The Characteristics of Tongue Inspection and Relationship between Tongue Inspection and Differenitiation of Syndrome (중풍초기환자의 설상(舌象) 분포와 변증의 유용성에 관한 임상고찰)

  • To assess the usefulness of tongue inspection for evaluating the Pattern identification in oriental medicine, we observed stroke patient's tongue and tongue coat and compared it with Pattern identification. The test group was composed of 85 acute stroke stage patients(within 72 hours of onset). Subjects were randomly selected from stroke patients admitted in the KyungHee University, Hospital of Oriental Medicine from December 1 1998 to June 30 1999. We took pictures of patient's tongue and tongue coat within 72hours from onset and checked Pattern identification at the same time. Tongues colored pale rose or red greatly outnumbered other colors. Tongue shape tended to be prickly or fissured, and tongue condition tended to be unflexible or deviated. Regarding tongue coat color, there were great amounts of yellow or clark yellow tongue coats, which were moist, thick or greasy in substance. The red tongue was significantly related to Fire-heat and deficiency of Yin syndrome, while faint white tongue to Damp syndrome(P=0.006). In terms of tongue coat, thin coat was related to Wind and Fire-heat syndromes, thick coat to Damp and Blood stasis syndrome, respectively (P=0.002). In conclusion, we thought that tongue inspection could be a useful Oriental medicine diagnosis in stroke.

Clinical Study on the Utilization of Yeoldahanso-tang and Cheongpyesagan-tang (열다한소탕(熱多寒少湯)과 청폐사간탕(淸肺瀉肝湯)의 활용에 대한 임상적 연구)

  • Objective: The purpose of this study is to examine the characteristics and the improvement in the symptoms of the patients who showed improvement after the administration of Yeoldahanso-tang and Cheongpyesagan-tang. Methods: The patients whose Sasang Constitution were examined by an expert at six Oriental medical hospitals and three regional Oriental-medicine clinics, 89 showed a predetermined symptom improvement when Yeoldahanso-tang and Cheongpysagan-tang were administered to them from November 2007 to July 2008. The traits of the patients and the effects of their treatment were examined. Results and Conclusions: 1. There were about as many male patients as there were female patients, but 21.3% of the patients who were female and who were in their 40's showed the most improvement, which makes them the biggest category in this study. 2. The main syndromes in which the patients showed improvement were hemi-paralysis, vertigo, and headache, and the ordinary symptoms in which the patients showed improvement were diarrhea, fatigue, and dry mouth. 3. The average food intake of the patients was highest and their feces had intermediate hardness (semi-soft). 4. The amount of the patients' sweat was either high(38.2%) or appropriate(38.2%) and they showed a dislike of heat over coldness. 5. Among the diseases that the patients previously had, high blood pressure. The above results show that Yeoldahanso-tang and Cheongpyesagan-tang can be considered beneficial to females in their 40's and to stroke patients whose food intake is normal, whose sweat level is either copious or appropriate, who dislike heat and who are suffering from high blood pressure.

Development of Instrument of Pattern Identification for Functional Dyspepsia (기능성 소화불량증 변증도구 개발 연구)

  • With the high prevalence of functional dyspepsia in the world, it was difficult to get objective diagnosis, treatment and assessment for the reason that there were many different symptoms and signs. The purpose of this study is to develop a standard instrument of pattern identification for functional dyspepsia which will be applied to clinical research. The items and structure of the instrument were based on review of published literature. The advisor committee on this study was organized by 11 oriental division of gastroenterology professors of oriental medical colleges nationwide. The experts discussed developing the instrument, and we also took professional advices by e-mail. We divided the symptoms and signs of functional dyspepsia into 6 pattern identification, such as disharmony of liver and stomach, retention of undigested food, damp-heat in the spleen and stomach, simultaneous occurrence of cold and heat syndromes, deficiency and cold of the spleen and the stomach, and insufficiency of stomach eum. We got the mean weights to each symptom of six pattern identification which had been scored on a 5-point scale ranging from 1 to 5 by the 11 experts. We made out the Korean instrument of the pattern identification composed of 45 questions for functional dyspepsia. Although there are some limitations in our study, the instrument is meaningful and certain worth of its own. We hope to improve the instrument through the further clinical studies and discussions.

