉매반응에 의해 PCP 분해정도를 조사하였다. 대부분 초기에 반응이 빠르게 진행되므로 time scale을 8시간 이내와 한달여기간동안 살펴보았다. 8시간동안의 PCP 분해정도는 일차함수로 동력학 계수가 -0.0233으로 나타났고, 이때 제거효율은 60.8%이고 one reactor의 경우 30일동안 80%의 제거효율을 보였다. PCP 회수율은 multi reactor의 경우 96.5($\pm$6.7)이고 one reactor(fan scale)의 경우 90.1%였다.

Effects of Ethanol and Phenobarbital on Hemoglobin Adducts Formation in Rats Exposed to Direct Black 38 (Direct Black 38 염료를 흰쥐에 투여 시 형성되는 헤모글로빈 부가체에 에탄올과 Phenobarbital이 미치는 영향)

  • Objectives : To evaluate the effects on the formation of benzidine-hemoglobin, and benzidine metabolite-hemoglobin adducts, caused by pretreatment with the known xenobiotic metabolism effectors, ethanol and phenobarbital, in rats administered Direct Black 38 dye. Methods : The experimental rats were divided into three groups: a control group, an ethanol group and a phenobarbital group. Rats were pretreated with ethanol (1g/kg) or phenobarbital (80mg/kg) 24 hours prior to the oral administration of Direct Black 38 (0.5mmol/kg), with the control group being administered the same amount of distilled water. Blood samples were obtained from the vena cava of 5 rats from each group prior to, and at 30 min, 3h, 5h, 9h, 12h, 24h, 48h, 72h, 96h, and 144h following the oral administration of Direct Black 38. Directly after sampling the blood was separated into hemoglobin and plasma, with the adducts being converted into aromatic amines by basic hydrolysis. Hydrolyzed benzidiene, monoacetylbenzidine and 4-aminobiphenyl were analyzed by reverse-phase liquid chromatography with an electrochemical detector, The quantitative amount of the metabolites was expressed by the hemoglobin binding index (HBI). Results : In the ethanol group, benzidine-, monoacetylbenzidine-, and 4-aminobiphenyl-HBI were increased to a greater extent than those in the control group. These results were attributed to the ethanol inducing N-hydrgxylation, which is related to the formation of the hemoglobin adduct, In the phenobarbital group, all the HBIs, with the exception of the benzidine-HBI, were increased to a greater extent than those of the control group. These results were attributed to the phenobarbital inducing N-hydroxylation related to the formation of the hemoglobin adduct. The N-acetylation ratio was only increased with the phenobarbital pretreatment due to the lower benzidine-HBI of the phenobarbital group compared to these of the control and ethanol groups. The N-acetylation ratios for all groups were higher than f for the duration of the experimental period. Although the azo reduction was unaffected by the ethanol, it was inhibited by the phenobarbital, The ratio of the benzidine-HBI in the phenobarbital group was lower than those of the ethanol the control groups for the entire experiment. Conclusion : Our results indicate that both ethanol and phenobarbital increase the formation of adducts by the induction of N-hydroxylation, but also induced N-acetylation. Phenobarbital decreased the formation of benzidine-HBI due to the decrease of the azo reduction. These results suggest that the effects or ethanol and phenobarbital need to be considered in the biochemical monitoring of Direct Black 38.

Electrochemical Detection of Hydrogen Peroxide based on Viologen Monolayers (Viologen 박막을 이용한 과산화수소의 전기화학적 검출 특성)

  • In this paper, we fabricated a biosensor for detecting hydrogen peroxide and investigated the sensing property. We prepared a viologen and hemoglobin modified gold electrode using self-assembly and layer by layer method. The electrochemical property of the viologen derivative was characterized in 0.1 M $NaClO_4$ electrolyte solution by cyclic voltammetry. The modified electrode showed reversible electrochemical properties and high stability. From the results, the viologen can act as a charge transfer mediator for access to the electrode surface. The catalytic characteristics of the designed sensor proved that hemoglobin has been kept in its natural structure and can retain its biological activity. The designed biosensor showed a fast amperometric response, excellent linearity and low detection limit. In addition, it had high sensitivity, good reproducibility and stability.

AVAILABILITY OF AUTOLOGOUS TRANSFUSION IN BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY FOR MANDIBULAR PROGNATHISM (하악전돌증 환자의 하악지 시상분할 골절단술에서 자가수혈의 유용성)

  • Objective: Autologous transfusion is frequently used with orthognathic surgery. The necessity of autologous transfusion has begun to question alone in bilateral sagittal split ramus osteotomy. The aim of this study was to evaluate the availability of autologous transfusion. Methods: The chart reviews of sixty patients who had undergone bilateral sagittal ramus osteotomy were done. The subjects were divided into two groups: In experimental group, 30 patients who autodonated 320ml in the preoperative 3 weeks actually received a transfusion in the postoperative 6 hours were included. In control group, 30 patients who underwent the same operation without preoperative donation and any other transfusion were included. Hemoglobin, hematocrit, and changes of these values in both groups were evaluated. Results: 1. From postoperative 3 hours to postoperative 1 day, the increases of hemoglobin(0.8g/dL in experimental group, 0.2g/dL in control group) and hematocrit(0.3% in experimental group, 0.6% in control group) were not statistically significant between both groups. 2. From postoperative 1 day to postoperative 1 week, the increase of hemoglobin(0.6g/dL in experimental group, 0.3g/dL in control group) was not statistically significant between both groups. But the increase of hematocrit(2.5% in experimental group, 1.0% in control group) was statistically significant between both groups over the same period(hematocrit p=0.043). 3. On postoperative 1 week, the values of hemoglobin(12.3% in both groups) and hematocrit(35.6% in experimental group, 36.8% in control group) were not statistically significant between both groups. Conclusion: The autologous transfusion in surgery of just a little blood loss was not effective. The most results show that there is little availability of autologous transfusion according to changes of hemoglobin and hematocrit in bilateral sagittal split ramus osteotomy.
