한국표준질병 사인분류에 따른 위염(胃炎)의 한의학적 변증 연구 (Study on Syndrome Differentiation of Gastritis by Korean Standard Classification of Dsease and Cause of Death)

  • This article is for understanding relations between the classifications of gastritis and syndrome differentiation types of Korean Medicine through research on syndrome differentiations of clinically applied gastritis and literature of Korean Medicine. Clinical papers were searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI) from 1995 to 2015. Conclusions are as follows. First, disease mechanism of chronic gastritis are qi stagnation, damp stagnation, heat obstruction, blood stasis obstruction, yin damage, damage to collaterals with healthy qi deficiency and pathogenic qi. And qi movement stagnation is shown through the status of chronic gastritis. Second, chronic superficial gastritis belongs to qi aspect syndrome and mainly pathogen excess syndrome. And the key mechanisms are congestion and disharmony of stomach qi sometimes combined with liver depression, food accumulation and dampness-heat. Third, chronic atrophic gastritis belongs to qi-blood syndrome and deficiency-excess complex syndrome with the root of spleen qi deficiency and stomach yin deficiency and the tip of blood stasis, qi stagnation. And key mechanism is damage to collaterals with healthy qi deficiency and toxin-blood stasis. Forth, pathogen excess syndromes are shown at the early stage of chronic gastritis and healthy qi deficiency syndromes after the middle stage. Qi deficiency is shown at the beginning of the disease and yin deficiency at the late stage. And qi deficiency is related with superficial gastritis and yin deficiency with atrophic gastritis.

한의 복진 정량화 연구 - 기능성 소화불량 환자의 복냉 진단을 중심으로 - (Study on the Possibility of Quantitative Measurement of Abdominal Examinations in Korean Medicine - A Focus on Diagnosis of Abdominal Coldness in Functional Dyspepsia Patients -)

  • Objective: This study was designed to investigate the possibility of quantification of the diagnosis of abdominal coldness (AC) in patients with functional dyspepsia (FD). Methods: Forty-four patients with FD were enrolled in this study. Three Korean medicine doctors each randomly examined all abdomens. Diagnosis of AC was made by consensus of at least two of the doctors. Body temperature (oral by digital oral thermometer) and skin temperature (by digital infrared thermal imaging [DITI]) were measured, followed by administration of the Cold and Heat questionnaire (CHQ) and the Instrument of Pattern Identification for Functional Dyspepsia (IPIFD). Results: Of the 44 patients with FD, 22 were assigned to the AC group and 22 to the non-AC group. The concordance rate of diagnosis among the three doctors was 63.6% (28/44), with a ${\kappa}$ of 0.504, indicating means moderate agreement). Neither the oral nor the skin temperatures showed statistically significant differences between the AC and non-AC groups. However, the CHQ scores and 'Simultaneous Occurrence of Cold and Heat Syndromes pattern' scores of the IPIFD were higher in AC group and showed statistically significant differences (p=0.010 and 0.009). Conclusions: This is the first study conducting quantitative measurements of abdominal coldness in patients with FD. Although oral and skin temperature showed no statistical significance between AC and non-AC groups, the concordance rate of diagnosis of AC among the three Korean Medicine doctors was moderate. The CHQ scores and 'Simultaneous Occurrence of Cold and Heat Syndromes pattern' scores of the IPIFD also suggest that diagnosis of AC is relevant to cold and heat patterns, and these questionnaires could be utilized as supportive data for the diagnosis of AC. Further studies should be conducted for the purpose of quantifying and standardizing abdominal examinations in Korean Medicine.

갈근나복자탕(葛根蘿葍子湯)의 기원(基源)과 창방(創方) 시기(時期)에 관한 고찰(考察) (A Study on The Origin and Background of GalgeunNabokja-Tang)

  • 1. Background: GalgeunNabokja-Tang(葛根蘿葍子湯) is suggested as treatment and prescription of Exterior Heat syndrome Diarrhea(表熱證泄瀉) in Donguisusebowon Tae-eumin Interior febrile disease induced form the liver affected by heat("東醫壽世保元" "太陰人肝受熱裏熱病論"), but is not recorded the treatment contents in Sinjungbang("新定太陰人病應用要藥二十四方"). 2. Methods: Accordingly, the writer surveyed, compared, and arranged the relevant contents by collecting books of doctors of Four-Constitutional Medicine(四象醫) since Lee Je-ma including Lee Je-ma's books. 3. Results: and Conclusions: 1) The treatment contents in GalgeunNabokja-Tang are thought to be valid to have the criterion as the contents of appearing in SanggyohyuntoDonguisusebowon Boyubang("詳校懸吐東醫壽世保元" "補遺方"). 2) The origin in GalgeunNabokja-Tang is GalgeunNabokja-Tang in Bimang-lok("備忘錄"). And the period of having made treatment for the first time may be estimated to be the one between Donguisuse- bowonSasangchobonguan("東醫壽世保元四象草本卷") and HamsansachonDonguisusebowonGabogubon ("咸山沙村東醫壽世保元甲午舊本"). 3) GalgeunNabokja-Tang is thought to be probably what Han Du-jeong recorded in Boyubang by searching for what Lee Je-ma modified some of GalgeunNabokja-Tang in Bimang-lok while writing Pathological Syndromes("病證論") in Donguisusebowon("東醫壽世保元").