A literature study about the comparison of Oriental-Occidental medicine on the Atopic dermatitis (아토피 피부염의 東西醫學的 文獻 考察)

  • The results of a literature study about the comparison of Oriental-Occidental medicine on the Atopic dennatitis were as follows. 1. In Oriental medicine, Atopic dermatitis belongs to the category of the Naesun(내선), Taeryumchang(胎斂瘡), Eczema(濕疹), Chimumchang(浸淫瘡). In Occidental medicine, the other names of Atopic dermatitis are allergic eczema, IgE dermatitis, flexual eczema etc. 2. In Oriental medicine, the definition of atopic dermatitis includes chang(瘡), sun(선), and pung(風). Occidental medicine, is one of the intrinsic eczema classifications. In fact eczema term circumscribes dermatitis in atopic patients. 3. In Oriental medicine, the etiology and pathogenesis of Atopic dermatitis arose from the state of cogenital defect(稟賦不足), internal accumulation of damp and heat(濕熱內蘊) at first, and then invaded pathogenic wind, damp, heat factors again, and combined endo-exoteric pathogenic factors. So it appears in skin. In Occidental medicine, the etiology and pathogenesis of Atopic dermatitis approaches in genetic, allergic and immunologic, pharmacophysioloic aspects. It is only a hypothesis but there is no known facts about radical aetiology. 4. In Oriental medicine, differentiation of syndromes classifies manifestation aspects, etiology and pathogenesis, and invasion period. In Occidental medicine, it divides into an invasion period, and clinical aspect etc. 5. In Oriental medicine, Internal theraphys of Atopic dermatitis are decoction of ingredients(湯劑), pills(丸), and tablet(片). So, it prescribes as treatments on the ground of differentition of syndrome. In Occidental medicine, there is no radical therapy because Atopic dispositions don't change. But steroid, antihistamine as symptomatic tre atments are generally used in Occidental medicine. 6. In Oriental medicine, external therapies are wet dressings(濕敷), lotion(洗劑), powder(散劑), adhesive plasters(膏劑), oil(油劑), smoking(烟薰法), warm over fire therapy(熱烘療法), acupunture and moxibustion therapy(鍼灸療法).

Study on Standard Symptoms and Gender Differences of Qi${\cdot}$Xue${\cdot}$Yin${\cdot}$Yang Asthenia Syndromes Based on Questionnaire Analysis (설문지 분석법에 의한 기허 혈허 기체 음허 양허증(氣虛 血虛 氣滯 陰虛 陽虛證)의 표준 증상 및 남녀차이 연구)

  • In order to establish the manifestational symptoms in men and women and highly frequent symptoms(HFS) of deficiency of Qi and blood(氣虛, 血虛), stagnation of Qi(氣滯), deficiency of Yin(陰虛), deficiency of Yang(陽虛), and, 546 questionaires were analyzed through Cronbach alpha value and Pearson's correlative efficient. Symptomatic differences of male and female specimens were as follows. HFS of Qi deficiency were similar in terms of inactivity like mental and physical fatigue, enervation in men and women. In blood deficiency, dizziness and light sleep were common in men and women, but mucosal symptoms of conjunctiva, lip, nail bed were more frequent in women than men. Emotional explosion was common in Qi stagnation, then women had more motivation-related symptoms than men. Inner heat was frequently arisen in Yin deficiency in both, and the heat was expressed more common in exterior and upper part in women but in interior abdominal part in men. In Yang deficiency, main symptoms of men were feeling coldness in peripheral, but Qi deficiency symptoms were more common in women former than coldness.

