개에서 발생한 결장직장 선암종 1례 (Colorectal Adenocarcinoma in a Dog)

  • An 8-year-old, female Pointer dog was presented with weight loss, hematochezia, and dyschezia. For the diagnosis, physical examination, complete blood counts, serum chemistry, radiography, ultrasonography, computed tomography, colonoscopy, cytology and histopathological examination were performed. The complete blood counts and serum biochemistry results were within the reference range. Ultrasonographic findings were presented with the thickened wall of the colon and rectum. Colonoscopy revealed irregular and ulcerated mucosal surface, mass and luminal narrowing of colorectal lesions. There were thickened wall and contrast enhancement of the lesion in colorectal region on the computed tomography. The cytologic examination suggested the adenocarcinoma. Postmortem histopathologic examination revealed adenocarcinoma. Based on these findings, the dog was diagnosed with annular colorectal adenocarcinoma.

  • For the investigation of bioactive natural products with hypoglycemic effect, we have evaluated various extracts of Pulsatillae Radix(Ranunculaceae), which has been used in traditional medicine for hematochezia due to intense evil heat, malaria, chills and fever, epistaxis and internal hemorrhoids. The ethylacetate extract of the radix of this plant was found to show a significant hypoglycemic effect on alloxan diabetogenic mice. Using bioactivity-guided chromatographic purification of the ethylacetate extract, hypoglycemic constituent: 2${\beta}$, 3${\beta}$, 14${\alpha}$, 20, 22R, 25-hexahydroxy-cholest-7-en-6-one was isolated and structurally identified by physico-chemical properties and spectroscopic evidences.

  • 일부 국가에서만 발병 예가 보고된 바 있는 신생아의 국한적인 원발성소장염전을 수술하여 생존시킨 2예를 보고하였다. 진단적인 소견은 갑자기 발병한 신생아 소장폐쇄 소견과 패혈증 및 1예에서만 보인 장관출혈 외에는 특기할 만 한 것이 없었으나 이러한 질환의 가능성을 염두에 두고 신속히 개복술을 시행함이 환자를 살리기 위하여 가장 중요한 점으로 판단되었다.

肛門病 中 痔의 範疇와 原因 症狀 및 治療에 對한 文獻的 考察 (A literatual studies on the chi-jil(痔疾).)

  • In oriental medicine, 'chi(痔)' is 'the prolapsed nodule' in 'Ku-gyu(九竅)', but in this paper, I will write about prolapsed nodule only in anus or around it, chi-jil(痔疾), it called hemorrhoid in western medicine. So in the literatual studies on chi(痔) in anus or around it, the results are as follows. 1. The etiology and pathogenesis of ch.i-jil(痔疾) is wind, wetness, dryness, and heat caused by inrregular diet habit, severe drinking and sexual action, deficiency of ki(氣) and hyeol(血). 2. Characteristic symptoms of chi-jil(痔疾) is the prolapsed nodule in the anus or around it, and general symptoms are hematochezia, pain, hernia, swelling, abcess, and mucosal secretion. 3. Chi-jil(痔疾) is classified eight types by characteristic symptom, shape, etiology and pathogenesis. They are mac-chi(脈痔), jang-chi(腸痔), ki-chi(氣痔), hyeol-chi(血痔), joo-chi(酒痔), mo-chi(牡痔), bin-chi(牡痔), and loo-chi(屢痔)(or young-chi(영痔), choong-chi(蟲痔)). Additionally, they are divided into two parts, internal and external chi-jil(痔疾), as likely as classification of internal and external hemorrhoid in western medicine. 4. Treatment of chi-jil(痔疾) is two methods, internal treatment and external treatment. Internal treatment is per os herb-pharmacotheraphy, external treatment is surgical or the other external pharmacotheraphy. There are several external treatment, these are fumigation-theraphy(熏痔法). irrigation-theraphy(洗痔法), paint-theraphy(塗痔法). withering-theraphy(枯痔法), bending-therphy(結紮法) and incisal -theraphy(切開法).

Double Primary Aortoenteric Fistulae: A Case Report of Two Simultaneous Primary Aortoenteric Fistulae in One Patient

  • Aortoenteric fistula is a rare but potentially fatal condition causing massive gastrointestinal bleeding. In particular, double primary aortoenteric fistulae are vanishingly rare. We encountered a 75-year-old male patient suffering from abdominal pain, hematochezia, hematemesis, and hypotension. His computed tomography images showed abdominal aortic aneurysm and suspected aortoenteric fistulae. During surgery, we found two primary aortoenteric fistulae. The one fistula was detected between the abdominal aorta and the third portion of the duodenum, and the other fistula was detected between the abdominal aorta and the sigmoid colon. We conducted the closure of the fistulae, the exclusion of the aneurysm, and axillo-bifemoral bypass with a polytetrafluoroethylene graft. The patient was discharged with no complications on the 21st postoperative day.