"상한론(傷寒論)"과 "온병조변(溫病條辨)"의 병인병기론적 비교 연구 (Comparative Study on Etiological Cause, Pathogenesis Mechanism of "Shanghanlun" and "Wenbingtiaobian")

  • We can understand "Shanghanlun(傷寒論)" and "Wenbingtiaobian(溫病條辨)" which are major books on externally contracted diseases well by making a comparative study of their similarities and differences. After studying etiological causes and characteristics of disease, disease pattern, syndrome differentiation, transmutation rules, following conclusions are derived. While cold is an etiological cause of Cold damage and harms Yang qi, heat is an etiological cause of Warm disease and harms Yin qi. Cold damage and Warm disease have something in common in the respect of damage to fluid and humor and Yang qi. Exuberant heat symptom of Yang brightness disease and lesser yin heat transformation pattern have similar damage to fluid and humor as Warm disease does. Warm disease can reach qi collapse syndrome through damage to Yang qi following fluid and humor damage. In the respect of water qi, as Cold damage makes water-dampness retain easily due to cold congealing, dampness-draining diuretic medicinal and warm yang medicinal are used together. As warm disease damages fluid and humor, yin-tonifying medicinal is used and dampness-draining diuretic medicinal can be used in the case of Warm disease with dampness. In the respect of disease pattern, cold syndromes arise mostly by Cold damage except heat syndrome of grater yang disease, chest bind syndrome, stuffiness syndrome, reverting yin disease and yang brightness disease. Warm disease is classified as pure heat syndrome and heat syndrome with bowel excess, damage to yin, qi collapse or damage to blood.

소화불량을 주소로 내원한 소양인(少陽人) 결흉증(結胸證)과 신열두통망음증(身熱頭痛亡陰證)의 소증(素證) 특성에 관한 단면연구 (The Cross-sectional Study on Characteristics between Soyangin Gyolhyungjeung and Soyangin Heat-related Diarrhea Accompanied by Headache in Patients of Indigestion)

  • 1. Objectives : The aim of this study was to survey the Original symptoms of Soyangin's and to compare the tendency of their symptoms between Soyangin Gyolhyungjeung and Soyangin Heat-related diarrhea accompanied by headache in patients of indigestion. 2. Methods : We recruited 76 Soyangin subjects in the Korean Medicine clinic and researched their original symptoms. Subjects were diagnosed by the Sasang Constitutional specialists and Sasang Constitutional Medication. Subjects were two groups, confirmed to Soyangin Gyolhyungjeung and Soyangin Heat-related diarrhea accompanied by headache. All subjects answered the questionnaire about their Original symptoms. We found the differences between 2 groups using the Mann-Whitney U Test. Analysis was conducted using SPSS 18.0 for Korean. 3. Results : In Original symptoms, there was no difference between 2 groups in sleep, appetite, digestion, eating habits, water temperature, function of bowel movement, urination, sweating, sensitivity to coldness and sensitivity to warmness. But there was difference between 2 groups in frequency of defecation & hardness of feces(p=0.027). 4. Conclusions : We found the differences of heat and cold in symptoms between two Soyangin syndromes.

결흉(結胸)의 병인병리에 대한 문헌적 고찰 (Literatural study on the Causal Pathogenesis and Pathology of Kyeolhyung)

  • This study was wrriten in order to to help understanding of Kyeolhyung(結洶). Kyeolhyung is a syndrome of epigastric pain and fullness due to accumulation of pathogen in the chest mostly caused by too early purgation of Taiyang(太陽) channel syndrome which leads to the exterior syndrome of heat from the exterior into the interior or combines with the original excessive fluid of the chest. The knotted chest can be divided into large, small, heat and substantice, cold and substantice, water and blood Kyeolhyung. Kyeolhyung is basically treated by purgation but practically it is treated by according to its own symptom and syndromes. And we were cured of the Kyeolhyung discriminating from Zanggyul(藏結) and Bijeung(비증).