  • After The Yellow Emperor's Canon of Internal Medicine, The text researches of pathologic change to the liver disease concluded the next, 1, The category of liver-disease(肝病) include the Symptoms of abnormality due to vital energy and blood motion, emotion and intention, muscular and reproductive function, and legions around descending liver channel. 2. In the theory that Liver-Yang energy(肝陽氣) is always overproducing, Liver-Yin blood(肝陰血) is always lacking, pathologic characteristics for liver disease is functional change of malfunction of the use of body(體用失調), So nourishing the liver and kidney is used for the principal aspects of a disease. regulating and calm the liver is used for the secondary aspects of a disease as the treatment plan, 3. Sthenia-syndrome of liver(肝實證) and liver-heat syndrome(肝熱證) is appered that overproducing and overflow of dispersion(疏泄太過) and discharge is responsible for overfunctioning of liver disease or some kinds of heat syndrome such as liver fire(肝火), Sthenia of liver-yang(肝陽上亢), the syndromes of sthenic liver heat(肝實熱) are appered. deficiency of the liver(肝虛證) and cold syndrome of liver(肝寒證) is classified pathologic characteristics of cold and heat, deficiency and excess that regression of sensory, motor, mental due to lack of dispersion and discharge(疏泄不及), or intruding of the cold miasma, are degected. 5. The liver is close relation of physiologic function and internal organ such as spleen, stomach, lung, heart, kidney, gall bladder by the meridian channels, because of property of wind Zang, rapid progession is classified by phthologic charateristics.

  • Objectives : To satisfy the demand of good treatment of constipation Methods : we investigated the literatures of Oriental Medicine about Constipation. Results: 1. There are three categories of etimological factors of constipation, that is, endogenous, exogenous and non-exo-endogenous factor. The endogenous factor is caused by seven emotions, called depression of Ki and stagnation of Ki. The exogenous factor is six excessive atmospheric influences, for example, wind, cold, dampness, heat and dryness. And the non-exo-endogenous factors are overfatigue, improper diet, stagnated blood and deficiency of Ki and blood that comes from old age, long disease and after delivery. 2. Classification: According to cause of disease it is classified by constipation due to cold, heat, wind, dryness, retention of undigested and phlegm. According to Internal Organs there are constipation due to deficiency syndrome of the stomach, excess syndrome of the stomach, deficiency syndrome of kidney and splenic constipation. And Differentiation of syndromes according to Ki and blood, there are constipation of deficiency type and excess type. There are constipation due to stagnation and deficiency of Ki, deficiency of blood and stagnated blood. 3. Principles and Methods of treatment 1) Herbal Medicine (1) Excess type [1] Constipation due to heat : Seunggitang(承氣湯) and Majainwhan(麻子仁丸) [2] Constipation due to stagnation of Ki : Samatang(四磨湯) and Yukmatang(六磨湯) (2) Deficiency type [1] Constipation due to deficiency of Ki : Whanggitang(黃?湯) [2] Constipation due to deficiency of blood: Yunjangwhan(潤腸丸) [3] Constipation due to cold : Jechunjun(濟川煎) and Banyuwhan(半硫丸) 2) Enema therapy: It is a method to induce defecation by honey or pig's bile juice for weak people. 3) Acupuncture and Moxibustion: Acupoints used to treat constipation are BL25, ST25 and TE6. Moxibustion at CV8, CV6 is good for constipation due to cold. (4) Diet therapy: It is very important that we eat meals regularly and defecate on the same time even if you don't. And we have to eat food like fruits, vegetables and beans. (5) finger pressure: Finger pressing around these points like ST25, SP25, BL25, BL31, BL32, BL33 and BL34 is good for it. (6) Kigong therapy: Abdominal breathing (7) Old man' s constipation: Hip bath or diet therapy is commended. Laxation with lubricant like Supungyunjangwhan(搜風潤腸丸) is used. (8) Women' s constipation: After delivery, we have to administer tonifying prescription Sipjundaebotang(十全大補湯) and enema can be used. Conclusion : We have to examine the cause of disease and bowl movement carefully. After comprehensive analysis of the data gained by the four methods of diagnosis, we diagnose and treat patients on the base of overall of symptoms and signs.