Lomustine Plus Hydroxyurea Chemotherapy for Primary Intramedullary Spinal Cord Tumor in a Maltese Dog

  • A 7-year-old, male Maltese dog with a body weight of 2.8 kg was presented with a history of hind limbs ataxia that progressed to tetraparesis over a one-month period. Based on physical and neurological examinations, tetraparesis with concomitant UMN signs, kyphosis and severe neck pain were identified. On MRI scan, we tentatively diagnosed this patient as a primary intramedullary spinal cord tumor. Therapy with lomustine plus hydroxyurea and prednisolone was initiated and the clinical signs rapidly improved. The patient was regularly checked by MRI scan and the range of the mass was gradually reduced to complete remission for 11 months. About 19 months after treatment, the patient showed anemia and hematochezia which suspected as adverse effects of chemotherapy. The condition was getting worse over 2 months and the patient suddenly expired 657 days after initial presentation. On histopathological examination, the spinal cord sample was identified as a neuronal atrophy without evidence of tumor cell.

Incidental finding of hemolymph nodes in a Holstein cow (Bos taurus taurus) with coccidiosis

  • This case report is about hemolymph nodes found in a dairy cow whose function is still not fully elucidated. A 4-month Holstein cow presented severe respiratory symptoms and hematochezia for a while with respiratory acidosis and metabolic alkalosis. Coccidiosis was diagnosed and treated immediately, but the cow died from respiratory acidosis and metabolic alkalosis. At necropsy, no abnormal appearance in thoracic and peritoneal organs was observed, but hemolymph nodes were observed being multifocally stuck on omasum serosa and the subcutaneous fascia of abdominal region, and the larger dark red lymph nodes were found along the omasum great curvature. Microscopically, lymphoid depletion and lymphadenitis in the lymph nodes were examined to point systemic infection, and in the hemolymph node, multifocally demarcated pale lesions with macrophage infiltration and fibrin deposition nearby subcapsular sinus. In subcapsular sinus of the hemolymph node, rod to linear gram-negative bacteria were found. Through this study, we might conclude that the hemolymph node is involved in pathogen phagocytosis.

Proven Cytomegalovirus Colitis Associated with Dasatinib Administration in Two Pediatric Allogeneic Hematopoietic Stem Cell Transplantation Recipients

  • Gastrointestinal (GI) bleeding is a rare adverse event of dasatinib, which is known to be caused by dasatinib-induced colitis, severe thrombocytopenia, and platelet dysfunction. We present two cases of pediatric patients who developed hematochezia during treatment with dasatinib after hematopoietic stem cell transplantation (HSCT). A colonic tissue biopsy was performed to differentiate the cause of GI bleeding. Both patients were diagnosed with proven cytomegalovirus (CMV) colitis, but only one was treated with ganciclovir. The patient who did not receive antiviral therapy experienced recurrent GI bleeding during dasatinib administration, leading to multiple treatment interruptions. During dasatinib therapy after HSCT, patients with GI bleeding and confirmed CMV colitis may benefit from antiviral therapy to reduce interruptions in dasatinib therapy.

Pancreatoduodenectomy following neoadjuvant chemotherapy in duodenal adenocarcinoma

  • A 51-year-old male patient had four times of massive hematochezia episode three days before arrival. Carbohydrate antigen (CA) 19-9 level was extremely elevated. Computed tomography, magnetic resonance imaging, and positron emission tomography-computed tomography identified 5.7 cm sized periampullary duodenal cancer with regional metastatic lymph nodes and vascular invasion to aberrant right hepatic artery, main portal vein, and superior mesenteric vein. Diagnosed as duodenal adenocarcinoma through endoscopic biopsy, 16 times of FOLFIRI (5-fluorouracil, leucovorin, irinotecan) was conducted. The regimen changed to XELOX (capecitabine, oxaliplatine), four times of administration was done, and the CA19-9 level dramatically decreased. The tumor decreased to 2.1 cm. After R0 laparoscopic pylorus preserving pancreatoduodenectomy, no adjuvant therapy was given. No sign of recurrence or metastasis was reported, and the patient reached complete remission after five years. We reported a case where neoadjuvant chemotherapy for locally advanced duodenal adenocarcinoma was shown to be effective.

Hookworm Infection Caused Acute Intestinal Bleeding Diagnosed by Capsule: A Case Report and Literature Review

  • Hookworm infections are rare causes of acute gastrointestinal bleeding. We report a middle aged man with primary nephrotic syndrome and pulmonary embolism. During the treatment with steroids and anticoagulants, the patient presented acute massive hemorrhage of the gastrointestinal tract. The results of gastroscopy showed red worms in the duodenum. Colonoscopy and CT angiogram of abdomen were unremarkable. Capsule endoscopy revealed fresh blood and multiple hookworms in the jejunum and ileum. Hookworms caused the acute intestinal bleeding. The patient responded well to albendazole. Hematochezia was markedly ameliorated after eliminating the parasites. Hence, hookworm infection should be considered in the differential diagnosis of a patient with obscure gastrointestinal bleeding. Capsule endoscopy may offer a better means of diagnosis for intestinal hookworm infections.