방유집(方有執)의 『상한론조변(傷寒論條辨)』 조문(條文) 구성에 대한 고찰(考察) (A Study on the Composition of Shanghanluntiaobian(傷寒論條辨))

  • Objectives : The purpose of this study is to investigate the method of setting up logic and estimate Fang Youzhi's work through Shanghanluntiaobian. Methods : Wang Shuhe's old edition Shanghanlun and Fang Youzhi's Shanghanluntiaobian were compared and several essential provisions were analyzed and addressed the positive and negative aspects of Fang Youzhi's revising old edition Shanghanlun mixed bamboo tablets. Results : Fang Youzhi's changing Bianmaifa("辨脈法") and Pingmaifa("平脈法") and placement Bianchishiyemaizhengbingzhi chapter after Liujingbingmaizhengbingzhi for discriminating convulsion dampness summer heat stroke from febrile diseases is appropriate. Fang Youzhi's deletion Shanghanli("傷寒例") chapter that has a guiding characteristics shows his editorial policy. Fang Youzhi's addition Bianwenbingfengwenzabingmaizhengbingzhi chapter is for discriminating febrile disease from epidemic febrile disease wind warmth syndrome miscellaneous disease and it indicates Shanghanlun is a book of treatment after differentiation of syndromes containing epidemic febrile disease and miscellaneous disease. Through Fang Youzhi's revising method I can see several positive aspects. First he tried to refine the logic of the content of Shanghanlun and accord with an actual situation. Second he shows Shanghanlun is a book of treatment after differentiation of syndromes containing epidemic febrile disease and miscellaneous disease and tried to see Shanghanlun clearly. A criticism of the Preserving Old Edition Shanghanlun group could interrupt flexibility and creative thinking reading Shanghanlun and a criticism to the Sangangdingli Principles of the several exegetists is a productive criticism because medicine treats human being so it cannot be completed by a logic. Conclusions : All of these processes are Fang Youzhi's endeavour for strengthening the system and logicality of the old edition Shanghanlun and it is meaningful to show a method to read Shanghanlun to beginners.

Objectives: The purpose of this study was to survey the Original symptoms(素證) of Soyangin's and to compare the tendency of their symptoms between Soyangin Heat-related diarrhea accompanied by headache(身熱頭痛亡陰證) and Soyangin Cold-related diarrhea accompanied by abdominal pain(身寒腹痛亡陰證). 2. Methods: We recruited 56 Soyangin subjects in the Korean Medicine clinic and researched their original symptoms. Subjects were diagnosed by the Sasang Constitutional specialists and Sasang Constitutional Medication. Subjects were two groups, confirmed to Soyangin Heat-related diarrhea accompanied by headache and Soyangin Cold-related diarrhea accompanied by abdominal pain. All subjects answered the questionnaire about their Original symptoms. We found the differences between 2 groups using the Independent-Samples T-test. Analysis was conducted using SPSS 12.0 for Korean. 3. Results and Conclusions: In Original symptoms, Tendency of overeating(p=0.01), Condition of digestion(p=0.04), Perspiration after exercise (p=0.02), Usual perspiration(p=0.00), Tiredness after perspiration(p=0.05) Cold intolerance on extremities(p=0.04), General edema(p=0.01) etc. are different between 2 groups. We found the differences of heat and cold in symptoms between two Soyangin syndromes.

황연해독탕가미방으로 호전시킨 중풍 환자 불면증 3예 (The Effect of Hwangyeonhaedogtang-Gamibang on Insomnia in Stroke - 3-Case Report -)

  • Insomnia is a disorder of initiaton and maintenance of sleep that results in daytime sleepiness and dysfunction, and arises from multiple psychological, physiologic and environmental factors. A number of stroke patients suffer from insomnia classified as one of the sleep disorders associated with physical illness and on the contrary insomnia may have profound deleterious effects on the natural course of stroke. We experiened three cases of stroke patients with insomnia. In the point of Differentiation of Syndromes, these subjects were diagnosed as Excessive heat-fire syndrome of stroke and were administrated with Hwangyeonhaedog-tang Gamibang. After treatment, both sleep patterns and other accompanied symptoms improved.

  • Seop-Gye divided up the constitution's strength and weakness, and so he cured disease by the 'cold and heat'. 'deficiency and excessiveness' means on the basis of the YINㆍYANG of constitution. He classified constitution with pathogenesis grounding in body formal distinction, flesh hardness and softness, face color and form, skin color. Therefore he grasped the rule of constitution connected with physiological function and pathological distinction, applied the rule of constitution in the diagnosis and cure of the chronic & epidemic febrile disease.