  • Objectives : First, we analyze the contents of Dong-Eui-Bo-Gam. Second, from the view of the 'Hyung-Sang(形象:body form)' theory, we try to interpret the acupuncture points chosen in each chapter with a base on the contents of the book. Methods : We study on the on 'Hyung-Sang(形象: form)' theory of acupuncture points in each chapter on 'Shin(神)' and 'Hyul(血)' in Dong-Eui-Bo-Gam(東醫寶鑑). Results & Conclusions : The acupuncture points on epilepsy in 'Shin(神)' chapter are come from the purview of 'Shin(神)' type, fire-form, bird-form & the flushing and feeble of lower-abdomen. The acupuncture points on amnesia in 'Shin(神)' chapter are come from the purview of the powerful intestines and deficient heart-lung. The acupuncture points on dementia in 'Shin(神)' chapter are come more from the purview of 'Shin(神)' type, fire-form, bird-form. And the acupuncture points on sudden palpitation in 'Shin(神)' chapter are come from the purview of phlegen form syndromes. As the epistaxis and hematemesis on 'Hyul(血)' happen more the heat of fire than extravasated blood, the acupuncture points on epistaxis and hematemesis in 'Hyul(血)' chapter are come from the purview of 'Shin(神)' type, fire-form, bird-form and Yang-Myeong body. And as well as the acupuncture points on hematemesis in 'Hyul(血)' chapter are come possibly from the purview of 'Wind-form'. The acupuncture points on feeble hematemesis in 'Hyul(血)' chapter are come from the purview of 'The body form on the deficiency of medium energy'. And the acupuncture points on melena in 'Hyul(血)' chapter are come from the purview of 'Hyul(血)' type.

  • Objectives : This study is performed to investigate the therapy on tinnitus through the literature of Eastern and Western medicine. Methods : This study review the definition, etiology, classification, internal and external methods of treatment of tinnitus based on 50 articles of Eastern medicine, 16 articles of Western medicine about the treatment of tinnitus. Results : The results were as follows. 1. In Eastern medicine, the cause and mechanism of disease of tinnitus arose from the state of qi-blood deficiency, internal dampness-heat and depression stagnation. In Western medicine, there are mainly caused by damage to the auditory system the lower induced changes in the upper. 2. In Eastern medicine, differentiation of syndromes classifies clinical aspects, and cause and mechanism of disease. In Western medicine, it divides into an generating region, clinical and associated symptoms aspects. 3. In Eastern medicine, internal method of treatments of tinnitus are divided into four treatments. In Western medicine, internal methods of tinnitus are vasodilators, blood-flow improvers, Metabolism improvers etc. 4. In Eastern medicine, external method of treatments of tinnitus are paste preparation method, powder preparation method, pill preparation method, acupuncture & moxibustion method. In Western medicine, external method of treatments of Tinnitus are divided into injection, other surgical therapies and adjuvant therapy. Conclusions : Until now, there is no perfect, effective single treatment. We think that Eastern medicine approach and treatment can be helpful to overcome the limitations of tinnitus cure.

A Study on Perspirations(汗出) in Daily Time Cycle (하루 중 시간변화(時間變化)에 따른 한출(汗出)의 기전(機轉)에 대한 연구(硏究))

  • This thesis intend to help the eastern medical doctor to understand body condition from interpretation of perspirations(汗出) in daily time cycle. The conclusion is followed. 1. In most Eastern Medical classic and clinic literatures, the time of fever and perspirations are described as a result of disease's position at human body. Following this description, in daytime the perspirations must come from the Gi phase and night time the perspirations must come from the blood phase. Because in daytime the skin pores are opening and the defensive Gi is going out to the superficial portion of the body. In night time the skin pores are shutting and the defensive Gi is going in to the five solid organs. So a sweat in daytime comes out from the Gi phase and superficial portion of the body. And in night time comes out from the blood phase and five solid organs. But in recent real clinic cases, in daytime, there are so many perspirations from the five solid organs. Comparatively, the perspirations from the superficial portion of body are very little. And in same daytime perspirations, when the heat pathogens mixed with moist, the symptom revelation time delay to the afternoon. Therefore it can be concluded that the time of perspirations are combination of disease's Gi or blood phase and characteristics of pathogens. The position of disease at human body cannot simply judge the symptom revelation time. 2. The exchange of climate following time cycle of a day effect to the condition of human body. At same time it activates or not activates the pathogens in human body. So we can consider the kinds and characteristics of pathogens by distinguishing the symptom revelation time. In general differentiation of syndromes[辨證] pathogen's kinds and location are generally judged. By understanding the characteristics of pathogen, doctor can devise more correct and delicate prescription.
